OBJECTIVE: To describe the incidence and main clinical characteristics of barotrauma during open lung ventilation (OLV).
DESIGN: A retrospective, observational, descriptive study was made of 100 patients with acute respiratory failure and bilateral pulmonary infiltrates.
INTERVENTIONS: 1) A lung recruitment maneuver (LRM) with fixed ventilation pressure and progressive positive end-expiratory pressure (PEEP) elevations was carried out, followed by stepwise decreases until establishing open-lung PEEP at the value associated to maximum respiratory compliance; 2) assisted/controlled pressure ve

Sinus barotrauma, secondary to mucosal disturbances, is a common finding within the aviation community. Multiple etiologies have all led to mucosal inflammation and thickening with potential obstruction of the sinus osteomeatal complex, especially during the barometric changes of flight. Obstruction can, therefore, lead to problems with sinus pressure equilibration with atmospheric pressure, and can lead to barosinusitis. We present a case of a U.S. Air Force Command Pilot with acute left sinus barotrauma during descent while flying a T-37 aircraft, along with a brief review of the pathophysio

Most reports describe reduction in proximal airway pressures with high-frequency jet ventilation. This led us to speculate that high-frequency jet ventilation might reduce barotrauma by providing alveolar ventilation at lower airway pressures. We describe a group of patients in whom a high incidence of barotrauma was observed after institution of high-frequency jet ventilation despite reduction in measured airway pressures. Fifteen hypoxic patients who could not be treated with conventional ventilation and who had no roentgenographic evidence of barotrauma were entered into the study. Airway p

Recurrent sinus barotrauma in an aviator is difficult to treat successfully. Exacerbations frequently result in marked aviator discomfort, cycles of temporary restriction from aviation duties, or even permanent disqualification for flying duties. Medical management and standard sinus operations are often ineffective, seldom curative, and have a disappointing record in returning the aviator to flying duties. Detailed computerized tomographic scanning of the paranasal sinuses coupled with the functional endoscopic sinus surgery approach directs treatment at the causative pathology. Sinus ventila

A left diaphragmatic hernia was created surgically in 20 fetal lambs between 93 and 110 days of gestation. Ten animals were alive with defects at cesarean section near term (135 to 140 days). These animals and two controls were submitted to various transpulmonary pressure gradients (inspiratory pressure minus pleural pressure). Hemodynamic and ventilatory studies were performed after the correction of the hernia. Morphometric analysis of the lung was carried out in all cases. The results showed a highly significant linear correlation between the transpulmonary pressure gradient employed and th

On the basis of our own experiences and the current literature, the following guidelines were established for the evaluation of scubadivers: 1. The ENT physical examination must include otoscopy and the valsalva manoeuver. The scubadiver should be able to promptly and symmetrically inflate his middle ear spaces. A central perforation is a relative contraindication, while a marginal ear drum perforation is an absolute contraindication for scubadiving. 2. Recommendations to the diver: Ear pressure equalibration should be performed continuously with increasing and decreasing water depth. Ear plug

BACKGROUND: Military free fall is distinct form of tactical parachuting being performed by paratroopers. During free-fall, the paratrooper descends at speeds more than 193 km/hour i.e. 1000 ft in 6 s from a height of around 10,000-12,000 feet. Human bodies are well adapted to bear small range of atmospheric pressure; however, a wide range of pressure difference resulting from sudden altitude change may lead to significant morbidity in the form of barotrauma in air-filled bodily cavities. Middle ear barotrauma appears as a result of uncorrectable rapid pressure difference between middle ear and

Intrahepatic gas (IHG) is commonly observed during early postmortem examinations of humans with upper or lower airway obstructions. We conducted a study to test the hypothesis that intrapulmonary gas could retrogradely spread to the hepatic vein following pulmonary barotrauma (PB). To establish a rat model of pulmonary barotrauma, we utilized a controllable pressure-vacuum pump to apply airway pressure (40, 60, or 80 mmHg). The rats were dissected directly at the end of the experiment, and histological analysis was performed through microscopic examination of the rats. Additionally, the rats w

Introduction. Pharyngoesophageal perforation secondary to barotrauma is a rare phenomenon that can have serious complications if identified late. It is challenging to detect due to nonspecific symptoms. We present a case in which detection proved difficult leading to delayed diagnosis. Case Report. A 27-year-old mechanic presented with haemoptysis, dysphonia, and odynophagia after a car tyre exploded in his face. Flexible nasoendoscopy (FNE) revealed blood in the pharynx, thought to represent mucosal haemorrhage. Initial treatment consisted of IV dexamethasone and antibiotics. After 3 days, od

INTRODUCTION: Barotitis media is known to be an adverse effect of altitude changes, but few studies have investigated this condition with respect to hypobaric chamber training and the resulting estimations of prevalence vary.
METHODS: In order to assess the prevalence of hypobaric chamber-related barotitis and evaluate a method of prevention, 335 healthy military pilots undergoing high altitude training were subject to clinical examination and tympanometry before entering the chamber. In order to minimize the risk of barotrauma, only subjects with normal preflight findings were cleared for al

Mallory-Weiss syndrome (MWS) is a well-defined entity in clinical medicine. However, the development of such a syndrome as a result of overpressure barotrauma of the stomach after repeated shallow-water scuba dives is rare. Also rare is the delayed onset of the MWS, approximately 20 hours after the dives. The causes of development of MWS in connection with scuba diving are discussed. The main causes seem to be the repeated changes of gas volume in the stomach with subsequent pressure forces toward the cardia in the course of repeated dives. The possibility of serious diving accident due to ove

A fit young man sustained a ruptured diaphragm during a recreational scuba dive three months after undergoing an uncomplicated laparoscopic Nissen fundoplication for gastro-oesophageal reflux disease. It is proposed that this rare occurrence was attributable to gastrointestinal barotrauma. The injury was treated by laparotomy, mobilisation of herniated abdominal viscera back into the abdomen, repair of the crura and gastropexy. He made a full postoperative recovery. It is concluded that scuba diving should be avoided in patients who have undergone fundoplication.

The presence of air in the interstitial pulmonary tissues is shown as interstitial emphysema and is often the first sign of barotrauma. It results from hyper-pressure in the airways occurring for the greater part of the time on some underlying pulmonary pathology. This extra alveolar air may diffuse into the interstitium and collect under the visceral pleura, appearing on radiographs as air cysts from 3 to 10 centimeters in diameter. We described 7 cases of air cysts occurring in adults on mechanical ventilation. The lesions seem to us to represent a radiological entity and evidence of barotra

Scuba diving exposes teeth, dental restorations and prosthetic devices to substantial hyperbaric mechanical stress. Dental barotrauma (pressure-induced structural injury) and barodontalgia (pressure-induced dental pain) are oral complications that remain underdiagnosed in routine dental practice, despite their capacity to cause acute pain, restoration failure and in-water emergencies. Gas entrapped within defective restorations, incompletely obturated root canals or poorly adapted prostheses responds to pressure change according to Boyle's law, generating forces that can fracture tooth structu

BACKGROUND: Sinus barotrauma, or aerosinusitis, occurs during rapid atmospheric changes in aviation, primarily affecting the frontal sinus. Mucosal swelling from a cold or allergic rhinitis (AR) can obstruct pressure equalization, leading to mucosal tears, cranial pain, and nasal bleeding. Despite its significance in aerospace medicine, high-quality imaging, nasal endoscopy, long-term outcomes, and the impact of AR management on sinus barotrauma remain inadequately documented in the literature.
CASE REPORT: A 29-yr-old healthy male healthcare provider experienced severe frontal sinus barotrau

BACKGROUND: Most of the cases of middle ear barotrauma in divers are due to the impassability of the Eustachian tube. The aim of our study is to compare the results of tympanometry and Valsalva part of Eustachian Tube Function test (ЕТF-test) with the ability of divers to compensate for the change in ambient pressure in a hyperbaric chamber.
MATERIAL AND METHODS: The study included 35 professional divers undergoing annual medical examination. For all subjects is measured first intratympanal pressure at rest, then after the maneuver of Valsalva with impedancemeter. Then a barofunction test (BF

BACKGROUND: This study was undertaken to establish a dynamic animal model of sinus barotrauma (SB).
METHODS: The right nasal cavities of 65 rabbits were filled with sponges to obstruct the right ostiomeatal complex (OMC), while in the left nasal cavities, the left OMC was kept clear. The rabbits were exposed to hypobaric chamber simulation. The right sinuses were assigned as the model group, randomly divided into 13 subgroups with 5 in each subgroup, while the left sinuses were assigned as the control group. The hypobaric chamber simulation involved 6 pairs of ascending/descending speeds (100

Pneumoperitoneum is usually secondary to perforation of gastrointestinal tract. Sometimes it is a barotrauma, especially in newborn infants undergoing mechanical ventilation, causing lung rupture and passage of air from airway into the peritoneum through the mediastinum. It seems less likely that the pneumoperitoneum occurs in a patient undergoing mechanical ventilation, in the absence of a demonstrable barotrauma and a bowel perforation. We present a preterm patient who, on her third day of life, while being subjected to mechanical ventilation, reports a massive pneumoperitoneum with the appa
