Saccular aneurysm at the basilar artery fenestration.

The authors describe a 19-year-old patient who was presented with a subarachnoid hemorrhage and was found to have a saccular aneurysm arising at the proximal end of the fenestration of the basilar artery. Previously reported cases are reviewed. Stereoscopic magnification angiography facilitated the analysis of the three-dimensional-anatomical relationship between the aneurysm and its fenestrated parent artery.

Endovascular treatment of basilar trunk aneurysm associated with fenestration of the basilar artery.

Basilar trunk saccular aneurysms associated with fenestration are infrequent, especially in the middle or distal portion of the basilar artery. Surgical treatment of the basilar trunk aneurysm is difficult, due to its anatomical environment and the complicated surgical exposure. A 46-year-old woman presenting with Hunt and Kosnik grade II subarachnoid hemorrhage was found to have a ruptured aneurysm arising at the proximal corner of the associated fenestration in the middle portion of the basilar artery. Because of surgical difficulties anticipated in approaching the aneurysm, it was decided t

Subarachnoid hemorrhage and granulomatous angiitis of the basilar artery: demonstration of the varicella-zoster-virus in the basilar artery lesions.

A 70-year-old man, with regional herpes zoster (C2) of 10 weeks duration, died following subarachnoid hemorrhage caused by the rupture of an aneurysm in the basilar artery. Granulomatous angiitis, with multinucleated giant cells, was found at autopsy in the wall of the aneurysm. Electron microscopy of the basilar artery disclosed intracytoplasmic viral particles with an envelope which measured 150-220 nm in diameter. Immunohistochemistry studies revealed varicella-zoster-virus-related antigen in the cytoplasm and/or in the nucleus of histiocytes in the vessel wall. These findings suggest that

[Traumatic basilar-cavernous fistula complicated by basilar artery aneurysm: report of one case].

A 19-years-old boy was admitted after brain trauma with subarachnoid hemorrhage, left oculomotor nerve palsy, elevated pressure in the cavernous sinus and intracranial noise. The diagnosis of traumatic basilar-cavernous fistula complicated with basilar artery aneurysm was established by digital subtraction angiography. After endovascular occlusion of the vessels twice using detachable coils, the patient was cured without relapse during the 2-year follow-up. It is at the time being second report globally and the first one in China.

Concomitance of Persistent Primitive Olfactory Artery and Basilar Artery Fenestration.

The cerebral arterial circle exhibits substantial morphological variability, occasionally encompassing rare congenital vascular variants. We report a unique case of a 64-year-old woman in whom computed tomography angiography (CTA) revealed the coexistence of a persistent primitive olfactory artery (PPOA) and basilar artery (BA) fenestration. The PPOA originated from the left anterior cerebral artery (ACA) and coursed anteroinferior toward the cribriform plate, where it formed a characteristic hairpin turn before resuming the typical A2 trajectory. Concurrently, a BA fenestration was identified

Acute superior mesenteric artery occlusion complicated by basilar artery occlusion.

Acute mesenteric ischemia is a rare cause of abdominal pain with a very high mortality rate. Vague presentation and often misleading clinical findings make the diagnosis elusive. Here, a unique case of complete superior mesenteric artery occlusion further complicated by basilar artery occlusion is illustrated.

Simultaneous unilateral internal carotid artery and occlusive basilar artery dissections.

We report a case of brain stem infarction. The case is interesting for simultaneous occurrence of basilar artery (BA) occlusion caused by BA dissection and left internal carotid artery (ICA) dissection after a minor cervical trauma. Stents were implanted at the ICA dissection, and BA occlusion was treated conservatively.

The Effect of the Vertebral Artery Stenosis on the Basilar Artery Tortuosity.

AIM: To investigate the effect of vertebral artery stenosis (VAS) on the basilar artery tortuosity. MATERIAL AND METHODS: A total of 158 patients (101 males; 57 females) were divided into two main groups depending on existence of the in the vertebral artery (VA). Then, these groups were divided into two sub-groups according to the stenosis severity. All these groups were compared via the following variables: the gender, age, stroke risk factors, and basilar artery deflection angle. RESULTS: The dominant VA was found more common on the left side; and most of the cases had an adverse direction

A vertebral artery dissection with basilar artery occlusion in a child.

This paper presents the case report of an 11-year-old boy with an acute dissection with thrombosis of the left vertebral artery and thrombosis of the basilar artery. The patient was treated with acute systemic thrombolysis, followed by intra-arterial thrombolysis, without any clinical improvement, showing left hemiplegia, bilateral clonus, hyperreflexia, and impaired consciousness. MRI indicated persistent thrombosis of the arteria basilaris with edema and ischemia of the right brainstem. Heparinization for 72 hours, followed by a two-week LMWH treatment and subsequent oral warfarin therapy, r

Small basilar artery fenestration mimicking an aneurysm.

Fenestration of the basilar artery is a well-known anatomical variant which can be misdiagnosed as arterial dissection. Misdiagnosis as an aneurysm is also possible and has been reported. A 41-year-old woman was referred to our hospital for evaluation of a suspected aneurysm of the proximal basilar artery trunk found on 1.5 T magnetic resonance angiography. More detailed 3 T magnetic resonance angiography revealed a small fenestration, not an aneurysm. The diameters of the vessels in the two limbs forming the fenestration were remarkably different: one limb had almost the same diameter as the

Endovascular Treatment of Symptomatic Basilar Artery Stenosis.

PURPOSE: While symptomatic basilar artery (BA) stenosis is associated with a higher risk of recurrent stroke or death, there is no consensus on the management of these patients who are refractory to antiplatelet therapy. This study retrospectively assesses the outcomes of endovascular treatment (EVT) for symptomatic BA stenosis. MATERIALS AND METHODS: We conducted a retrospective review of patients with symptomatic BA stenosis who underwent EVT, including angioplasty or stenting, from 2006 to 2018. A total of 15 patients, who experienced transient ischemic attacks or strokes despite dual anti

Basilar Artery Occlusion: Diagnosis and Acute Treatment.

PURPOSE OF REVIEW: To provide the latest updates in the diagnosis, management, and prognosis in patients with basilar artery occlusion (BAO). RECENT FINDINGS: The diagnosis of BAO requires a high index of suspicion among patients with sudden onset depressed level of consciousness, quadraparesis (or hemiparesis), and cranial nerve dysfunction. Computed topography with angiography is currently the diagnostic modality of choice for the diagnosis of BAO. Given the often catastrophic nature of BAO, intravenous thrombolysis combined with mechanical thrombectomy should be offered in eligible patient

Basilar stroke from a persistent hypoglossal artery.

Occlusion of the basilar artery can be treated with endovascular thrombectomy, although the results have not been well studied. Persistent fetal cerebrovascular anatomy can lead to unusual presentation of carotid atherosclerotic disease and can be a barrier to successful mechanical thrombectomy if not recognized. This case presents a rare persistent carotico-basilar anastomosis which resulted in basilar occlusion via the persistent hypoglossal artery and coincident absence of contralateral vertebral arterial access due to a left vertebral artery terminating in the left posterior inferior cereb

Delay in Diagnosis of Basilar Artery Stroke.

BACKGROUND: Basilar artery stroke causes substantial morbidity and mortality. Although its unusual clinical presentation potentially contributes to a delay in diagnosis, this problem has not been systematically studied. We compared intervals between symptom onset, initial presentation, and diagnosis in stroke due to basilar artery (BA) versus left middle cerebral artery (LMCA) occlusion to determine the presence of and potential reasons for diagnostic delay in BA stroke. METHODS: We retrospectively identified 21 consecutive adult patients diagnosed with BA stroke between 2009 and 2011 from ou
