Boarded patients, blurred benchmarks: an opportunity for CMS.

The Centers for Medicare & Medicaid Services recently introduced 2 clinical quality measures that intend, in part, to disincentivize hospital boarding: a patient safety concern that involves holding admitted patients in the emergency department while an inpatient bed is unavailable. The Age-Friendly Hospital Measure (AFHM), a structural measure, asks hospitals to attest to having protocols in place to reduce boarding for older patients. The Emergency Care Access and Timeliness (ECAT) measure, an intermediate outcome measure, will require hospitals to report on the proportion of boarded patient

A website calculator to benchmark the carbon footprint of haemodialysis.

BACKGROUND: Haemodialysis (HD) contributes vastly to greenhouse gas (GHG) emissions. Recognizing this, the German Society for Nephrology initiated a web-based carbon footprint assessment tool to benchmark emissions. METHODS: This study collected data from five pilot HD centres between 2015 and 2023. Emission categories appropriate for HD were defined and included transportation, energy consumption, manufacturing/disposal and other operational factors. RESULTS: The all-centre, all-period average was 3.72 ± 0.44 tons of carbon dioxide equivalents per patient per year, with manufacturing/dispos

Achievable Benchmarks of Care in Pediatric Emergency Departments.

IMPORTANCE: Achievable benchmarks of care (ABCs) are a process by which performance is anchored on a select high-achieving group. ABCs can motivate systemic quality improvement toward realistically achievable goals. The establishment of multisite ABCs in pediatric emergency medicine has been limited. OBJECTIVE: To report ABCs for pediatric emergency care management (encounters with asthma, infections, or pain), readiness (vital measurement, timeliness, and throughput), and quality (return visits). DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study analyzed visits at 12 emergency d

Conversion Rate in Laparoscopic Cholecystectomy as a Critical Benchmark.

BACKGROUND: Conversion from laparoscopic to open cholecystectomy is associated with increased morbidity. A low conversion rate, together with a low complication rate, may serve as a surrogate marker of surgical quality. This study aimed to analyze the conversion rate at a secondary referral center in relation to bile duct injuries and to identify risk factors associated with conversion. METHODS: We performed a retrospective analysis of all laparoscopically initiated cholecystectomies between January 2013 and December 2022. Demographic and clinical data, surgical indication, timing, difficulty

BUSIS: A Benchmark for Breast Ultrasound Image Segmentation.

Breast ultrasound (BUS) image segmentation is challenging and critical for BUS computer-aided diagnosis (CAD) systems. Many BUS segmentation approaches have been studied in the last two decades, but the performances of most approaches have been assessed using relatively small private datasets with different quantitative metrics, which results in a discrepancy in performance comparison. Therefore, there is a pressing need for building a benchmark to compare existing methods using a public dataset objectively, to determine the performance of the best breast tumor segmentation algorithm available

Simple benchmark for mesothelioma projection for Great Britain.

BACKGROUND: It is of considerable interest to forecast the future burden of mesothelioma mortality. Data on deaths are available, whereas no measure of asbestos exposure is available. METHODS: We compare two Poisson models: a response-only model with an age-cohort specification and a multinomial model with epidemiologically motivated frequencies. RESULTS: The response-only model has 5% higher peak mortality than the dose-response model. The former performs slightly better in out-of-sample comparison. CONCLUSIONS: Mortality is predicted to peak at about 2100 deaths around 2017 among males in

Characterization of a benchmark database for myoelectric movement classification.

In this paper, we characterize the Ninapro database and its use as a benchmark for hand prosthesis evaluation. The database is a publicly available resource that aims to support research on advanced myoelectric hand prostheses. The database is obtained by jointly recording surface electromyography signals from the forearm and kinematics of the hand and wrist while subjects perform a predefined set of actions and postures. Besides describing the acquisition protocol, overall features of the datasets and the processing procedures in detail, we present benchmark classification results using a var

Benchmark dose of lead inducing anemia at the workplace.

To estimate the critical dose of lead inducing anemia in humans, the effects of lead on hemoglobin (Hb) and hematocrit (Hct) levels and red blood cell (RBC) count were examined in 388 male lead-exposed workers with blood lead (BPb) levels of 0.05-5.5 (mean 1.3) micromol/L by using the benchmark dose (BMD) approach. The BPb level was significantly related to Hb (regression coefficient beta=-0.276), RBC (beta=-11.35), and Hct (beta=-0.563) among the workers (p < 0.001) when controlling for age and working status. The average BPb levels were significantly higher in the workers with anemia (1.85 m

Models of neurotoxicity: extrapolation of benchmark doses in vitro.

In risk assessment, no observed exposure level (NOAEL) and benchmark dose (BMD) are usually derived either from epidemiological studies in humans or from animal experiments. In many in vitro studies, concentration-effect/response curves have been analyzed using different mathematical models finalized to the identification of EC50. In the present article, we propose a model to fit dose-response curves in vitro. The BMD approach has been used to compare the cell viability (MIT assay) of different rat (C6 and PC12, glial and neuronal, respectively) and human cell lines (D384 and SK-N-MC, glial an

Benchmark dose calculation from epidemiological data.

A threshold for dose-dependent toxicity is crucial for standards setting but may not be possible to specify from empirical studies. Crump (1984) instead proposed calculating the lower statistical confidence bound of the benchmark dose, which he defined as the dose that causes a small excess risk. This concept has several advantages and has been adopted by regulatory agencies for establishing safe exposure limits for toxic substances such as mercury. We have examined the validity of this method as applied to an epidemiological study of continuous response data associated with mercury exposure.

Benchmarks and methods for 3D medical image retrieval.

The increasing use of medical imaging in healthcare settings presents a significant challenge due to the additional workload for radiologists, yet it also offers opportunity for enhancing healthcare outcomes if effectively leveraged. Artificial Intelligence (AI)-based 3D medical image retrieval holds the potential to alleviate radiologists' burden by offering evidence-based diagnostics and predictions that can enhance the scale and accuracy of radiologists' work, while simultaneously supporting output verification for safety and regulatory compliance. Despite its promise, the field of 3D medic

BAGLS, a multihospital Benchmark for Automatic Glottis Segmentation.

Laryngeal videoendoscopy is one of the main tools in clinical examinations for voice disorders and voice research. Using high-speed videoendoscopy, it is possible to fully capture the vocal fold oscillations, however, processing the recordings typically involves a time-consuming segmentation of the glottal area by trained experts. Even though automatic methods have been proposed and the task is particularly suited for deep learning methods, there are no public datasets and benchmarks available to compare methods and to allow training of generalizing deep learning models. Benchmarks in organism performance and their use in comparative analyses.

Evaluating the response of organisms to stress assumes that functional benchmarks are available against which the response can be gauged, but this expectation remains unfulfilled for many taxa. As a result, attempts to describe the organismic effects of environmental degradation and physiological stress can prove misleading. Functional benchmarks and the effects of stress are particularly germane to coral reefs that globally are exposed to significant environmental challenges, and in this study, we compiled data on scleractinian corals to describe a reference against which stress responses can

Thermodynamic benchmark study using Biacore technology.

A total of 22 individuals participated in this benchmark study to characterize the thermodynamics of small-molecule inhibitor-enzyme interactions using Biacore instruments. Participants were provided with reagents (the enzyme carbonic anhydrase II, which was immobilized onto the sensor surface, and four sulfonamide-based inhibitors) and were instructed to collect response data from 6 to 36 degrees C. van't Hoff enthalpies and entropies were calculated from the temperature dependence of the binding constants. Application of the benchmark method to risk assessment of trichloroethene.

An alternative approach for risk assessment of nongenotoxic substances, the benchmark method, has been evaluated and applied to trichloroethene as a test case. The benchmark dose is the dose that corresponds to a specific increase in risk, normally 1 or 10%. Experimental data from the literature on trichloroethene were used for these calculations. Eighty sets of data on effects on liver, kidney, the central nervous system, and tumors were analyzed. All non-observed-effect levels (NOELs) were higher than the benchmark dose corresponding to 1% extra risk, and 42% of the NOELs and 93% of the lowe

Benchmarking AI-driven protocols for cyclic peptide conformer prediction and ranking.

Generative AI is being used in peptide design to optimize binders from early research stages. One notable application is with respect to cyclic peptides, wherein AI assists in modelling conformers in bound or solution states. This work benchmarks state-of-the-art protocols' ability to predict and rank conformers of cyclic peptide binders alone and in complex with their targets, using sequence or all-atom representations. The datasets consist of cyclic peptides with experimental structures available in public repositories. Various tools were included in the benchmark: HighFold, cyclic-aware Alp
