Informed consent for emergency surgery in infants and children.

The informed consent process for emergency surgery in children poses a challenge for pediatric surgeons because the child and his/her surrogates (usually parents) must make medical decisions in a relatively short period. The unique circumstances of a surgical emergency create potential barriers to achieving the central goals of the informed consent process, respect for patient autonomy and beneficence. The purpose of this review is to provide a practical guide for pediatric surgeons on the informed consent process as it applies to emergency surgery in pediatric patients. We will also discuss i

Proxy decision making in Alzheimer disease research: durable powers of attorney, guardianship, and other alternatives.

Individuals in advanced stages of Alzheimer disease are incapable of giving or refusing legally or ethically valid consent to participation in biomedical and behavioral research protocols. Yet, many research protocols aimed at better treatment of persons with this disease can be conducted usefully only if such individuals are themselves involved as human subjects. This dilemma raises difficult issues of personal autonomy and beneficent motives. This article discusses several potential proxy decision-making mechanisms regarding research participation in light of the relevant legal and ethical c

Ethical Perspectives in Renal Genetic Testing and Counseling.

This report examines ethical considerations surrounding genetic testing in nephrology. More than 14% of the U.S. population lives with CKD, including approximately 800,000 individuals receiving treatment for end-stage kidney disease, and an estimated 20% of CKD cases have a genetic etiology. As genetic testing becomes integrated into routine care, careful evaluation of its risks and benefits is essential. Using the Georgetown principles-autonomy, beneficence, nonmaleficence, and justice-as a framework, we analyze contemporary ethical challenges in renal genetics. We emphasize the importance of

Ethical considerations in the design, execution, and analysis of clinical trials of chronic pain treatments.

INTRODUCTION: In the field of pain research, clinical trials may randomize over 500 subjects and include more than 150 sites spanning over a dozen countries. METHODS: This review examines the ethical considerations affecting clinical trial design, execution, and analysis of trials for chronic pain. The Belmont Report has been the touchstone for human studies protection efforts since 1979. Commissioned by the U.S. government in response to ethical failures in medical research, such as the Tuskegee Syphilis Study, the report emphasizes 3 basic principles: respect for persons, beneficence, and j

Magnetic fluid hyperthermia therapy for malignant brain tumors--an ethical discussion.

UNLABELLED: This paper describes the worldwide first nanoparticle-based therapy against brain tumors termed magnetic fluid hyperthermia (MFH) or Nano-Cancer therapy, which now is under investigation in a phase II study. The preliminary results show evidence of a local effectiveness and with only minor to moderate therapy-associated side effects. The evaluation, according to the principles of medical ethics (autonomy, beneficence, nonmaleficence, justice), supports the further development of the MFH therapy. FROM THE CLINICAL EDITOR: This paper describes the worlwide first nanoparticle-based t

Using Electroconvulsive Therapy (ECT) in emergency clinical situations against patient's autonomy: A case study analysis.

In this paper, we present a comprehensive case study analysis of a specific legal order issued by the Court of First Instance Number 6 of Santiago de Compostela on March 21, 2023. The focal point of the case study is the utilization of Electroconvulsive Therapy (ECT), a specific neurotechnology, in an emergency clinical situation. The objective of this article is to examine the legal and bioethical dimensions surrounding the use of ECT, shedding light on the ethical implications and decision-making processes involved in such cases. The analysis delves into the legal aspects of the case, consid

New Intravenous Calcimimetic Agents: New Options, New Problems. An Example on How Clinical, Economical and Ethical Considerations Affect Choice of Treatment.

BACKGROUND: Dialysis treatment is improving, but several long-term problems remain unsolved, including metabolic bone disease linked to chronic kidney disease (CKD-MBD). The availability of new, efficacious but expensive drugs (intravenous calcimimetic agents) poses ethical problems, especially in the setting of budget limitations. METHODS: Reasons of choice, side effects, biochemical trends were discussed in a cohort of 15 patients (13% of the dialysis population) who stared treatment with intravenous calcimimetics in a single center. All patients had previously been treated with oral calcim

Ethical considerations and decision making in opioid prescribing for chronic pain: A case study in rheumatology practice.

Dilemmas regarding opioid prescribing for chronic pain frequently occur within health care settings. The ethical principles of autonomy, beneficence, nonmaleficence, and justice, as well as the principles of care ethics, can assist in addressing these opioid-related dilemmas. The purpose of this clinical case study is to provide a case study highlighting an opioid prescribing dilemma and then identify opioid-related transition considerations; address ethical questions that nurse practitioners (NPs) may encounter in clinical practice when providing care for individuals living with chronic pain

Children and the Opioid Crisis: We Can Make a Difference.

The use of opioid-sparing and opioid-free strategies in children can provide adequate analgesia while decreasing the risk of adverse events and contributing to the ongoing battle against the opioid crisis. However, every child must be evaluated individually so that a safe and efficacious perioperative pain management plan can be created. A working knowledge of the risks and benefits of opioids, nonopioid adjuncts, and regional anesthesia along with the ethical considerations for balancing stewardship and beneficent care is essential to the success of these strategies. As perioperative practiti

[Effects of different treatments for osteoporosis on prevention of fractures: toward and individualized and economic approach].

The aim of this paper is to review the benefice of various antiosteoporotic treatment (calcium and vitamin D, raloxifene, bisphosphonates, calcitonin, PTH, strontium ranelate) and to discuss for which women with osteoporosis these treatments are efficient. The age, previous fractures, and bone mass measurement (T-score) identify women at high risk for subsequent fracture. The development of assessment tools for predicting fracture risk, the use of the NNT (number needed to treat), and economical analyses including QALY (quality-adjusted life-year) are particularly helpful. Economical analyses

Ethical principles and parental choice: treatment options for neonates with hypoplastic left heart syndrome.

Nurses caring for children with congenital heart disease face unique challenges, especially when caring for neonates diagnosed with hypoplastic left heart syndrome (HLHS). The treatment options for these neonates present difficult choices for the child's decision makers and are not without significant life-altering consequences. In order to assist in the decision-making process, nurses as patient and family advocates should acknowledge the unique role they play in the informed consent process, while simultaneously identifying specific ethical principles that are components of this process. Ethical considerations of transparency, informed consent, and nudging in a patient with paediatric aortic stenosis and symptomatic left ventricular endocardial fibroelastosis.

A 9-year-old boy who was born with bicuspid aortic stenosis underwent two unsuccessful aortic valvuloplasty interventions, and by 2 years of age he developed restrictive cardiomyopathy caused by left ventricular endocardial fibroelastosis and diastolic dysfunction. The attending cardiologist referred the patient to a high-volume, high-profile congenital cardiac surgical programme 1000 miles away that has a team with considerable experience with left ventricular endocardial fibroelastosis resection and a reputation of achieving good results. Owing to problems with insurance coverage, the parent

An Ethical Reflection on Assisted Dying, Inequity Within Palliative Medicine Services and Access to Care in the United Kingdom: A Resident Doctor's Perspective.

With the passing of the Assisting Dying Act in the United Kingdom, there are growing concerns from within and outside the Palliative Care community. The conflicting ethical issues at play are complex and often contradictory. I present a short summary of several ethical issues and a narrative argument in opposition to assisted dying in our current system, citing autonomy, beneficence, arguments of justice, inequity and a rights-based approach. Overall, I will argue that the disparity between availability and demand for access to specialist palliative care creates an inequitable playing field fo

[The actual place of molecular analysis for donors and recipients in red blood cell immuno-hematology].

Molecular testing in red blood cell immuno-haematology is used extensively since the last 5 past years because of the technical developments and the possibility to use commercial kits. But these analyses have a high cost and rely on dedicated laboratories. Therefore, serology remains a reference technique for many laboratories in France, and in the world. Molecular analyses are used to resolve problems that cannot be resolved by serology. In this review, we will detail the main indications of molecular analysis, in donors and recipients, taking into account the technical tools that we can use,

Ethical implications of high attrition in AI-based mental health interventions: a systematic review and meta-analysis.

BACKGROUND: The integration of AI-based conversational agents (CAs) in mental health aims to bridge the global "treatment gap." However, high user attrition poses a critical ethical challenge, potentially compromising care continuity and distributive justice. This study quantifies attrition rates in AI-driven mental health interventions and evaluates their ethical implications for user autonomy and research integrity. METHODS: Following PRISMA 2020 guidelines and registered with PROSPERO, this systematic review and meta-analysis synthesized evidence from six databases through September 2025.
