Topical intrapocket anesthesia during scaling and root planing: a randomized clinical trial.

Although the use of injectable anesthesia prior to subgingival scaling and root planing (SRP) reduces pain, many patients report fear and prolonged numbness of adjacent tissues. The aim of the present study was to compare the effects of a eutectic mixture containing 25 mg/g of lidocaine and 25 mg/g of prilocaine, injectable 2% lidocaine, topical 2% benzocaine and a placebo substance on reducing pain during SRP. In this randomized, split-mouth, masked clinical trial, thirty-two patients presenting more than two teeth with probing depth and clinical attachment level ≥ 5 mm in at least 4 sextants

Central cyanosis on a psychiatric unit treated at the Salford Royal Hospital.

We describe a case of acquired methaemoglobinaemia due to frequent use of the 'legal high' known as 'Pink Panthers'. This contains 5,6-Methylenedioxy-2-aminoindane and 2-Aminoindane, both amphetamine analogues with the potential to cause methaemoglobinaemia. Furthermore, the most common 'cutting agent' for legal highs in the UK is benzocaine, also known to cause methaemoglobinaemia. Given the increasing prevalence of legal highs, particularly those containing added benzocaine, such presentations may become more common. Furthermore, in one case series, benzocaine gel used for toothache was the

Methemoglobinemia caused by topical teething preparation: a case report.

Methemoglobinemia (MetHb) remains an uncommon, but potentially fatal disorder. Benzocaine (ethyl aminobenzoate), a topical anesthetic, has been reported to cause acquired MetHb when used during endoscopic or other ambulatory procedures. Reports of severe MetHb following benzocaine-containing preparations in the community, however, are very rare. We discuss this entity by describing an unusual case of severe MetHb in a 5-year-old child, caused by unattended self-use of a benzocaine-containing, pain-relief gel for teething. This case story illustrates the potential lethal risk of over-the-counte

Contrasting actions of cocaine, local anaesthetic and tetrodotoxin on discharge properties of rat aortic baroreceptors.

1. Effects of cocaine, lignocaine, benzocaine and tetrodotoxin (TTX) on the simultaneously measured pressure- and diameter-discharge frequency relations of single fibre baroreceptors were compared in rat in vitro aortic arch-aortic nerve preparations. 2. Between 1 and 10 microM, cocaine produced selective increases in the pressure threshold shifting the pressure-response curve without altering the gain or threshold frequency. At near-blocking concentrations, gain was depressed as well. Cocaine experiments were done in nitroprusside (NP, 1 microM). Neither NP or NP with cocaine altered diameter

Screening of Anaesthetics in Adult Zebrafish (Danio rerio) for the Induction of Euthanasia by Overdose.

Zebrafish are often euthanized by overdose of anaesthesia. However, fish may have aversion towards some anaesthetics, and protocol efficacy varies between species. Using wild type adult Danio rerio, we assessed time to loss of opercular beat, righting, and startle reflexes during induction of anaesthetic overdose by either tricaine (0.5 g/L or 1 g/L), benzocaine (1 g/L), 2-phenoxyethanol (3 mL/L), clove oil (0.1%), isoeugenol (540 mg/L), lidocaine hydrochloride (1 g/L), or etomidate (50 mg/L). Initial screening demonstrated that benzocaine and buffered lidocaine hydrochloride achieved the fast

Clinical studies with phenylpropanolamine: a metaanalysis.

The combined analysis of parallel double-blind studies of prescription anorectic drugs in 1973 showed 0.23 kg/wk more weight loss than placebo at the end of the studies. In 1985, the combined studies of phenylpropanolamine (PPA) and PPA with caffeine gave 0.27 kg/wk more weight loss than placebo, but since 1985, this difference has dropped to 0.14 kg/wk in the combined studies with PPA. The incidence of adverse reactions in the combined studies with PPA were 19% and 14% for placebo (P less than 0.02). These adverse reactions were mild, however, and ceased with drug discontinuation. The two non

Effects of intraluminal local anesthetic on upper gastrointestinal motor, sensory, and peptide hormone responses to intraduodenal glucose.

OBJECTIVE: Enterally administered glucose modifies gut sensation, diminishes hunger, and slows gastric emptying by suppressing antral motility and stimulating pyloric pressures. We aimed to clarify the mechanism of small intestinal glucose sensing. METHODS: We studied eight healthy males twice, in random order. An antroduodenal manometry catheter was positioned with a sleeve sensor across the pylorus. Benzocaine, or vehicle alone, was given into the proximal duodenum as a bolus, followed by continuous infusion for 105 min (T=-15 to 90 min). Glucose was also infused into the proximal duodenum

Allergic contact dermatitis to cinchocaine.

A case of allergic contact dermatitis to cinchocaine is presented to highlight the confusion regarding the classification of, and cross-reactivity between, topical anaesthetic agents and the inadequacy of the use of benzocaine as a single screening agent for topical anaesthetic contact allergy. Our patient presented with a 10-day history of acute perianal dermatitis. Allergic contact dermatitis to Proctosedyl ointment, which was applied for 3 weeks prior, was suspected. On patch testing, he reacted to cinchocaine, a constituent of Proctosedyl ointment, and to the actual product, but had no rea

Methemoglobinemia induced by local anesthetics.

A case of methemoglobinemia secondary to topical local anesthetic agents is reported. A 28-year-old, 80-kg white man was admitted to a hospital for respiratory distress two days after an accident. During intubation, Cetacaine (benzocaine, butamben, and tetracaine) spray and Americaine (benzocaine) ointment were used topically for local anesthesia. Ninety minutes later, the patient was cyanotic despite ventilation, and an arterial blood sample was chocolate-brown and contained 29 d/dl methemoglobin (normal less than 1.7 g/ dl). Methylene blue, 80 mg as a 1% solution in 5% dextrose in water, was

Determination of cutting agents in seized cocaine samples using GC-MS, GC-TMS and LC-MS/MS.

Cocaine is usually sold as a white powder and can contain several adulterants and diluents, known as cutting agents. The cutting agents play an important role in the identification of trafficking routes, and they can also modify or intensify signs and symptoms of drug intoxication increasing the risk to the health's user. The purpose of this work was to quantify cocaine and cutting agents in 116 illicit samples from NMS Labs, Willow Grove, PA, U.S. Gas chromatography - mass spectrometry (GC-MS) and handle-portable gas chromatography toroidal ion trap mass spectrometry (GC-TMS) were used as scr

Evaluation and refinement of euthanasia methods for Xenopus laevis.

The most common method of euthanasia for Xenopus species is by immersion in tricaine methane sulfonate solution (MS222). A wide range of doses of MS222 (0.5 to 5 g/L) have been recommended, but few reports describe dose-response testing, the time to loss of consciousness, or the reliability of euthanasia. The objective of this study is to evaluate the efficacy of immersing individual and groups of frogs in MS222 at concentrations ranging from 1 to 5 g/L for euthanasia and of 3 less-common methods: intracoelomic injection of MS222, intracoelomic injection of sodium pentobarbital with phenytoin,

Severe methemoglobinemia after transesophageal echocardiography.

Methemoglobinemia, an increased concentration of methemoglobin in the blood, is an altered state of hemoglobin whereby the ferrous form of iron is oxidized to the ferric state, rendering the heme moiety incapable of carrying oxygen. This can cause hypoxia, cyanosis, or even death. Severe methemoglobinemia resulting from oral benzocaine spray before endoscopic procedures has been reported as a rare complication. We report a case of severe acquired methemoglobinemia resulting from topical benzocaine use before transesophageal echocardiography. This case serves to highlight the severity of methem

Effects of local anaesthetics on the activity of the Na,K-ATPase of canine renal medulla.

The purpose of this study is to characterize the effects of local anaesthetics on Na,K-ATPase activity. The ATPase activity of Na, K-ATPase-enriched membranes from canine renal medulla was determined in the absence and in the presence of lidocaine, procaine, tetracaine, benzocaine, bupivacaine, prilocaine, and procainamide at 37 and 25 degrees C. All of these local anaesthetics, except benzocaine, inhibit the activity of the Na,K-ATPase of canine renal medulla at both 25 and 37 degrees C. Benzocaine inhibits Na,K-ATPase activity at 37 degrees C, but stimulates activity at 25 degrees C. The inf

Health risks of cocaine adulteration: local anesthetics as modulators of monoamine and organic cation transporters.

INTRODUCTION: Local anesthetics (LAs) are frequently used as adulterants in cocaine sold on the illicit market, sometimes in higher quantities than cocaine itself. These agents can mimic cocaine's anesthetic effect, masking the products reduced purity. While reports suggest that LAs influence monoaminergic neurotransmission, systematic evidence remains limited. We examined three LAs commonly detected in cocaine samples submitted for drug checking from Vienna, Austriaprocaine, benzocaine, and lidocainefor their activity on uptake-1 monoamine transporters (DAT, NET, SERT) and uptake-2 organic ca

Duration and Intensity of Anesthetic Effect Produced by Canker Sore Medications.

Canker sore medications are often recommended to patients for relief of pain associated with aphthous ulcers or other minor irritations of the mouth. The anesthetic effect of these medications is achieved by incorporation of local anesthetics, such as benzocaine, into the products' formulations. To determine the duration and intensity of the anesthetic effect of benzocaine-containing products, three topical oral health care medications, Red Cross Canker Sore Medication, Maximum Strength Anbesol and Orajel Mouth Aid, were compared. In this double blind cross-over study, each product was tested

pH-dependent effects of local anaesthetics in perturbing lipid membranes.

The effects of local anaesthetics lidocaine, benzocaine, carbisocaine and carbisocaine derivatives, KaQ-7 and Ka-O, in perturbing bovine brain lipid membranes or egg lecithin membranes were compared at pH 6.0; 7.0; and 8.0. The electron spin resonance method with stearic acid labeled at carbon position 16 as the spin probe was employed. The perturbation effects of lidocaine and Ka-O were found to increase with increasing pH of the sample, whereas the effect of carbisocaine decreased with increasing pH. The perturbation effects of benzocaine and KaQ-7 were independent of pH. The pH-dependent pe

General and local anaesthetics perturb the fusion of phospholipid vesicles.

The effects of general and local anaesthetics on Ca2+-induced fusion of negatively charged lipid vesicles have been investigated. Vesicles composed of phosphatidylcholine and phosphatidic acid (2:1 molar ratio) were induced to fuse using 5 mM free Ca2+. Fusion, assessed by an increase in size using gel filtration techniques and confirmed by electron microscopy, displayed a dependence on Ca2+ and Mg2+ concentration and on temperature. The inhalational anaesthetics halothane, methoxyflurane and diethyl ether enhanced fusion as did the uncharged local anaesthetic benzocaine. In contrast, the char
