Helical blade versus sliding hip screw for treatment of unstable intertrochanteric hip fractures: a biomechanical evaluation.

OBJECTIVE: To compare the fixation stability in the femoral head with sliding hip screw versus helical blade designs for unstable, intertrochanteric hip fractures. METHODS: A simulated, unstable intertrochanteric hip fracture was created in six pairs of cadaveric femurs. One of each pair was treated using an intramedullary nail with a sliding hip screw (ITST) for femoral head fixation and the other was treated with a nail with a helical blade (TFN). Each specimen was cyclically loaded with 750N vertical loads applied for 10, 100, 1000 and 10,000 cycles. Measurements for femoral head displacem

Cement distribution and initial fixability of trochanteric fixation nail advanced (TFNA) helical blades.

INTRODUCTION: Early fixation and rehabilitation is the gold standard treatment for intertrochanteric femur fractures. To avoid postoperative complications such as cut-out or cut-through, cement augmentation with perforated helical blades has been developed. The purpose of this study was to evaluate the distribution of injected cement at the head-neck portion of proximal femur using computed tomography (CT) and to examine its initial fixability and clinical outcomes. PATIENTS AND METHODS: Elderly patients who had intertrochanteric fractures were treated with a helical blade only (BO group) or

Femoral metastatic fractures treated with intramedullary nailing.

Between 1993 and 1998, 49 consecutive patients with impending or complete pathological fractures of the femur due to metastatic bone disease were treated with intramedullary nailing. Twenty-four were treated with a long Gamma nail and 32 with an AO nail with a cephalomedullary spiral blade. Both implants gave a good functional result with relief of pain and improved mobility with no difference in morbidity between either group (P>0.05).

[Posterior lumbar fusion using spongious-metal pins in lumbar instability and spondylolisthesis].

A new technique of posterior lumbar interbody fusion (PLIF) using a cast cancellous metal blade is presented. A new shape of the metal implant which insures an immediate lumbar fusion is designed by the authors. This technique is used in 8 patients with spondylolisthesis and in 11 patients with lumbar instability. In all patients good and immediate fusion is achieved as well as satisfactory clinical effect. The operative technique is presented and advantages of this technique are discussed.

Modified fabrication techniques lead to improved centrifugal blood pump performance.

The authors are developing an implantable centrifugal blood pump for short- and medium-term (1-6 months) left ventricular assist. They hypothesized that the application of result dependent modifications to this pump would lead to overall improved performance in long-term implantation studies. Essential requirements for pump operation, such as durability and resistance to clot formation, have been achieved through specialized fabrication techniques. The antithrombogenic character of the pump has been improved through coating at the cannula-housing interfaces and the baffle seal, and through cha

Is new always better: comparison of the femoral neck system and the dynamic hip screw in the treatment of femoral neck fractures.

BACKGROUND: Hip fractures in the elderly population are common and the number of patients is rising. For young and geriatric patients with undisplaced fractures osteosynthesis is the primary type of treatment. The dynamic hip screw (DHS) is around for many years and proved its value especially in displaced fractures. Since 2018 the femoral neck system (FNS) is available as an alternative showing promising biomechanical results. The aim of this study is to evaluate clinical results of the FNS and compare it to the DHS. MATERIALS AND METHODS: Patients older than 18 years with Garden I-IV fractu

Salvage procedure for cut-through after surgical fixation of trochanteric fractures with TFN.

INTRODUCTION: Closed reduction and fixation using a cephalomedullary nail represents the accepted management of unstable intertrochanteric fractures. Cut-through has been described as a complication associated with the treatment. Although a hip arthroplasty may be the most predictable revision method, a non-prosthetic option can lead to similar results. The objective is to describe a non-prosthetic revision procedure in cases of cut-through. METHOD: We performed a retrospective analysis of our Institutional Registry for Hip Fractures in elderly patients (RIAFC) from January 2010 to June 2018

Recurrent implantation failure in assisted reproduction: how to counsel and manage. B. Treatment options that have not been proven to benefit the couple.

The success of assisted reproduction, although gradually increasing over the years, is still less than satisfactory. Many couples have benefited from this treatment; however, many have also been left frustrated following multiple failed attempts. Couples who fail to conceive after multiple IVF/intracytoplasmic sperm injection (ICSI) treatments often seek treatment options that are new and that have not been offered before. Some of these include immunological testing and treatment, allogenic lymphocyte therapy, intratubal transfer of zygotes and embryos, blastocyst transfer, sequential embryo t

Injection of fat for soft tissue augmentation.

Perinephric rabbit fat was divided into small particles with scissors and razor blades and then injected subcutaneously into the donor rabbit. The injected fat was compared with surgically implanted fat. Subcutaneous augmentation with both types of fat resulted in similar findings. Grossly, 40% of injection and implantation sites were palpable after 16 to 36 weeks of observation. Histologically, fat was present at practically every site, but with a variable amount of fat cell destruction. This study demonstrates that the author's method for dividing and injecting fat produces comparable result

Increased failure rates after the introduction of the TFNA proximal femoral nail for trochanteric fractures: implant related or learning curve effect?

Background and purpose - Trochanteric fractures are often treated using intramedullary fixation. In our institution, the TFN-Advanced Proximal Femoral Nailing System (TFNA) was introduced as replacement for the Gamma Trochanteric Nail (GTN3) for the treatment of these fractures as a result of a hospital-driven change of trauma implant supplier. We compared trochanteric fracture fixation failure rate between these 2 intramedullary nails. Patients and methods - All trochanteric fractures treated surgically from 2011 to 2019 were retrospectively reviewed for fixation failure. From 2016 only the T

Is implant choice associated with fixation strength for displaced radial neck fracture: a network meta-analysis of biomechanical studies.

The multitude of fixation options for radial neck fractures, such as pins, screws, biodegradable pins and screws, locking plates, and blade plates, has led to a lack of consensus on the optimal implant choice and associated biomechanical properties. This study aims to evaluate the biomechanical strength of various fixation constructs in axial, sagittal, and torsional loading directions. We included biomechanical studies comparing different interventions, such as cross/parallel screws, nonlocking plates with or without augmented screws, fixed angle devices (T or anatomic locking plates or blade

Abrupt flow decline as the initial indicator of early HeartMate 3 pump thrombosis.

Pump thrombosis shortly after HeartMate 3 (HM3) implantation is rare. We report a man in his 50s who developed pump thrombosis 14 days after HM3 implantation. The patient initially presented with an abrupt decline in pump flow accompanied by only mild laboratory abnormalities, while remaining clinically stable. Contrast-enhanced computed tomography raised suspicion of thrombus formation within the outflow graft, prompting emergent pump exchange. Subsequent manufacturer analysis demonstrated thrombus attached to the metallic edge at the inflow aspect of the pump, with no thrombus identified in

Inter-Defect Septal Puncture Technique For Single Device Closure of Wide-Spaced Multi-Hole Atrial Septal Defect.

Closure of a wide-spaced multi-hole secundum atrial septal defect (MHASD) using a single occluder is difficult to accomplish. Multiple occluder implantation has risks such as incomplete endothelialisation, device embolisation, and residual shunt. Blade or balloon septotomy enables single device occlusion; however, the aforementioned may cause a short circumferential rim with subsequent device instability. This paper describes an inter-defect septal puncture technique for single device closure of different layouts of wide-spaced MHASDs via per-atrial or percutaneous approach under exclusive tra

The comparison of usefulness prosthetic rehabilitation with removable and fixed suprastructures on endosseous implants.

Advanced atrophy bone in edentulous jaws is a serious problem for conventional prosthodontics. The advent of implantology, as well as guided reconstruction of bones, has opened new possibilities for conventional prosthodontic suprastructures. The turning point in implantology occurred in the sixties and was promoted by a Swedish orthopaedist Brånemark. Nowadays, implants are inserted routinely without creating any significant problems to a skillful maxillofacial surgeon. This study was performed in 73 patients who had 77 prosthodontic suprastructures (fixed suprastructures in 58 and removable

Optimal position of intramedullary nails in the femoral head following spiral blade resection for revision of intertrochanteric fractures: a biomechanical analysis.

PURPOSE: No biomechanical studies have yet investigated the optimal position of extramedullary hip screws or intramedullary nails within the femoral head after revision surgery for helical blade cut-out in intertrochanteric fractures. The purpose of this study was twofold: 1) to compare the stability of different fixation methods in the revision of femoral intertrochanteric fractures, and 2) to determine the optimal placement of the lag screw in a novel proximal femoral bionic intramedullary nail (PFBN) within the femoral head during such revision procedures. METHODS: Using finite element ana

A Rare Case of Solitary Fibrosus Tumor (SFT) of the Nasal Septum.

INTRODUCTION: Solitary fibrous tumor (SFT) is a rare mesenchymal tumor that usually arises from the pleura but can also occur in extrapleural sites, such as the sinonasal region. It causes aspecific symptoms, including nasal obstruction and discharge, postnasal drip, anosmia, epistaxis, and headache. It may be difficult to distinguish these symptoms from those caused by other mesenchymal lesions that usually occur in this site, especially when the tissues undergo iatrogenic damage following surgical removal. CASE REPORT: This case report shows a rare right nasal septal solitary fibrous tumor,
