Blood alcohol concentration in the clinical laboratory: a narrative review of the preanalytical phase in diagnostic and forensic testing.

The analysis of blood alcohol concentration (BAC), a pivotal toxicological test, concerns acute alcohol intoxication (AAI) and driving under the influence (DUI). As such, BAC presents an organizational challenge for clinical laboratories, with unique complexities due to the need for forensic defensibility as part of the diagnostic process. Unfortunately, a significant number of scientific investigations dealing with the subject present discrepancies that make it difficult to identify optimal practices in sample collection, transportation, handling, and preparation. This review provides a syste

Assessing the applicability of cerebrospinal fluid collected from the spinal cord for the determination of ethyl alcohol in post-mortem toxicology.

This paper presents the results of a study on the applicability of cerebrospinal fluid (CSF) collected from the spinal canal in the post-mortem determination of ethyl alcohol. The present study reviewed data of autopsy cases (n = 45), in which ethyl alcohol was detected in CSF using gas chromatography with a flame ionization detector (HS-GC-FID), to investigate ethyl alcohol concentrations in CSF, compared with blood. As a result of statistical analysis of the obtained data, a high positive correlation was found between blood ethanol concentration and cerebrospinal fluid collected from the spi

Predictors of Breath Alcohol Profiles: A Multiple-Study Lab-Based Approach.

BACKGROUND: Oral alcohol administration studies often show a range of breath alcohol content (BrAC) following alcohol ingestion, though examination of BrAC trajectories as distinct profiles has yet to be explored as well as potential predictors of these profiles. METHODS: Across two laboratory studies of oral alcohol administration with target blood alcohol concentrations (BACs) of 0.08% (N = 89) and 0.10% (N = 92) of emerging adults (Mage = 23.03) with a history of binge drinking, BrAC was captured every 15 min for seven intervals. Repeated measure latent profile analysis (RMLPA) was used to

Assessing uncertainty in forensic alcohol calculations: The case for using percentile ranges.

Forensic alcohol calculations are used to determine the amount of alcohol consumed based on (a) a relevant biological sample (i.e., breath or blood), (b) the blood alcohol concentration after a certain period following the consumption of a known quantity of alcohol, or (c) the extrapolation of the analytical result of an alcohol analysis to a legally relevant time. Current best practice guidelines recommend the use of percentile ranges for the parameters that contribute the most uncertainty to the results derived from forensic alcohol calculations. These parameters are volume of distribution (

Metacognitive judgments of alcohol-related intoxication and impairment: The Mellanby effect and alcohol myopia in low-risk drinkers.

INTRODUCTION: The decision to drive after alcohol consumption is largely influenced by an individual's interpretation of their alcohol intoxication and level of driving impairment. However, such perceptions may be influenced by the level and duration of intoxication. This study investigated the effect of alcohol intoxication on drivers' judgments of their impairment and driving capabilities across different blood alcohol concentration (BAC) levels. METHOD: One hundred and sixty blinded participants were randomly allocated into low (∼.025%), mid (∼.055%), or high (∼.085%) BAC condition groups

Available markers of excessive alcohol use.

The need for objective diagnostic tools in people with alcohol intake abuse is one of the major needs in daily clinical practice. Determination of blood alcohol concentration is commonly used in cases of suspected acute alcohol intoxication, especially in the emergency room. A dose-dependent correlation between alcohol consumption and mean corpuscular volume (MCV) is a known index of excessive alcohol intake. Serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) levels are frequently elevated (2-4 times above normal) in patients with alcohol use disorder and an AST/ALT rati

Intoxicated aggression: Do alcohol and stimulants cause dose-related aggression? A review.

RATIONALE: Violence and drug use are significant public health challenges that are strongly linked. It is known that alcohol plays a major role in the causation of unnatural deaths and that stimulants like cocaine and amphetamine are often implicated in aggressive acts or violence. However, a clear causal relationship between these substances and aggression, and more specifically a blood concentration threshold at which intoxicated aggression emerges is lacking. In case of a crime and subsequent law enforcement, knowledge about dose-response relationships could be of pivotal importance when ev

Preloading, exit intoxication related to energy drink usage, and gender differences within night-time entertainment districts for young people.

BACKGROUND: Governments frequently introduce legislation to reduce drunkenness and violence in night-time entertainment districts (NEDs). However, there is no study which has looked at inebriation of a large sample of young people exiting a NED. We investigated Breath Approximated Blood Alcohol Concentration (BrAC; grams per decilitre, g/dl) levels as people left Brisbane (Australia) NEDs and measured the effects of preloading, energy drink usage, time of night and gender differences on exit intoxication. METHODS: We systematically assessed patrons between November 2015 to March 2019 by admin

The effect of alcohol hangover on the ability to ride a bicycle.

To investigate the effects of alcohol on the ability to ride a bicycle, practical cycling tests were carried out at different blood alcohol concentrations (BAC). For this purpose, various alcoholic beverages could be consumed from around 2 p.m. until 11 p.m. Afterwards, the test persons spent the night on the trial site and were provided with dormitory sleeping accommodation. On the following morning, beginning at around 8 a.m., a final cycling test was performed. The performances of those test persons who had returned to state of soberness and of those with residual blood alcohol levels were

Elucidating the alcohol-sleep-hangover relationship in college students using a daily diary approach.

This daily diary study expands knowledge of the pharmacological alcohol-sleep relationship using a multilevel modeling approach. The interplay between alcohol and sleep on hangover susceptibility is also explored. College students (n = 337; 52 % female) provided 2976 days of self-reported alcohol use. We regressed sleep duration onto accumulated sleep debt, prior night sleep duration, and estimated blood alcohol concentration (eBAC) at bedtime; linear mixed models disaggregated day and person-level effects. Binomial models, assessing days after drinking when eBAC = 0 % versus when eBAC>0 % at

Cannabis Adaptation During and After Alcohol Ignition Interlock Device Installation: A Longitudinal Study.

OBJECTIVE: A common intervention to prevent alcohol-impaired driving are alcohol ignition interlock devices (IIDs), which prevent drivers with a blood alcohol concentration greater than .025% from starting the car. These devices force drivers to adapt their drinking to accommodate the device. Prior studies indicated a transfer of risk as some drivers with an IID may increase cannabis use as they decrease alcohol use. This study examines whether this increase in cannabis use persists after IID removal when alcohol use reverts to pre-IID levels. METHOD: The data are from the Managing Heavy Drin

Effect of co-administration of cassava (Manihot esculenta Crantz) rich diet and alcohol in rats.

The effects of co-administration of a cassava rich diet and alcohol in rats were investigated. The animals were divided into four groups (1) Control, (2) Alcohol, (3) Cassava and (4) Alcohol + Cassava. Consumption of alcohol along with cassava reduced the alcohol induced toxicity which was evidenced by the lower activities of GOT, GPT, GGT, acid phosphatase and alkaline phosphatase in the liver and serum of co-administered group. The pyruvate content in the blood increased while the lactate content, lactate/pyruvate ratio and the activity of LDH decreased in the blood due to co-administration.
