Microscopic metallic wear and tissue response in failed titanium hallux metatarsophalangeal implants: two cases.

The membranes present at the implant-bone interface were retrieved from two patients with titanium single stem hallux implants that had failed. Both patients had pain and valgus deformity of the hallux, and radiographs showed a radiolucent shadow around the implant stem, with thinning of the dorsal cortex of the proximal phalanx in one patient. After removal of the implants, arthrodesis of the first metatarsophalangeal (MP) joint was performed. Histologic analysis of the membrane tissue at the implant-bone interface showed a synovial-like appearance. There was a fibrous tissue stroma adjacent

Biocompatibility: a biomechanical and biological concept in total hip replacement.

The insertion of any implant or prosthesis into bone usually changes the biomechanical environment and, thus, alters the stresses and strains applied to the bone. Both bone overload and excessive stress protection can result in bone resorption. The material and geometry of any implant should be designed to avoid excessive flexural mismatch. Incompatibility of materials may result in interface and mechanical failure, with the consequent generation of particulate debris, subsequent osteolysis and implant failure. Particulate debris can be generated from articulating surfaces and from any other m

Osseocutaneous integration of an intraosseous transcutaneous amputation prosthesis implant used for reconstruction of a transhumeral amputee: case report.

Exoprosthetic replacement with an artificial limb is the main option for reconstruction after traumatic amputation of an upper limb. Direct skeletal attachment using an osseointegrated implant improves the ease of fixation of the exoprosthesis to the amputation stump. We now report the use of an intraosseous transcutaneous amputation prosthesis that is designed to achieve osseocutaneous integration. Osseocutaneous integration differs from osseointegration because the aim is to create a stable interface among the implant, the bone, and the soft tissues. This reduces the risk of soft tissue infe

Biomechanical, histological and ultrastructural analyses of laser micro- and nano-structured titanium implant after 6 months in rabbit.

Short-term, experimental studies of partly laser-modified implants with nano-scale surface topographical features have recently shown a considerable increase in the biomechanical anchorage to bone. The aim of this study is to evaluate the biomechanical and bone-bonding ability of partly laser-modified implants compared with machined implants after a healing period of 6 months in a rabbit model. The results showed a 170% increase in removal torque. Histology and scanning electron microscopy demonstrated osseointegration for both implant types, but also revealed a different fracture pattern at t

Use of Patient-Specific Finite Element Models in Dental Rehabilitation to Investigate Stresses of a Fibula Free Flap for Mandibular Reconstruction.

PURPOSE: This study investigated the effect of implant length, diameter, and surface contact on the stresses developed in a fibular free flap. MATERIALS AND METHODS: Finite element (FE) models for dental implants placed in a patient-specific fibula were created using a patient-specific fibula CT scan and geometry files of commercially available dental implants. The FE models involved nine dental implants of different lengths and diameters: 3.5, 4.3, and 5.0 mm in diameter and 10.0, 11.5, and 13.0 mm in length. Three contact conditions between the implant and the fibular flap were investigated

Analysis of 16 retrieved proximally cemented femoral stems.

Sixteen proximally cemented, collared, and distally splined, Bridge Hip femoral stems with a matte proximal surface and smooth distal surface were retrieved because of loosening. Electron microscopy, with correlated elemental analysis, identified titanium particulate embedded in the internal surface of the cement mantle. Data supported the observations that loosening of the femoral stems was related to proximal debonding at the cement-implant interface, loosening at the proximal cement-bone interface, and inherent rotational instability. Cement-implant interface debonding resulted in the proxi

A FE study on the effect of interference fit and coefficient of friction on the micromotions and interface gaps of a cementless PEEK femoral component.

The use of a more compliant material, such as polyetheretherketone (PEEK), for a cementless femoral component is a potential solution to prevent aseptic loosening caused by peri-prosthetic stress-shielding. Long-term fixation of a cementless femoral component is achieved by a proper primary fixation of the bone-implant interface, which is influenced by the interference fit and frictional properties of the implant surface. This computational study investigates the sensitivity of micromotions and interface gaps of a cementless PEEK femoral component to the interference fit and coefficient of fri

In vitro and in vivo performance of bioactive Ti6Al4V/TiC/HA implants fabricated by a rapid microwave sintering technique.

Failure of the bone-implant interface in a joint prosthesis is a main cause of implant loosening. The introduction of a bioactive substance, hydroxyapatite (HA), to a metallic bone-implant may enhance its fixation on human bone by encouraging direct bone bonding. Ti6Al4V/TiC/HA composites with a reproducible porous structure (porosity of 27% and pore size of 6-89 μm) were successfully fabricated by a rapid microwave sintering technique. This method allows the biocomposites to be fabricated in a short period of time under ambient conditions. Ti6Al4V/TiC/HA composites exhibited a compressive str

A novel drill-milling technology for robot-assisted implant osteotomy preparation: simulation and an ex vivovalidation study.

OBJECTIVES: Excessive frictional heat generated at the drill-bone interface during implant osteotomy can compromise osseointegration and lead to implant failure. This study's objective was to use a helical milling technique for dental implant osteotomy preparation to mitigate thermal damage to the bone. METHODS: This study introduces a novel helical milling technique designed to minimize thermal damage to bone during dental implant osteotomy preparations. Finite element simulations were conducted to compare the thermal distribution and cutting stress of a conventional twist drill and the newl

Bone adaptation to a polyester fiber anterior cruciate ligament replacement.

A series of polyester fiber ACL implants was studied in ovine stifle joints up to 2 years postimplantation. The implants were linked to the bone-tunnel wall by oriented fibrous tissue. Cross-sections of the tunnels showed bone ingrowth among the implant fibers at 2 years. A human trial of the Apex implant yielded a series of retrievals, some associated with gross bone-tunnel enlargement. There was no evidence of bone ingrowth in the human implants. It was hypothesized that-tunnel enlargement resulted from fretting at the implant-tissue interface in response to cyclic loads in use.

Compliant Intramedullary Stems for Joint Reconstruction.

The longevity of current joint replacements is limited by aseptic loosening, which is the primary cause of non-infectious failure for hip, knee, and ankle arthroplasty. Aseptic loosening is typically caused either by osteolysis from particulate wear, or by high shear stresses at the bone-implant interface from over-constraint. Our objective was to demonstrate feasibility of a compliant intramedullary stem that eliminates over-constraint without generating particulate wear. The compliant stem is built around a compliant mechanism that permits rotation about a single axis. We first established s

Production of platelet-derived growth factor in aseptic loosening of total hip replacement.

Aseptic loosening is the predominant cause of total hip implant failure. It has been assumed that a layer or membrane, containing macrophages, fibroblasts and vascular endothelial cells, of synovial-like tissue develops at the implant-to-bone interface almost invariably and, with time, somehow leads to loosening of the components from the surrounding bone. These cells produce a variety of cytokines and proteolytic enzymes which stimulate bone resorption. Platelet derived growth factor (PDGF) may be one of the cytokines which stimulate bone resorption and contribute to aseptic loosening in tota

Effect of load on the bone around bone-anchored amputation prostheses.

Osseointegrated transfemoral amputation prostheses have proven successful as an alternative method to the conventional socket-type prostheses. The method improves prosthetic use and thus increases the demands imposed on the bone-implant system. The hypothesis of the present study was that the loads applied to the bone-anchored implant system of amputees would result in locations of high stress and strain transfer to the bone tissue and thus contribute to complications such as unfavourable bone remodeling and/or elevated inflammatory response and/or compromised sealing function at the tissue-ab

Improved bone-screw interface with hydroxyapatite coating: an in vivo study of loaded pedicle screws in sheep.

STUDY DESIGN: An in vivo sheep model with loaded pedicle screws was used, wherein each animal served as its own control. OBJECTIVES: To examine the effects of hydroxyapatite (HA) coating on the bone-to-implant interface in loaded spinal instrumentations. SUMMARY OF BACKGROUND DATA: Spinal instrumentation improves the healing rate in spinal fusion, but screw loosening constitutes a problem. HA coating of other implants has resulted in favorable effects on the bone-to-implant interface. METHODS: Nine sheep were operated on with destabilizing laminectomies at two levels: L2-L3 and L4-L5. Postoperative periprosthetic fractures associated with compressive osseointegration: a systematic review of the Zimmer Biomet Compress® device.

The Zimmer Biomet Compress® Compliant Pre-Stress (CPS) device uses a compressive osseointegration technique for endoprosthetic fixation and provides an alternative to stemed implants. Despite having benefits, such as bone stock retention, periprosthetic fractures continue to remain a concern. Therefore, this systematic review presents the prevalence, risk factors, and outcomes of the CPS device and periprosthetic fractures after treatment. Medical databases (PubMed, Embase, and Google Scholar) were searched to identify studies reporting complications related to the CPS device. Peer-reviewed st

Marginal Bone Loss and Peri-Implant Clinical Outcomes of Internal Hexagonal and Internal Conical Implant Abutment Connections: A Narrative Review.

The implant-abutment connection is the mechanical and biological interface between the implant and the prosthesis, and its geometry influences mechanical stability, sealing of the microgap, stress distribution, and the peri-implant tissue response. This narrative review compares internal hexagonal connections with internal conical connections, of which the Morse taper is a specific deep-cone, friction-locking type, focusing on marginal bone loss and peri-implant clinical outcomes, and distinguishes laboratory and mechanistic evidence from clinical evidence. Laboratory, finite element, and micr
