Surgical repair of brachial plexus injury.

This preliminary report surveys the authors' experience in the treatment of 66 patients with brachial plexus traction injuries. The results of nerve reconstructive surgery obtained in 44 patients with a follow-up of at least 2 years were disappointing; on average, 10% of upper limb function was added. The role of several factors involved in this type of injury and the major draw-backs of treatment are discussed in combination with future possibilities. Current techniques of neurotizations are presented in one case.

Brachial plexus injury caused by impalement.

Open injuries of the brachial plexus are rare. One such case, that of a 68-year-old impaled on a fence spike, is presented here. Certain principles to guide evaluation and treatment are discussed. Concomitant injury to the pleura or to vascular structures requires immediate attention; the extent and type of plexus damage may be determined from physical findings and the nature of injury. The results of plexus reconstruction are variable and routine exploration may be detrimental. The Brooks classification is reviewed.

Brachial plexus injury following liver transplantation.

We examined 7 adults with brachial plexus injury following liver transplantation; they were among 120 transplants performed over a 10-month period (incidence, 5.8%). The lesion was right-sided in 5 patients, left-sided in 1, and bilateral in 1. The affected side correlated with the axillary veno-venous shunt in 3 lesions only. All noted weakness and sensory loss shortly after recovery from general anesthesia. The EMG examination indicated axonal lesions, and recovery was protracted.

Brachial plexus injury following median sternotomy.

Brachial plexus injuries are annoyingly common after median sternotomies and vary from those causing minor symptoms to those producing major disability. We compared two groups of patients operated upon with the arms either abducted to a 90 degree angle or at the sides and found no difference in the incidence of brachial plexus injury. However, the finding of Horner's syndrome in one patient and the finding in another (at autopsy) of brachial plexus penetration by a fractured first rib caused us to question traction on the brachial plexus as the correct pathogenesis of the injury. A concomitant

Direct electrical injury to brachial plexus.

Electrical current can cause neurological damage directly or by conversion to thermal energy. However, electrical injury causing isolated brachial plexus injury without cutaneous burns is extremely rare. We present a case of a 17-year-old boy who sustained accidental electrical injury to left upper extremity with no associated entry or exit wounds. Complete motor and sensory loss in upper limb were noted immediately after injury. Subsequently, the patient showed partial recovery in muscles around the shoulder and in ulnar nerve distribution at 6 months. However, there was no improvement in mus

Ultrasonography for neonatal brachial plexus palsy.

Ultrasonography has previously been reported for use in the evaluation of compressive or traumatic peripheral nerve pathology and for its utility in preoperative mapping. However, these studies were not performed in infants, and they were not focused on the brachial plexus. The authors report a case in which ultrasonography was used to improve operative management of neonatal brachial plexus palsy (NBPP). An infant boy was born at term, complicated by right-sided shoulder dystocia. Initial clinical evaluation revealed proximal arm weakness consistent with an upper trunk injury. Unlike MRI or C

Primary brachial plexus neoplasia in cats.

Conditions affecting the brachial plexus and its branches can cause lameness and/or neurological deficits. There are few reports of schwannomas in cats. In reported cases, the tumours arose from the dermis or subcutis of the limbs, head and neck and thorax, but there are no reports of primary tumours that arose from the brachial plexus itself. The purpose of this study is to present the clinical, radiological, ultrasonographical and pathological findings of primary brachial plexus tumour in three cats.

Severe brachial plexus injuries in rugby.

We describe the mechanisms, pattern of injuries, management and outcomes of severe injuries to the brachial plexus sustained during the play of rugby. Thirteen cases of severe injury to the brachial plexus caused by tackles in rugby had detailed clinical assessment, and operative exploration of the brachial plexus. Seventeen spinal nerves were avulsed, two were ruptured and there were traction lesions in continuity of 24 spinal nerves. The pattern of nerve lesion was related to the posture of the neck and the forequarter at the moment of impact. Early repair by nerve transfer enabled some func

Causes of neonatal brachial plexus palsy.

The causes of brachial plexus palsy in neonates should be classified according to their most salient associated feature. The causes of brachial plexus palsy are obstetrical brachial plexus palsy, familial congenital brachial plexus palsy, maternal uterine malformation, congenital varicella syndrome, osteomyelitis involving the proximal head of the humerus or cervical vertebral bodies, exostosis of the first rib, tumors and hemangioma in the region of the brachial plexus, and intrauterine maladaptation. Kaiser Wilhelm syndrome, neonatal brachial plexus palsy due to placental insufficiency, is p

Brachial plexus injury following median sternotomy.

OBJECTIVES: Brachial plexus injury is a rare complication after median sternotomy. We investigated that injury to the brachial plexus was retrospectively assessed in the results of three patients who underwent median sternotomy for open heart surgery. MATERIALS AND METHODS: All patients were placed in the hands-up position after right internal jugular vein cannulation, and the internal mammary artery was prepared for all of those. Nerve conduction measurements and electromyography were performed besides neurological examination. RESULTS: Brachial plexus injury was detected in three cases (0.

Management of infant brachial plexus injuries.

Management of brachial plexus injuries is geared toward normalization of limb function, primarily through optimization of nerve regeneration and mechanical increase in elbow flexion and shoulder stabilization. Changes in the skeletal muscles and the osteous structures of the upper extremity are ongoing throughout the course of treatment, mandating continual assessment and aggressive rehabilitation. In patients who present too late for microsurgical intervention, irreversible changes take place in skeletal muscles, highlighting the importance of early referral. However, secondary procedures hav

Sonographic evaluation of brachial plexus pathology.

Pre-operative US examinations of the brachial plexus were performed with the purpose of exploring the potential of this technique in recognizing lesions in the region and defining their sonographic morphology, site, extent, and relations to adjacent anatomic structures, and comparing them to the surgical findings to obtain maximal confirmation. Twenty-eight patients with clinical, electro-conductive, and imaging findings suggestive of brachial plexus pathology were included in this study. There were four main etiology groups: post-traumatic brachial plexopathies; primary tumors (benign and mal

Spontaneous brachial plexus hemorrhage-case report.

BACKGROUND: Shoulder hemorrhage resulting in brachial plexus neuropathy is a rare occurrence most often seen in cases of traumatic injury or anticoagulation therapy. We report a unique case of spontaneous brachial plexus hemorrhage. CASE DESCRIPTION: This is the first report of a spontaneous shoulder hemorrhage in which a 48-year-old jackhammer operator presented to the emergency department with a sudden onset of right shoulder pain and upper extremity pain and numbness. Imaging studies revealed a hematoma in the right axilla and chest wall. Without evidence of active bleeding or worsening ne

Management of obstetric brachial plexus palsy.

OBPP is a disease with deleterious medical, psychological, and socioeconomic sequelae for the patient and his or her family. The majority of patients show acceptable spontaneous recovery with nonoperative management, which includes aggressive physical rehabilitation and electrical stimulation. There are patients with guarded prognosis, however, who have no chance of recovery unless they undergo early aggressive surgical reconstruction of the injured brachial plexus. It is of great importance to diagnose the injury type as early as possible, especially if there is minimal recovery in the early

[Brachial plexus injuries in newborn infants].

39 children with brachial plexus palsy during the period 1/1 1990 30/6 1995 were identified retrospectively. The incidence was 1.8/1,000 (95% confidence interval 1.3-2.5/1,000). Shoulder dystocia was noted in 74%, and 44% were delivered by vacuum or forceps. Mean birth weight was 4,300 g. The outcome is known for all children. Seven children show permanent sequelae; in three of these there is marked loss of function. In the remaining children, normalization occurred within one month in 41%, within three months in 69% and within six months in 83%. Surgical repair is now a treatment option if el
