Vertical infraclavicular brachial plexus block: needle redirection after elicitation of elbow flexion.

BACKGROUND: In vertical infraclavicular brachial plexus block, success depends on distal flexion or extension response. Initially, elbow flexion (lateral cord) is generally observed. However, specific knowledge about how to reach the medial or posterior cord is lacking. We investigated the mid-infraclavicular area in undisturbed anatomy and tested the findings in a clinical setting. METHODS: Along a length of 35 mm around the mid-infraclavicular point, cryomicrotomy sections of 5 shoulders from cadavers were used to determine the topography of the cords in relation to one another and the axil

Ultrasound examination for the optimal head position for interscalene brachial plexus block.

BACKGROUND: Surface anatomic landmarks have traditionally been used to locate the brachial plexus in the interscalene groove. Head rotation can affect the orientation of neck vessels and may possibly affect the brachial plexus. The optimal degree of head rotation has been specified for better internal jugular vein cannulation but not for interscalene brachial plexus block. The purpose of this study was to evaluate the influence of head rotation on interscalene brachial plexus block. METHODS: We simulated the needle insertion in interscalene approach to brachial plexus with the ultrasound prob

[Case report: bilateral block of the brachial plexus--approaches, dosage and effectiveness].

A young man squeezed both hands in a bending machine with the result of complex hand injuries. Because of a high risk for general anesthesia, operation as well as postoperative analgesia was performed under bilateral brachial plexus block with the axillary approach. We discuss the considerations concerning the possible approaches with bilateral anesthesia of the brachial plexus under consideration of side effects and effectiveness. The needed dosage of the local anesthetics will be discussed.

Case report: ropivacaine neurotoxicity at clinical doses in interscalene brachial plexus block.

PURPOSE: To describe a case of ropivacaine toxicity following an ultrasound guided interscalene block and discuss the possible mechanisms involved. CLINICAL FEATURES: A 76-yr-old woman with multiple myeloma was scheduled for open reduction and internal fixation following a pathological fracture of her left upper humerus. She developed central nervous system toxicity with ropivacaine 15 min after a carefully placed ultrasound-guided interscalene catheter. The dose of ropivacaine was within recommended limits and there was no evidence that the catheter was intravascular. Surgery proceeded uneve

[Brachial plexus block. Effect of low interscalenic approach on phrenic nerve paresis].

BACKGROUND: The aim of the paper was to determine the influence of the brachial plexus block on phrenic nerves paralysis in distal side of interscalenic space (between 1/3 medium and 1/3 inferior), with little volume of local anesthetic drug. Comparative, prospective, randomized study. METHODS: Fourty-three patients, ASA I-II, submitted to surgical operation at superior arm, shoulder excluded. The patients were randomized into two groups: group A, 21 patients, were submitted to brachial plexus block by armpit tract; group B, 22 patients, were submitted to brachial plexus block by low intersca

Dose-range effects of clonidine added to lidocaine for brachial plexus block.

BACKGROUND: Although addition of clonidine to local anesthetics can prolong pain relief after peripheral nerve block, a dose-range effect has not been determined. METHODS: Fifty-six outpatients undergoing carpal tunnel release were randomly assigned to receive in a double-blind fashion 45 ml of a mixture containing either 400 mg lidocaine plus saline or 400 mg lidocaine plus 30, 90 or 300 microg clonidine for axillary nerve block. In each group (n = 14), blocks were evaluated at regular time intervals to determine sensory and motor functions in the five nerve regions of the hand and forearm.

Ultrasound-guided Assessment of Phrenic Nerve Involvement after Interscalene Brachial Plexus Block.

BACKGROUND AND AIM: Phrenic nerve is usually involved while administering intersalene brachial plexus block (ISBPB), resulting in partial/total hemidiaphragmatic paresis (Hdp) that can affect respiratory mechanics. The aim was to determine the incidence of diaphragm excursion before and after administration of ISBPB using ultrasonography. METHODS: Sixty-two patients received ISBPB for anesthesia in the shoulder region who consented to be a part of a prospective observational study. Pre- and postblock values for diaphragmatic excursions were recorded for quiet and deep breathing (DB) using a 3

Fatal streptococcal necrotizing fasciitis as a complication of axillary brachial plexus block.

A 74-yr-old diabetic woman developed necrotizing fasciitis of the right upper limb after axillary brachial plexus block for carpal tunnel decompression. Clinical signs included oedema, diffuse swelling and bullae; rapidly followed by toxic shock syndrome and multiorgan failure. The patient died 48 h after hospital admission, despite broad-spectrum antibiotics, surgical treatment and supportive measures for the management of shock and multiorgan failure. Cultures yielded group A Streptococcus. Delay in antibiotic and surgical treatment probably affected the outcome. Early diagnosis and treatmen

Perioperative intravenous adenosine infusion to extend postoperative analgesia in brachial plexus block.

BACKGROUND AND OBJECTIVE: Adenosine infusions have been shown to reduce requirements of anaesthetics, to decrease the need for postoperative analgesics and to attenuate hyperaesthesia related to neuropathic pain. We decided to investigate the effects, beneficial or otherwise, of an adenosine infusion administered during surgery. A brachial plexus block was used to produce anaesthesia for the surgery. METHODS: Sixty adults undergoing upper extremity surgery were included in the study. Brachial plexus block was performed via an axillary approach with lidocaine 1.25% and epinephrine 1/200 000 (4

Clinical perception of phantom limb sensation in patients with brachial plexus block.

BACKGROUND AND OBJECTIVE: Patients' perception of limb position during regional anaesthesia is frequently incorrect. The incidence and nature of this phenomenon has not yet been completely described. The aim of this prospective study was to assess phantom sensation in patients undergoing a brachial plexus block. METHODS: Axillary block was performed in 81 patients with the aid of a peripheral nerve stimulator. Immediately after the block, patients were allocated randomly to two groups (Group 1, n = 40; Group 2, n = 41) to have the blocked limb placed either on their thorax or in abduction. Fi

Cutaneous sympathetic vasoconstrictor reflexes for the evaluation of interscalene brachial plexus block.

BACKGROUND: Although signs of sympathetic blockade following interscalene brachial plexus block include Horner's syndrome, increased skin temperature and vasodilatation, the degree of sympathetic blockade is not easily determined. The aim of this study was, therefore, to use activation of cutaneous finger pad vasoconstrictor reflexes for description and quantification of the degree of sympathetic blockade following unilateral interscalene brachial plexus block. METHODS: Eight patients scheduled for acromioplasty under general anesthesia were studied. An interscalene plexus catheter was insert

Supraclavicular brachial plexus block using a nerve stimulator and an insulated needle.

A technique employing a nerve stimulator and an insulated needle was used for supraclavicular brachial plexus block in 71 patients using 0.5% plain bupivacaine 15-20 ml. The mean minimal stimulating current to produce paraesthesia was 0.09 mA. The plexus was identified at a mean depth of 27 mm below the skin. The block was successful in 98% of patients when the stimulation was felt in the index, middle or ring finger, but was often incomplete when felt in the thumb or little finger.

Combined thoracic paravertebral block and interscalene brachial plexus block for modified radical mastectomy: A case report.

BACKGROUND: Modified radical mastectomy (MRM) is the most common surgical treatment for breast cancer. General anesthesia poses a challenge in fragile MRM patients, including cardiovascular instability, insufficient postoperative pain control, nausea and vomiting. Thoracic paravertebral block (TPVB) is adequate for simple mastectomy, but its combination with interscalene brachial plexus block (IBPB) has not yet been proved to be an effective anesthesia method for MRM. CASE SUMMARY: We describe our experience of anesthesia and pain management in 10 patients with multiple comorbidities. An ultr
