Elevated lactate as an early marker of brain injury in inflicted traumatic brain injury.

BACKGROUND: Traumatic brain injury is a major cause of disability and death in the pediatric population. The metabolic and neurochemical abnormalities that underlie traumatic brain injury remain poorly understood, but hypoxia-ischemic injury might play an important role. OBJECTIVE: This study evaluated children with inflicted traumatic brain injury using magnetic resonance spectroscopy (MRS). We postulated that children with hypoxic-ischemic injury indicated by elevated lactate in the acute phase of injury will have worse early neurological status and short-term clinical outcomes than those w

Existing evidence of self-management interventions in acquired brain injuries and traumatic injuries: a scoping review.

PURPOSE: Patients with acquired brain injuries (ABI) and severe traumatic injuries face many persisting and overlapping injury-related consequences. Despite a growing number of self-management (SM) interventions targeting these conditions, no comprehensive reviews exist. This scoping review aims to map the existing SM interventions across these injury populations. MATERIALS AND METHODS: Five databases (MEDLINE, EMBASE, PsycINFO, CINAHL, PubMed) were systematically searched. Studies were included if they adhered to the operational definition by Jonkman et al. and targeted adults with ABI and t

Toward rational use of repeat imaging in children with mild traumatic brain injuries and intracranial injuries.

OBJECTIVE: Limited evidence exists on the utility of repeat neuroimaging in children with mild traumatic brain injuries (mTBIs) and intracranial injuries (ICIs). Here, the authors identified factors associated with repeat neuroimaging and predictors of hemorrhage progression and/or neurosurgical intervention. METHODS: The authors performed a multicenter, retrospective cohort study of children at four centers of the Pediatric TBI Research Consortium. All patients were ≤ 18 years and presented within 24 hours of injury with a Glasgow Coma Scale score of 13-15 and evidence of ICI on neuroimaging

Preterm brain Injury: White matter injury.

Despite the advances in neonatal intensive care, the preterm brain remains vulnerable to white matter injury (WMI) and disruption of normal brain development (i.e., dysmaturation). Compared to severe cystic WMI encountered in the past decades, contemporary cohorts of preterm neonates experience milder WMIs. More than destructive lesions, disruption of the normal developmental trajectory of cellular elements of the white and the gray matter occurs. In the acute phase, in response to hypoxia-ischemia and/or infection and inflammation, multifocal areas of necrosis within the periventricular white

Pediatric traumatic brain injury and ocular injury.

PURPOSE: Traumatic brain injury (TBI) is a leading cause of pediatric disability and mortality. Together with sight-threatening ocular injuries, TBIs may lead to devastating consequences in developing children and complicate rehabilitation. We sought to investigate this relationship in pediatric patients admitted with major trauma. METHODS: The records of pediatric patients admitted with ocular injury and concomitant TBI were reviewed retrospectively using the National Trauma Data Bank (2008-2014). RESULTS: Of 58,765 pediatric patients admitted for trauma and also had ocular injuries, 32,173

Brain injury guidelines for the management of traumatic brain injury: a systematic review and meta-analysis.

OBJECTIVES: The Brain Injury Guidelines (BIG) serve to predict the severity of traumatic brain injuries resulting in radiologic findings. The use of BIG promises significant time and resource savings, over routine neurosurgical consultation and repeat imaging. After more than a decade of validation, studies have demonstrated BIG to reliably predict adverse outcomes. We sought to analyze the existing literature to determine the pooled efficacy of BIG and its variations. METHODS: A systematic review and meta-analysis were conducted in accordance with PRISMA guidelines (PROSPERO ID: CRD420234811

The development of a brain injury survivor patient and public involvement group by a brain injury survivor.

BACKGROUND: Patient and public involvement (PPI) in research is seen as key to ensuring applicability and impact. Undertaking PPI in people after brain injury has long been seen to be a challenge. In 2020 The NIHR Brain Injury MedTech Cooperative developed a programme with the aim of improving PPI involvement, impact and diversity in this population. METHODS: Through a process of iterative development, a PPI programme was created. It built on an existing underutilised database of people after brain injury and their carers who were interested in engaging with PPI and utilised video-calling sof

A Pediatric Brain Injury Guideline Allows Safe Management of Traumatic Brain Injuries by Trauma Surgeons.

BACKGROUND: We implemented a pediatric Brain Injury Guideline (pBIG) to stratify traumatic brain injuries (TBI) and allow management of less severe cases without repeat CT imaging or neurosurgical consultation. Injuries were classified as mild (pBIG1), moderate (pBIG2), severe (pBIG3) or isolated skull fracture (ISF) based on neurologic status, size and number of bleeds. We hypothesize that pediatric TBIs can be safely managed with this guideline. METHODS: Isolated TBIs (<18 years) were queried from the Pediatric Trauma Registry after pBIG implementation from July 2021-March 2023. Datasets in

Aging with Traumatic Brain Injury: Effects of Age at Injury on Behavioral Outcome following Diffuse Brain Injury in Rats.

Development and aging are influenced by external factors with the potential to impact health throughout the life span. Traumatic brain injury (TBI) can initiate and sustain a lifetime of physical and mental health symptoms. Over 1.7 million TBIs occur annually in the USA alone, with epidemiology suggesting a higher incidence for young age groups. Additionally, increasing life spans mean more years to age with TBI. While there is ongoing research of experimental pediatric and adult TBI, few studies to date have incorporated animal models of pediatric, adolescent, and adult TBI to understand the

Prevalence of comorbidities post mild traumatic brain injuries: a traumatic brain injury model systems study.

Traumatic brain injury (TBI) is associated with an increased risk of long-lasting health-related complications. Survivors of brain trauma often experience comorbidities which could further dampen functional recovery and severely interfere with their day-to-day functioning after injury. Of the three TBI severity types, mild TBI constitutes a significant proportion of total TBI cases, yet a comprehensive study on medical and psychiatric complications experienced by mild TBI subjects at a particular time point is missing in the field. In this study, we aim to quantify the prevalence of psychiatri

Altered brain activation in military personnel with one or more traumatic brain injuries following blast.

Explosive blast is a frequent cause of traumatic brain injury (TBI) among personnel deployed to Afghanistan and Iraq. Functional magnetic resonance imaging (fMRI) with an event-related stimulus-response compatibility task was used to compare 15 subjects with mild, chronic blast-related TBI with 15 subjects who had not experienced a TBI or blast exposure during deployment. Six TBI subjects reported multiple injuries. Relative to the control group, TBI subjects had slightly slower responses during fMRI and increased somatic complaints and symptoms of post-traumatic stress disorder (PTSD) and dep

Assessment of Sport-Related Brain Injuries with Rapid Objective Perimetry.

Sport-related mild traumatic brain injury (mTBI) or concussion is common and has long-term implications. The lack of diagnostically accurate, rapid, and easy-to-administer tests exacerbates the problem. We evaluated the objectiveFIELD Analyser® (OFA®) for mTBI. This cross-sectional study included athletes who had sustained a concussion, and two groups of controls: the putative control group (pCG) comprising rugby players who claimed they had never had a concussion, and a non-rugby normal control group (nCG). Two OFA tests, the 8 min (OFA30) and the rapid 90 s (OFA30-12), were performed. Discri

Stem Cell-Derived Extracellular Vesicle Therapy in Ischemic Brain Injuries.

Ischemic brain injury (IBI), including stroke and cardiac arrest-induced global cerebral injury, presents a significant clinical challenge due to its high morbidity and incidence of neurological deficits. Currently, effective strategies for neurological repair remain limited. Extracellular vesicles (EVs) are a diverse group of cell-derived, lipid-bound nanoparticles that encapsulate RNAs, proteins, lipids, metabolites, growth factors, and cytokines. EVs play an essential role in intercellular communication and are involved in various physiological and pathological processes. Stem cell-derived
