The influence of perinatal factors on breast feeding.

The effect on breast feeding of various factors connected with delivery and the immediate post partum period were investigated in a group of 1701 parturients. Children born by caesarean section or assisted delivery, those of low birth weight or asphyxiated at birth started breast feeding significantly less often than healthy children delivered normally. The length of breast feeding was not affected by these factors once it was started, 49% breast feeding for six months or more. Exceptionally young and old mothers breast fed less well than mothers in general. Mothers whose husbands attended the

Breast-feeding problems after epidural analgesia for labour: a retrospective cohort study of pain, obstetrical procedures and breast-feeding practices.

Various clinical practices have been found to be associated with breast-feeding problems. However, little is known about the effect of pain, obstetrical procedures and analgesia on breast-feeding behaviour. We designed a retrospective study with a questionnaire concerning pain, obstetrical procedures and breast-feeding practices mailed to 164 primiparae in Lapland. Altogether 99 mothers (60%) returned completed questionnaires that could be included in the analysis, which was carried out in two steps. Firstly, all accepted questionnaires were grouped according to the success or failure to breas

Promotion of breast feeding: effect on neonates of change of feeding routine at a maternity unit.

The effect on the health of neonates of a change in neonatal routine care, including general rooming-in, breast feeding on demand and avoidance of supplementary bottle feeding was studied in conjunction to a breast feeding campaign at a maternity unit. There was an accentuated weight loss in the neonatal period during ad libitum breast feeding. The mean serum bilirubin of clinically jaundiced infants was slightly higher on a 4-hourly feeding schedule with supplementary bottles than on the new regimen. There was a similar high (32-33%) incidence of bilirubin levels greater than 205 mumol/l (12

Exclusive Breast Feeding in Low Birth Weight Babies.

BACKGROUND: Human milk is the ideal source of nutrition for full term neonates, but there is controversy regarding its use as a nutrition source for preterm and low birth weight infants. However current data supports the use of human milk for low birth weight infants. This study was undertaken to evaluate the growth of both preterm and term small for gestational age infants nursed on exclusive breast milk from birth to four months of age. METHODS: This was a prospective, longitudinal study carried out in a recognized baby-friendly hospital where exclusive breast-feeding is practiced in all ne

Breast-feeding influences on later life--cardiovascular disease.

Current evidence, almost exclusively from observational studies, provides a rather mixed picture. From the few studies that have been able to look at fatal or non-fatal cardiovascular events, there is little indication that breast-feeding is associated with either an increased or decreased risk. With respect to blood pressure, the meta-analyses suggest a small but statistically significant lowering of around 1 mmHg SBP associated with having been breast-fed in infancy. However, there is a strong indication from the meta-analyses that even this small effect may partly be accounted for by public

Epidural analgesia: breast-feeding success and related factors.

OBJECTIVE: to compare the early breast-feeding behaviours of full-term newborns whose mothers had received epidural analgesia (EDA) during an uncomplicated labour, with a group of newborns whose mothers had not received EDA. DESIGN AND SETTING: a retrospective comparative study design was used and the study was carried out in a labour ward in Stockholm, Sweden between January 2000 and April 2000. The ward has about 5500 deliveries per year. PARTICIPANTS: all maternity records of women who had received EDA during labour (n=585) were included in the study. For each EDA record, a control record

Breast-feeding after vertical mammaplasty with medial pedicle.

A retrospective study was performed in which the breast-feeding success of women of childbearing age (15 to 40 years) with macromastia but no prior breast surgery was compared with that of women of similar age who had undergone medial pedicle/vertical pattern reduction mammaplasty. All women completed a self-administered questionnaire that provided information on their breast-feeding success. The control group consisted of 149 women with macromastia (mean age, 27 years) who had been evaluated for possible breast reduction surgery and who had children before their consultation. The study group

[Mother-infant rooming-in and exclusive breast feeding].

OBJECTIVE: To determine the influence of joined mother-child hospital lodging (JL) on the frequency of exclusive human lactation (EHL) from birth to four months of age. MATERIAL AND METHODS: Non-randomized cohort study performed at the lactation clinic of the "Luis Castelazo Ayala" Hospital of the Mexican Social Security Institute from January 1993 to October 1994 which included 178 healthy mother-child pairs (MCP) with term pregnancy. The jointly lodged group (JL) consisted of 90 MCP in which mothers received assessment for lactation; 88 MCP were lodged separately and no assessment was provi

Does breast feeding protect against non-gastric infections?

There is convincing evidence that breast-feeding is protective against gastro-enteritis and diarrhoea, but for other infections the situation is less clear cut. There is evidence that breast-fed infants are at increased risk of one infection (infant botulism). They are probably not significantly protected from upper respiratory tract infections (other than otitis media.), but they may be at a decreased risk of lower respiratory tract infections, particularly those associated with respiratory syncytial virus. There is strong evidence that Haemophilus influenzae B infection is more likely in the

Breast-feeding counselling in a diarrhoeal disease hospital.

Lactation counsellors were trained to advise mothers of partially breast-fed infants who were admitted to hospital because of diarrhoea, so that they could start exclusive breast-feeding during their hospital stay. Infants (n = 250) up to 12 weeks of age were randomized to intervention and control groups. Mothers in the intervention group were individually advised by the counsellors while mothers in the control group received only routine group health education. During follow-up at home by the counsellors a week later, only the mothers in the intervention group were counselled. All the mothers

[Some recent determinants of breast feeding in Chile].

Four factors that could explain in great part breast milk production difficulties in Chilean mothers are analyzed according to recent studies. Maternal nutritional status, positively influences breast milk volume and duration, thus adequate weight increment during pregnancy should be encouraged. Maternal work has been negatively correlated with breast-feeding duration in various local studies, by the contrary post-partum rest and direct care of infant show protective effects. Mothers that smoke during pregnancy and lactation have shorter breast-feeding periods than non smokers, meanwhile the l

[Breast feeding and hypernatremic dehydration. 3 case studies].

Despite the well-recognized advantages of breast-feeding, a rare but life-threatening complication may occur with failure to thrive resulting in severe malnourishment or hypernatremic dehydration. We report 3 more cases. Two of them with severe hypernatremic dehydration and cachexia were treated using peritoneal dialysis rather than intravenous fluids in order to rapidly correct the severe metabolic acidosis but slowly the serum osmolarity. Although insufficient breast-milk volume appears to prevail over positive sodium balance in the pathway to hypernatremic dehydration, high sodium concentra

[Breast feeding: the natural means of health promotion].

Breast-feeding has long been accepted as the best way of feeding young children. Promotion of breast-feeding in hospitals has increased the proportion of women beginning to breast feed. The maintenance of lactation however needs other support, which can be provided by self help groups. "La Leche Liga"'s activity centers around this problem. Breastfeeding circles take place in Switzerland too and provide an opportunity for sharing experience and learning to solve everyday problems.

The Pima Infant Feeding Study: breast feeding and gastroenteritis in the first year of life.

In 1978, a retrospective study of infant feeding was conducted among 257 Pima Indian women of reproductive age, who were residing on the Gila River Reservation, Arizona. With data on infant feeding patterns from 683 infants born 1950-1977 to the Pima Indian women, infants were classified into one of five feeding groups based on the duration breast-fed and time of introduction and duration bottle-fed. History of severe diarrhea/diarrhea with dehydration/diarrhea and vomiting was abstracted from the infant's medical record and classified as a case of gastroenteritis. The risk of developing a fir

Therapy of psoriasis during pregnancy and breast-feeding.

There have been multiple systemic drugs approved for the therapy of psoriasis vulgaris and psoriasis arthritis (PsA) in the last decade. However, treatment decisions are difficult to make in women planning a pregnancy and in pregnant and lactating women due to the paucity of data for such cases. The strongest evidence for psoriasis therapy during pregnancy exists for topical corticosteroids. Medically controlled use of UVB-therapy is also considered safe. The best evidence regarding systemic therapy during pregnancy and lactation is available for the group of TNF-alpha inhibitors, which is als

Breast-feeding influences cognitive development in Filipino children.

The importance of breast-feeding (BF) for cognitive development has been researched widely over the past several decades. Although scholars agree that children who breast-feed are generally more intelligent, it is uncertain whether this advantage is due to BF effects or to other accompanying healthy characteristics of women who breast-feed. This is a problem in nearly every study, and even in studies controlling for known confounding variables, residual confounding remains a concern. This study tried a new approach, evaluating the relation between BF and cognitive development or ability in a p

Hypernatraemic dehydration and breast feeding: a population study.

As part of a population based regional review of all neonatal readmissions, the incidence of dehydration with hypernatraemia in exclusively breast fed infants was estimated. All readmissions to hospital in the first month of life during 1998 from a population of 32 015 live births were reviewed. Eight of 907 readmissions met the case definition, giving an incidence of at least 2.5 per 10 000 live births. Serum sodium at readmission varied from 150 to 175 mmol/l. One infant had convulsions. The sole explanation for hypernatraemia was unsuccessful breast feeding in all cases. The eight cases are
