Long-term milrinone therapy as a bridge to heart transplantation: Safety, efficacy, and predictors of failure.

BACKGROUND: Studies of long-term inotrope use in advanced HF have previously provided limited and conflicting results. This study aimed to evaluate the safety and efficacy of long-term milrinone use and identify predictors of failure to bridge to orthotropic heart transplant (OHT) in a cohort of end-stage heart failure (HF) patients listed for heart transplantation and receiving inotrope therapy. METHODS: The study included 150 adults listed for OHT at a single center from 2001 to 2017 who received milrinone therapy for ≥30 days. Multivariate Cox proportional hazards models were used to ident

Antiviral T Cells for Adenovirus in the Pretransplant Period: A Bridge Therapy for Severe Combined Immunodeficiency.

Viral infections can be life threatening in patients with severe combined immunodeficiency (SCID) and other forms of profound primary immunodeficiency disorders both before and after hematopoietic stem cell transplantation (HSCT). Adoptive immunotherapy with virus-specific T cells (VSTs) has been utilized in many patients in the setting of HSCT, but has very rarely been attempted for treatment of viral infections before HSCT. Here we describe the use of VSTs in an infant with RAG1 SCID who had developed disseminated adenovirus which failed to improve on cidofovir. Adenovirus cleared following

Mitraclip procedure as a bridge therapy in a patient with heart failure listed for heart transplantation.

Functional mitral regurgitation (MR) is frequently detected in patients with dilated cardiomyopathy and advanced heart failure, worsening quality of life and predicting poor survival. However, the optimal treatment of patients with advanced heart failure and severe MR has been controversial. We present the case of a 55-year-old man with previous aortic valve replacement, severe MR with high-grade pulmonary hypertension, and refractory heart failure (HF). He was listed for cardiac transplant and underwent percutaneous MitraClip implantation as bridge therapy. The postoperative course was uneven

Bridge therapy or standard treatment for urgent surgery after coronary stent implantation: Analysis of 314 patients.

Intravenous administration of a short acting glycoprotein IIb/IIIa inhibitor has been proposed as a bridge to surgery in patients on dual antiplatelet treatment, but data in comparison with other treatment options are not available. We conducted a retrospective analysis of consecutive patients who underwent un-deferrable, non-emergency surgery after coronary stenting. The bridge therapy was performed after discontinuation of the oral P2Y12 inhibitor by using i.v. tirofiban infusion. Net Adverse Clinical Events (NACE) was the primary outcome. We analyzed 314 consecutive patients: the bridge str

Acute ventricular septal defect treated with an Impella recovery as a 'bridge therapy' to heart transplantation.

We present the case of a 59-year-old male, admitted to hospital for cardiogenic shock due to massive infero-lateral myocardial infarction. Angiography showed occlusion of the right coronary artery and widespread critical lesions of both the anterior descending and circumflex artery. Echocardiography showed inferior akinesia with a large posterior ventricular septal defect (VSD). The haemodynamic instability induced us to use a left ventricular assist device (L-VAD) like Impella for easiness of its percutaneous implantation and for its duration. We obtained the stabilisation of the patient and

Acceptance of a pump-driven infusion therapy with prostaglandin E1 as a bridge to heart transplantation.

Ambulatory pump-driven intravenous infusions are a novel and - compared with hospitalization-cost-effective procedure to bridge refractory heart failure patients to cardiac transplantation. In the present study 13 patients received chronic infusions with prostaglandin E1 alone or in conjunction with catecholamines and the acceptance of this bridging therapy was investigated over a period of seven weeks. Prostaglandin E1 was uptitrated from 2.5 ng/kg/min to a maximum of 40 ng/kg/min, according to individual tolerance. 50% of the maximum tolerated dose of prostaglandin E1 was used for chronic in

Immune checkpoint targeted drug conjugate therapies: Bridging preclinical and clinical innovation for solid tumor therapy.

Solid tumors remain a significant therapeutic challenge due to their complex biology, heterogeneous tumor microenvironment (TME), and frequent development of resistance to standard treatments. Although immune checkpoint inhibitors (ICIs) have redefined tumor therapy by restoring antitumor immunity, their clinical benefit in Solid tumors is often constrained by low response rates (LRR), immune-related toxicities, and adaptive resistance. Immune checkpoint targeting drug conjugates (IDCs) have recently emerged as an innovative approach that combines checkpoint blockade with the targeted delivery

Efficacy and safety of bridging therapy prior to CAR T-cell therapy in relapsed or refractory multiple myeloma.

Disease burden at the time of BCMA-directed chimeric antigen receptor (CAR) T-cell infusion is a key determinant of outcome in relapsed or refractory multiple myeloma (RRMM). Bridging therapy is frequently administered between leukapheresis and infusion to prevent disease progression, yet its clinical impact remains unclear. We conducted a multicenter real-world cohort study of 399 patients with RRMM treated with BCMA-directed CAR T-cell therapy, including 348 (87%) who received bridging therapy. Bridging therapy recipients had more advanced and biologically adverse disease, including higher r

Chronic oral anticoagulant therapy in carotid artery stenting: the un-necessity of perioperative bridging heparin therapy.

INTRODUCTION: Chronic oral anticoagulant therapy (OAT) is of widespread use, and usually its management in patients undergoing carotid artery stenting (CAS) is through perioperative bridging heparin therapy. Aim of the present study is to analyze a single center experience of CAS in patients maintaining OAT without perioperative bridging heparin therapy. MATERIALS AND METHODS: A retrospective evaluation of consecutive patients submitted to CAS was performed. Clinical anatomical characteristics and chronic OAT were evaluated to find a correlation with stroke, death, myocardial infarction and b

Selective internal radiation therapy as a bridge to curative-intent surgery in bulky colorectal liver metastases.

Achieving secondary resectability remains a major challenge in patients with initially unresectable colorectal liver metastases (CRLM). We report a 46-year-old patient with rectal adenocarcinoma and three large synchronous unresectable liver metastases who achieved a durable response following an intensive multimodal strategy. FOLFOX plus panitumumab induced partial regression, while selective internal radiation therapy (SIRT) led to extensive tumor necrosis, enabling curative-intent extended left hepatectomy combined with microwave ablation. The patient remains in remission without systemic t

Prolonged Terlipressin Therapy as a Bridge to Curative TIPS for HRS-AKI in a Nontransplant Candidate.

Hepatorenal syndrome-acute kidney injury (HRS-AKI) is a severe complication of advanced cirrhosis with high mortality and limited treatment options. Terlipressin is currently the only FDA-approved therapy for HRS, though recurrence is common upon discontinuation in patients without access to liver transplantation. Transjugular intrahepatic portosystemic shunt (TIPS) may provide definitive treatment for HRS-AKI in nontransplant candidates or in the setting of prolonged waitlists. This case illustrates the safety and success of terlipressin prior to TIPS for definitive treatment of HRS-AKI in a

Transcatheter Mitral Repair as a Bridge to Cancer Therapy in a Young Patient With Marantic Endocarditis.

BACKGROUND: Nonbacterial thrombotic endocarditis (NBTE) can cause valvular destruction and severe mitral regurgitation (MR). In multimorbid cancer patients, surgery is often not feasible, complicating timely oncologic therapy. CASE SUMMARY: A 27-year-old woman presented with systemic embolization and was found to have NBTE involving the aortic and mitral valves caused by ROS1-positive lung adenocarcinoma. She underwent mechanical aortic valve replacement and mitral debridement. Progressive MR led to cardiac decompensation. Mitral valve transcatheter edge-to-edge repair (M-TEER) achieved hemod

Drug-Eluting Bead Transarterial Chemoembolization as Bridge Therapy for Hepatocellular Carcinoma Before Living-Donor Liver Transplantation.

BACKGROUND: The majority of hepatocellular carcinoma (HCC) patients is diagnosed in late stages and therefore becomes ineligible for potentially curative treatment such as resection or liver transplantation. Transarterial chemoembolization (TACE) with drug-eluting beads (DC beads, Biocompatibles, Farnham, United Kingdom) has been proven with less side effects and better efficacy than conventional TACE, especially among patients with poor liver function. PURPOSE: The aim of this study is to evaluate outcomes of HCC patients who received TACE with DC beads, which resulted to eligibility for liv

Successful usage of extracorporeal membrane oxygenation as a bridge therapy for acute pulmonary embolism between hospitals.

A 50-year-old man presented to a nearby hospital with loss of consciousness. Investigation revealed thrombus formation at the tricuspid valve. Due to suspected pulmonary embolism, the patient underwent contrast-enhanced computed tomography during which he went into a shock with sudden drop in functional oxygen saturation (SpO(2)). Extracorporeal membrane oxygenation (ECMO) was introduced for cardiovascular and respiratory support, and he was transferred to our hospital for further treatment. The patient was treated by surgical thromboembolectomy and was dismissed from the hospital without majo

Disulfide-Bridged Ru Complex-Mediated Photo-Disulfidptosis for Colorectal Cancer Therapy.

Despite numerous therapeutic strategies targeting specific programmed cell death pathways, effectively eliminating malignant colorectal cancer (CRC) remains a significant challenge. Disulfidptosis, a newly identified form of cell death, is characterized by rapid NADPH depletion and abnormal disulfide bond formation in cytoskeletal proteins in cells with high SLC7A11 expression under glucose starvation. Given the aberrant expression of SLC7A11 and the hypermetabolic phenotype of CRC cells, targeting the disulfidptosis pathway offers a promising therapeutic approach for CRC treatment. Here, we d

CardioGAP+: bridging the gap between patient identification and therapy implementation in cardiovascular risk.

INTRODUCTION: In the face of escalating medical complexity, the development of simple strategies that allow the optimization of resources becomes more urgent than ever. This is particularly evident in cardiovascular diseases (CVD), where despite successful risk stratification and mitigation strategies, elevated mortality rates persist among socioeconomically disadvantaged groups. Efforts to prevent CVD events rely on early identification of at-risk individuals and timely implementation of risk-mitigating measures. Persistent challenges in health systems to identify and intervene with high-risk

Bridging the gaps in fibroblast activation protein targeted radionuclide therapy: a translational perspective.

Fibroblast activation protein-targeted radionuclide therapy (FAP-TRT) has emerged as a novel strategy for modulating the tumour microenvironment (TME) by selectively eradicating FAP-expressing cancer-associated fibroblasts (CAFs). Although preclinical studies have demonstrated promising results across various tumour models using diverse radiolabelled FAP inhibitors, clinical translation remains limited by modest efficacy, short tumour retention, and highly heterogeneous responses. This review aims to provide an overview of recent advances in radiopharmaceutical design to enhance tumour targeti

Bridging neurotransmission and tumor biology: the emerging focus on CHRM1 in skin cancer therapy.

INTRODUCTION: Skin cancer is one of the most prevalent types of cancer and has shown an upward trend recently. Among various types, melanoma skin cancer is known to be the most dangerous, exhibiting resistance to treatment, and is also prone to metastasis. Targeted and immunotherapeutic approaches, despite their potential, exhibit limitations. Recently, the research focus has shifted to various neurotransmitter-mediated pathways acting as a key mediator between tumor development and neuronal signaling. Among them, CHRM1 has garnered traction as a target that supports tumor growth and prolifera

Natural Plant Bioactives as Regulators of Histone Modifications: Bridging Epigenetics and Anticancer Therapy.

Cancer remains a leading cause of global mortality, with tumor heterogeneity, recurrence, metastasis, and drug resistance posing significant challenges to conventional therapies, which are often limited by toxicity and efficacy. Epigenetic regulation, particularly histone modifications such as methylation, acetylation, phosphorylation, and lactylation, plays a pivotal role in orchestrating gene expression and tumor progression, representing promising yet complex therapeutic targets. Bioactive compounds derived from natural plants, characterized by multi-target activity and low toxicity, have e
