The relationships between host factors, viral shedding, illness severity, and antibody response in respiratory syncytial virus (RSV)-induced bronchiolitis are poorly defined. These relationships were prospectively evaluated in 77 infants hospitalized with RSV bronchiolitis in multicenter, double-blind, placebo-controlled trials of RSV immunoglobulin therapy. Severity of illness was influenced by age and host risk factors but was not influenced by RSV neutralizing antibody titer or by the amount of virus in nasal secretions at enrollment. Virus recovery in nasal secretions was variable but was

BACKGROUND: Neurally Adjusted Ventilatory Assist (NAVA) is a patient-triggered ventilatory mode requiring intact respiratory drive. We compared 2 intravenous morphine infusion doses in infants with acute viral bronchiolitis receiving NAVA. The objectives were, to evaluate (1) impact on neural respiratory drive, measured by electrical activity of the diaphragm (EAdi) and estimated airway occlusion pressure (P0.1) and (2) tolerability, via the COMFORT behavioral scale and physiological variables.
METHODS: Randomized, nonblinded, crossover (morphine dose), feasibility trial. Infants likely requi

OBJECTIVES: Viral bronchiolitis is an acute infection and inflammatory disease of the respiratory tract, with infants typically presenting with the most severe symptoms. Medical management of bronchiolitis is mostly supportive. Several preliminary studies suggest potential benefit from the use of high-flow nasal cannula systems. Although high-flow nasal cannula is a well-established modality in the newborn intensive care unit, its use in the pediatric intensive care unit for acute respiratory failure is far less established. The objective of this study was to identify any laboratory and clinic

Although there is increasing evidence showing that infants with viral bronchiolitis exhibit a high degree of heterogeneity, a core uncertainty shared by many clinicians is with regard to understanding which patients are most likely to benefit from bronchodilators such as albuterol. Based on our review, we concluded that older infants with rhinovirus (RV) bronchiolitis, especially those with a nasopharyngeal microbiome dominated by Haemophilus influenzae; those affected during nonpeak months or during non-respiratory syncytial virus (RSV) predominant months; those with wheezing at presentation;

BACKGROUND: Acute viral bronchiolitis is the leading cause of hospitalization among infants during the first year of life. Most infants hospitalized for bronchiolitis do not present risk factors and are otherwise healthy. Our objective was to determine the genetic features associated with the risk and a severe course of bronchiolitis.
METHODS: We prospectively evaluated 181 infants with severe bronchiolitis admitted at three hospitals over a 2-year period, who required oxygen therapy. The control group consisted of 536 healthy adults. Patients were evaluated for the presence of comorbidities

BACKGROUND: Bronchiolitis is a prevalent viral respiratory illness in infants, often caused by respiratory syncytial virus (RSV). It is the leading cause of hospitalization in children under 1 year of age. The introduction of Nirsevimab, a long-acting monoclonal antibody approved in 2022, aimed to reduce RSV-related hospitalizations and severe cases. This study assesses the impact of Nirsevimab on bronchiolitis incidence, severity, and viral etiology in Sicily from 2021 to 2025.
METHODS: This observational study included all pediatric patients hospitalized for bronchiolitis at the University

A 36-year-old Danish man, living in Asia, was diagnosed with Pneumocystis pneumonia (PCP) and HIV in 2013 (CD4+ count: 6 cells/µL; viral load: 518 000 copies/mL). He initiated combination antiretroviral therapy. Later that year, he was also diagnosed with granulomatosis with polyangiitis and was treated with prednisolone. Despite complete viral suppression and increasing CD4+ count (162 cells/µL), he was readmitted with PCP in April 2015. Subsequently, he returned to Denmark (CD4+ count: 80 cells/µL, viral suppression). Over the following months, he developed progressive dyspnoea. Lung functio

Pneumonia remains a leading cause of morbidity and mortality in developing countries. Comprehensive surveillance data are needed to review the prevention and control strategies. We conducted active surveillance of acute lower respiratory infections among children aged <2 years hospitalized at two hospitals of Islamabad, Pakistan. Viral etiology was determined using real-time PCR on respiratory specimens collected during March 2011-April 2012. The overall mean age was 7.83 ± 5.25 months while no statistical difference between age or sex distribution of patients with positive and negative viral

INTRODUCTION: High-flow nasal cannula (HFNC) is commonly used as first step respiratory support in infants with moderate-to-severe acute viral bronchiolitis (AVB). This device, however, fails to effectively manage respiratory distress in about a third of patients, and data are limited on determinants of patient response. The respiratory rate-oxygenation (ROX) index is a relevant tool to predict the risk for HFNC failure in adult patients with lower respiratory tract infections. The primary objective of this study was to assess the relationship between ROX indexes collected before and 1 h after

The relationship between viral infection, host immune response in infants with respiratory syncytial virus (RSV) bronchiolitis and subsequent wheezing is discussed. We measured RSV-RNA load and interferon-λ1-3 expression in the nasopharyngeal washings from 68 infants hospitalized for RSV bronchiolitis, and wheezing was assessed 36 months after the first episode of bronchiolitis. Higher RSV-RNA load and higher interferon-λ2/3 levels were found in children with recurrent wheezing at 36-month follow-up.

BACKGROUND: Acute viral bronchiolitis is the most common lower respiratory tract infection in infants < 2-years of age. Airway Clearance Techniques (ACTs) aim to reduce airway obstruction, thereby decreasing airway resistance, improving gas exchange, and reducing respiratory load, all of which can lead to improved clinical stability.
METHODS: One hundred ninety-two infants were randomly allocated to one of the following ACTs using flow-based techniques: Assisted Autogenic Drainage (AAD; n = 62), Prolonged Slow Expiration (PSE; n = 63), or a control group (n = 67). The sessions were performed
