[Evolution of CD4 and CD8 lymphocyte subsets in HIV seronegative and seropositive hemophiliacs].

PURPOSE: To asses the evolution over time of the CD4, CD8 and total lymphocyte counts in both HIV seronegative and HIV seropositive haemophiliacs. PATIENTS AND METHODS: A total of 124 haemophilic patients (77 HIV seropositive and 47 HIV seronegative) regularly controlled at our Hospital were studied. All patients have had several (range 2-15) CD4 and CD8 lymphocyte counts, with a minimum interval of 2 or more years between the first count and the last one (median 7.7 years). A linear regression analysis of the serial cell counts against the time was made and the corresponding slopes were esti

The trajectories of CD4 T lymphocytes over time in patients who have defaulted on treatment for tuberculosis in a cohort of people living with HIV, Recife/PE.

BACKGROUND: The CD4 T lymphocyte count in people living with HIV (PLHIV) is a predictor for the progression of the disease (AIDS), survival and response to antiretroviral treatment (ART). A CD4 T lymphocyte count of less than 200 cells/mm3 is indicative of a greater risk for the onset of opportunistic diseases and death. Defaulting on treatment for tuberculosis (TB) may impact immune recovery in PLHIV who are taking ART. The aim of this study was to investigate an association of the CD4 lymphocyte with TB treatment Trajectory and with death. METHODS: A cohort of PLHIV over eighteen years of a

Progressive multifocal leukoencephalopathy in an HIV patient receiving successful long-term HAART.

Progressive multifocal leukoencephalopathy (PML) has been traditionally associated to severe immunosuppression and described mainly in highly active antiretroviral therapy (HAART)-naïve patients with a low lymphocyte CD4+ count. In the last years, some cases of PML have been described in HIV patients with a higher CD4+ count shortly after initiation of HAART and in association with the immune reconstitution inflammatory syndrome (IRIS). We report on a rare case of PML, not IRIS associated, that occurred in a HIV-positive patient with a lymphocyte CD4+ count greater than 700/µl and with an unde

How do inflammatory marker dynamics shift with acute calculous cholecystitis severity?

INTRODUCTION: Gallstone may cause complications of cholecystitis, gallbladder gangrene, perforation, and related sepsis. This study aims to identify how CRP and immune cells change in patients with acute calculous cholecystitis based on the severity of disease. METHOD: Patients with acute calculous cholecystitis were categorized into three main groups-mild, moderate, and severe-based on the Tokyo guidelines. CRP, neutrophil, lymphocyte, helper T cells, cytotoxic T lymphocytes, and HLA-DR expression on CD14+ monocytes were measured using flow cytometry at the time of hospitalization from all p

Clinical features and prognostic factors of hospitalized patients with acute exacerbation of interstitial lung disease secondary to dermatomyositis/polymyositis.

INTRODUCTION/OBJECTIVES: Acute exacerbation of interstitial lung disease secondary to dermatomyositis or polymyositis (DM/PM-AEILD) is an intractable, life-threatening disease with unsatisfactory therapeutic efficacy, particularly in patients positive for anti-melanoma differentiation-associated gene 5 (anti-MDA5) antibody (Ab). Identifying prognostic risk factors for DM/PM-AEILD might help clinicians to stratify high-risk patients at an early stage and deliver targeted standardized treatment. METHODS: We retrospectively collected clinical data of 35 hospitalized patients diagnosed with DM/PM

Tuberculosis in the Context of Idiopathic CD4 Lymphocytopenia: A Case Report and Literature Review.

BACKGROUND Idiopathic CD4 lymphocytopenia is a rare immunodeficiency syndrome characterized by a persistent reduction in CD4 lymphocyte count to fewer than 300 cells/µL or less than 20% of total T lymphocytes on multiple occasions. This diagnosis is made in the absence of HIV infection or other known immunodeficiencies. Herein, we present a case report of idiopathic CD4 lymphocytopenia in an adult patient, contributing to the limited literature on the management challenges of this rare immunodeficiency. CASE REPORT A 42-year-old male patient presented with shortness of breath, productive cough

Idiopathic CD4+ lymphocyte deficiency. Report of an unusual case associated with atopic dermatitis and allergic contact dermatitis and review of the literature.

BACKGROUND: Idiopathic CD4+ lymphocyte deficiency is a newly described entity of apparently non-human immunodeficiency virus-associated helper T-cell depletion. The clinical spectrum continues to evolve, but, to date, it has included patients ranging from those with minimal symptoms to those who have died with acute opportunistic infections. Several individual cases have been previously reported, many with distinctive, primarily infectious, cutaneous manifestations. OBSERVATIONS: We describe a patient with idiopathic CD4+ lymphocyte deficiency distinguished by a unique clinical presentation o

Impact of pulmonary tuberculosis on survival of HIV-infected adults: a prospective epidemiologic study in Uganda.

BACKGROUND: Retrospective cohort studies of tuberculosis suggest that active tuberculosis accelerates the progression of HIV infection. The validity of these findings has been questioned because of their retrospective design, diverse study populations, variable compliance with anti-tuberculous therapy and use of anti-retroviral medication. To assess the impact of tuberculosis on survival in HIV infection we performed a prospective study among HIV-infected Ugandan adults with and without tuberculosis. METHODS: In a prospective cohort study, 230 patients with HIV-associated tuberculosis and 442

Prevalence of pulmonary tuberculosis and immunological profile of HIV co-infected patients in Northwest Ethiopia.

BACKGROUND: In sub-Saharan Africa, as high as 2/3 of tuberculosis patients are HIV/AIDS co-infected and tuberculosis is the most common cause of death among HIV/AIDS patients worldwide. Tuberculosis and HIV co-infections are associated with special diagnostic and therapeutic challenges and constitute an immense burden on healthcare systems of heavily infected countries like Ethiopia. The aim of the study was to determine the prevalence of pulmonary tuberculosis and their immunologic profiles among HIV positive patients. METHODS: A cross sectional study was conducted among adult HIV-positive p

Prevalence of Risk Factors Associated With Poor Quality of Sleep in People Living with HIV and the Correlation between Quality of Sleep and Cd4+ T Lymphocyte Reconstitution: A Cross-Sectional Study from Turkey.

INTRODUCTION: The prevalence of sleep disorders in people living with HIV (PLWH) is higher than in the general population. Even if viral suppression is achieved with Antiretroviral Therapy (ART), the chronic immune activation and increased inflammation due to immune reconstitution persist. The aim of our study was to determine the prevalence of poor quality of sleep (QoS) and associated risk factors in PLWH and to investigate the relationship between poor QoS and CD4 T lymphocyte count and CD4 reconstitution. METHODS: PLWH ≥18 years old, attending for routine HIV monitoring were recruited. PL

Acute rheumatic fever in human immunodeficiency virus infection.

A 25-year-old homosexual man with a childhood history of rheumatic heart disease presented with painful joints, fever and chest pain. He was diagnosed as having acute rheumatic fever and was found to be HIV antibody-positive. His illness responded to conventional treatment but he had a persistently low CD4 lymphocyte count and was started on zidovudine. Interpretation of the significance of a low CD4 lymphocyte count is problematic in a patient with coincident rheumatic fever and HIV infection as both conditions can cause CD4 lymphopenia.

Extensive brain masses and cavitary lung lesions associated with toxoplasmosis and acquired immunodeficiency syndrome.

Toxoplasmosis is an important cause of enhancing brain lesions in patients with acquired immunodeficiency syndrome (AIDS), and it is typically associated with low CD4-lymphocyte counts. Extensive toxoplasma encephalitis when the CD4-lymphocyte count is above 100 cells/µl is unusual. Cavitary lung lesions are also not typically associated with toxoplasmosis. Here, we present a case of toxoplasmosis associated with extensive brain masses and cavitary lung lesions, both of which improved with directed toxoplasmosis therapy, in an AIDS patient with a CD4 cell count of 120 cells/µl.

[Paraproteins and lymphoma in HIV positive patients].

BACKGROUND AND AIM: Compared to healthy subjects there is a higher incidence of monoclonal immunoglobulins (= paraproteins = PP) in patients infected with the human immunodeficiency virus (HIV). High-grade B-cell non-Hodgkin's lymphomas (NHL) are the second most common neoplasms in these patients. Our aim was to determine whether paraproteins would be of diagnostic significance regarding an underlying or developing NHL. PATIENTS AND METHODS: The sera of 202 HIV-positive patients were tested for the presence of monoclonal or oligoclonal bands by using high-resolution-electrophoresis (HRE) and
