The classical sub-types are: the syndromes of Capgras, Fregoli,Intermetamorphosis (mentioned in 3) and Subjective doubles. These syndromes occur in a clear sensorium and shouldbe differentiated from the banal transient misidentifications occurring in confusional states and in mania and from thenon-delusional misidentifications (e.g.

True vignettes: interesting, illustrative examples of behavioral abnormalities in people with Parkinson's disease.

BACKGROUND: Parkinson's disease (PD) and dementia with Lewy bodies (DLB) are neurobehavioral disorders in which behavioral abnormalities are common but often not recognized. OBJECTIVE: To bring to life the actual descriptions rendered by our patients of their unusual experiences. METHODS: Members of the Parkinson Study Group were asked to provide illustrative examples of particularly memorable abnormal behaviors that their own PD patients or family members reported. RESULTS: These recollections were deidentified and sorted into categories. These disorders include the classically described s

'Clonal pluralization of the self': a new form of delusional misidentification syndrome.

The authors present a patient with paranoid schizophrenia, who has the delusion that he exists in plural numbers. The patient declares these doubles to be both psychologically and physically completely identical to him, and he believes 'them' to be in fact women. In connection with the case, the authors discuss the phenomena of reduplicative paramnesia and clonal pluralization, and they suggest introducing the psychopathological term 'clonal pluralization of the self' for the reported phenomenon.

Misidentified political figures: an underappreciated danger.

A series of twelve patients is presented in which each patient suffered from one or more misidentification syndromes and also misidentified one or more political figures. The fact that misidentification syndromes have been associated with physical violence and that the majority of the patients studied had a history of physical violence suggests that these individuals could pose a significant danger of physical harm to others, including political figures. Persons who threaten political figures should be evaluated for misidentification syndromes.

Psychiatric disorders associated with acquired brain pathology.

Acquired brain pathology can be associated with diverse psychiatric manifestations. Three major types of psychiatric disorders potentially found in cases of acquired brain pathology are examined: (1) psychosis, (2) mood disorders, and (3) personality disorders with special emphasis in so-called "acquired psychopathy." Two types of psychotic manifestations are reviewed: (a) Schizophrenia-like psychosis; (b) Other delusional disorder, specifically, somatoparaphrenia and delusional misidentification syndromes, which include reduplicative paramnesias, Capgras syndrome, Frégoli syndrome, and "doubl

Psychiatric symptoms typical of patients with dementia with Lewy bodies - similarity to those of levodopa-induced psychosis.

We examined psychiatric symptoms in eight cases with dementia with Lewy bodies (DLB), which included visual hallucination of persons or small animals, visual illusion, metamorphosia, leibhaftige Bewusstheit, personal or topographical misidentification, Capgras' syndrome and reduplicative paramnesia as well as depressive state and delusion of persecution. These psychiatric symptoms are identical to those of levodopa-induced psychosis, although these symptoms appeared before medication with anti-Parkinson drugs. Alzheimer's disease or plaque disease? Two cases at the frontier of a definition.

Atypical dementias confront the adequacy of current diagnostic concepts. The two patients with atypical dementia syndromes described here shared common postmortem features of numerous neocortical neuritic (senile) plaques and microvascular amyloid, sparing of hippocampus and substantia nigra, and the virtual absence of neurofibrillary tangles. Microscopically, the two differed only by the presence of a few subcortical Lewy bodies in case 1. These similar morphologic features were associated with dramatically different clinical presentations. In the first patient, visual hallucinations, Capgras

Delusional disorders are a naturally occurring 'experimental psychosis'.

A delusion can only be 'explained' if it can be correlated with a demonstrable brain event, and this is only beginning to be possible. The misidentification syndromes are leading the way in demonstrating a close relationship between quite specific brain abnormalities and highly characteristic delusional symptoms. Although delusional disorder (DSM-IV 297.10) and its treatment are well described, virtually no investigations have been carried out on the neuropathological substrate. This article suggests that delusional disorder is a naturally occurring 'model psychosis' which would reward experim

Religious conversion in an older male with longstanding epilepsy.

Religious experiences in epilepsy patients have provoked much interest with suggestions that hyperreligiosity is associated with temporal lobe seizures. Extreme varieties of religious behavior may be more frequent in epilepsy patients during ictal activity or during post-ictal psychotic episodes. We report a 75 year-old man with epilepsy who developed a progressive decline in cognition and behavior following a religious conversion 15 years earlier. He subsequently developed religious delusions of increasing severity and symptoms of Capgras syndrome. Brain imaging revealed bilateral posterior c

Delusions and hallucinations of cocaine abusers and paranoid schizophrenics: a comparative study.

We compared delusions and hallucinations of 100 cocaine abusers and 100 paranoid schizophrenic subjects admitted to an East Texas state psychiatric hospital. Subjects in both groups feared that individuals or organized groups might harm them in some way, but delusions of the paranoid schizophrenic subjects were more often bizarre than those of the cocaine abuse subjects. "Cocaine bugs" (parasitosis) were more often found in the cocaine abuse subjects. Command hallucinations were found in both groups, but the commands of the schizophrenic group more often related to harming or killing others. C

Disorders of face perception and recognition.

Face recognition is one of the most complex visual tasks performed by the human brain. Data from monkeys suggest that area IT may play a key role in identifying faces, and functional imaging research suggests that the human homologue of IT may be located in the medial occipitotemporal cortex, where a FFA has been located. Damage to medial occipitotemporal structures on the right or bilaterally leads to prosopagnosia, the failure to recognize facial identity. Prosopagnosia is not a single functional disorder but a family of dysfunctions, with different patients having different degrees of impai

Domperidone-induced tardive dyskinesia and withdrawal psychosis in an elderly woman with dementia.

OBJECTIVE: To report a case of probable domperidone-induced tardive dyskinesia and withdrawal psychosis in an elderly woman. CASE SUMMARY: A 75-year-old woman was first assessed for cognitive decline and personality changes. On clinical examination, diffuse choreoathetoid movements were noted. Chronic domperidone use seemed the most likely cause for the movement disorder and was abruptly discontinued. Within a few days, she developed a severe psychotic syndrome with Capgras delusions; the movement disorder continued unabated. Both the movement disorder and psychotic symptoms promptly remitted

Facial processing in schizophrenia and delusional misidentification: cognitive neuropsychiatric approaches.

The human face is of great importance as a stimulus in view of the detailed information it contains--information vital to understand in order to survive in the complex social world. There are different aspects to facial processing, including: affect analysis, recognition of individuality and recognition of familiarity, all of which have been incorporated into the cognitive model of Bruce and Young (1986). The cognitive neuropsychiatric approach has recently been employed in the study of facial processing in schizophrenia. It has become apparent that schizophrenic subjects are impaired in facia

The behavioral presentation of an individual with a disordered sense of self.

There is increased recognition that the brain processes a "sense of self" (best understood as a sense of "mineness") in the right hemisphere association area and which can be distorted due to neurologic injury. Although there are numerous descriptions of types of disorders of the self (e.g., asomatognosia, anosognosia, mirror misidentification disorder, Capgras syndrome, schizophrenia), there are few descriptions of the subjective experiences of individuals with traumatic brain injury (TBI) who experience a reduced sense of self. This article presents a longitudinal case study of a woman in he

Neuroanatomical correlates of clinical misidentification and misperception in senile dementia of the Alzheimer type.

BACKGROUND: Impaired frontal lobe and/or nondominant hemisphere lesions have been described in patients with typical misidentification syndromes (e.g., Capgras' syndrome or reduplicative paramnesia) who had various underlying psychiatric or neurologic disorders. In a prospective long-term study, the authors examined computed tomography measurements of patients with senile dementia of the Alzheimer type (DAT) to determine whether the forms of misidentification they suffered might correlate with organic factors. METHOD: One hundred twenty-eight consecutive DAT patients were examined by means of

An unusual delusion of duplication in a patient affected by Dementia with Lewy bodies.

BACKGROUND: Dementia with Lewy bodies (DLB) is the second most frequent diagnosis of progressive degenerative dementia in older people. Delusions are common features in DLB and, among them, Capgras syndrome represents the most frequent disturbance, characterized by the recurrent and transient belief that a familiar person, often a close family member or caregiver, has been replaced by an identical-looking imposter. However, other delusional conditions near to misidentification syndromes can occur in DLB patients and may represent a major psychiatric disorder, although rarely studied systematic

Two Cases of Late-Onset Anti-NMDAr Auto-Immune Encephalitis After Herpes Simplex Virus 1 Encephalitis.

Context: Encephalitis due to herpes simplex virus 1 (HSV-1) was described as a potential trigger for the development of anti-N-methyl-D-aspartate receptor (NMDAr) auto-immune encephalitis (AIE) within a few days to a few weeks after the infection. Methods: We assessed clinical, radiological, and biological diagnoses process, treatment response, and evolution.
