[Shifts in the diameter of continuous circular capsulorhexis based on the Gullstrand eye model with the Callisto Eye System during cataract surgery].

Objective: To investigate the disparity between the set value of the capsulorhexis ring diameter in ophthalmic surgical navigation systems and the actual capsulorhexis ring diameter projected onto the lens surface. Methods: It was a cross-sectional study. Based on the Gullstrand eye model, the theoretical diameter of the capsulorhexis ring projected onto the lens plane through the cornea was calculated using the law of refraction in the Callisto Eye System. When the anterior chamber depth (ACD) ranged from 2.0 to 4.0 mm and the corneal curvature ranged from 40.00 to 48.00 D, the capsulorhexis

Twenty-nine-gauge dual-chandelier retroillumination for the non-open-sky continuous curvilinear capsulorhexis in the penetrating keratoplasty triple procedure.

We describe a technique for the penetrating keratoplasty (PKP) triple procedure that uses 29-gauge dual-chandelier illumination during creation of a non-open-sky continuous curvilinear capsulorhexis (CCC). The chandeliers are inserted through the pars plana into the vitreous cavity through the bulbar conjunctiva at the 3 o'clock and 9 o'clock positions. We compared this approach with that of a core vitrectomy, in which a single 25-gauge port is inserted into the vitreous cavity transconjunctivally through the upper temporal pars plana. The area of halation around the corneal opacity was signif

Color-Contrast Adjustment With a 3D Head-Up System to Aid Capsulorhexis After ICG Staining: Single-Patient Case Report From a Tertiary Center.

We report a single case in which continuous curvilinear capsulorhexis (CCC) was safely completed using NGENUITY in 3D head-up surgery (HUS) during cataract surgery. Visibility of the dye-stained anterior capsule was improved during the CCC procedure. An 84-year-old female underwent a planned extracapsular extraction on December 2022, in HUS using NGENUITY for a Grade 4 cataract classified by the Emery-Little classification. After staining the anterior capsule with 0.5% indocyanine green (ICG) for 5 s, the anterior chamber was washed to reduce the risk of endothelial damage. Then, CCC was perfo

Posterior capsulorhexis combined with optic buttonholing: an alternative to standard in-the-bag implantation of sharp-edged intraocular lenses? A critical analysis of 1000 consecutive cases.

BACKGROUND: Current after-cataract prevention relies on optimizing the natural barrier effect of the optic rim against lens epithelial cell (LEC) migration. However, deficiencies in circumferential capsular bag closure caused by the intraocular lens (IOL) haptic or delayed secondary re-division of the fused capsules by Soemmering s ring formation lead to primary or secondary barrier failure. Consequently, surprisingly high posterior laser capsulotomy rates have been reported long-term, even with optimal capsular surgery and the most widespread hydrophobic acrylic IOLs, considered to be the mos

Clear lens phacoemulsification with continuous curvilinear capsulorhexis and foldable intraocular lens implantation for the treatment of a patient with bilateral anterior lenticonus due to Alport syndrome.

The case of a 29-year-old man is reported who presented with a gradually progressive, painless decrease in vision over 10 years. Anterior segment examination with a slit lamp revealed anterior lenticonus in both eyes. The patient had previously been diagnosed with bilateral sensorineural deafness, however investigations revealed microscopic haematuria and renal insufficiency that subsequently led to a diagnosis of classical Alport syndrome. Since the patient's quality of vision was severely affected by the bulging anterior lens capsule, surgical treatment was required. Clear lens phacoemulsifi

Digitalization of surgical features improves surgical accuracy via surgeon guidance and robotization.

The evolution of surgical techniques aims to augment surgeons' capabilities through digital guidance and robotization for higher precision and consistency. Currently, surgeries heavily rely on surgeon's experience and visual judgment, causing operation variations. Artificial intelligence (AI) offers a solution by extracting and digitizing surgical trajectories and features from videos to provide digital guidance. In this study, we collected 17,538 videos of capsulorhexis, a crucial step in cataract surgery, to create an AI-driven system named Meta Surgery (MetaS). MetaS evaluates and identifie
