Long-term captopril therapy: evolving hemodynamic effects.

Nine patients with resistant hypertension received captopril for 12 months. Five received captopril alone, four required additional therapy. In the former, mean blood pressure fell from 109 +/- 4.2 mm Hg to 84 +/- 7.5 mm Hg (P less than 0.025) after seven days. A rise to 101 +/- 19 mm Hg was noted at six and 12 months. Total peripheral resistance fell at seven days but returned to levels above control at six and 12 months. Cardiac index was 3.21 +/- 0.55 liters/min/m2 before treatment, 3.27 +/- 0.56 liters/min/m2 at seven days, and 2.17 +/- 4.0 liters/min/m2 (P less than 0.025) at 12 months. H

[Captopril in congestive heart failure (author's transl)].

Captopril (Lopirin, Squibb, von Heyden) is an inhibitor of the angiotensin-I converting enzyme. In congestive heart failure angiotensin mediated vasoconstriction and aldosterone secretion is reversed by captopril. Both effects of captopril induce reduction of cardiac preload and afterload. The clinical state of patients with congestive heart failure therefore improves since cardiac output increased and pulmonary pressures decrease. Captopril acts orally and the dosage used for the treatment of congestive heart failure ranges from 50 to 150 mg daily. After oral ingestion of a single dose the ma

European Captopril Radionuclide Test Multicenter Study. Preliminary results. Inspective renographic analysis. The European Captopril Radionuclide Test Multicenter Study Group.

Captopril radionuclide test (CRT) has been introduced in clinical practice as a screening test for renovascular hypertension, since it allows the detection of the decrease of glomerular filtration rate that may be induced by angiotensin converting enzyme inhibition (25 mg oral captopril) in kidneys ipsilateral to a renal artery stenosis. However, due to the low prevalence of the disease, experiences in single centers may hardly satisfy the need for representative series of patients to validate the test. Nineteen centers participated in a collaborative study (CRT European Multicenter Study) tha

Topical captopril: a promising treatment for secondary lymphedema.

Transforming growth factor-beta 1 (TGF-β1)-mediated tissue fibrosis is an important regulator of lymphatic dysfunction in secondary lymphedema. However, TGF-β1 targeting can cause toxicity and autoimmune complications, limiting clinical utility. Angiotensin II (Ang II) modulates intracellular TGF-β1 signaling, and inhibition of Ang II production using angiotensin-converting enzyme (ACE) inhibitors, such as captopril, has antifibrotic efficacy in some pathological settings. Therefore, we analyzed the expression of ACE and Ang II in clinical lymphedema biopsy specimens from patients with unilate

The effect of captopril on endometriotic implants in a rat model.

OBJECTIVE: To determine the effects of captopril on experimentally induced endometriosis in a rat model. STUDY DESIGN: Twenty-four adult, mature female Wistar-Albino rats in which endometriotic implants were induced by transplanting autologous uterine tissue to ectopic sites on the peritoneum. After the endometriotic implants were formed surgically, the 24 rats were randomly divided into three groups. Group 1 (captopril group, eight rats) were given 50 mg kg(-1)d(-1) of oral captopril for 21 d. Group 2 (leuprolide acetate group, eight rats) were given a single 1 mg kg(-1) subcutaneous injecti

Antithrombotic effect of captopril and enalapril in old rats.

In the present study, we have shown considerably accelerated thrombosis in old rats in comparison with adult rats, which may be related to the impaired hemostatic balance in these animals. In old rats, captopril and enalapril caused a marked reduction of venous thrombus weight. The mechanism of antithrombotic action of these drugs seems to be dependent on the suppression of coagulation cascade and the enhancement of the fibrinolytic processes.

Antithrombotic effect of captopril and enalapril in young rats.

Angiotensin converting enzyme inhibitors (ACE-Is) are the main drugs used in the treatment of essential hypertension and congestive heart failure in adults. Recently, we have demonstrated the antithrombotic effect of captopril (CAP) and enalapril (ENA) in venous thrombosis model in adult rats. One might also suggest the beneficial effect of those drugs on hemostasis in young individuals. Two months old male Wistar rats were used in the study. Acute administration of CAP at a dose of 50 and 100 mg kg(-1) significantly reduced the venous thrombus weight. Dose-dependent reduction in the thrombus

Bartter's syndrome and captopril scintigraphy: a case report.

We report a case of a woman who came to our attention because of hypokalemia, hyperreninemia and hyperaldosteronemia but with normal blood pressure. Under suspicion of a normotensive renal artery stenosis captopril and baseline scintigraphies were performed. Captopril scintigraphy demonstrated a bilateral progressive retention of radiopharmaceutical without significant excretion. The baseline study revealed a complete normalization of the scintigraphyc picture. A Magnetic Resonance Angiography (Angio-MRI) performed to evaluate renal arteries gave completely normal results. On the basis of the

Indapamide is as effective as captopril in the control of microalbuminuria in diabetes.

Microalbuminuria is a predictor of overt diabetic nephropathy and macrovascular disease. Thirty-one diabetic patients with persistent urinary albumin excretion rate (AER) of 20-200 mu g min-1 were randomised to receive indapamide 2.5 mg or captopril 37.5 mg daily for 12 weeks. After a 4-week washout, patients received the alternate agent for 12 weeks. Resting blood pressure (BP), AER, cholesterol, triglycerides, and HbA1c were measured at baseline, after 6 and 12 weeks of each treatment, and after a 4-week washout period following each treatment arm. Results from patients who completed at leas

Interpretation of captopril renography by nuclear medicine physicians.

UNLABELLED: This study was designed to assess intra- and interobserver variability and diagnostic accuracy of nuclear medicine physicians in their evaluation of baseline and captopril renograms. METHODS: The diagnostic performance of three experienced nuclear medicine physicians according to their interpretation of baseline, captopril and paired renograms was assessed. To this end, the readers evaluated the renograms of 28 hypertensive patients in whom a diagnosis of renovascular hypertension was suspected on the basis of clinical clues. All patients also underwent angiography. The readers we

Ramipril vs captopril in mild to moderate hypertension.

Ramipril 5 mg once daily was compared to Captopril 50 mg twice daily in a randomised, double-blind, parallel group study in 60 patients with a diastolic blood pressure between 95 to 120 mmHg over a period of 2 months. Both drugs in the dose regimen used in this study exerted a similar anti-hypertensive effect at the end of 2 months of treatment resulting in a fall of supine diastolic blood pressure with Ramipril = 19.27 +/- 3.34 mmHg and Captopril = 19.15 +/- 2.63, in patients receiving the drugs without the diuretic. Provocative captopril testing in the diagnosis of renovascular hypertension.

Firm recommendations concerning the testing procedures, radionuclides of choice, and diagnostic criteria for RVHT are somewhat difficult, given the lack of rigorous standardization of these complex protocols. Nevertheless, review of the existing data indicates that post-captopril renography alone appears to be a safe and effective means of noninvasively screening hypertensive patients for renovascular disease. A normal study after captopril suggests with a reasonable degree of certainty that potentially reversible RVHT is unlikely. A positive study in an appropriately screened hypertensive pat

Effects of lisinopril and captopril on calcium in rat heart.

We studied the effects of lisinopril (Lis) and captopril (Cap), two angiotensin-converting enzyme inhibitors, on calcium in ischemia/reperfusion and normal rat hearts. Ischemia/reperfusion hearts were subjected to 15 min ischemia followed by 1 or 30 min reperfusion. Lis 0.1 mumol.L-1 and Cap 200 mumol.L-1 decreased the concentration of calcium in ischemia/reperfusion hearts (the content of calcium in reperfusion 1 min heart were reduced from 4.0 +/- 0.6 to 2.7 +/- 0.5 and 3.0 +/- 0.9 mumol/g dry wt respectively). In cultured cell of neonatal rat heart, both drugs inhibited the uptake of 45Ca2+

[Renal scintigraphy with captopril in the diagnosis of renovascular hypertension].

Altogether 16 patients with arterial hypertension (AH) were examined. Of these, 11 were with stenosis of renal arteries, 4 had essential hypertension (EH) and I with nephrogenous parenchymatous AH. To estimate the influence of captopril on the total and separate renal function in patients with and without stenosis of renal arteries and the possibility of the use of the given pharmacological test in the diagnosis of renovascular hypertension (RVH), all the patients underwent renal scintigraphy with 99Tc-DTPA on admission to the hospital and 1-7 days after a single intake of 25 mg captopril per

Screening for renovascular disease with captopril-enhanced renography.

Changes in renal function caused by angiotensin-converting-enzyme (ACE) inhibitors can be detected on 99mTc-DTPA renography so that DTPA scanning before and after a single dose of captopril can be used to screen for renovascular disease. We have performed captopril-DTPA scans with renal arteriography on 104 patients, of whom 27 had renal artery stenosis, all due to atheroma. Using a 5% fall in divided function or a delay of greater than 15 min in time to peak activity on one side after captopril, or the finding of greater than 90% divided function on one side before captopril as criteria for a

Limitations of the captopril test in the diagnosis of renovascular hypertension.

The captopril test has been suggested as a sensitive and specific noninvasive tool for the diagnosis of renovascular hypertension. We applied the test to 72 consecutive hypertensive patients in whom renal artery stenosis was suspected. The test was positive in seven cases (9.7%), of whom four were true positive and three false positive. The captopril test has value in screening for renovascular hypertension, but its limitations should be recognized. False positive results are not uncommon and false negative results have been reported under certain circumstances. Nevertheless, when properly app
