Risk Stratification of Genetic, Dilated Cardiomyopathies Associated With Neuromuscular Disorders: Role of Cardiac Imaging.

The etiology of dilated cardiomyopathy (DCM) can be grouped as either genetic or nongenetic. More than 50 pathogenic genes have been described, with sarcomeric and lamin A/C mutations being the most common. Mutation carriers for genetic DCM are often asymptomatic until cardiac disease manifests with heart failure, arrhythmias, or sudden cardiac death. Preventive strategies are promising but can only be applied and tested adequately if genetic DCM can be diagnosed at an early stage. Early diagnosis of mutation carriers that may develop overt DCM requires advanced imaging techniques that can det

The role of magnetic resonance imaging in fetal congenital heart disease.

Fetal echocardiography is the preferred imaging method for prenatal diagnosis of congenital cardiovascular anomalies. With the development of rapid magnetic resonance imaging (MRI) and electrocardiographic gating techniques, fetal cardiac MRI has become an adjunct to ultrasound, which can not only detect congenital cardiovascular anomalies, but also quantify fetal cardiovascular structures, cardiac function, and hemodynamics. This article summarizes fetal cardiac MRI sequences, describes normal imaging features, and presents a systematic approach to looking for MRI appearances of various conge

Reverse cardiac remodelling and dysfunction in A97S transthyretin cardiac amyloidosis after tafamidis treatment.

Transthyretin cardiomyopathy (ATTR-CM) is an under-recognized cause of heart failure, but it has received increasing attention due to the availability of treatment options. We present a case of hereditary transthyretin cardiomyopathy (A97S, an under-represented variant in current clinical studies) who presented with heart failure. Timely diagnosis and intervention with tafamidis demonstrated reversed cardiac remodelling via multiple imaging techniques (echocardiography, cardiac magnetic resonance imaging and technetium-99m pyrophosphate scintigraphy). The echocardiography and cardiac magnetic

Multimodality Noninvasive Imaging in the Monitoring of Pediatric Heart Transplantation.

Orthotopic heart transplantation is a well-established and effective therapeutic option for children with end-stage heart failure. Multiple modalities, including noninvasive cardiac imaging, cardiac catheterization, angiography, and endomyocardial biopsy, are helpful to monitor these patients for graft dysfunction, rejection, and vasculopathy. Because of morbidities associated with invasive monitoring, noninvasive imaging plays a key role in the surveillance and evaluation of symptoms in pediatric transplant recipients. Echocardiography with or without stress augmentation may provide serial da

The role of cardiac magnetic resonance in diagnosis of cardiac sarcoidosis.

Sarcoidosis is a systemic granulomatous disease with a high prevalence of cardiac involvement in autopsic studies. Cardiac sarcoidosis is associated with increased cardiovascular morbidity and mortality. Endomyocardial biopsy is a specific technique, but unfortunately not sensitive enough. Non-invasive cardiac imaging has an important role in the evaluation of patients with suspected or confirmed cardiac sarcoidosis. Echocardiography remains the first choice imaging technique because of its availability and low cost. However, this method could not provide tissue characterization or evaluation

Comparison of Maximal Wall Thickness in Hypertrophic Cardiomyopathy Differs Between Magnetic Resonance Imaging and Transthoracic Echocardiography.

Left ventricular (LV) wall thickness is a prognostic marker in hypertrophic cardiomyopathy (HC). LV wall thickness ≥30 mm (massive hypertrophy) is independently associated with sudden cardiac death. Presence of massive hypertrophy is used to guide decision making for cardiac defibrillator implantation. We sought to determine whether measurements of maximal LV wall thickness differ between cardiac magnetic resonance imaging (MRI) and transthoracic echocardiography (TTE). Consecutive patients were studied who had HC without previous septal ablation or myectomy and underwent both cardiac MRI and

Imaging of common coronary artery anomalies and imaging features important for clinical decision-making.

Anomalous coronary arteries are associated with sudden cardiac death, although only in a relatively small percentage. Although this has been a field of study for years and there are many ongoing studies of larger patient cohorts and longer term follow-up, important questions remain at present in clinical decision-making, particularly whether or not to have surgery. Advanced imaging techniques including coronary computed tomography (CT) and cardiac magnetic resonance (MR) play an important role in the workup of patients with known or suspected anomalies including delineating origin and course a

Indications for coronary and cardiac computed tomographic angiography.

Cardiovascular disease remains the leading cause of death and disability in the developed world. Over the last several years, many noninvasive imaging techniques have been used to enhance the diagnosis of cardiac disease. One of these advances, multidetector computed tomographic angiography (CTA), has transformed computed tomography from a single-slice transaxial imaging modality to a true 3-dimensional imaging technique with the ability to acquire submillimeter volumetric data. These benefits enable novel applications of CTA for the assessment of cardiac and coronary anatomy that were not pre

Review of Multimodal Imaging in Cardiac Sarcoidosis.

Sarcoidosis is a multisystemic disease that causes organ dysfunction through the formation of non-caseating granulomas. Cardiac sarcoidosis (CS) is a phenotype associated with a poor prognosis and benefits from timely diagnosis and management. A nonspecific clinical presentation and the lack of a reliable diagnostic gold standard make obtaining a definitive diagnosis challenging. Advanced cardiac imaging techniques, such as cardiac magnetic resonance imaging (CMR) and 18F-labeled fluorodeoxyglucose (18F-FDG)-positron emission tomography (PET), play an essential role in CS assessment and have b

Recent Advances in Cardiovascular Imaging Relevant to the Management of Patients with Suspected Cardiac Amyloidosis.

Cardiac amyloidosis is a form of infiltrative cardiomyopathy typically presenting with progressive heart failure. The clinical presentation and morphological findings often overlap with other cardiovascular diseases, and frequently results in misdiagnosis and consequent under-reporting. Cardiovascular imaging is playing an increasingly important diagnostic and prognostic role in this referral population, and is reducing the reliance on endomyocardial biopsy as a confirmatory testing. Advancements across multiple cardiac imaging modalities, including echocardiography, magnetic resonance imaging

Novel Insights into Non-Invasive Diagnostic Techniques for Cardiac Amyloidosis: A Critical Review.

Cardiac amyloidosis (CA) is a cardiac storage disease caused by the progressive extracellular deposition of misfolded proteins in the myocardium. Despite the increasing interest in this pathology, it remains an underdiagnosed condition. Non-invasive diagnostic techniques play a central role in the suspicion and detection of CA, also thanks to the continuous scientific and technological advances in these tools. The 12-lead electrocardiography is an inexpensive and reproducible test with a diagnostic accuracy that, in some cases, exceeds that of imaging techniques, as recent studies have shown.

Optimizing Object Detection Algorithms for Congenital Heart Diseases in Echocardiography: Exploring Bounding Box Sizes and Data Augmentation Techniques.

BACKGROUND: Congenital heart diseases (CHDs), particularly atrial and ventricular septal defects, pose significant health risks and common challenges in detection via echocardiography. Doctors often employ the cardiac structural information during the diagnostic process. However, prior CHD research has not determined the influence of including cardiac structural information during the labeling process and the application of data augmentation techniques. METHODS: This study utilizes advanced artificial intelligence (AI)-driven object detection frameworks, specifically You Look Only Once (YOLO)

Predicting adverse outcomes after cardiac surgery and structural interventions: the role of right ventricular function.

Right ventricular (RV) function is a major determinant of clinical outcomes in patients undergoing cardiac surgery and transcatheter interventions. Although left ventricular morphology and function have been the traditional focus of preprocedural assessment, RV dysfunction is now recognized as an important predictor of morbidity and mortality. Therefore, inclusion of RV-related parameters in preprocedural risk assessment is on the rise. This review summarizes current evidence on the role of RV function in various cardiac diseases that require surgery or interventions. Conventional two-dimensio
