Changes in short-term variation of antenatal cardiotocography to identify intraamniotic infection: a historical cohort study.

INTRODUCTION: Intraamniotic infection (IAI) is one of the main possible complications of preterm prelabor rupture of membranes (PPROM) and can lead to severe consequences for the neonate, such as early onset neonatal sepsis (EONS). Available diagnostic tools for IAI have poor diagnostic performance, which may result in both over- and underdiagnoses of IAI. In a search for better diagnostic tools, we have examined short-term variation (STV) in fetal heart rate. We have previously shown that in IAI exposed pregnancies, the STV was more than 20% lower in the last cardiotocography trace before the

Cardiotocography versus intermittent auscultation of fetal heart on admission to labour ward for assessment of fetal wellbeing.

BACKGROUND: The admission cardiotocograph (CTG) is a commonly used screening test consisting of a short (usually 20 minutes) recording of the fetal heart rate (FHR) and uterine activity performed on the mother's admission to the labour ward. This is an update of a review published in 2012. OBJECTIVES: To compare the effects of admission cardiotocography with intermittent auscultation of the FHR on maternal and infant outcomes for pregnant women without risk factors on their admission to the labour ward. SEARCH METHODS: We searched the Cochrane Pregnancy and Childbirth Group's Trials Register

Continuous cardiotocography (CTG) as a form of electronic fetal monitoring (EFM) for fetal assessment during labour.

BACKGROUND: Cardiotocography (CTG) records changes in the fetal heart rate and their temporal relationship to uterine contractions. The aim is to identify babies who may be short of oxygen (hypoxic) to guide additional assessments of fetal wellbeing, or determine if the baby needs to be delivered by caesarean section or instrumental vaginal birth. This is an update of a review previously published in 2013, 2006 and 2001. OBJECTIVES: To evaluate the effectiveness and safety of continuous cardiotocography when used as a method to monitor fetal wellbeing during labour. SEARCH METHODS: We search

Admission Cardiotocography in Term Pregnancies for Predicting Intrapartum Interventions and Early Neonatal Outcomes: A Systematic Review.

Cardiotocography (CTG) is widely applied for intrapartum fetal surveillance to identify early signs of fetal compromise and guide timely intervention. Despite extensive clinical use, uncertainty persists regarding its effectiveness in improving neonatal outcomes, particularly when used routinely at admission in low-risk term pregnancies. This systematic review addresses inconsistencies in existing evidence concerning the predictive value and clinical utility of admission CTG. The objective was to evaluate its association with intrapartum interventions and early neonatal outcomes. A systematic

Correlation of suspicious and pathological cardiotocography with umbilical cord blood gas parameters: A prospective cohort study.

INTRODUCTION: Fetal blood sampling is considered the best option in overcoming the increased false positivity of cardiotocography (CTG) but is not used in many developing countries due to logistics difficulties. Hence, obstetricians highly rely on CTG for diagnosing fetal hypoxia. Studies of suspicious and pathological CTG correlating with umbilical blood parameters are limited. Lactate can be a reliable marker to assess intrapartum hypoxia. However, the correlation of suspicious and pathological CTG with lactate is least studied. MATERIAL AND METHODS: A prospective observational cohort study

Cardiotocography-Based Experimental Comparison of Artificial Intelligence and Human Judgment in Assessing Fetal Asphyxia During Delivery.

Cardiotocography (CTG) has long been the standard method for monitoring fetal status during delivery. Despite its widespread use, human error and variability in CTG interpretation contribute to adverse neonatal outcomes, with over 70% of stillbirths, neonatal deaths, and brain injuries potentially avoidable through accurate analysis. Recent advancements in artificial intelligence (AI) offer opportunities to address these challenges by complementing human judgment. This study experimentally compared the diagnostic accuracy of AI and human specialists in predicting fetal asphyxia using CTG data.

Early Diagnosis and Classification of Fetal Health Status from a Fetal Cardiotocography Dataset Using Ensemble Learning.

(1) Background: According to the World Health Organization (WHO), 6.3 million intrauterine fetal deaths occur every year. The most common method of diagnosing perinatal death and taking early precautions for maternal and fetal health is a nonstress test (NST). Data on the fetal heart rate and uterus contractions from an NST device are interpreted based on a trace printer's output, allowing for a diagnosis of fetal health to be made by an expert. (2) Methods: in this study, a predictive method based on ensemble learning is proposed for the classification of fetal health (normal, suspicious, pat

Computerized fetal cardiotocography analysis in early preterm fetal growth restriction - a quantitative comparison of two applications.

Background We developed an open-source software for the computerized analysis of antenatal fetal cardiotocography (CTG) without limitation of duration of the registration, enabling batch processing and adaptation to any digital storage system. Methods STVcalc was developed based on literature about the FetalCare system (Huntleigh Healthcare Ltd, Cardiff, UK). For comparison with FetalCare, we selected the CTGs of all women who delivered in 2011 a small-for-gestational-age (SGA) fetus between 24 and 31 weeks by cesarean section (CS) for fetal distress, or had fetal death, before labor onset. Cardiotocography patterns are stronger predictors of neonatal acidemia than decision-to-delivery interval in emergency cesarean deliveries.

OBJECTIVES: The aim of the study was to evaluate whether non-reassuring cardiotocography (CTG) patterns or the decision-to-delivery interval is a stronger predictor of neonatal outcomes, including umbilical arterial pH and Apgar scores, in infants delivered via emergency cesarean section for suspected fetal distress. MATERIAL AND METHODS: In this retrospective cohort study, the medical records of patients who underwent emergency cesarean delivery for suspected fetal distress were reviewed. Patients were categorized according to the type of non-reassuring CTG pattern, and the association of th

Optimizing fetal health prediction: Ensemble modeling with fusion of feature selection and extraction techniques for cardiotocography data.

Cardiotocography (CTG) captured the fetal heart rate and the timing of uterine contractions. Throughout pregnancy, CTG intelligent categorization is crucial for monitoring fetal health and preserving proper fetal growth and development. Since CTG provides information on the fetal heartbeat and uterus contractions, which helps determine if the fetus is pathologic or not, obstetricians frequently use it to evaluate a child's physical health during pregnancy. In the past, obstetricians have artificially analyzed CTG data, which is time-consuming and inaccurate. So, developing a fetal health categ

The present and future of intrapartum computerized cardiotocography: role of pattern recognition incorporating single vs. multiple parameters.

Computer assisted cardiotocography holds a great promise in minimizing human errors thereby improving perinatal outcome. Despite exponential growth (Moore's law) in computing power for decades, this promise remains unrealized. The systematic analyses of studies on computerized cardiotocography offer little guide to future. This analytical review presents a more qualitative discussion of available evidence as well as concepts regarding the development and acceptance of computerized cardiotocography. To begin with, a workable approach would be for computer algorithms to follow the most scientifi
