The intraurethral catheter: long-term follow-up in patients with urinary retention due to infravesical obstruction.

Insertion of 43 intraurethral catheters (IUC) was performed on 31 patients in order to replace an indwelling catheter for urinary retention due to benign prostatic hyperplasia. The insertion of the IUC was done under local anaesthesia with lignocaine jelly in the out-patient clinic. The device has been in place for 3 to 14 months (mean 7), in 14 cases for greater than 3 months and in 29 cases for greater than 6 months. In 7 of the 43 inserted IUCs the device had to be removed earlier than intended (after 4.2-8 months) for reasons related to the device. Of the remaining 36 devices which were fu

Alcohol sclerosis for polycystic liver disease and obstructive jaundice: use of a nasobiliary catheter.

We report the use of a nasobiliary catheter in the management of a 55-yr-old female with autosomal dominant polycystic kidney disease who developed obstructive jaundice from a hepatic cyst. The patient presented with a 2-wk history of fatigue, jaundice, nausea, vomiting, and abdominal pain. Physical examination was remarkable for tender hepatomegaly. Computerized tomography revealed multiple hepatic cysts and dilated intrahepatic biliary radicles. Endoscopic stent placement failed to relieve the obstruction. Computerized tomography guided percutaneous aspiration of the obstructing hepatic cyst

Potential pitfalls in the obstructive renal scan in patients with double-pigtail ureteral catheters.

In 21 patients with a long-standing internal ureteral double-pigtail catheter we demonstrated the adverse effect of a full bladder on upper system drainage. A pseudo-obstructive pattern noted in these patients on routine scans was immediately rectified by mere bladder emptying and a normal pattern was demonstrated. We suggest the bladder be emptied in all patients with indwelling internal ureteral stents before renographic evaluation.

Combined endovascular and surgical recanalization after central venous catheter-related obstructions.

Central venous occlusion in children is a challenging problem that can occur after a central venous catheter insertion. Long-term catheter-related complications include sepsis and venous thrombosis with consequent loss of central access. We describe 2 cases of children younger than 1 year who were dependent on a central venous catheter for total parenteral nutrition. They developed a chronic extensive obstruction of the right and left brachiocephalic veins with a superior vena cava syndrome. The patients' survival was dependent on the restoration of central venous access until the planned inte

[Preliminary application of multiple silk thread ligation and traction technique in endoscopic catheter placement for intestinal obstruction].

Objective: To investigate the efficacy and safety of the multiple silk ligation and traction technique in endoscopic placement of intestinal obstruction catheters. Methods: Silk ligations were performed between the silicone beads of the leading tip, behind the anterior balloon, behind the posterior balloon, and 20 cm distal to the posterior balloon of the catheter. The silk threads were grasped with biopsy forceps through the endoscopic channel, and the catheter was gradually tractioned to the distal descending duodenum. Subsequently, sterile water was injected into the anterior balloon, the g

Closed-Loop Bowel Obstruction Induced by Ventriculoperitoneal Shunt Catheter Coiling at the Sigmoid Colon: A Case Report.

Intestinal obstruction is a rarely encountered complication in patients with ventriculoperitoneal (VP) shunt. The most common causes of bowel obstruction in this subset of patients include volvulus, formation of a spontaneous knot, and adhesions. Herein, we report a 21-year-old bedridden male with a history of congenital hydrocephalus on VP shunt, spina bifida, neurogenic bladder, and paraplegia who presented with a seven-day history of abdominal discomfort, distention, constipation, vomiting, and intolerance to oral intake. Abdominal x-ray showed dilated bowel loops. Computed tomography (CT)

Placement of a permanent cystostomy catheter to relieve urine outflow obstruction in dogs with transitional cell carcinoma.

Permanent cystostomy catheters were placed in 7 dogs to relieve urine outflow obstruction from presumed transitional cell carcinoma of the bladder trigone and urethra. The catheters were easily managed at home by the owners. Complications were minimal. The most frequent complication was urinary tract infection. Two owners complained of difficulty in draining the bladder 1 week before euthanasia of their dogs. The primary reason for euthanasia of all dogs was progression of the tumor. Survival times of these dogs were similar to those previously reported for dogs with transitional cell carcinom

Stent-based and catheter-based endobiliary radiofrequency ablation for malignant biliary obstruction: an ex-vivo comparative study.

Endobiliary radiofrequency ablation (RFA) is used as an adjunct therapy to improve local tumor control and stent patency; however, catheter-based RFA may result in non-uniform ablation in curved or tortuous ducts because of incomplete electrode-wall contact. This study compared stent-based electrodes with conventional catheter-based electrodes using a curved endoluminal phantom and ex-vivo porcine liver tissue. In the curved phantom, the catheter-based electrode showed curvature-dependent asymmetry, with higher macroscopic ablation grades at the lesser curvature than at the greater curvature (

Percutaneous thrombolysis via cholecystostomy catheter to dissolve biliary clots causing obstructive jaundice.

Haemobilia, or bleeding within the biliary tree, is rare. It can cause biliary obstruction secondary to blood clots. A comorbid 87-year-old was admitted to hospital with acute cholecystitis, choledocholithiasis, and an Escherichia coli bacteremia. He had a partial pancreatectomy and gastrojejunostomy 35 years prior for severe pancreatitis. He was treated with antibiotics and a percutaneous cholecystostomy. He developed atrial fibrillation and was subsequently commenced on warfarin. He re-presented 5 days after discharge with abdominal pain and fevers. Liver function tests revealed cholestasis

Atypical placement of hemodialysis catheters in patients with complete and irreversible obstruction of central venous vessels.

PURPOSE: This article describes the approach to atypical placement of central venous catheters (CVC) in dialysis patients with complete untreatable obstruction of central venous vessels. METHODS: Five patients with complete obstruction of central venous vessels underwent CT venography and digital venous angiography. After ultrasound-guided and radioscopic-assisted cannulation of the internal jugular vein, permanent CVCs were placed in atypical locations: in two patients a preliminary venous angioplasty was performed to facilitate the catheter positioning in a mediastinal enlarged collateral v

Safety and utility of the alpha-replacer for treatment of intraluminal obstruction of peritoneal catheters by fibrin clots.

BACKGROUND: The Moncrief-Popovich technique of peritoneal catheter implantation has beneficial effects for peritoneal dialysis (PD) initiation. However, it might increase the risk of peritoneal catheter obstruction by fibrin clots, because the catheter is buried under the skin for several weeks to months. Effects of treatment of intraluminal occlusion of PD catheters with tissue plasminogen activator, recommended by the International Society for Peritoneal Dialysis guidelines/recommendations are reportedly limited. We investigated the effectiveness of the 'alpha-replacer' (JMS, Tokyo, Japan) f

Shunt Testing In Vivo: Illustration of Partially Obstructed Ventricular Catheter by In-Growing Choroid Plexus.

Assessing shunt function in vivo presents a diagnostic challenge. Infusion studies can be a cost-effective and minimally invasive aid in the assessment of shunt function in vivo. We describe a case of a patient who after a foramen magnum decompression for type I Chiari malformation developed bilateral posterior fossa subdural hygromas and mild hydrocephalus, eventually necessitating insertion of a ventriculoperitoneal shunt. The patient returned with symptoms that were concerning for infection of the shunt. A bedside infusion study helped confirm that the ventricular catheter was partially obs

A novel approach in the use of radiofrequency catheter ablation of septal hypertrophy in hypertrophic obstructive cardiomyopathy.

OBJECTIVE: Alcohol septal ablation (ASA) is a therapeutic alternative to surgical myectomy in patients with hypertrophic obstructive cardiomyopathy (HOCM). However, the anatomical variability of the septal branch, risk of complete heart block, and late onset ventricular arrhythmias are limitations to its therapeutic usage. There is recent interest in the use of radiofrequency catheter ablation (RFCA) as a therapeutic option in HOCM. We aimed to assess the safety and efficacy of RFCA in the treatment of symptomatic HOCM. METHODS: Seven patients with symptomatic HOCM (mean age 43.7±15.6 years,

Worsening of Obstructive Sleep Apnea Associated with Catheter-Related Superior Vena Cava Syndrome.

There is growing evidence that fluid accumulation in the neck contributes to the pathogenesis of obstructive sleep apnea (OSA). We describe a case of catheter-related superior v ena cava (SVC) thrombosis revealed by rapid onset of typical symptoms of OSA. A marked improvement in OSA severity was observed after central venous catheter removal, anticoagulant therapy, and SVC angioplasty.
