[A case of pulmonary artery injury which might have been caused by a pulmonary artery catheter during the mitral valve replacement].

We experienced a pulmonary artery injury which might have been caused by a pulmonary artery catheter (PAC). A 66-year-old woman after mitral valve replacement, developped massive tracheal bleeding during weaning from the cardiopulmonary bypass. Transesophageal echocardiography revealed the air bell in the right pulmonary artery synchronized with ventilation. We speculated pulmonary artery injury and performed pulmonary artery angiography; however, it could not clarify the bleeding point. Surgical approach found the air leak from the erasure of the bronchus intermedius and the lobectomy led to

The appropriateness of the pulmonary artery catheter in cardiovascular surgery.

PURPOSE: The pulmonary artery catheter (PAC) is commonly used in anesthesiology and critical care, but its appropriate (where benefit exceeds risk) application is unknown. This study describes current clinical practice attitudes among anesthesiologists in cardiac and vascular surgery in an effort to determine the most appropriate indications for use of the PAC. METHODS: Anonymous, cross-sectional, mailed survey of anesthesiologists in Canada and the USA. Opinions of anesthesiologists about the appropriateness of PAC application were assessed in 36 clinical scenarios, using a nine-point Likert

Hemodynamic monitoring.

The goal of hemodynamic monitoring is to maintain adequate tissue perfusion. Classical hemodynamic monitoring is based on the invasive measurement of systemic, pulmonary arterial and venous pressures, and of cardiac output. Since organ blood flow cannot be directly measured in clinical practice, arterial blood pressure is used, despite limitations, as estimate of adequacy of tissue perfusion. A mean arterial pressure (MAP) of 70 mm Hg may be considered a reasonable target, associated with sign of adequate organ perfusion, in most patients. In the approach to hypotension, which is the most comm

Asymptomatic primary pulmonary hypertension associated with liver cirrhosis.

We report a case of asymptomatic primary pulmonary hypertension associated with liver cirrhosis (type B) and portal hypertension found by chance during a preoperative Swan-Gantz catheterization study. Our experience suggests that the actual prevalence of primary pulmonary hypertension associated with liver cirrhosis may be greater than that previously reported. During the follow-up of liver cirrhosis with portal hypertension, we should consider primary pulmonary hypertension, even if the patient is free of symptoms, and a chest X-ray check may be necessary.

Misplacement of a Swan-Ganz catheter after insertion through the left internal jugular vein.

We report a case of perforation of the left brachiocephalic vein after insertion through the left internal jugular vein. Entrapment of air in the balloon occurred, possibly due to high surrounding tissue pressure at its base, preventing deflation. We emphasize the potential hazards of catheterization via the left internal jugular vein.

Aberrant course of Swan-Ganz catheter revealing persistent left superior vena cava.

The presence of a persistent left superior vena cava (PLSVC) is the most common congenital disorder of thoracic venous anomaly. Failure of the left anterior cardinal vein to degenerate results in persistent left superior vena cava. It is usually discovered during central venous catheterization or surgical repair of congenital heart disorder. Although most patients with isolated PLSVC or double SVC are asymptomatic, there are some important clinical implications in certain situations, such as central venous catheterization or cardioplegia, which may result in serious complications. In this arti

Balloon catheter test in patients with atrial septal defect and patent ductus arteriosus.

Since 1973, the balloon catheter test with a Swan-Ganz, Dotter-Lukas or Fogarty catheter has been attempted in our department in cases of atrial septal defect (ASD) and patent ductus arteriosus (PDA) during cardiac catheterization and operation. Accurate informations concerning size, number and location of ASD were obtained during cardiac catheterization studies by using the balloon catheter. The difference between the size of the defect measured at catheterization and at operation was less than 1 mm in diameter. Size of the defect ranged from 13 to 44 mm in diameter (25.7 mm on the average) a

Right Heart Catheterization-Related Complications: A Review of the Literature and Best Practices.

Right heart catheterization (RHC) is considered to be the gold standard for the measurement of pulmonary artery pressure and has a class 1 indication to confirm the diagnosis of pulmonary arterial hypertension and to guide treatment decisions. Associated complications with RHC in contemporary practice are uncommon. In this review, we aim to summarize the complications that have been published in case reports or case series with updated management and techniques to prevent and mitigate complications. We searched the PubMed database for the following reports: "right heart catheterization," "Swan

Spring coil embolization of a patent ductus arteriosus in an adult.

To date, coil embolization has been reserved for occlusion of the small patent ductus arteriosus (PDA) because of potential dislodgement of the coils. We report a case of a larger, hemodynamically significant PDA in an adult which was successfully closed by two spring coils using a "crossed catheter" technique for coil delivery.

Nosocomial blood-borne infection secondary to intravascular devices.

A total of 143 patients with 159 episodes of intravascular device, blood-borne infection were studied. All infections were confirmed by the same organism being recovered from blood culture and by semiquantitative culture of the catheter tip. Sites of infection included the peripheral venous line (n = 72), central catheter (n = 49), arterial line (n = 18), subclavian dialysis catheter (n = 12), Swan-Ganz catheter (n = 4), Broviac catheter (n = 3), and transvenous pacemaker wires (n = 1). Staphylococcus aureus (n = 78) and Staphylococcus epidermidis (n = 33) predominated as pathogens. Excessive

[Intra- and postoperative complications in infrarenal abdominal aortic aneurysms].

UNLABELLED: Patients scheduled for operation of an abdominal aortic aneurysm are a challenge to the anesthesiologist due to multiple coexisting diseases and serious intraoperative hemodynamic changes caused by cross-clamping. The aim of this study was to investigate the incidence of intra- and postoperative complications and to analyze the coexisting diseases in order to estimate complications and risks. PATIENTS AND METHOD: The charts of 72 patients scheduled for resection of an abdominal aortic aneurysm in 1984 and 1985 were retrospectively analysed. The patients are divided into 6 groups:

Femoral catheters and deep venous thrombosis: a prospective evaluation with venous duplex sonography.

Femoral vein cannulation (FVC) with 8.5F Swan-Ganz catheter introducers allows expeditious intravenous access and rapid volume infusion; however, the incidence of associated iliofemoral deep venous thrombosis (DVT) is unknown. Trauma patients (n = 76) requiring unilateral FVC were prospectively entered into a study where they were resuscitated via FVC and serially evaluated with bilateral venous duplex sonography (VDS). The incidence of DVT in catheterized femoral veins was compared with that in uncatheterized femoral veins. Catheters were removed promptly and VDS was performed within 24 hours

Central venous cannulation done by house officers in the intensive care unit: a prospective study.

Central venous cannulation (CVC) is a procedure frequently performed by house staff in the intensive care units of teaching hospitals. In the medical ICUs of our two hospitals, CVC was successfully done by house officers in 172 cases requiring 231 attempts (one operator at one insertion site), for a success rate of 74%. There were 14 complications (6.1%), five requiring intervention, but none fatal. The overall success rate was higher for the internal jugular approach and lower for the external jugular approach than for other sites. The success rate for Swan-Ganz catheterization was higher for

Transvenous electrical cardioversion of equine atrial fibrillation: patient factors and clinical results in 72 treatment episodes.

BACKGROUND: Transvenous electrical cardioversion (TVEC) has been developed for treatment of atrial fibrillation (AF) in horses. The relationship among patient variables, treatment response, and outcome in a typical referral population has not been evaluated. HYPOTHESIS: Patient variables such as age, sex, weight, and duration of arrhythmia affect prognosis for response to treatment and the energy level at which cardioversion occurs. ANIMALS: TVEC was applied to 72 episodes of lone AF in 63 client-owned performance horses, with the majority (54) being Standardbred racehorses. METHODS: Cathet

Prospective Evaluation of a Multibeat Analysis Cardiac Index Estimation in Patients With Cardiogenic Shock.

OBJECTIVES: The decision algorithm for managing patients in cardiogenic shock depends on cardiac index (CI) estimates. Cardiac index estimation via thermodilution (CI-TD) using a pulmonary artery catheter is used commonly for obtaining CI in these patients. Minimally invasive methods of estimating CI, such as multibeat analysis (CI-MBA), may be an alternative in this population. DESIGN: A prospective, observational study. SETTING: Cardiac intensive care unit. PARTICIPANTS: Twenty-two subjects in cardiogenic shock provided 101 paired CI measurements. INTERVENTIONS: Measurements were obtaine

Aortic pulsatility index predicts clinical outcomes in heart failure: a sub-analysis of the ESCAPE trial.

AIMS: Aortic pulsatility index (API), calculated as (systolic-diastolic blood pressure)/pulmonary capillary wedge pressure (PCWP), is a novel haemodynamic measurement representing both cardiac filling pressures and contractility. We hypothesized that API would better predict clinical outcomes than traditional haemodynamic metrics of cardiac function. METHODS AND RESULTS: The Evaluation Study of Congestive Heart Failure and Pulmonary Artery Catheterization Effectiveness (ESCAPE) trial individual-level data were used. Routine haemodynamic measurements, including Fick cardiac index (CI), and the

Pulmonary artery flow catheters for directing management in pre-eclampsia.

BACKGROUND: Gestational hypertension and pre-eclampsia can cause fluid shifts. Pulmonary oedema and renal failure can result from these shifts. Fluid management is crucial in managing pre-eclampsia, especially in the context of pulmonary oedema and renal failure. Pulmonary artery catheterisation may be a method of effectively monitoring fluid status and thus aid in the management of renal failure and pulmonary oedema in the context of pre-eclampsia. OBJECTIVES: To assess the safety and efficacy of pulmonary flow catheters in women with severe pre-eclampsia in preventing and managing of renal

Pulmonary artery catheter insertion in a patient of dextrocardia with anomalous venous connections.

In a young adult patient having situs solitus with dextrocardia the attempted pulmonary artery catheter placement for emergency mitral valve replacement required an unduly long length (50cm) of catheter insertion to get into right ventricle and then into pulmonary artery. Although catheter coiling was suspected initially, chest x-ray taken after successfully placement revealed an uncommon congenital anomalous venous connection i.e. right internal jugular opening into left sided superior vena cava then into inferior vena cava after running all along the left border of the heart. With the result
