Decline in plasma retinol in unconjugated hyperbilirubinemia treated with bilirubin adsorption using an anion-exchange resin.

When an anion-exchange resin column (Plasorba BR-350) was used for the treatment of unconjugated hyperbilirubinemia, we found an unexpected decrease in plasma retinol (vitamin A) concentrations in a patient with type I Crigler-Najjar syndrome. The purpose of our study was to investigate the mechanism of this decrease in plasma retinol. When the patient's serum bilirubin exceeded the bilirubin binding capacity of 14.7 mumol bilirubin/g serum albumin (i.e., 720 mumol/L of bilirubin), abrupt deterioration of the patient's neurologic status (suppression in his gait and speech) occurred, so the nee

Ultrapure Water Production by a Saline Industrial Effluent Treatment.

A membrane system was applied for ultrapure water production from the treatment of saline effluent from the canned food industry. The industrial effluent presented a high saline concentration, including sodium chloride, calcium carbonate, calcium sulfates, and magnesium. The effluent was treated using a system of reverse osmosis (RO) and a post-treatment process consisting of ion exchange resins (IEXRs). The RO was accompanied by the addition of a hexametaphosphate dose (2, 6, and 10 mg/L) as an antiscalant to avoid the RO membrane scaling by minerals. In turn, IEXRs were used for water deioni

A closed-cycle and low-cost strategy for recovery and reuse of toxic lead from discarded perovskite photocatalysts.

Lead halide perovskites are promising photocatalysts, while high toxicity of lead results in challenging post-treatment. In this paper, an ion exchange-directional precipitation strategy is developed to recycle and reuse lead as PbBr2. CsPbBr3 perovskite-based catalysts are dissolved to produce Cs+ , Pb2+ and Br- ions, with Cs+ and Pb2+ extracted via multiple adsorption cycles using cation exchange resin. Gradient HNO3 solutions are used to elute the resin to maximize release of metal ions. The released Pb2+ is directionally precipitated as Pb(OH)2 through pH adjustment, followed by dissolving

3D Printed Microfluidic Chromatographic Column for Fast Downstream Processing Development.

3D printing is emerging as a promising fabrication technique for microfluidic devices. In this work, this technology was exploited in the development of a microfluidic chromatographic column with nominal volume of 54 µL. The microcolumn was packed with a cation exchange resin and characterized, using potassium iodide as a tracer, in terms of porosity (ε = 0.72), plate number, and asymmetry factor (0.8 < AS < 1.8 for flowrates >50 µL/min). To showcase the potential of this microdevice, it was exploited in the characterization of the chromatographic behavior of lysozyme. The measured saturation

Closed-loop recycling of lead iodide precursor from waste organic solvents in the production of perovskite solar cells: Toward sustainable photovoltaics.

Perovskite solar cells (PSCs), characterized by high efficiency, have demonstrated significant potential in solar energy harvesting. However, the PSCs production process usually generates substantial waste containing polar organic solvents, i.e., N,N-dimethylformamide (DMF) and dimethyl sulfoxide (DMSO), which are contaminated with high-concentration Pb(Ⅱ). This study introduces a closed-loop recycling approach to separate and recover Pb(Ⅱ) from waste organic solvents using a cation-exchange resin, enabling lead reuse in PSCs manufacturing. The resin efficiently adsorbs and desorbs Pb(Ⅱ) from

[Kalimate-Associated Gastric Ulcer].

Kalimate (calcium polystyrene sulfonate) is a cation-exchange resin commonly used in clinical practice to treat hyperkalemia. However, Kalimate has been demonstrated to also cause serious gastrointestinal injuries, such as colonic necrosis, ulcerations, and perforations, in a subset of patients with chronic renal failure; these cases have been reported with and without the administration of hypertonic sorbitol. These lesions usually occur in the large or small intestine; lesions occurring in the stomach are rarely reported. We present the case of a 62-year-old woman with very large Kalimate-in

Effect of acetaldehyde and acetylsalicylic acid on HbA1c chromatography in the FPLC method with Mono S cation exchanger.

The effects of alcohol and aspirin on HbA1c chromatography in the Mono S method were studied in vitro and in vivo. A modified chromatography with enhanced resolution was used, making possible detailed examination of minor interfering peaks included in the routine HbA1c value. Incubation with acetylsalicylic acid increased a hemoglobin fraction separate from HbA1c. In vivo this fraction was elevated by 0.1% of the total hemoglobin during therapeutic aspirin ingestion for one month. In vitro acetaldehyde generated two labile hemoglobin fractions and slightly increased a minor stable fraction whi

Hyperkalemia management: a multidisciplinary expert panel's perspective on the role of new potassium binders.

Hyperkalemia is a potentially life-threatening condition frequently encountered in clinical practice, particularly among patients with chronic kidney disease, heart failure, diabetes, and hypertension and those undergoing treatment with renin-angiotensin-aldosterone system inhibitors (RAASi). The management of chronic and acute hyperkalemia is complex and requires timely intervention to prevent severe complications such as cardiac arrhythmias and sudden death. Traditional therapeutic approaches to chronic hyperkalemia, including dietary potassium restriction, use of diuretics, and administrati

The effect of sodium zirconium cyclosilicate on acid-base balance in chronic kidney disease.

Hyperkalemia is a common electrolyte abnormality in chronic kidney disease (CKD). Sodium zirconium cyclosilicate (SZC) is a novel selective cation exchanger that is used to treat hyperkalemia and may also capture ammonium, which is of a similar size to potassium. We investigated the effect of SZC on acid-base balance in CKD patients. This retrospective study surveyed 20 patients with CKD whose serum potassium levels were maintained within the normal range by treatment with polysulfonate resin, which was replaced with SZC during the clinical course. We compared clinical parameters before and af

Mechanistic evaluation of factors affecting compound loading into ion-exchange fibers.

Donnan theory was applied to gain mechanistic understanding on the factors affecting drug loading process, compound-fiber affinity and subsequent release from fibrous ion-exchangers. Impact of initial loading solution concentration on fiber occupancy and loading efficiency of compounds were assessed experimentally and theoretically. Relative affinity towards the anion-exchange fibers was studied by dual loading of monovalent salicylic acid and either more lipophilic 3-isopropylsalicylic acid or divalent 5-hydroxyisophthalic acid. The effect of fiber framework on compound binding was evaluated

Highly Efficient Extraction of 212 Pb/212Bi from the Decay Chain of 232Th-Based on Anion Exchange in Bromide Medium.

Although 212Pb and 212Bi have significant applications in targeted α-nuclide therapy for malignant tumors, their use is limited by the scarcity of 212Pb/212Bi resources. To address this bottleneck problem, an advanced strategy for selectively extracting 212Pb and 212Bi from the decay chain of 232Th is first proposed by using an efficient porous silica-supported anion exchanger in bromide medium. The selective adsorption mechanisms of the resin for Pb(II) and Bi(III) in bromide medium are first clarified through spectroscopic analyses and theoretical calculations. Using the novel method and exc

Treatment of hyperkalemia: something old, something new.

Treatment options for hyperkalemia have not changed much since the introduction of the cation exchange resin, sodium polystyrene sulfonate (Kayexalate, Covis Pharmaceuticals, Cary, NC), over 50 years ago. Although clinicians of that era did not have ready access to hemodialysis or loop diuretics, the other tools that we use today-calcium, insulin, and bicarbonate-were well known to them. Currently recommended insulin regimens provide too little insulin to achieve blood levels with a maximal kalemic effect and too little glucose to avoid hypoglycemia. Short-acting insulins have theoretical adva

Rhabdomyolysis, myoglobinuria and exercise.

Muscle membrane injury is a predictable consequence of extreme exertion. The risk is compounded if an untrained individual performs eccentric exercise in a hot environment, or there is any preceding infectious disease, drug ingestion or an underlying metabolic disorder. Once the integrity of the membrane is breached, a constellation of physiological changes follows. Cell contents leak out and extracellular components leak in. Muscle pain and weakness ensue. Myoglobin is but one substance that is liberated into the bloodstream. When dehydration, hypovolaemia and acidosis are added to the myoglo

Hierarchical Biobased Macroporous/Mesoporous Carbon: Fabrication, Characterization and Electrochemical/Ion Exchange Properties.

With the goal of improving the mechanical properties of porous hierarchical carbon, cellulosic fiber fabric was incorporated into the resorcinol/formaldehyde (RF) precursor resins. The composites were carbonized in an inert atmosphere, and the carbonization process was monitored by TGA/MS. The mechanical properties, evaluated by nanoindentation, show an increase in the elastic modulus due to the reinforcing effect of the carbonized fiber fabric. It was found that the adsorption of the RF resin precursor onto the fabric stabilizes its porosity (micro and mesopores) during drying while incorpora

Hydrogen Storage and Release via Carbon Dioxide Hydrogenation to Formate Salts under High-Pressure Conditions with Ir Complex and Subsequent Formic Acid Dehydrogenation.

To investigate the potential of formic acid (FA) as a hydrogen carrier, we examined hydrogen storage and production through formate salts generated via CO2 reduction under supercritical fluid conditions. Formate salts were synthesized using Cp*Ir homogeneous catalysts to reduce CO2 under supercritical conditions (CO2: 12 MPa; H2: 0.5 MPa; total 12.5 MPa), achieving turnover frequency (TOF) of 10,240 h-1 and a turnover number (TON) of 20,480 within 2 h at 50 °C. The maximum formate concentration reached 0.81 mol/L after 18 h. The resulting formate salt solution (0.81 mol/L) was subsequently con

Novel way of separating polyfluorocarboxylic acids by ion-exclusion chromatography.

Ion-exclusion chromatography has been successfully applied to the separation of a number of polyfluorocarboxylic acids. The separation of various mono- and dibasic polyfluorocarboxylic acids having a different alkyl group was investigated using a polymethacrylate-based weakly acidic cation-exchange resin (TSK gel OApak-A) in the H -form and conductimetric detection. When water was used as the eluent, polyfluorocarboxylic acids could not be resolved. When an aqueous solution of sulfuric acid, benzoic acid, o-phthalic acid, 1,3,5-benzenetricarboxylic acid, 1,2,4,5-benzenetetracarboxylic acid, an

Management of hyperkalaemia consequent to mineralocorticoid-receptor antagonist therapy.

Mineralocorticoid-receptor antagonists (MRAs) reduce blood pressure and albuminuria in patients treated with angiotensin-converting-enzyme inhibitors or angiotensin-II-receptor blockers. The use of MRAs, however, is limited by the occurrence of hyperkalaemia, which frequently occurs in patients older than 65 years with impaired kidney function, and/or diabetes. Patients with these characteristics might still benefit from MRA therapy, however, and should not be excluded from this treatment option. This limitation raises the question of how to optimize the therapeutic use of MRAs in this populat
