Transesophageal Echocardiography in Cardiogenic Embolic Cerebral Infarction.

OBJECTIVE: To evaluate the diagnostic values of transesophageal echocardiography (TEE) and transthoracic echocardiography (TTE) in cardiogenic embolic cerebral infarction. METHODS: Fifty patients with occult cerebral infarction who were admitted to the hospital between June 2015 and June 2016 were selected as research subjects. The patients were diagnosed by transesophageal echocardiography and transthoracic echocardiography. Diagnostic data were compared to analyze the values of the two diagnostic methods. RESULTS: Sixteen out of fifty patients were diagnosed as cardiogenic embolic cerebral

Multiple cerebral infarction and cardiomyopathy with pheochromocytoma.

A 44-year-old woman was admitted to our hospital with altered mental status and weakness in the left upper and lower limbs. A brain magnetic resonance imaging indicated multiple cerebral infarctions in the bilateral frontal and parietal lobes and in the left occipital lobe. Magnetic resonance angiography indicated overall arterial wall irregularity and stenosis. An electrocardiogram showed negative T waves, troponin I was elevated at 0.60 ng/mL, and an echocardiogram showed severe hypokinesis, leading to a diagnosis of ischemic heart disease; however, no stenosis was found at cardiac catheteri

[Mechanism underlying thrombus formation in cerebral infarction].

For thrombus formation, three important factors, blood flow, blood component and blood vessels, have been recognized as Virchow's triad. In cardiogenic embolism with atrial fibrillation, stagnation of blood in the left atrium causes fibrin-rich thrombus. Anticoagulation is the only effective drug for prevention of this type of thrombus. In atherothrombotic and lacunar infarction, injury of endothelium and arterial vessels and platelet play a crucial role of formation of platelet-rich thrombus. Antiplatelet drugs such as aspirin, clopidogrel and cilostazole are effective for prevention of arter

[Cryptogenic cerebral infarction: from classification to concept].

Since the foundations laid by Sacco and Mohr in 1989, from the Stroke data bank, cryptogenic infarctions have had a predominant place among the causes of ischemic strokes. In that study, they accounted for approximately 40% of the stroke causes. Cryptogenic infarctions are infarctions without a defined cause, despite a complete work-up; they differ from infarctions of undetermined causes, which may involve overlapping causes or an incomplete investigation. The size of this group will probably shrink as knowledge advances. Patent foramen ovale (PFO), with or without a septal aneurysm, is more f

Polyneuropathy and cerebral infarction complicating scrub typhus.

We describe a 64-year-old man with scrub typhus who presented with both polyneuropathy and cerebral infarction. A eurological examination revealed a confused mental state, stiff neck, hearing impairment, symmetric weakness, sensory loss, and ataxia. Electrophysiologic studies showed demyelinating sensorimotor polyneuropathy and sensorineural hearing loss. Brain magnetic resonance imaging showed multiple infarctions. Brain involvement or polyneuropathy associated with scrub typhus has been rarely reported, and the pathogenic mechanism underlying the multiple neurological complications remains t

Neuroplasticity and cellular therapy in cerebral infarction.

Stroke is the second to third most common cause of death in adults, and more than a third of people who survive a stroke will have severe disability. Therapeutic options currently centre on fibrinolytic treatment, but its limitations restrict use to a small proportion of patients. Although a wide range of neuroprotective substances has been effective in experimental models, they have repeatedly failed in clinical trials because of toxicity or loss of effectiveness. Recent strategies based on neuroplasticity and cellular therapy have shown significant efficacy in improving functional recovery i

Clinical features of postoperative cerebral venous infarction.

There is a potential risk of sacrificing the cortical vein during neurosurgical operations, particularly in the interhemispheric or subtemporal approach. An impaired cortical vein might cause cerebral venous circulatory disturbances (CVCDs) resulting in venous infarction. In this article, we have reviewed the management and results of eight cases with symptomatic postoperative venous infarction. We have encountered eight cases with symptomatic postoperative venous infarction (0.3%) during the past 5 years. The series is composed of 3 males and 5 females, with ages that ranged from 43 to 76 yea

Diagnosis and prevention of atherosclerotic cerebral infarction.

Atherosclerotic disease accounts for approximately 25% of ischemic strokes. Atherosclerotic stroke is caused mainly by embolic events from the carotid artery bifurcation or the aortic arch, although intracranial thrombosis can occur, more often in African Americans, Asians, and diabetes patients. Primary prevention of stroke is critical for patients with risk factors for atherosclerosis, including hypertension, diabetes, smoking and hypercholesterolemia. Stroke can be prevented in patients with established atherosclerotic disease by identification and management of patients with carotid artery

Cerebral infarction following pituitary apoplexy--case report.

A 29-year-old man presented with lethargy, headache, high fever, and visual disturbance. Neurological examination showed mydriatic pupil, ptosis, diminished light reflex, and ophthalmoplegia on the left. Magnetic resonance (MR) imaging showed the typical findings of pituitary apoplexy, and cerebral angiography disclosed mild narrowing of the A1 segment of the left anterior cerebral artery (ACA). Transsphenoidal tumor resection was performed. Transient severe right hemiparesis occurred directly after the operation. Computed tomography demonstrated cerebral infarction in the territory of the lef

Portal vein thrombosis causing neonatal cerebral infarction.

Neonatal cerebral infarction often occurs in the absence of known risk factors. Two such cases are described in which portal vein thrombosis was documented during two dimensional echocardiography. In both cases, infarcts were consistent with embolic events. A novel mechanism is proposed, which may explain some cases of "idiopathic" neonatal cerebral infarction.

Patent foramen ovale and cryptogenic cerebral infarction.

The patent foramen ovale (PFO) has been increasingly implicated in the aetiology of stroke, particularly in young patients with no other identifiable cause (cryptogenic stroke). The mechanism is by the passage of venous clots through the patent foramen into the arterial circulation, enabling cerebral embolism. Such patients with cryptogenic stroke and PFO are often treated with life-long anticoagulants or antiplatelet agents in an attempt to decrease the risk of a recurrence. Less commonly, surgical closure of the PFO has been undertaken in these patients. However, the recent evolution of clos

Clinical effects of anterior cerebral artery infarction.

Anterior cerebral artery infarction is uncommon. We studied the topographical distribution of the lesions and the resulting clinical effects for a better understanding of their relationship and the functional outcome. There were 17 patients; the mean age of the cohort was 71 years. There were 10 men and 7 women. Two clinical syndromes were identified in accordance to the two anatomic areas of distribution. The paracentral lobule syndrome (group 1) was characterized by contralateral motor weakness, the leg more than the arm. The second group involved mainly the motor and supplementary motor are

[Lacunar cerebral infarction in a young woman].

HISTORY AND FINDINGS: A 29-year-old woman was admitted to hospital with an acute right-sided hemiplegia and sensory disorders, as well as upper right quadrant anopsia. There were no other significant abnormalities. She had previously been healthy and was free of any predisposing risk factors for thromboembolism. Neurological examination elicited a homonymous right upper quadrant hemianopsia, dysesthesia of the right half of the face and hypesthesia and hypalgesia of the right side of the body. In addition there was paresis of the right arm and a positive right Babinski reflex. INVESTIGATIONS:

[Non-valvular atrial fibrillation and cerebral infarction].

Non-valvular auricular fibrillation (NVAF) is linked to a high rate of cerebral embolism (CE), being one of the main causes of cerebral infarction in elderly people. The real risk of these patients suffering a cerebral embolism depends on various factors such as the type of fibrillation (chronic as opposed to paroxistic) and on the coexistence or not of other cerebrovascular risk factors. In recent years five random essays have been published on the primary prophylaxis of CE in patients with NVAF (AFASAK, SPAF, BAATAF, CAFA and SPINAF) and one on its secondary prophylaxis (EAFT), which have ev

Reduplication after right middle cerebral artery infarction.

Reduplication and misidentification syndromes are similar and peculiar phenomena of disorientation and beliefs with delusional qualities. They are more frequent among neurologic and psychiatric patients than previously thought. Organic factors play a definitive role in their occurrence. A case is presented which exemplifies reduplication of place and person, following a right hemisphere focal lesion. Current theories are briefly reviewed and possible roles of nortriptyline therapy and psychological factors are discussed in the pathogenesis of reduplicative phenomena in this patient.

Watershed cerebral infarction associated with perioperative hypotension.

The pathogenesis of perioperative stroke is not clear from the literature. To explore the influence of various risk factors we examined the clinical, Duplex ultrasound and computerised tomography findings of all cases suffering cerebral infarction within 24 hours of surgery in a prospective series of 358 coronary or peripheral vascular reconstructive operations. Four patients (1.1%) had cerebral infarcts within 24 hours of surgery, all associated with perioperative systolic blood pressures of less than 90 mmHg. The other significant risk factor was previous cerebral ischaemic symptoms. Haemody
