Evaporative cooling for low-cost monitoring of flow of cerebrospinal fluid through shunts in patients with hydrocephalus.

Hydrocephalus, a neurological disorder caused by an abnormal accumulation of cerebrospinal fluid (CSF) in the brain, manifests in symptoms such as headaches, blurred vision, and balance issues. While ventriculoperitoneal shunting is a common treatment, it has high failure rates, especially in pediatric patients. Recent progress in continuous, non-invasive monitoring using skin-mounted sensors based on anemometric techniques and transient plane source methods offer significant promise. Here, we introduce an advanced device of this general type, configured for ultralow power operation and cost-e

Survival and failure trends of cerebrospinal fluid shunts with distal slit valves: comparative study and literature review.

OBJECTIVE: The malfunction rates of and trends in various cerebrospinal fluid (CSF) shunt designs have been widely studied, but one area that has received little attention is the comparison of the peritoneal distal slit valve (DSV) shunt to other conventional valve (CV) type shunts. The literature that does exist comes from older case series that provide only indirect comparisons, and the conclusions are mixed. Here, the authors provide a direct comparison of the overall survival and failure trends of DSV shunts to those of other valve type shunts. METHODS: Three hundred seventy-two new CSF s

Ceftaroline Diffusion Into Cerebrospinal Fluid of Children With Ventriculitis Due to Ventriculoperitoneal Shunt Infection.

BACKGROUND: Data on ceftaroline cerebrospinal fluid (CSF) levels in children are limited. This study examined ceftaroline diffusion into CSF among children with ventriculoperitoneal shunt (VPS)-related ventriculitis and external ventricular drains. METHODS: Patients received a single dose of 15 mg/kg of iv ceftaroline (maximum 600 mg) over 2 hours. Simultaneous plasma and CSF samples were obtained at 3 time points to measure ceftaroline concentration: end of infusion (0 hours), and 2 and 6 hours postinfusion. The areas under the curve (AUCs) for plasma and CSF were calculated using linear int

Late cerebrospinal fluid shunt infections: a Hydrocephalus Clinical Research Network study.

OBJECTIVE: Infection is a significant complication of CSF shunt surgery, with most infections occurring early in the postoperative course. While standardized perioperative protocols have successfully reduced early infections, the epidemiology, risk factors, and clinical patterns of late shunt infections are poorly characterized. This study aimed to determine the incidence, risk factors, clinical features, and outcomes of late shunt infections in a large, multicenter pediatric cohort. METHODS: The Hydrocephalus Clinical Research Network (HCRN) Core Data Project was queried for all CSF shunt pr

Epidemiology of cerebrospinal fluid shunt surgery in Australia and New Zealand (2017-2024).

BACKGROUND: Hydrocephalus-related procedures constitute approximately one-third of neurosurgical interventions annually, with incidence rates ranging from 30 to 300 per 100,000 population, varying by region, age and aetiology. Despite advancements in surgical techniques and shunt devices, cerebrospinal fluid (CSF) shunts exhibit high complication and failure rates, necessitating frequent revisions due to issues such as overdrainage, underdrainage, infections and mechanical failures. This study leverages data from the Australasian Shunt Registry (ASR) to define the epidemiology of shunt surgery

Ventriculoperitoneal Shunt Failure and Cerebrospinal Fluid Protein: A Meta-Analysis and Systematic Review.

Ventriculoperitoneal shunts (VPS) are used to manage hydrocephalus but suffer from high failure rates. Our objectives were to (1) conduct a meta-analysis to objectively weigh this conflicting evidence, and (2) conduct a systematic review compiling and synthesizing what is known about the association between CSF proteins and shunt failure. A literature search was performed in compliance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The Embase, PubMed, and CENTRAL databases were searched from inception to June 2023. The articles were screened based on the in

Dual catheter and double-lumen cerebrospinal fluid shunt systems with backflow mechanisms.

PURPOSE: We previously developed a novel functional benchtop apparatus to simulate catheter occlusion in vitro utilizing avian vitelline membrane and chalaza to test catheter designs and de-obstruction techniques. Here, we study the integration of double-lumen catheter-mediated backflow in the shunt system assembly and its potential for an in-line de-obstruction of an obstructed ventricular catheter. METHODS: A double-lumen catheter was connected to a standard proximal shunt catheter for all trials. One limb of the double-lumen catheter was connected to the backflow mechanisms and allowed to

Cerebrospinal fluid as vaginal discharge: ventriculoperitoneal shunt migration following Cesarean section.

Congenital hydrocephalus was once a permanently disabling and even fatal disease. With the advent of ventriculoperitoneal shunts, affected women are now surviving to their reproductive years and beyond. Pregnancy outcomes in this population are generally positive. However due to possible shunt complications, including infection, migration, and organ perforation, perinatal care for pregnant individuals with a ventriculoperitoneal shunt is complex and requires input from both obstetric and neurosurgical providers. We present the case of a 28-year-old G1P1 with a history of congenital hydrocephal

Cerebrospinal fluid shunting protocol for idiopathic intracranial hypertension for an improved revision rate.

OBJECTIVE: Cerebrospinal fluid (CSF) shunting in idiopathic intracranial hypertension (IIH) is associated with high complication rates, primarily because of the technical challenges that are related to small ventricles and a large body habitus. In this study, the authors report the benefits of a standardized protocol for CSF shunting in patients with IIH as relates to shunt revisions. METHODS: This was a retrospective study of consecutive patients with IIH who had undergone primary insertion of a CSF shunt between January 2014 and December 2020 at the authors' hospital. In July 2019, they imp

Value of Cerebrospinal Fluid Lactate Levels in Diagnosing Shunt Infections in Pediatric Patients.

OBJECTIVE: The diagnosis and timely treatment of shunt infections (SI) in children is of paramount importance. In some cases, the standard cerebrospinal fluid (CSF) variables will not be sufficient for an accurate diagnosis of SI. CSF lactate (LCSF) has been found to assist in differentiating bacterial from nonbacterial meningitis in non-neurosurgical patients. To the best of our knowledge, the use of lactate in diagnosing or confirming the presence of SI has not yet been discussed. The goal of the present study was to describe the role of LCSF levels in children with shunts and Ommaya reservo

Vaginal Migration of Ventriculoperitoneal Shunt Catheter and Cerebrospinal Fluid Leak as a Complication of Hysterectomy.

BACKGROUND: Ventriculoperitoneal (VP) shunting is a common neurosurgical procedure to treat hydrocephalus that diverts cerebrospinal fluid from the cerebral ventricles to the peritoneal cavity for reabsorption. The distal catheter may potentially migrate through any potential or iatrogenic opening in the peritoneal cavity. Increasingly successfully management of childhood hydrocephalus and adult-onset conditions leading to hydrocephalus, such as subarachnoid hemorrhage, is leading many adult female patients harboring VP shunts needing to undergo hysterectomy. Hysterectomy creates a potential d

Impact of cerebrospinal fluid shunting for idiopathic normal pressure hydrocephalus on the amyloid cascade.

The aim of this study was to determine whether the improvement of cerebrospinal fluid (CSF) flow dynamics by CSF shunting, can suppress the oligomerization of amyloid β-peptide (Aβ), by measuring the levels of Alzheimer's disease (AD)-related proteins in the CSF before and after lumboperitoneal shunting. Lumbar CSF from 32 patients with idiopathic normal pressure hydrocephalus (iNPH) (samples were obtained before and 1 year after shunting), 15 patients with AD, and 12 normal controls was analyzed for AD-related proteins and APLP1-derived Aβ-like peptides (APL1β) (a surrogate marker for Aβ). Favorable outcome of neonatal cerebrospinal fluid shunt-associated Candida meningitis with caspofungin.

Invasive Candida infections associated with medical devices are very difficult to cure without device removal. We present a case of neonatal cerebrospinal fluid shunt-associated Candida meningitis, in which removal of the device was precluded, that was successfully treated with caspofungin. Pharmacokinetic assessment of caspofungin concentrations in cerebrospinal fluid showed that exposure was adequate in the presence of a high systemic exposure. In complex cases of neonatal Candida infections involving medical devices, the addition of caspofungin might be beneficial.
