Imaging of Congenital Anomalies and Developmental Abnormalities Involving the Arteries of the Head and Neck.

The embryonic development of the cerebrovascular system is a highly intricate process that gives rise to the arterial network supplying the brain, one of the most metabolically demanding organs in the human body. A hallmark of this system is the presence of extensive collateral circulation, exemplified by the circle of Willis, which can preserve cerebral perfusion in the event of arterial occlusion, severe stenosis, or dissection. Disruptions in the embryologic development of this system can lead to a spectrum of congenital and developmental arterial anomalies. While many of these variants are

Occipital artery-posterior inferior cerebellar artery bypass for anterior inferior cerebellar artery stenosis with repeated cerebellar ischemic stroke: a case report.

BACKGROUND: Vertebrobasilar insufficiency revascularization has long been a preventive treatment for cerebral infarction. However, no studies have demonstrated the efficacy of revascularization in patients with cerebellar ischemia. CASE PRESENTATION: We present the case of a 77-year-old Japanese man who experienced seven recurrent cerebellar infarctions over 2 years. Severe stenosis was noted at the origin of the common trunk of the posterior inferior cerebellar artery and anterior inferior cerebellar artery. Patients with cerebral infarctions are resistant to medical treatment. Blood flow ev

New information on the value of plaque characterisation--relation to symptoms.

This paper reviews the literature on the significance of carotid plaque echomorphology and degree of stenosis in relation to the different types of cerebrovascular symptomatology (amaurosis fugax, hemispheric transient ischaemic attacks, stroke) and the asymptomatic status. It provides evidence that amaurosis fugax is associated with hypoechoic and severely stenosed plaques, the hemispheric transient ischaemic attacks and stroke are associated with plaques of intermediate echodensity and stenosis while the asymptomatic status is associated with hyperechoic and moderately stenosed plaque. It le

Gestational Hypoxia and Blood-Brain Barrier Permeability: Early Origins of Cerebrovascular Dysfunction Induced by Epigenetic Mechanisms.

Fetal chronic hypoxia leads to intrauterine growth restriction (IUGR), which is likely to reduce oxygen delivery to the brain and induce long-term neurological impairments. These indicate a modulatory role for oxygen in cerebrovascular development. During intrauterine hypoxia, the fetal circulation suffers marked adaptations in the fetal cardiac output to maintain oxygen and nutrient delivery to vital organs, known as the "brain-sparing phenotype." This is a well-characterized response; however, little is known about the postnatal course and outcomes of this fetal cerebrovascular adaptation. I

[Cerebrovascular Diseases and Autonomic Failure].

Cerebral vessels are innervated by sympathetic and parasympathetic nerves, as well as the trigeminal nerve. Under physiological conditions, neural regulation contributes to coupling of cerebral blood flow (CBF) and metabolism and autoregulation of CBF against systemic blood pressure. It also plays a key role in vasodilatation distal to narrowed vascular lesions associated with atherosclerosis, hyperperfusion syndrome that occurs after vascular reconstruction in patients with chronic ischemic diseases, reversible cerebral vasoconstriction syndrome, and migraine. Moreover, cerebrovascular diseas

Anesthetic management of patients undergoing intracranial bypass procedures.

Cerebral revascularization is used to augment or replace cerebral blood flow in patients at risk of developing cerebral ischemia. These include patients with moyamoya disease, occlusive cerebrovascular disease, skull base tumors, and complex aneurysms. Our aim in this review is to provide a comprehensive update of both surgical and anesthetic aspects of cerebral revascularization procedures. The anesthetic concerns for most patients presenting for different types of bypass procedures are similar and include the maintenance of adequate cerebral perfusion to prevent cerebral ischemia. Patients w

[Roles of cerebrovascular innervations and their neurotransmitters in various pathological conditions of central nervous system].

The concept of neurogenic control of the cerebral circulation began to develop only as recent as the middle of the 20(th) century, when it was elucidated that the cerebral blood vessels are supplied by a very dense plexus of adrenergic nerves innervating the vascular bed. After then, already in the course of several decades, more than ten types of neurotransimitters including acetylcholine, serotonin, substance P, and calcitonin gene-related peptide, etc., have been observed in these nerves. To determine the origins and pathways of cerebrovascular parasympathetic nerves and sensory nerves, ret

New techniques in the evaluation of cerebrovascular disease.

Numerous reports in the literature indicate that various noninvasive vascular techniques can now be used to evaluate atherosclerosis at the carotid bifurcation. This article reviews noninvasive screening techniques currently available and being developed. Particular emphasis has been placed on the practicality of these techniques as well as their limitations. Our conclusions are that noninvasive techniques cannot be used as definitive screening tests for cerebrovascular disease. Although these tests are frequently useful when positive, the false negative rate of these tests would appear to be

Selective cerebral revascularization as an adjunct in the treatment of giant anterior circulation aneurysms.

Cerebral revascularization, an indispensable component of neurovascular surgery, has been performed in the treatment of cranial base tumors, complex cerebral aneurysms, and occlusive cerebrovascular disease. The goal of a revascularization procedure is to augment blood flow distally. It can therefore be used as an adjunctive measure in the treatment of complex neurosurgical disease processes that require parent artery sacrifice for definitive treatment. In the treatment of giant anterior circulation aneurysms, for instance, a cerebral revascularization procedure may be considered in patients i

Disruption of the Mouse Blood-Brain Barrier by Small Extracellular Vesicles from Hypoxic Human Placentas.

Cerebrovascular complications, including cerebral edema and ischemic and hemorrhagic stroke, constitute the leading cause of maternal mortality associated with preeclampsia. The underlying mechanisms of these cerebrovascular complications remain unclear. However, they are linked to placental dysfunction and blood-brain barrier (BBB) disruption. Nevertheless, the connection between these two distant organs is still being determined. Increasing evidence suggests that the placenta releases signaling molecules, including extracellular vesicles, into maternal circulation. Extracellular vesicles are

Siltation and tidal effects: a comprehensive, clinical, and didactic approach to reasoning about vertebrobasilar hypoflow conditions.

OBJECTIVES: To propose a clinically applicable model for understanding vertebrobasilar hypoperfusion by categorizing its causes into two mechanisms: the siltation effect, related to local vascular obstruction, and the tidal effect, related to systemic reduction in posterior circulation blood flow. METHODS: A non-systematic narrative review was conducted using selected literature in the fields of neurotology, vascular neurology, and cardiovascular medicine. The analogy of coastal rivers affected by sedimentation and tides was used to explain the proposed classification. Clinical examples were

Impaired cerebrovascular reactivity in chronic obstructive pulmonary disease.

Impaired cerebrovascular reactivity (CVR) is associated with stroke. Cerebrovascular diseases are common comorbidity in chronic obstructive pulmonary disease (COPD) patients. The aim of our study was to quantify CVR in the anterior and posterior cerebral circulation during voluntary breath-holding in COPD patients according to airflow limitation severity. In this cross-sectional study, we compared 90 COPD patients without previous cerebrovascular disease and 30 age- and sex-matched healthy volunteers (mean age 67 ± 7.9, 87 males). Using transcranial Doppler ultrasound and breath-holding index

Vertebrobasilar infarcts and ischemia.

This article discusses the clinical presentation of vertebrobasilar ischemia and infarcts. Pertinent dizziness intake questions are presented, as well as key components of the physical examination. The anatomy of the posterior cerebral circulation is discussed as are syndromes associated with cerebrovascular infarcts in the posterior circulation. A high incidence of recurrence of ischemic attacks or infarcts in vertebrobasilar insufficiency is noted. The authors note that recent developments in imaging and interventions may indicate need for referral or consultation with specialists in some ca

Peripheral macrophages in the development and progression of structural cerebrovascular pathologies.

The human cerebrovascular system is responsible for maintaining neural function through oxygenation, nutrient supply, filtration of toxins, and additional specialized tasks. While the cerebrovascular system has resilience imparted by elaborate redundant collateral circulation from supportive tertiary structures, it is not infallible, and is susceptible to developing structural vascular abnormalities. The causes of this class of structural cerebrovascular diseases can be broadly categorized as 1) intrinsic developmental diseases resulting from genetic or other underlying aberrations (arterioven

[Doppler ultrasonography in Takayasu syndrome].

The authors by means of ultrasound Doppler examined 26 patients with non-specific aortoarteritis involvement of the aortic arch branches. The use of compressive tests permitted accurate diagnosis of the routes of blood overflow to the affected segment of the arterial basin of the brain. It was established that the main route of blood inflow in patients with non-specific aortoarteritis was the system of intracranial anastomoses; extracranial blood supply of the brain was noted in occlusion of the internal carotid artery.

Alzheimer disease as a vascular disorder: nosological evidence.

BACKGROUND: The main stumbling block in the clinical management and in the search for a cure of Alzheimer disease (AD) is that the cause of this disorder has remained uncertain until now. SUMMARY OF REVIEW: Evidence that sporadic (nongenetic) AD is primarily a vascular rather than a neurodegenerative disorder is reviewed. This conclusion is based on the following evidence: (1) epidemiological studies showing that practically all risk factors for AD reported thus far have a vascular component that reduces cerebral perfusion; (2) risk factor association between AD and vascular dementia (VaD); (

The role of neuroeffector mechanisms in cerebral hyperperfusion syndromes.

Cerebral hyperperfusion, a state in which blood flow exceeds the metabolic needs of brain, may complicate a number of neurological and neurosurgical conditions. It may account for the propensity with which hemorrhage, cerebral edema, or seizures follow embolic stroke, carotid endarterectomy, or the excision of large arteriovenous malformations, and for some of the morbidity that accompanies acute severe head injury, prolonged seizures, and acute severe hypertension. Hyperperfusion syndromes have in common acute increases in blood pressure, vasodilatation, breakdown of the blood-brain barrier,
