Superficial and deep cervical plexus blocks versus patient-controlled analgesia in total laryngectomy; a randomized trial.

INTRODUCTION: The cervical plexus block (CPB) delivers analgesia for surgeries in the head and neck . Patient-controlled analgesia (PCA) has been utilized to enhance pain management. This study evaluates combined bilateral superficial and deep CPB versus PCA in postoperative pain management after total laryngectomy. MATERIALS AND METHODS: Randimized two equal groups: the CPB group (n = 25), who received combined bilateral US-guided superficial and deep CBP, and the PCA group (n = 25), who received PCA. The primary outcome was postoperative visual analog scale (VAS) . The secondary outcomes we

IgG4-Related Peripheral Neuropathy with Unilateral Cervical Nerve Root and Brachial Plexus Swelling: A Case Report.

A 64-year-old man presented with left upper limb weakness and dysesthesia for 4 months. Magnetic resonance imaging demonstrated swelling from the 6th-8th left cervical nerve roots to the left brachial plexus. The serum IgG4 level was elevated (762.7 mg/dL). 18F-FDG-PET showed high uptake in the mediastinal lymph nodes, and biopsy revealed infiltration of IgG4-positive plasma cells. We diagnosed IgG4-related neuropathy, and steroid therapy administration improved the symptoms. IgG4-related disease should be considered in the differential diagnosis of peripheral nerve swellings. If biopsy of the

Subclavian transcatheter aortic valve implantation (TAVI): superficial cervical plexus block combined with low-dose interscalene block.

PURPOSE: Transcatheter aortic valve implantation (TAVI) has become a widely used technique for treating aortic stenosis. Subclavian access may be warranted in the presence of poor vasculature that precludes femoral access. Conscious sedation is increasingly being adopted with some evidence suggesting better outcomes compared with those of general anesthesia. We describe the use of two regional anesthetic techniques to facilitate subclavian access for TAVI. CLINICAL FEATURES: Our case report involves the successful management of a challenging patient with severe peripheral vasculopathy and res

Lumbar Hypobaric Spinal Anesthesia Combined With Intermediate Cervical Plexus Block for Upper Abdominal Laparoscopy: A Case Report.

Thoracic spinal anesthesia offers targeted anesthesia for upper abdominal surgery but raises safety concerns related to thoracic intrathecal access and potential catastrophic complications. We describe a physiology-based alternative combining lumbar hypobaric spinal anesthesia with an intermediate cervical plexus block to achieve similar coverage to thoracic spinal anesthesia (T3-T4) without thoracic puncture. A 46-year-old woman undergoing laparoscopic cholecystectomy received a unilateral ultrasound-guided cervical plexus block and intrathecal 4 mL hypobaric ropivacaine with dexmedetomidine

Efficacy Comparison Between Interscalene Block with and Without Superficial Cervical Plexus Block for Anesthesia in Clavicle Surgery.

BACKGROUND: Clavicle fractures account for over one-third of shoulder injuries and up to 3.3% of all fractures in adults. While the majority of these fractures can be managed non-surgically, there are instances where surgical intervention is performed. Regional anesthesia (RA) can be a preferred alternative to general anesthesia (GA) to avoid complications and high costs in this surgery. Moreover, the identification of the most optimal approach for RA remains challenging. OBJECTIVES: This study aimed to compare the efficacy of interscalene block (ISB) with and without superficial cervical ple

Effectiveness of Intermediate Cervical Plexus Block in Whiplash-Associated Disorder: A Prospective Observational Trial in Fifty Patients.

BACKGROUND: Whiplash trauma can result in a range of symptoms, including chronic neck pain, headache, facial pain, upper back pain, and tinnitus, which comprises whiplash-associated disorder (WAD). Intermediate cervical plexus block (iCPB) is a novel intervention that targets the upper cervical nerves and anecdotal reports suggest benefits in WAD. OBJECTIVES: We hypothesized that the cervical plexus may have a role in the pathogenesis of WAD and blocking the cervical plexus may provide analgesia. STUDY DESIGN: Prospective observational trial. SETTING: Tertiary pain medicine unit at a univer

[Neurocutaneous flaps applied on head and neck surgery: Anatomic study of feasibility using superficial cervical plexus branches].

OBJECTIVE: To evaluate the feasibility of neurocutaneous flaps for head and neck surgery using the cervical superficial plexus branches. METHODS: Anatomic study on cervical superficial plexi (focusing on the great auricular and cervical transverse nerves) of five fresh specimens, which were injected with green colored latex. RESULTS: A constancy of anatomic landmarks has been observed with little inter-individual variability. The cervical superficial plexus branches were easily isolated, their length and course not varying much. Each cervical superficial plexus nerve is accompanied by a peri

A novel strategy for repairing preganglionic cervical root avulsion in brachial plexus injury by sural nerve grafting.

OBJECT: In this study, the authors evaluated the efficacy of a new surgical strategy for reconnecting the injured brachial plexus with the spinal cord using fibrin glue containing acidic fibroblast growth factor as an adhesive and neurotrophic agent. METHODS: Eighteen patients with preganglionic brachial plexus injuries, each with varying degrees of upper limb dysfunction, underwent cervical laminectomy with or without sural nerve grafting. The treatment of each avulsed root varied according to the severity of the injury. Some patients also underwent a second-stage operation involving supracl

High cervical spinal cord stimulation after failed dorsal root entry zone surgery for brachial plexus avulsion pain.

BACKGROUND: Many patients had intractable pain after brachial plexus avulsion. This article describes a feasible method of high cervical SCS for a patient who failed in DREZ surgery to treat the pain. CASE DESCRIPTION: A 70-year-old man had right brachial plexus avulsion after a car accident 15 years ago. He complained of chronic spontaneous pain over his right upper limb since then. Two DREZ surgeries failed to relieve the pain. The pain was successfully controlled by high cervical SCS. CONCLUSION: Stimulating the high cervical spinal cord is effective for treating intractable pain after br

Seventh cervical nerve root transfer from the contralateral healthy side for treatment of brachial plexus root avulsion.

Cervical root nerve transfer from the contralateral side has been used for the treatment of brachial plexus root avulsion in 49 patients. Resection of C7 root from the healthy side has produced no long-term symptoms or signs. Nine patients with ten recipient nerves have been followed up for more than two years and seven have obtained a functional recovery. This operation offers a new approach for the treatment of brachial plexus root avulsion.

A minimally painful, comfortable, and safe technique for hemodialysis catheter placement in children: superficial cervical plexus block.

The superficial cervical plexus block (SCPB) is utilized in pediatric patients to perform certain surgical procedures, but there is no evidence supporting its use in hemodialysis catheter placement. We evaluated the analgesic effectiveness, intraoperative complications, and patient satisfaction associated with SCPB for pediatric patients in renal failure undergoing emergent dialysis catheterization. A total of 52 patients ranging from 1 to 17 years old that required emergent dialysis catheter placement and received SCPB were included in this study. During the catheterization, intraoperative pa

MR visualization and clinical significance of the anterior longitudinal epidural venous plexus in cervical extra-axial lesions.

A thin band of intermediate signal intensity behind the cervical vertebral bodies, demonstrated on sagittal MR images, was attributed to the anterior longitudinal epidural venous (AEVP). Percentage visualization and clinical significance of this structure were evaluated in extra-axial lesions of the spinal cord. In review of 50 normal controls, the percentage visualization of this structure at each cervical vertebral level was 25.5% on T1 weighted sagittal SE images, while that of T2 or proton density weighted SE images was 26.0%. AEVP was demonstrated by sagittal MRI in 29 of 128 extra-axial

Dissection Report of Combined Variations of the Facial Nerve, Cervical Plexus, and Superficial Venous System in the Cervicofacial Region.

The cervicofacial region contains densely layered neural and venous structures with clinically significant anatomic variability. Variations involving the extratemporal facial nerve (FN), its communications with the cervical plexus (CP), and the superficial venous system may increase surgical risk during parotid, submandibular, and upper cervical procedures. A stepwise cervicofacial dissection was performed on an 82-year-old male donated cadaver. The branching pattern of the extratemporal FN, intrafacial interconnections, communications with CP branches, and relationships to the retromandibular

The efficacy of electroacupuncture for cervical nerve edema and movement disorder caused by the brachial plexus injury: a case report.

UNLABELLED: The brachial plexus injury (BPI) is one of the most severe types of peripheral nerve injuries, often caused by upper limb traction injury. In clinic, the surgery is widely used to treat the BPI. However, surgery may need to be performed multiple times at different stages, which carries risks and brings heavy economic burden. In non-surgical treatment, splinting, local injection of corticosteroids, and oral corticosteroids can achieve significant short-term benefits, but they are prone to recurrence and may cause complications of mechanical or chemical nerve damage. In this report,

The analgesic effects of bilateral superficial cervical plexus block in thyroid surgery: A systematic review and meta-analysis.

BACKGROUND AND AIMS: Thyroid surgery is moderately painful, and many techniques to reduce postoperative pain have been studied. Regional techniques are a part of multimodal analgesia employed for various surgical cases. Bilateral superficial cervical plexus block (BSCPB) is a commonly used regional anaesthesia technique for analgesia for thyroid surgery. A previous meta-analysis by this group had left questions about some facets of the technique, to which further trials have contributed. METHODS: The systematic review and meta-analysis was registered on the International Prospective Register

An anatomical study of the pharyngeal plexus: Application to avoiding postoperative dysphagia following anterior approaches to the cervical spine.

We aimed to localize the pharyngeal branches of the pharyngeal plexus to preclude postoperative complications such as dysphagia resulting from injury to those branches. Cranial nerves IX and X and the sympathetic trunk were dissected on 10 sides in the necks of embalmed adult cadavers of European descent to identify the pharyngeal branches so that anatomical landmarks could be identified and injury thereby avoided. In all sides, the pharyngeal branches originated from the glossopharyngeal and vagus nerves and the superior cervical ganglion and entered the posterior pharyngeal wall at the C2-C4

Modified longitudinal intermediate cervical plexus block for supraclavicular nerve analgesia: a randomized, double-blind trial in healthy volunteers.

BACKGROUND: The superficial cervical plexus block (SCPB) is routinely employed for clavicle surgery due to the dominant innervation of the surgical field by the supraclavicular nerves (SN). However, evidence directly comparing the blocking effect of different SCPB approaches-particularly the novel modified longitudinal intermediate cervical plexus block (MU)-on SN coverage and phrenic nerve safety remains limited. This randomized, double-blind, controlled trial in healthy volunteers aimed to evaluate the efficacy of MU relative to landmark-based and ultrasound-guided intermediate approaches in
