Arterial thoracic outlet syndrome by a commonly overlooked anomaly, the cervical rib: A case report.

Thoracic outlet syndrome (TOS) is a constellation of symptoms that occur due to the compression of neurovascular structures traversing the thoracic outlet. TOS manifests in 3 distinct forms: neurogenic, venous, and arterial. Among these, arterial TOS is the rarest. A 32-year-old man presenting with severe right arm pain was referred for CT angiography. The imaging revealed bilateral cervical ribs, with the right side showing more pronounced development and fusion with the first thoracic rib. At the fusion site of the right cervical rib and the first thoracic rib, a pseudoaneurysm of the right

An unusual case of the syndrome of cervical rib with subclavian artery thrombosis and cerebellar and cerebral infarctions.

BACKGROUND: Cerebellar and cerebral infarctions caused by the syndrome of cervical rib with thrombosis of subclavian artery are very unusual. CASE PRESENTATION: We report the case of a 49-year-old male patient with a right cervical rib compression leading to subclavian arterial thrombosis and both cerebellar and cerebral infarctions secondary to retrograde thromboembolisation. Follow-up imaging revealed partial resolution of the thrombosis after combined anti-coagulant and anti-platelet therapy. The cervical rib and first costa were surgically removed to prevent additional events. CONCLUSION

[The superior thoracic outlet compression syndrome: a report of a case in childhood with complete cervical rib].

Authors report a paediatric patient with thoracic outlet syndrome (TOC) and complete cervical rib. The symptoms at the time of presentation result from pressure on either the subclavian vessels or the lower trunk of the brachial plexus. TOC is infrequent in young people and usually the symptomatology does'nt need a surgical approach. Some Authors affirm that there are necessary impulsive moments like growth, increased muscular mass and rib ossification, decreased elasticity of ligaments, vessels, muscles and nerves, for outcoming the clinical manifestations. Paediatric competention has risen u

Ultrasound-Guided Hydrodissection for Neurogenic Thoracic Outlet Syndrome Caused by a Cervical Rib: A Case Report.

Thoracic outlet syndrome (TOS) is an underdiagnosed condition that is frequently misattributed to cervical spine pathology. Neurogenic TOS (nTOS), the most common subtype, often presents nonspecific upper extremity pain and paresthesia, making diagnosis challenging. We report a case of nTOS associated with a cervical rib that was successfully treated with ultrasound-guided hydrodissection (HD). Dynamic ultrasound examination demonstrated brachial plexus tethering with symptom reproduction. Serial HD resulted in immediate and sustained symptom relief, suggesting that this minimally invasive tec

A Never Described Variant of the Cervical Rib Causing Arterial Thoracic Outlet Syndrome: World's First Case.

Cervical ribs, also known as Eve's ribs, are rare and found in 1% of population. They are more common in females and more common on right side. They are asymptomatic in 90% of cases. Cervical rib fused with transverse process of sixth vertebra is rarer. We present a case of dry gangrene of lateral three fingers with right radial and subclavian artery thrombosis with rest pain, due to right cervical rib fused with transverse process of sixth vertebra. After development of line of demarcation of the dry gangrene, patient was operated for excision of cervical rib and sixth cervical vertebral tran

Thoracic outlet syndrome: a rare case with bilateral cervical ribs and bilateral anterior scalene hypertrophy.

Thoracic outlet syndrome (TOS) is a rare neurovascular disorder generally caused by the presence of a cervical rib or hypertrophic scalene anterior muscle that can compress the brachial plexus and/or subclavian vessels. In the vascular form, the symptoms are caused by the compression of the artery and/or the subclavian vein. In the first case, the compression is caused by the cervical rib and leads to hypo-perfusion with cooling and cyanosis of the upper limb, while in the second case, the compression is caused by the anterior scalene muscle and leads to congestion, cyanosis, swelling and pain

Unilateral thoracic outlet syndrome in a case with bilateral cervical ribs, does it always produce compression?

BACKGROUND: The cervical rib can be a rare finding in asymptomatic subjects. When it produces compression, it affects variably the vasculo-nervous structures of the brachial plexus; the clinical history is progressive. The imaging studies indicate the site of osteo-neuro-vascular conflict and electrophysiological studies indicate the degree of neural involvement. CASE REPORT: We present a patient with an eight-month history of paresis and right dysesthesias, with an image of bilateral cervical ribs with different anatomical characteristics. This case discusses how a variant of the anatomy pro

The management of thoracic outlet syndrome induced by bilateral cervical ribs in young, female athlete - a case report.

INTRODUCTION: The presence of additional cervical ribs is a rare and relatively unknown pathology. The brachial plexus is most often compressed. Thoracic Outlet Syndrome (TOS) is the one of discussed of mixed compression syndromes, due to diagnostic difficulties and the lack of evidence to resolve the effectiveness of surgical treatment over conservative treatment. CASE REPORT: Presentation of the case of a 22-year-old female athlete with cervical ribs with neurogenic TOS symptoms and presentation of the rehabilitation model and its results. Functional diagnosis included TOS specific clinical

Thoracic outlet syndrome with right subclavian artery dilatation in a child - transaxillary resection of the pediatric cervical rib.

Cervical rib is a well-documented congenital anomaly, and thoracic outlet syndrome (TOS) with vascular compression is extremely rare in childhood. The subclavian artery, subclavian vein, brachial plexus, or a combination of these can be affected in this syndrome. We present a case of TOS in an eight-year-old boy and describe the rare finding of subclavian artery compression with post-stenotic dilatation and severe brachial plexus compression symptoms. The patient underwent right transaxillary resection of the first thoracic and cervical rib. The presentation is unique and may be elusive.

Arterial thoracic outlet syndrome caused by a cervical rib: a combined thoracoscopic and supraclavicular approach for 'en bloc'-resection.

A cervical rib is the cause of ∼5% of thoracic outlet syndromes (TOS). We report the case of a patient with arterial TOS due to the presence of a cervical rib, managed by combined thoracoscopic and supraclavicular approach. An 18-year-old female patient presented with symptoms of arterial TOS. Magnetic resonance imaging and computed tomography angiography scans showed a tortuous subclavian artery due to a fused cervical and 1st rib. Three-port thoracoscopy was performed, which allowed to mobilize the ventral part of the 1st rib. Supraclavicular access allowed mobilization as well as central di

Cervical Rib Prevalence and its Association with Thoracic Outlet Syndrome: A Meta-Analysis of 141 Studies with Surgical Considerations.

BACKGROUND: Cervical ribs (CR) are supernumerary ribs that arise from the seventh cervical vertebra. In the presence of CR, the boundaries of the interscalene triangle can be further constricted and result in neurovascular compression and thoracic outlet syndrome (TOS). The aim of our study was to provide a comprehensive evidence-based assessment of CR prevalence and their association with TOS as well as surgical approach to excision of CR and surgical patients' characteristics. METHODS: A thorough search of major electronic databases was conducted to identify any relevant studies. Data on th

Exercise-induced thoracic outlet syndrome and concomitant osteomyelitis in cervical rib with a possible familial origin: A case report.

Cervical ribs are rare and usually asymptomatic. Occasionally, they can cause nerve impingements and compressive symptoms. In cervical ribs, osteomyelitis secondary to trauma is unheard of. We report such a case made more interesting by the familial presence of bilateral cervical ribs in two generations, indicating a familial origin.

Bilateral cervical ribs causing cerebellar stroke and arterial thoracic outlet syndrome: a case report and review of the literature.

Stroke is an exceedingly rare presentation of arterial thoracic outlet syndrome (aTOS). This report describes a case of cerebellar stroke secondary to aTOS and reviews the literature. A 56-year-old woman with no previous history of stroke or arm ischemia presented with vertigo. Computed tomography (CT) and magnetic resonance imaging confirmed a left cerebellar ischemic stroke. She subsequently developed ischemia of her left arm, which was treated by a thromboembolectomy. CT angiography revealed bilateral cervical ribs along with bilateral subclavian artery aneurysms. Staged resection of the ce

Syndrome of ichthyosis congenita, neurosensory deafness, oligophrenia, dental aplasia, brachydactyly, clinodactyly, accessory cervical ribs and carcinoma of the thyroid.

We report the case of a 15-year-old girl with a uneventful family history. Her skin condition was clinically, histologically and ultrastructurally compatible with the diagnosis of ichthyosis congenita. She suffered from neurosensory deafness and oligophrenia. Further findings included dental aplasia, brachydactyly, clinodactyly and accessory cervical ribs. At the age of 14, a thyroid carcinoma was diagnosed. Therapy with a retinoid derivative (Ro 10-9359) resulted in a marked improvement of the ichthyosis. We assume a genetic syndrome with autosomal-recessive inheritance.
