Estrogen and visual hallucinations in a patient with Charles Bonnet syndrome.

PURPOSE: To describe the occurrence of visual hallucinations in a patient with Charles Bonnet syndrome associated with estrogen intake. METHOD: Case report. RESULTS: An 84-year-old woman with poor visual acuity secondary to bilateral, nonexudative, age-related macular degeneration had nonthreatening visual hallucinations 2 weeks after starting oral estrogen for osteoporosis. The estrogen was stopped, and the hallucinations subsided. The patient was given estrogen twice more and each time the hallucinations recurred. CONCLUSION: We report a case of Charles Bonnet syndrome associated with est

EEG changes associated with hallucinations caused by Charles Bonnet Syndrome.

Charles Bonnet syndrome (CBS) is characterised by the presence of visual hallucinations following visual loss in many patients. The neuropathophysiology of CBS is poorly understood. We used electroencephalography (EEG) in individuals with frequent hallucinations in order to identify changes in neural activity that co-occur with hallucination onset. We found reduced α power in occipital electrodes at the onset of the hallucinations compared to offset (p < 0.05), suggesting that intermittent periods of low endogenous alpha may create the neural conditions for hallucination to emerge in CBS patie

Awareness of Charles Bonnet syndrome among ophthalmologists: a survey study.

PURPOSE: Charles Bonnet Syndrome (CBS) is a condition characterized by complex visual hallucinations in individuals with significant visual impairment. Despite its prevalence, CBS remains under-recognized, leading to delayed diagnoses and unnecessary distress for patients and their families. Awareness among ophthalmologists is critical for early identification and management. This study aims to evaluate the awareness and understanding of CBS among ophthalmologists and identify gaps in knowledge, diagnosis, and management practices. METHODS: A cross-sectional survey of 322 ophthalmologists was

Charles Bonnet Syndrome Prevalence in a Younger Ophtalmology Outpatient Population.

BACKGROUND: In the literature, most of the studies on Charles Bonnet Syndrome (CBS) were performed on older populations. This study aims to determine the prevalence and epidemiology of CBS and the nature of the hallucinations of the younger group of patients from an ophthalmology outpatient clinic of a university hospital. SUBJECTS AND METHODS: Twenty-eight patients were randomly chosen from the outpatient ophthalmology clinic of a university hospital, and those who agreed to participate in the study were included. First, the patients were asked if they had ever seen visions that other people

[A case of Charles Bonnet syndrome following syphilitic optic neuritis].

Charles Bonnet syndrome refers to visual hallucinations in patients with visual acuity loss or visual field loss without dementia. We report a case of Charles Bonnet syndrome following syphilitic optic neuritis. A 62-year-old man was admitted to our hospital suffering acute bilateral visual loss in a few months. On admission, he was almost blind and his optic discs were found to be atrophic on fundoscopy. In addition to increased cell counts and protein concentration in cerebrospinal fluid (CSF), serum and CSF rapid plasma reagin tests were positive. A diagnosis of syphilitic optic neuritis wa

What associates Charles Bonnet syndrome with age-related macular degeneration?

Charles Bonnet syndrome (CBS) is a condition related to patients with visual loss due to age related macular degeneration or glaucoma that are having complex visual hallucinations. The CBS was first described by Swiss physician Charles Bonnet in 1760. Affected patients, who are otherwise mentally healthy people with significant visual loss, have vivid, complex recurrent visual hallucinations (VHs). One characteristic of these hallucinations is that they usually are "Lilliputian hallucinations" as patients experience micropsia (hallucinations in which the characters or objects are distorted and

Rehabilitation in Charles Bonnet syndrome: a review of treatment options.

People with vision loss sometimes experience visual hallucinations associated with Charles Bonnet syndrome. The appearance of these hallucinations often causes anxiety for the sufferer and can be difficult for the attending eye care professional to manage. A review of the literature highlighted a range of visual, pharmacological and social management regimes that may alleviate these hallucinations, albeit using small samples in uncontrolled trials. Eye care practitioners should be aware of methods of rehabilitation in Charles Bonnet syndrome that may lead to resolution of the visual hallucinat

[Complex visual hallucinations in partially sighted elderly: Charles Bonnet syndrome].

In two patients, women aged 85 and 80 years, who suffered from visual hallucinations involving living creatures, cars and buildings, Charles Bonnet syndrome was diagnosed. Both were worried that these signs were an indication of a severe psychiatric condition. In diagnosing the Charles Bonnet syndrome the following criteria can be used: presence of complex visual hallucinations, retention of insight, absence of hallucinations in other sensory modalities and absence of delusions. Hallucinations in dementia, delirium, migraine, epilepsy, or in extreme states of exhaustion or sensory deprivation,

The Charles Bonnet syndrome: a type of organic visual hallucinosis.

The Charles Bonnet syndrome consists of visual hallucinations usually described in patients with bilateral eye pathology. The hallucinations are vivid and usually of a pleasant nature. Patients ordinarily maintain insight into the unreal nature of the visions. The syndrome appears to have an ophthalmologic and neurologic disease etiology. Treatment involves correction of the decreased visual acuity.

The Charles Bonnet syndrome. Visual perceptive dysfunction in sensory deprivation.

We present a series of four patients with the Charles Bonnet syndrome, which is characterized by recurrent vivid visual hallucinations in the presence of normal cognition and insight. It usually occurs in elderly people with ophthalmic pathology causing severe visual sensory deprivation. Our experience suggests that this syndrome is frequently overlooked or misdiagnosed by both ophthalmologists and psychiatrists. To our knowledge this is only the second series of cases of the syndrome in the American literature.

An overview of psychological and social factors in Charles Bonnet syndrome.

Charles Bonnet syndrome (CBS) is a condition where cognitively normal individuals with sight impairment experience simple and/or complex visual hallucinations. The exact pathogenesis of CBS is unknown; however, deafferentation is often recognised as a causal mechanism. Studies have provided insight into the multifaceted impact of CBS on wellbeing. Onset of CBS may cause distress among those believing visual hallucinations are indicative of a neurological condition. Hallucinatory content is often congruent with the emotional response. For example, hallucinations of a macabre nature typically re

The elephant in the room: understanding the pathogenesis of Charles Bonnet syndrome.

PURPOSE: Charles Bonnet syndrome (CBS) is a syndrome characterised by complex visual hallucinations in individuals who are cognitively normal, though often elderly and visually impaired. Although first described over 250 years ago, the condition remains poorly understood and difficult to treat. RECENT FINDINGS: Our understanding of CBS pathogenesis has advanced little since it was first described, and much of the recent literature consists of case studies strikingly similar to the first published account of CBS. However, imaging studies have provided some indication as to the cortical areas i

Charles Bonnet syndrome: evidence for a generative model in the cortex?

Several theories propose that the cortex implements an internal model to explain, predict, and learn about sensory data, but the nature of this model is unclear. One condition that could be highly informative here is Charles Bonnet syndrome (CBS), where loss of vision leads to complex, vivid visual hallucinations of objects, people, and whole scenes. CBS could be taken as indication that there is a generative model in the brain, specifically one that can synthesise rich, consistent visual representations even in the absence of actual visual input. The processes that lead to CBS are poorly unde

Of Roman chariots and goats in overcoats: the syndrome of Charles Bonnet.

Charles Bonnet syndrome (CBS) is a widely under-recognised disorder typically characterised by complex visual hallucinations in the visually impaired. The lack of consensus over a uniform definition for CBS has much to do with the unresolved pathophysiology of the disorder. A leading hypothesis proposes that complex hallucinations arise from visual association cortical areas following their de-afferentation from the central visual pathway. While treatment aimed at improving the visual deficit can limit the symptoms of CBS, at present there is no reliably effective pharmacotherapy for the disor

[The Charles Bonnet syndrome: a case report and a brief review].

The Charles Bonnet syndrome (CBS) is a rare disease that also seems to be generally misdiagnosed. Initially described in the XVIII century by the philosopher with the same name, it consist in complex visual hallucinations in elderly people who suffer sensory deprivation with no other psychopathology. The hypothesis on the neurophysiology of hallucinations suggests, as in other diseases that present hallucinations, some implication of thalamus-cortex pathway release. Some authors have proposed CBS as an early marker of dementia and Parkinson's disease. Nevertheless, the results in functional ne

The experiences of visually impaired military veterans with Charles Bonnet syndrome.

BACKGROUND: Charles Bonnet syndrome (CBS) refers to the experience of visual hallucinations occurring secondary to sight loss. Although there is an increasing amount of research on this phenomenon, CBS remains a lesser-known outcome of visual impairment, with limited research into the impact on the patient. OBJECTIVES: To explore the experiences and opinions of visually impaired military veterans with CBS regarding the impact of visual hallucinations. DESIGN: Semi-structured qualitative interviews. METHODS: Forty-six telephone interviews were conducted with members of Blind Veterans UK, a c

[Complex visual hallucinations in the visually impaired, the Charles Bonnet syndrome].

Charles Bonnet syndrome (CBS) is characterised by recurrent, complex and vivid visual hallucinations in the absence of cognitive dysfunction. Individuals with CBS usually maintain insight into the unreal nature of their hallucinatory experiences. There is a strong association between CBS and acquired visual loss and the most commonly described ocular pathology is age-related macular degeneration. However, CBS has been described in the context of visual impairment due to pathologies anywhere along the visual pathway. It is widely agreed that CBS is an underreported diagnosis caused by patients'
