Drug-Induced Liver Injury. Cholestatic Injury, Acute and Chronic.

Cholestasis is the failure of bile to reach duodenum due to three different mechanisms: a. alteration of bile secretion by hepatocytes into the canaliculus with or without liver cell damage; b. obstruction of the intrahepatic bile ducts caused by diseases of ductules or small/medium bile ducts; c. obstruction of extrahepatic bile ducts. This short review focuses on drugs which may induce cholestasis by any of these mechanisms.

[Cholestasis caused by Fasciola hepatica].

Human infestation with Fasciola hepatica is observed occasionally. Cholestasis due to parasitic obstruction of the common duct is an infrequent complication. A case of fascioliasis is described in a 56 year-old male with symptoms of biliary colic and biochemical cholestasis. The diagnosis was based on the indirect hemagglutination test and the evidence of Fasciola hepatica in the extrahepatic biliary tract shown by ultrasound and ERCP. The patient presented an acute pancreatic reaction during an attempt at therapeutic endoscopic sphincterotomy. Medical treatment was initiated then with Bithion

What is cholestasis in 1985?

Bile secretion involves a number of specific transport systems in the hepatocytes. These are located at the baso-lateral membrane (bile acid/sodium co-transport, sodium/proton exchange), intracellularly or at the canalicular membrane (bile acid secretory carrier, bicarbonate transport system). In addition, the paracellular (tight junction) pathway allows passage of inorganic ions and waters. Cholestasis may result from an alteration of these transport systems (hepatocellular cholestasis), from obstruction of intrahepatic bile ducts or obstruction of extrahepatic bile ducts. The main proposed m

Drug disposition in cholestasis: An important concern.

Cholestasis, a chronic liver condition, disrupts bile acid homeostasis and complicates drug disposition, posing significant challenges in medicating cholestatic patients. Drug metabolism enzymes and transporters (DMETs) are pivotal in drug clearance. Research indicates that cholestasis leads to alterations in both hepatic and extrahepatic DMETs, with changes in expression and function documented in rodents and humans. This review synthesizes the modifications in key drug disposition components within cholestasis, focusing on cytochrome P450 (CYP450), drug transporters, and their substrates. Ad

[Transient hypoplasia of the intrahepatic bile ducts. Clinical and morphologic studies].

Two boys were thriving normally up to the age of three and nine weeks respectively when obstructive cholestasis developed. Liver biopsy revealed paucity of intrahepatic bile ducts. Extrahepatic bile ducts were not occluded. In both cases cholestasis completely disappeared after five and nineteen weeks respectively. Control liver biopsis showed normal numbers of intrahepatic bile ducts. We conclude that paucity of intrahepatic bile ducts may be transient.

Use of rifampin for severe pruritus in children with chronic cholestasis.

BACKGROUND: Rifampin has been proposed to reduce pruritus in children and adults with chronic cholestasis; however, there is a paucity of published data regarding the use of rifampin in children. METHODS: In an open trial, 24 children were evaluated during a 6-year period. Diagnoses included 13 patients with extrahepatic biliary atresia (54%), six with Alagille's syndrome, three with Byler's disease, and one each with primary sclerosing cholangitis and alpha1-antitrypsin deficiency. All patients had severe pruritus that had not responded adequately to at least 2 months of therapy with ursodeo

The role of endoscopic retrograde cholangiopancreatography in cholestatic infants.

Conjugated hyperbilirubinemia in infants is a serious problem, and one that requires further investigation. Early differentiation between extrahepatic and intrahepatic causes of cholestasis is essential for the proper management of cholestasis in infancy. A successful outcome in patients with extrahepatic biliary atresia is significantly influenced by the age at which the Kasai procedure (hepatoportoenterostomy) is performed. However, extensive diagnostic workup may fail to distinguish definitively between intrahepatic and extrahepatic disease, and exploratory laparotomy and intraoperative cho

Primary Sclerosing Cholangitis in a Young Female.

Primary sclerosing cholangitis (PSC) is a liver disease characterized by inflammation and fibrosis of both intrahepatic and extrahepatic bile ducts resulting in cholestasis. Due to nonspecific symptoms it is difficult to diagnose until complication arises. It is common in male and usually associated with other autoimmune diseases. Here, we report a case of PSC in a young female which was initially thought to be drug induced cholestasis without presence of any other autoimmunity.

Pathogenesis and medical therapy of primary sclerosing cholangitis. Any news?

Primary sclerosing cholangitis is characterized by inflammation and fibrosis of the intra- and extrahepatic bile ducts. Medical therapy has focused on anticholestatic, antiinflammatory and immunosuppressive drugs. Although inflammation, fibrosis and cholestasis may all occur at the same time, inflammation dominates the early phase and fibrosis and cholestasis the later stages. With inflammatory bowel disease and rheumatoid arthritis as stimulants, research on anti-inflammatory drugs is in a very active stage, with an emphasis on cytokines and cytokine antagonists. For patients with primary scl

[Diagnosis and treatment of intrahepatic cholestasis syndrome].

Prompted by the histomorphological aspect of ductopenia, chronic intrahepatic liver diseases are increasingly being subsumed under the term vanishing bile duct syndrome. Classification by cholestasis syndromes in adults (e.g. primary biliary cirrhosis, primary sclerosing cholangitis) and in the newborn or children (e.g. alphal antitrypsin deficiency, cystic fibrosis) makes good sense. Decisive for the diagnosis are, depending on the disease presenting, a typical constellation of laboratory results, detection of autoantibodies, imaging procedures (e.g. ERC, MRI), liver biopsy where indicated, o

Persistent cholestasis resulting from duodenal papillary carcinoma in an adolescent male: A case report.

RATIONALE: Cholestasis in pediatric patients has diverse etiologies and can be broadly classified as intrahepatic or extrahepatic. The common causes of extrahepatic cholestasis are bile duct calculus, inflammation, or pancreatitis. Malignant tumor is a rare cause of bile ducts obstruction in adolescent. Here we report a 14-year-old male patient with cholestasis due to poorly differentiated adenocarcinoma. PATIENT CONCERNS: A 14-year-old male patient with cholestasis was admitted because of jaundice, weakness, weight loss, and stomach pain for 2 months. The patient had been diagnosed with epil

Covered versus uncovered double bare self-expandable metal stent for palliation of unresectable extrahepatic malignant biliary obstruction: a randomized controlled multicenter trial.

BACKGROUND AND AIMS: In a recent randomized controlled trial, a double bare metal stent (DBS) showed better stent patency than single-layer metal stents. However, clear evidence comparing the efficacy of uncovered (UCDBS) and partially covered (PCDBS) DBSs for distal malignant biliary obstruction (MBO) is lacking. Therefore, we compared the clinical outcomes including stent patency of UCDBSs versus PCDBSs. METHODS: A multicenter, randomized study was performed in patients with distal MBO. The primary endpoint was stent patency. Secondary endpoints were the proportion of patients with patent s

[Status of ultrasound diagnosis in differential cholestasis diagnosis].

Differentiating between the hepatocellular "internal" and the "surgical" or "endoscopic" cholestasis syndrome has become a domain of sonography. There is a 92 to 99% success rate, due to the possibility of diagnosing by means of sonography dilations of the gall bladder, the extrahepatic biliary tract and the intrahepatic bile ducts, as well as numerous diseases of the hepatobiliary system and of the pancreas. Congested intra- and extrahepatic bile ducts indicate the presence of a deep-seated distal obstruction, such s a prepapillary concrement, an inflammatory or malignant papillary stenosis o

Drug-induced cholestasis.

Acute, drug-induced hepatocellular cholestasis (either pure or cholestatic hepatitis) is a common manifestation of drug-induced hepatic injury. The drugs most frequently responsible are hormonal steroids and psychopharmacological agents (in particular phenothiazines and some antidepressants). Cholestasis usually subsides without sequelae in less than six months. Acute, drug-induced ductular cholestasis is uncommon and can resemble biliary tract obstruction. Complete recovery occurs promptly after the withdrawal of the causative drug in most cases. The pathogenetic mechanism may be immunoallerg

Hepatobiliary imaging using Tc-pyridoxylideneglutamate in the diagnosis of obstructive jaundice.

In 77 of 114 consecutive patients with suspected hepatobiliary disease undergoing cholescintigraphy a firm clinical or operative diagnosis was possible. These patients were classified as normal, or as having extra-hepatic biliary obstruction (partial or complete) or hepatocellular disease. On a double-blind basis cholescintigraphy correctly interpreted 18 of 20 (90%) normal controls, 12 of 14 (86%) of those with partial obstruction, 16 of 16 (100%) of those with complete obstruction and 23 of 27 (85%) of those with hepatocellular disease giving an overall diagnostic accuracy of 69 of 77 (90%).

[LP-X in the first month of life (author's transl)].

An abnormal lipoprotein can be detected in the sera of patients suffering from intra- and extrahepatic cholestasis. The specificity of LP-X detection for the diagnosis of cholestasis in adult persons was proved by extensive investigations. The aim of this study was to examine the specificity of LP-X with regard to cholestasis in the postnatal phase of premature and newborn infants. 1056 sera of 270 newborns were tested and in 75% of these cases LP-X was found to be positive. Three hypothetical explanations are given concerning the appearance of LP-X in the sera of premature and newborn infants

Drug-induced cholestasis.

Intrahepatic cholestasis, defined as arrested bile flow, mimics extrahepatic obstruction in its biochemical, clinical and morphological features. It may be due to hepatocyte lesions of which there are three types, termed canalicular, hepatocanalicular and hepatocellular, respectively; or it may be due to ductal lesions at the level of the cholangiole or portal or septal ducts. Defective bile flow due to hepatic lesions reflects abnormal modification of the ductular bile. Defective formation of canalicular bile may involve bile acid-dependent or independent flow. It appears to result most impor

[Transient dilatation of intra- and extra-hepatic bile ducts. A case].

A case of transient dilatation of intra and extrahepatic bile ducts in the course of an infectious disease is described in a three-year-old girl. The ultrasonic bile duct abnormalities disappeared after three months. The mechanism involved remains uncertain; the hypothesis of a transient inflammatory obstruction is evoked.

Left lobe of the liver: percutaneous biliary drainage.

Left-sided biliary drainage was performed in 89 of 678 (13%) patients undergoing percutaneous biliary drainage procedures during a 9-year period. The most common indication was Klatskin tumor, followed by sclerosing cholangitis, carcinoma of the gallbladder, postoperative benign stricture, and metastasis to the porta hepatis. Left-duct drainage was successful in 82 of the 89 patients (92%) in whom it was attempted. One patient sustained a major hemorrhage from a hepatic artery aneurysm, which was managed with transcatheter embolization.
