Patient perception of chronic illness care in a large inflammatory bowel disease cohort.

BACKGROUND: Improvements in care for inflammatory bowel diseases could use the Chronic Care Model, an evidence-based approach that has improved patient outcomes and reduced costs in other illnesses. Specific aims include (1) to explore patient perception of chronic illness care in a large inflammatory bowel disease cohort and (2) to determine whether demographic factors, medication adherence, quality of life, disease type, and activity were associated with perception of chronic illness care. METHODS: We randomly selected 1000 participants from the Crohn's and Colitis Foundation of America Par

The Chronic CARe for diAbeTes study (CARAT): a cluster randomized controlled trial.

BACKGROUND: Diabetes is a major challenge for the health care system and especially for the primary care provider. The Chronic Care Model represents an evidence-based framework for the care for chronically ill. An increasing number of studies showed that implementing elements of the Chronic Care Model improves patient relevant outcomes and process parameters. However, most of these findings have been performed in settings different from the Swiss health care system which is dominated by single handed practices. METHODS/DESIGN: CARAT is a cluster randomized controlled trial with general practi

Assessing eligibility of Medicare beneficiaries with chronic conditions for the ACCESS model.

INTRODUCTION: The Advancing Chronic Care with Effective Scalable Solutions (ACCESS) model is a new outcome-aligned payment model intended to support technology-enabled chronic disease management among Traditional Medicare (TM) beneficiaries. However, the national population potentially eligible for ACCESS remains poorly characterized, particularly those with multiple chronic conditions. METHODS: Using nationally representative survey data from the 2023 Medical Expenditure Panel Survey, we described the health characteristics and health care utilization patterns of TM beneficiaries potentially

Vertical integrated service model: an educational intervention for chronic disease management and its effects in rural China - a study protocol.

BACKGROUND: Chronic diseases are becoming a huge threat to the Chinese health system. Although the New Round of Medical Reform aims to improve this, the chronic disease management in rural China is still worrying as it relies highly on hospital care instead of primary care. The vertical integrated care model has proven to be effective for chronic disease patients in many high-income countries, while few studies have been conducted in China. In this project, vertical integrated care will be applied to optimize the care of patients with type 2 diabetes mellitus (T2DM) and primary hypertension in

Cost-Effectiveness of a Lifestyle and Behavioral Care Model Targeting Cardiometabolic Disease Progression.

Chronic diseases drive healthcare costs, and employers seek scalable strategies to improve health outcomes and control expenses. Telehealth behavioral care shows promise for managing chronic conditions, but its long-term economic value in employer populations is still unclear. We assessed the cost-effectiveness and ROI of a behavioral care (LBC) model using a Markov model in a custom analytic tool. The model simulated disease progression, healthcare utilization, and QALYs over five years from the employer perspective. Transition probabilities, costs, and mortality risks were obtained from the

The after discharge care management of low income frail elderly (AD-LIFE) randomized trial: theoretical framework and study design.

Interdisciplinary care management is advocated for optimal care of patients with many types of chronic illnesses; however, few models exist that have been tested using randomized trials. The purpose of this report is to describe the theoretical basis for the After Discharge Management of Low Income Frail Elderly (AD-LIFE) trial, which is an ongoing 2-group randomized trial (total n = 530) to test a chronic illness management and transitional care intervention. The intervention is based on Wagner's chronic illness care model and involves comprehensive posthospitalization nurse-led interdiscipli

Community ownership of biopsychosocial model of care: a qualitative study in the Katana health district, Democratic Republic of Congo.

BACKGROUND: The WHO recommends the Person-Centred Care approach, based on the biopsychosocial (BPS) model with community participation, to streamline the management of chronic diseases in Primary Health Care (PHC) activities and reduce their growing burden, even in Africa. The Democratic Republic of Congo's (DRC) experience of community participation in implementing the BPS model for chronic diseases has been little explored. OBJECTIVE: To describe community ownership of the biopsychosocial model of chronic disease care in PHC facilities in South Kivu province, DRC. METHODS: A basic interpre

The Need for Comprehensive Care for Persons with Chronic Immune Thrombocytopenic Purpura.

Chronic platelet disorders (CPD), including chronic immune thrombocytopenic purpura (cITP), thrombotic thrombocytopenic purpura (TTP) and platelet function disorders are among the most common bleeding disorders and are associated with morbidity and mortality. The clinical phenotype and complexity of cITP is much like that of hemophilia. In cITP and hemophilia, bleeding is problematic for many, complicating employability, insurability and overall quality-of-life (QoL). While myriad drug therapies are available for cITP and hemophilia, each are variable in their effectiveness, very few (except f

Payment Model Evolution for Spinal Fusions.

Value-based payment reform in the United States is rapidly evolving, with new models from the Centers for Medicare & Medicaid Services targeting a range of inpatient and outpatient procedures. To trace this evolution, we examine spinal fusion-a procedure consistently included in past and emerging models-as a lens to compare model characteristics, payment structure, quality measurement, and primary care integration. We analyze Bundled Payments for Care Improvement-Advanced (BPCI-A, 2018 to 2025), Transforming Episode Accountability Model (TEAM, launched in 2026), Advancing Chronic Care with Eff

The Embedded Model for Bridging Essential Delivery of Care in Assisted Living Facilities-EMBED-ALF.

Older adults residing in assisted living facilities (ALFs) often face fragmented medical care, especially when managing complex chronic conditions, cognitive impairment, or end-of-life needs. This article describes an interprofessional provider-led, integrated care model embedded within an ALF in a major metropolitan area that includes memory care and home hospice. The model integrates geriatrics and palliative medicine-trained providers into routine care, supported by structured communication, tele-emergency services, and collaboration with hospice and hospital teams. While formal outcomes ar

The contribution of respiratory microbiome analysis to a treatable traits model of care.

The composition of the airway microbiome in patients with chronic airway diseases, such as severe asthma, chronic obstructive pulmonary disease (COPD), bronchiectasis and cystic fibrosis (CF), has the potential to inform a precision model of clinical care. Patients with these conditions share overlapping disease characteristics, including airway inflammation and airflow limitation. The clinical management of chronic respiratory conditions is increasingly moving away from a one-size-fits-all model based on primary diagnosis, towards care targeting individual disease traits, and is particularly

Self-Care Processes as Predictors of Quality of Life in Adults With Moyamoya Disease: A Structural Equation Modeling Study.

Background: Moyamoya disease is a chronic cerebrovascular condition requiring ongoing self-management to prevent complications. Riegel's theory of self-care in chronic illness posits that factors such as disease knowledge, self-efficacy, social support, and access to care influence self-care maintenance, monitoring, and management, which in turn affect health outcomes and quality of life (QoL). Aim: To test a structural equation model of factors influencing self-care and outcomes in adults with moyamoya disease based on Riegel's self-care theory. Methods: A cross-sectional survey of 211 adults

The Lived Experience of Pain Services: A Comparison of Service Users' and Service Providers' Experience of Irish Health Services.

Background: A supportive healthcare experience that implements a biopsychosocial model of care can empower a person with chronic pain to make informed decisions and engage in self-management behaviours. Despite the positive influence of supportive healthcare, little is known about the presence of healthcare support in under-resourced chronic pain services. This idiographic study explores the lived experience of service users and providers participating in chronic pain services with a specific focus on autonomy support and self-management skills.
