Open versus arthroscopic distal clavicle resection.

PURPOSE: The purpose of this systematic review was to critically evaluate the available literature in an attempt to compare the outcome of open versus arthroscopic distal clavicle resection in the treatment of acromioclavicular joint pathology. METHODS: From January 1966 to December 2008, Medline was searched for the following key words: "acromioclavicular joint arthritis," "acromioclavicular osteolysis," "distal clavicle excision," "acromioclavicular joint excision," "Mumford," and "clavicle." Inclusion criteria included studies that compared the outcome of open versus arthroscopic distal cl

Complications after open distal clavicle excision.

Isolated distal clavicle excision performed as an open procedure has been considered safe and, in the literature, has been considered the standard for comparison with arthroscopic distal clavicle excisions. However, we noticed isolated open distal clavicle excision was associated with a number of complications. We therefore raised two questions about the complication rate in a cohort of our patients who had undergone this procedure: (1) What was the complication rate and how did it compare to that in the existing literature on this subject? and (2) Were the complications in our cohort similar

Development of the Utrecht Score for clavicle fractures: a short and complete clavicle score with patient-reported and objective measures.

PURPOSE: The purpose of this study was to develop a clavicle-specific questionnaire with patient-reported and objective measures. METHODS: The present study used data of DASH and Constant scores from a previously performed randomized-controlled trial comparing plate and intramedullary pin fixation of clavicle fractures. Exploratory factor analysis was used to identify the most relevant items and the underlying structure of the questionnaires. To optimize the applicability to patients with a clavicle fracture, the selected items were reformulated. If relevant themes were underexposed, an addit

Transfer of the coracoid and conjoined tendon for failed, unstable, short clavicle following excessive outer clavicle resection.

Nineteen patients presented with an unsightly deformity of the shoulder, pain or discomfort at the medial scapular border, pseudo nonparalytic scapular winging, and thoracic outlet symptoms after excessive resection of the clavicle for either complete acromioclavicular separation or displaced fracture of the outer clavicle, which allows the scapula and shoulder to rotate anteriorly and inferiorly on the chest creating traction on the medial scapular muscles and the brachial plexus resulting in pseudo nonparalytic winging and thoracic outlet symptoms. RESULTS: All underwent transfer of the cora

Virtual surgical planning and mirrored, 3-dimensionally printed guides for corrective clavicle osteotomies in clavicle malunions and nonunions.

BACKGROUND: The objective of this study was to retrospectively review clinical and radiographic outcomes of patients who underwent corrective osteotomies for clavicle malunion and internal fixation for nonunion using a combination of virtual surgical planning, patient-specific 3-dimensional (3D)-printed clavicles, and 3D-printed cutting guides manufactured at the point of care. METHODS: Between 2015 and 2021, 18 patients underwent corrective osteotomy for a clavicle malunion (7 shoulders) or internal fixation for a clavicle nonunion (11 shoulders). There were 11 male and 7 female individuals

Conceptual finite element study for comparison among superior, anterior, and spiral clavicle plate fixations for midshaft clavicle fracture.

Open reduction internal fixation technique has been generally accepted for treatment of midshaft clavicle fractures. Both superior and anterior clavicle plates have been reported in clinical or biomechanical researches, while presently the spiral clavicle plate design has been introduced improved biomechanical behavior over conventional designs. In order to objectively realize the multi-directional biomechanical performances among the three geometries for clavicle plate designs, a current conceptual finite element study has been conducted with identical cross-sectional features for clavicle pl

Hydropneumothorax in an Isolated Midshaft Clavicle Fracture.

BACKGROUND: Clavicle fractures are the fourth most common fracture. These fractures are either managed conservatively or surgically. Pneumothorax is a rare complication of clavicle fractures and potentially can be missed if this rare complication is not known. CASE REPORT: A 76-year-old female presents with a 2-day history of pain and bruising over the right clavicular prominence following a fall from a standing height. Subsequent radiographs show the development of pneumothorax, despite having no respiratory symptoms. DISCUSSION: Pneumothorax as a complication of isolated clavicle fractures

Treatment of a Recalcitrant Non-union of the Clavicle.

BACKGROUND: Plate fixation is the treatment of choice for a midshaft clavicle non-union. Those non-unions that require >1 surgical procedure to heal are termed recalcitrant non-union. Regardless of the number of previously failed procedures, our surgical strategy is aimed at achieving an optimal mechanical and biological environment to facilitate healing. MATERIALS AND METHODS: We performed a retrospective analysis of 14 patients with a recalcitrant clavicle non-union treated with open reduction and plate fixation combined with graft augmentation when indicated. Healing rates after index surg

The emergency medicine management of clavicle fractures.

BACKGROUND: Clavicle fractures are common. An emergency physician needs to understand the diagnostic classifications of clavicle fractures, have a plan for immobilization, identify associated injuries, understand the difference between treating pediatric and adult patients, and have an approach to multimodal pain control. It is also important to understand when expert orthopedic consultation or referral is indicated. OBJECTIVE OF THE REVIEW: To provide an evidence-based review of clavicle fracture management in the emergency department. DISCUSSION: Clavicle fractures account for up to 4% of

Emphysematous osteomyelitis of the clavicle: a pleural process?

Emphysematous osteomyelitis (EO) is a rare infection associated with intraosseous gas. EO is an often fatal disease with an estimated 34% mortality. We present a case of a 63-year-old man with sternoclavicular EO with pleural involvement and significant subcutaneous emphysema diagnosed by CT. Extension of intraosseous gas into the pleural cavity is an extremely interesting presentation that has not been previously reported. The patient underwent a multidisciplinary treatment approach with surgical debridement and an extended antibiotic course. Intraoperative cultures of the pectoralis muscle a

Structural effects of variation in the human clavicle.

OBJECTIVES: Purported evolutionary shifts in shoulder structure have been linked to changes in hominin behavior and adaptation. Researchers use clavicle morphology to infer these shifts. However, there is a lack of empirical data underlying such predictive relationships. This study investigates how clavicle morphology affects articulated shoulder girdle and upper thorax configuration in humans. MATERIALS AND METHODS: Landmarks and scalar measurements on the clavicle, scapula, and ribs 1-3 were collected from three-dimensional computed tomographic scans of living humans. Covariation between di

Management of Midshaft Clavicle Fractures in Adults.

Fractures of the clavicle are common injuries that occur across all age groups but are most frequently seen in the young, active patient population. Among the different types of clavicle fractures, those occurring in the middle third of the clavicular shaft are the most common. Historically, most of these fractures were treated by closed means even when notable displacement was present. Recently, there has been a renewed interest in assessing the best treatment option for these patients. Although nonsurgical treatment is a reliable method for treating many of these fractures, more recent data

Diagnosis and Management of Distal Clavicle Osteolysis.

Distal clavicle osteolysis is an uncommon condition that most commonly affects weight lifters and other athletes who perform repetitive overhead activity. Although this condition most commonly presents in young active men, it is becoming increasing more common in women with the rise in popularity of body building and extreme athletics. Distal clavicle osteolysis can be debilitating, especially in those with rigorous training regimens, preventing exercise because of pain with activities such as bench presses and chest flies. Aside from a careful history and physical examination, radiographic ev

Plating of a periarticular medial clavicle fracture.

This article describes a case of a 23-year-old man who sustained a far medial fracture dislocation of the medial clavicle. We performed an open reduction and internal fixation of the fragment back to the remaining medial fragment using a small locking plate. This fracture can be problematic and usually requires reduction due to the potential vascular or airway compromise. Because the fracture often occurs at the medial physis, it is difficult to secure in place to prevent displacement. Historically, fixation of this fracture has been problematic with sutures and K-wires. Failure of fixation an

Stress fracture of the clavicle in a collegiate diver.

PURPOSE: This is the first reported case of a stress fracture of the clavicle in a collegiate diver. CASE SURVEY: A 20-year-old collegiate platform diver developed left clavicular pain after training for several months. Radiographs and radionuclide bone scan supported a diagnosis of a stress fracture of the left midclavicle. Complete fracture healing followed a period of active rest. DISCUSSION: A literature review of clavicle stress fractures is discussed. The open-hand diving technique used by this diver for water penetration may have caused repetitive stresses to radiate up the arm to the

Aneurysmal bone cyst of the clavicle in a child.

We report a case of an aneurysmal bone cyst (ABC) of the clavicle in a 9-year-old boy, which initially presented as a pathological fracture of a benign cystic lesion. ABC of the clavicle is rare in children less than 10 years old and radiological diagnosis may prove difficult in the early stages of presentation.

Atraumatic osteolysis of the distal clavicle. A review.

Atraumatic osteolysis of the distal clavicle (AODC) in athletes is a stress failure syndrome of the distal clavicle. It is related to intolerable exercise doses. For some athletes, the acromioclavicular joint is the weak link in the musculoskeletal system. There is never a history of a major injury to the acromioclavicular joint. It occurs principally in young athletes who have a long history of training and performance. It is further characterised by athletes who generally have an associated intense strength training programme. The condition will inexorably progress to decrease the level of p

Nonunion of the clavicle and thoracic outlet syndrome.

Fifteen adult patients with clavicular nonunions were evaluated over a 10-year period. Fourteen of these patients were treated initially with a "figure-of-eight" clavicle strap. Seven presented with narrowing of the costoclavicular space, and symptoms of intermittent brachial plexus impingement or thoracic outlet syndrome. Two of these resulted from resection of the mid-portion of the clavicle with subsequent impingement from hypertrophy of the lateral stump. Most commonly, hypertrophic nonunion of the clavicle caused the thoracic outlet syndrome, frequently many months after the initial injur
