We emphasize the benefits of a multidisciplinary team approach to the systemic management of patients with cleft lip and cleft palate (CL/CP) and their families. An ideal team offers support to families at each stage of their child's development and adapts to address new issues that are encountered throughout growth. We suggest the addition of a registered dietitian to the initial phase of the care team to ensure the child consumes adequate macronutrient intake, and to maximize growth and development. An ideal cleft palate care team focuses on a comprehensive, collaborative, and family-centere

Severe periodontitis lesions can harbor several hundred-thousand copies of active cytomegalovirus, and this paper proposes that cytomegalovirus in maternal periodontitis can infect the fetus. Cleft lips and palates may be oral examples of congenital cytomegalovirus infection. Anti-cytomegalovirus periodontal treatment is indicated for high-risk women who exhibit severe periodontitis and weakened immune system and are contemplating pregnancy or are in the first trimester of pregnancy.

INTRODUCTION: Currently, there is no consensus regarding the role of opioids in the management of perioperative pain in children undergoing cleft lip/palate repair.
METHOD: The present study evaluated opioid prescribing patterns of surgeon members within the American Cleft Palate-Craniofacial Association surgeons utilizing an anonymous survey.
RESULTS: Respondents performing cleft lip repair typically operate on patients 3 to 6 months of age (86%), admit patients postoperatively (82%), and discharge them on the first postoperative day (72%). Comparatively, respondents performed palatoplasty

OBJECTIVE: We conducted a comprehensive review of state laws and regulations that require private health insurance plans to cover the services needed by children born with cleft lip and/or cleft palate (CL/P). The goal is to better understand how states are reducing the barriers children with CL/P face when seeking recommended health care services.
DESIGN: We identified all state laws and regulations mandating insurance coverage of services for children with CL/P by private insurance carriers from 1999 through 2017 using Westlaw legal database. We categorized laws and regulations into ten ser

Cleft palate following cleft lip may include a developmental disorder during palatogenesis. CL/Fr mice fetuses, which develop cleft lip and palate spontaneously, have less capability for in vivo cell proliferation in palatal mesenchyme compared with CL/Fr normal fetuses. In order to know the changes of signaling molecules contributing to cleft palate morphogenesis following cleft lip, the mRNA expression profiles were compared in palatal shelves oriented vertically (before elevation) in CL/Fr fetuses with or without cleft lip. The changes in mRNA profile of cleft palate morphogenesis were pres

Real-time ultrasonography in pregnancy can accurately detect gestational age, position of the fetus, position of the placenta, sex of the fetus, and many congenital anomalies before birth. To date, however, there has been no report of detection of a cleft lip-palate deformity by ultrasound. Two cases, in approximately 200 scans, have been found in patients with no previous family history of cleft lip-palate. The first case was a bilateral cleft lip-palate visualized at 28 weeks gestation. The second was a unilateral cleft lip-palate detected at 33 weeks gestation. Diagnosis of cleft lip-palate

PURPOSE OF REVIEW: Cleft lip repair requires multidisciplinary follow-up throughout a child's life and often requires lip revision surgery in adolescence to restore function and symmetry of the lip. There is significant variability in the approaches taken for lip repair and therefore a review of current techniques and subsequent guidance to secondary cleft lip repair is warranted.
RECENT FINDINGS: New methods of secondary reconstruction can be divided into superficial or muscle related. Recent suggestions for superficial reconstruction include botulinum toxin injection, silicone gel sheeting,

PURPOSE OF REVIEW: The optimal timing and techniques utilized to address the nasal deformity of the cleft lip continue to raise challenges and debate for the surgical community. There has been a shift in the paradigm addressing the nasal deformity to a more proactive approach. The goal of this article is to provide an update of the latest techniques for primary cleft rhinoplasty.
RECENT FINDINGS: A medical literature search was performed specifically targeting primary cleft rhinoplasty in order to review the current strategies implemented, including presurgical orthopedics, surgical incisions

The timing of repair of a cleft lip continues to be debated. One of the reasons for delaying operation has been the belief that there is an increased risk of anaesthesia during the neonatal period. As a contribution to this debate we have analysed the anaesthetic and surgical complications of neonatal cleft lip repair undertaken at a single centre. We made a retrospective study of 99 babies consecutively referred for repair of cleft lip over a five-year period (January 1995-December 1999). In contrast to other series, all babies were considered for neonatal surgery and no exclusion criteria we

Midline cleft of the lower lip is defined as a midline vertical cleft of the soft tissue of the lower lip. It may present with a midline cleft of the mandible. It may also be accompanied by other congenital anomalies such as a cleft tongue, ankyloglossia, a heart lesion, and absence of the hyoid bone. The etiologic cause is thought to be a failure of mesodermal penetration into the midline structures of the first branchial arch. This case report is on a female child who presented with an incomplete midline cleft of the soft tissue of the lower lip. It was surgically corrected with a vertical w

We report a case of Nager acrofacial dysostosis, whose clinical examination disclosed some undescribed features, as a contribution to a better delineation of this syndrome. Previously unreported features include lobulated tongue, cleft lip and mental retardation. Fetal alcohol syndrome is discussed. We propose to reunificate Richieri-Costa syndrome and Nager syndrome.

This is a modification of Millard's rotation-advancement principle in closing unilateral lip clefts, published in 1971. In selected cases, if there is enough soft tissue at the base of the columella to fill the defect between the columella base and the rotated short part of the philtrum, a slightly curved scar will be formed resembling the normal philtral column. From 1967 till the end of 1981, out of a total number of 1305 unilateral cleft lip repairs, 189 were operated in this manner using the described modification of Millard's technique with good results. For most of the other cases and es

LEARNING OBJECTIVES: After studying this article, the participant should be able to: 1. Understand the embryologic and anatomical basis of the nose in a unilateral cleft lip. 2. Identify key challenges in achieving nasal symmetry and balance in cleft rhinoplasty coinciding with primary lip reconstruction. 3. Evaluate the advantages and limitations of different surgical approaches to the nasal tip, including open, closed, and conservative techniques. 4. Recognize the role of septoplasty, alar base repositioning, and nasal tip manipulation in improving outcomes. 5. Appreciate the importance of p

Unilateral cleft lip is a common congenital defect of the head and neck, which ranges from incomplete to complete forms, with or without cleft palate. Early intervention is crucial to address feeding difficulties and unwanted sequalae. Advanced ultrasonography enables prenatal diagnosis and coordinated planning with multidisciplinary cleft repair teams. Postnatal care includes feeding strategies, growth monitoring, and early interventions like nasoalveolar molding. In this article, surgical techniques, such as Millard, modified Millard or Mohler, and Fisher, are highlighted. Emphasis is placed
