The Cleft palate-craniofacial journal : official publication of the American Cleft Palate-Craniofacial Association2021eng
INTRODUCTION: Currently, there is no consensus regarding the role of opioids in the management of perioperative pain in children undergoing cleft lip/palate repair.
METHOD: The present study evaluated opioid prescribing patterns of surgeon members within the American Cleft Palate-Craniofacial Association surgeons utilizing an anonymous survey.
RESULTS: Respondents performing cleft lip repair typically operate on patients 3 to 6 months of age (86%), admit patients postoperatively (82%), and discharge them on the first postoperative day (72%). Comparatively, respondents performed palatoplasty

OBJECTIVE: We conducted a comprehensive review of state laws and regulations that require private health insurance plans to cover the services needed by children born with cleft lip and/or cleft palate (CL/P). The goal is to better understand how states are reducing the barriers children with CL/P face when seeking recommended health care services.
DESIGN: We identified all state laws and regulations mandating insurance coverage of services for children with CL/P by private insurance carriers from 1999 through 2017 using Westlaw legal database. We categorized laws and regulations into ten ser

Cleft palate following cleft lip may include a developmental disorder during palatogenesis. CL/Fr mice fetuses, which develop cleft lip and palate spontaneously, have less capability for in vivo cell proliferation in palatal mesenchyme compared with CL/Fr normal fetuses. In order to know the changes of signaling molecules contributing to cleft palate morphogenesis following cleft lip, the mRNA expression profiles were compared in palatal shelves oriented vertically (before elevation) in CL/Fr fetuses with or without cleft lip. The changes in mRNA profile of cleft palate morphogenesis were pres

Cleft lip and palate are among the most common congenital anomalies in humans. The treatment of this group of patients is best conducted by a multidisciplinary team approach. This article discusses the accepted treatment algorithm and timeline, as well as special considerations for this patient group when performing orthognathic surgery. Patients with cleft lip and palate often present with significantly more technical and challenging procedures, so clinicians should familiarize themselves with these special considerations before attempting to care for these individuals.

Orthodontic treatment of children with cleft palates continues through the periods of the three dentitions: temporary, mixed, and adult. Using examples, this paper deals with the different difficulties that can be encountered during treatment of this malformation. Orthodontic interception at the time of the temporary dentition corrects the heart of the problem, the transverse insufficiency, but also addresses moderate maxillary retrusion. Cooperation with a speech therapist at this stage is essential. In the mixed dentition, orthodontists correct incisal malalignment and, depending upon the se

Real-time ultrasonography in pregnancy can accurately detect gestational age, position of the fetus, position of the placenta, sex of the fetus, and many congenital anomalies before birth. To date, however, there has been no report of detection of a cleft lip-palate deformity by ultrasound. Two cases, in approximately 200 scans, have been found in patients with no previous family history of cleft lip-palate. The first case was a bilateral cleft lip-palate visualized at 28 weeks gestation. The second was a unilateral cleft lip-palate detected at 33 weeks gestation. Diagnosis of cleft lip-palate

Modern neonatal care and advanced plastic surgical correction have led to the survival of most newborns with oral clefts. These children are likely to reproduce. A slight increase in the incidence of oral clefts may be expected in the future. The genetics of cleft lip and cleft palate is reviewed. The inheritance is usually multifactorial. With normal parents the risk of having a first affected child with cleft lip is about one per thousand, the risk of having a second affected child 4 per cent and the risk of having a third affected child 10 per cent. If a parent has already a cleft lip, the

Ninety-three girls with cleft palate (53 submucous cleft palate (SMCP) and 40 isolated cleft palate (ICP)), mean age 6.2 years (range 5.5-7.5), were compared retrospectively from lateral cephalograms. Forty-three patients with SMCP had had surgical treatment at the mean age of 3.4 years (range 0.9-6.8), 10 of the SMCP patients were unoperated. Twenty-six patients with ICP had clefts of the hard and soft palate, and 14 had clefts of the soft palate only. Palatal closure of ICP had been done at the mean age of 1.5 years (range 1.0-2.1). The skeletal craniofacial morphology was similar in SMCP an

UNLABELLED: Although cleft lip and cleft palate are among the most common congenital malformations, the presence of an isolated congenital palatal fistula along with a submucous cleft is very rare. This appears as an oval-shaped, full-thickness fenestration in the palatal midline that does not fully extend anteriorly or posteriorly, accompanied by the findings of a submucous cleft. Because of the uncommon nature of this entity, there is controversy about its etiology, diagnosis, and management.
METHODS: Two cases of children with congenital palatal fistulae and a submucous cleft palate are pr

Cleft palate repairs often require secondary surgeries and/or revisions for a variety of reasons. The most common causes are symptomatic oronasal fistulas and velopharyngeal insufficiency. Complications from primary surgery, such as wound dehiscence, infection, and hematomas, contribute to the relatively high rate of revision surgery. Prevention of postoperative complications that may lead to fistula or velopharyngeal insufficiency is key, and many techniques have been described that have reportedly decreased the incidence of secondary surgery. Management varies depending on the nature of the

Dealing with this type of malformation, the orthodontist is confronted with a three-level problem: inter-personal, social, and technical. Because families consider the birth of a cleft palate child to be a disaster, orthodontists who undertake their treatment should understand the genesis of the turbulence that families will have to endure. Most parents learn to deal with these difficulties, but the affected children are going to have to submit to the stares of those they encounter. Adolescence is a critical period for them and it is also the time when they will visit their orthodontist most f

Cleft lip with or without cleft palate (CL/CP) is one of the most common structural birth defects, with treatment including multiple surgeries, speech therapy, and dental and orthodontic treatments over the first 18 years of life. As a result of the treatment interventions, pediatric nurses may be required to care for these children beginning in infancy through adolescence. Providing care for these patients and families can include educating patients and parents about the genetics of CL/CP, as well as meeting their immediate medical needs. A basic overview of normal lip and palate development,

In this study, the variation in palatal dimensions, particularly for height, width, and depth, were evaluated in subjects with different types of cleft palate using the moiré technique. The sample consisted of 95 subjects with cleft palate and 68 noncleft individuals. Effects of cleft type, gender, developmental stage of the dentition, missing teeth, and 11 skeletal and pharyngeal cephalometric variables on palatal dimensions were assessed using multiple-regression analyses. The typing of clefts had an effect on the all palatal dimensions. The palate was shallower in subjects with clefts invol
