Appropriateness of Lumbar Spine Magnetic Resonance Imaging Requests: Guideline Concordance, Diagnostic Yield, Artificial-Intelligence Assessment and Derivation of a Simple Clinical Decision Rule.

Background/Objectives: Lumbar magnetic resonance imaging (MRI) is widely overused in low back pain. We assessed the appropriateness of lumbar MRI requests against three international guidelines and two independent large language model (LLM)-based artificial intelligence (AI) assessments, related appropriateness to diagnostic yield, and derived a simple decision rule to identify imaging likely to change management. Methods: In this single-centre retrospective study, 147 consecutive adults undergoing lumbar MRI for low back pain were analysed. Each request was classified using pre-imaging clinic

AnemiaScore: An Investigator-Developed Rule-Based Clinical Decision-Support Framework for Cancer-Related Anemia-A Single-Center Retrospective Proof-of-Concept Concordance Study.

Background/Objectives: AnemiaScore is an investigator-developed, deterministic, rule-based clinical decision-support framework for cancer-related anemia. This initial proof-of-concept study evaluated agreement with recorded local oncologist decisions, used as a clinical reference comparator rather than an independent standard. Methods: This single-center retrospective study analyzed 180 directly comparable A-C cases in the primary three-category analysis. An exploratory full-cohort analysis included 188 eligible cases and descriptively incorporated Category D decisions arising either from the

Evaluation of a clinical decision rule to guide antibiotic prescription in children with suspected lower respiratory tract infection in The Netherlands: A stepped-wedge cluster randomised trial.

BACKGROUND: Optimising the use of antibiotics is a key component of antibiotic stewardship. Respiratory tract infections (RTIs) are the most common reason for antibiotic prescription in children, even though most of these infections in children under 5 years are viral. This study aims to safely reduce antibiotic prescriptions in children under 5 years with suspected lower RTI at the emergency department (ED), by implementing a clinical decision rule. METHODS AND FINDINGS: In a stepped-wedge cluster randomised trial, we included children aged 1-60 months presenting with fever and cough or dysp

Clinical decision support system for detecting right ventricular dysfunction in acute pulmonary embolism: Explainable a new Binary Rule Search (BRS) ensembles and robustness evaluation.

BACKGROUND: Right-ventricular dysfunction (RVD) in acute pulmonary embolism (PE) carries excess short-term mortality; fast, transparent risk stratification is needed. OBJECTIVE: To develop and evaluate an interpretable Binary Rule Search (BRS) framework for RVD detection on a fully binarized clinical dataset, emphasizing conservative robustness. METHODS: We analyzed a single-center cohort (N = 363; development = 250, external validation = 113) recoded into 0/1 predictors. For subset sizes k = 1-5, a Rust BRS searched bit-mask rules maximizing Matthews correlation coefficient (MCC) on develop

Prediction of risk of recurrence of venous thromboembolism following treatment for a first unprovoked venous thromboembolism: systematic review, prognostic model and clinical decision rule, and economic evaluation.

BACKGROUND: Unprovoked first venous thromboembolism (VTE) is defined as VTE in the absence of a temporary provoking factor such as surgery, immobility and other temporary factors. Recurrent VTE in unprovoked patients is highly prevalent, but easily preventable with oral anticoagulant (OAC) therapy. The unprovoked population is highly heterogeneous in terms of risk of recurrent VTE. OBJECTIVES: The first aim of the project is to review existing prognostic models which stratify individuals by their recurrence risk, therefore potentially allowing tailored treatment strategies. The second aim is

Development of a Clinical Decision Rule for the Early Safe Discharge of Patients with Mild Traumatic Brain Injury and Findings on Computed Tomography Brain Scan: A Retrospective Cohort Study.

International guidelines recommend routine hospital admission for all patients with mild traumatic brain injury (TBI) who have injuries on computed tomography (CT) brain scan. Only a small proportion of these patients require neurosurgical or critical care intervention. We aimed to develop an accurate clinical decision rule to identify low-risk patients safe for discharge from the emergency department (ED) and facilitate earlier referral of those requiring intervention. A retrospective cohort study of case notes of patients admitted with initial Glasgow Coma Scale 13-15 and injuries identified

Prospective derivation of a clinical decision rule for thoracolumbar spine evaluation after blunt trauma: An American Association for the Surgery of Trauma Multi-Institutional Trials Group Study.

BACKGROUND: Unlike the cervical spine (C-spine), where National Emergency X-Radiography Utilization Study (NEXUS) and the Canadian C-spine Rules can be used, evidence-based thoracolumbar spine (TL-spine) clearance guidelines do not exist. The aim of this study was to develop a clinical decision rule for evaluating the TL-spine after injury. METHODS: Adult (≥15 years) blunt trauma patients were prospectively enrolled at 13 US trauma centers (January 2012 to January 2014). Exclusion criteria included the following: C-spine injury with neurologic deficit, preexisting paraplegia/tetraplegia, and

Prospective observational study to assess the performance accuracy of clinical decision rules in children presenting to emergency departments with possible cervical spine injuries: the Study of Neck Injuries in Children (SONIC).

INTRODUCTION: Paediatric cervical spine injury (CSI) is uncommon but can have devastating consequences. Many children, however, present to emergency departments (EDs) for the assessment of possible CSI. While imaging can be used to determine the presence of injuries, these tests are not without risks and costs, including exposure to radiation and associated life-time cancer risks. Clinical decision rules (CDRs) to guide imaging decisions exist, although two of the existing rules, the National Emergency X-Radiography Low Risk Criteria and the Canadian C-Spine Rule (CCR), focus on adults and a n

Improving Medication Safety in Chronic Kidney Disease Using Rule-Based and Artificial Intelligence-Based Clinical Decision Support Systems: A Systematic Review of Randomized Controlled Trials.

BACKGROUND: Optimization of renal drug dosing to avoid drug toxicity is essential in Chronic Kidney Disease (CKD), yet prescribing errors are common. CDSS with rule-based and AI/ML based tools are used to address this safety gap; however, their impact remains uncertain. METHODS: We performed a PRISMA-guided systematic review and meta-analysis of RCTs comparing rule-based or AI/ML CDSS with usual care comparators among adults with CKD or at risk of CKD-related prescribing errors. The primary outcome was a medication safety endpoint aligned with the CDSS logic (appropriate renal dosing, potenti

Identifying Predictor Variables for a Future Clinical Decision Rule for Ordering of Chest X-Rays (CXR) for Non-Traumatic Chest Pain in Adult Emergency Department (ED) Patients.

OBJECTIVE: We sought to identify predictor variables for a future clinical decision rule for CXR ordering in non-traumatic chest pain. METHODS: Emergency Medicine Physicians were asked to select variables that would result in a CXR being ordered. Inter-rater reliability (using Fleiss' kappa) was calculated for the most commonly agreed upon. RESULTS: Of the 40 candidate variables, 10 were most agreed upon; however, only three had perfect inter-rater agreement and reliability. CONCLUSIONS: In this study, only an abnormal lung examination, a suspicion of pneumonia or of acute pulmonary oedema

External validation of the REDUCTION clinical decision aids for ruling out fractures in patients with midfacial and mandibular injuries in the emergency department: a protocol for a prospective multi-centre cohort study.

BACKGROUND: Computed Tomography (CT) is the gold standard for diagnosing midfacial and mandibular fractures in emergency department (ED) patients. However, its increasing availability has led to more frequent use, resulting in higher healthcare costs and unnecessary radiation exposure. Although various clinical parameters can assist in diagnosis of maxillofacial fractures, none are reliable enough to rule them out individually. To support clinical decision-making, and to reduce unnecessary imaging, the REDUCTION studies developed four decision aids based on combinations of clinical parameters.

Use of a penicillin allergy clinical decision rule to enable direct oral penicillin provocation: an international multicentre randomised control trial in an adult population (PALACE): study protocol.

INTRODUCTION: Penicillin allergies are highly prevalent in the healthcare setting and associated with the prescription of second-line inferior antibiotics. More than 85% of all penicillin allergy labels can be removed by skin testing and 96%-99% of low-risk penicillin allergy labels can be removed by direct oral challenge. An internally and externally validated clinical assessment tool for penicillin allergy, PEN-FAST, can identify a low-risk penicillin allergy without the need for skin testing; a score of less than 3 has a negative predictive value of 96.3% (95% CI, 94.1 to 97.8) for the pres

Impact of a clinical decision rule on antibiotic prescription for children with suspected lower respiratory tract infections presenting to European emergency departments: a simulation study based on routine data.

BACKGROUND: Discriminating viral from bacterial lower respiratory tract infections (LRTIs) in children is challenging thus commonly resulting in antibiotic overuse. The Feverkidstool, a validated clinical decision rule including clinical symptoms and C-reactive protein, safely reduced antibiotic use in children at low/intermediate risk for bacterial LRTIs in a multicentre trial at emergency departments (EDs) in the Netherlands. OBJECTIVES: Using routine data from an observational study, we simulated the impact of the Feverkidstool on antibiotic prescriptions compared with observed antibiotic

Which patients with unstable angina or non-Q-wave myocardial infarction should have immediate cardiac catheterization? A clinical decision rule for predicting who will fail medical therapy.

Unstable angina and non-Q-wave myocardial infarction (MI) are common and costly clinical events, but there is considerable uncertainty about optimal clinical management of these syndromes. We developed a prediction rule to help clinicians determine which patients with unstable angina or non-Q-wave MI are likely to "fail" medical therapy and ultimately require cardiac catheterization within 6 weeks of presentation. Subjects were 733 patients presenting with unstable angina or non-Q-wave MI enrolled in the TIMI IIIB trial and randomized to initial medical management. We developed a prediction ru

Diagnostic Value of In Vitro Tests for Peanut Allergy in Children Without Clinical Exposure: A High-Specificity Rule-In Decision Pathway-Preliminary Findings from a Single-Center Study in Polish Children.

Background: Diagnosing peanut allergy (PA) in children without known exposure remains challenging due to the need to distinguish true clinical allergy from asymptomatic sensitization. This study aimed to evaluate the diagnostic performance of individual and combined in vitro markers, particularly sIgE to Ara h 2, and to develop a multistage decision pathway that may reduce reliance on oral food challenge (OFC). Methods: Eighty children with suspected peanut allergy were prospectively enrolled. All participants, including healthy controls, underwent skin prick testing (SPT), measurement of sIgE

Clinical prediction rules in acute appendicitis: which combination of variables is more effective at predicting?

BACKGROUND: Clinical prediction rules have been designed to reduce variability and improve the diagnostic process. However, there are no unanimous criteria regarding which of them is the most efficient for the diagnosis of acute appendicitis. AIM: The primary aim of this study was to assess the diagnostic efficacy of the most commonly used clinical prediction rules. The second aim was to identify the combination of the smallest number of clinical and analytical variables that would allow a cost-effective diagnostic approach. METHODS: A retrospective observational study was conducted of 458 p

The "Petechiae in children" (PiC) study: evaluating potential clinical decision rules for the management of feverish children with non-blanching rashes, including the role of point of care testing for Procalcitonin & Neisseria meningitidis DNA - a study protocol.

BACKGROUND: Children commonly present to Emergency Departments (ED) with a non-blanching rash in the context of a feverish illness. While most have a self-limiting viral illness, this combination of features potentially represents invasive serious bacterial infection, including meningococcal septicaemia. A paucity of definitive diagnostic testing creates diagnostic uncertainty for clinicians; a safe approach mandates children without invasive disease are often admitted and treated with broad-spectrum antibiotics. Conversely, a cohort of children still experience significant mortality and morbi

Diagnostic clinical prediction rules for categorising low back pain: A systematic review.

BACKGROUND: Low back pain (LBP) is a common complex condition, where specific diagnoses are hard to identify. Diagnostic clinical prediction rules (CPRs) are known to improve clinical decision-making. A review of LBP diagnostic-CPRs by Haskins et al. (2015) identified six diagnostic-CPRs in derivation phases of development, with one tool ready for implementation. Recent progress on these tools is unknown. Therefore, this review aimed to investigate developments in LBP diagnostic-CPRs and evaluate their readiness for implementation. METHODS: A systematic review was performed on five databases
