From Evaluation to Implementation: A Delphi Consensus Framework with Domain-Weighted Decision Models for Robotic Surgery Adoption.

Background: Adoption of surgical robotic systems is accelerating globally, driven by advances in clinical capability, ergonomics, and workflow efficiency. However, robotic surgery programs introduce complex organizational challenges related to patient safety, workforce capability, infrastructure readiness, governance, vendor dependency, procurement, and long-term sustainability. Hospitals often lack structured, leadership-oriented frameworks to support systematic, context-appropriate evaluation, adoption, and governance of these technologies. Objective: To develop a multidisciplinary, consensu

Examining gaps in institutional policies for clinical genomic data sharing: A cross-jurisdictional study.

The sharing of data generated by clinical genetic and genomic testing without explicit consent is important for timely diagnosis and treatment. While many jurisdictions permit the sharing of identifiable data for direct clinical care, institutional policies vary in how clearly they specify key elements, including when sharing is permitted, what data are covered, and what safeguards apply. Greater clarity around these elements may support responsible data sharing while balancing timely care with transparency and appropriate protections. We conducted a mixed-methods content analysis of data-shar

Medical-Bioethical Mediation as a Potential Governance Innovation for Sustainable, Efficient, and Resilient Healthcare Systems.

BACKGROUND/OBJECTIVES: Healthcare systems increasingly operate through dense networks of clinicians, managers, public health authorities, institutions, and communities. Disagreement is no longer confined to bedside decisions; it also affects coordination, legitimacy, and implementation at system level. Bioethics, administrative governance, and consultation processes address important parts of this problem, but they do not by themselves provide a dedicated process for handling persistent disagreement, mistrust, and competing values across actors and institutions. This paper proposes medical-bio

Corporatization of Academic Medical Centers and Its Impact on the "Esprit-De-Corps" of Surgical Teams.

BACKGROUND: Accelerating corporatization of US healthcare has reshaped academic medical center governance, with for-profit investor-owned organizations comprising 23.7% of market share in 2,023 and 56.1% of hospitals owned by corporations with 3 or more hospitals. Although intended to enhance financial sustainability, centralized, productivity-driven management may conflict with the professional culture underpinning high-performing surgical teams. STUDY DESIGN: Narrative perspective synthesizing literature in health policy, organizational psychology, and surgical culture to examine how corpor

Black box no more: a scoping review of AI governance frameworks to guide procurement and adoption of AI in medical imaging and radiotherapy in the UK.

Technological advancements in computer science have started to bring artificial intelligence (AI) from the bench closer to the bedside. While there is still lots to do and improve, AI models in medical imaging and radiotherapy are rapidly being developed and increasingly deployed in clinical practice. At the same time, AI governance frameworks are still under development. Clinical practitioners involved with procuring, deploying, and adopting AI tools in the UK should be well-informed about these AI governance frameworks. This scoping review aimed to map out available literature on AI governan

Governance and Prioritization of Human Spinal Cord Tissue Procurement for Research in an Organ Donation Setting.

INTRODUCTION: Procurement of human spinal cord tissue for research is rare and typically occurs through deceased organ donation pathways. Existing tissue banking guidance, largely derived from surgical pathology practice, does not adequately address the ethical, anatomical, and operational constraints unique to donation-integrated spinal cord procurement. OBJECTIVES: The objective of this article is to describe institutionally implemented governance and prioritization practices for donation-integrated procurement and research banking of human spinal cord tissue within a tertiary academic hosp

Do tanning salons adhere to new legal regulations? Results of a simulated client trial in Germany.

In August 2009 and January 2012, two regulations were passed in Germany to limit UV exposure in the general population. These regulations state that no minors are allowed to use tanning devices. Personnel of tanning salons is mandated to offer counseling regarding individual skin type, to create a dosage plan with the customer and to provide a list describing harmful effects of UV radiation. Furthermore, a poster of warning criteria has to be visible and readable at all times inside the tanning salon. It is unclear whether these regulations are followed by employees of tanning salons in German

Guidance for laboratory implementation, governance and continuous assurance of artificial intelligence in histopathology.

Artificial intelligence (AI) is rapidly transforming histopathology, with applications ranging from workflow optimisation and quality assurance to tumour diagnosis, grading, biomarker assessment and estimation of prognosis. While numerous AI algorithms have demonstrated promising analytical and clinical performance, pathology laboratories are increasingly adopting commercially available AI systems with regulatory-approval rather than developing their own algorithms. Existing guidance largely focuses on AI development, validation and regulatory approval, with comparatively little practical dire

AEGIS: an operational infrastructure for post-market governance of adaptive medical AI under US and EU regulatory frameworks.

Medical AI requires governance preserving safety during continuous improvement. FDA Predetermined Change Control Plans (PCCP) and EU post-market surveillance address iterative updates but lack operational tooling. We present AEGIS, an internal governance and post-market-monitoring infrastructure designed to support the documentation of predetermined governance pathways for PCCP and Medical Devices Regulation alignment. AEGIS comprises dataset assimilation and retraining, model monitoring, and conditional decision-making modules. It utilizes a four-category taxonomy (APPROVE, CONDITIONAL APPROV

A Multiple Source Qualitative Case Study of Person-Centered Care in HIV Care Delivered to Adults Living With HIV by Chile's Public Health Care System.

In Chile, little research exists depicting how a person-centered care (PCC) model is translated into clinical practice. A multiple-source qualitative case study design was used to analyze which elements of PCC are implemented in care for people living with HIV (PLWH). Data collection included documentary analysis of national governance documents and semi-structured interviews conducted within two public hospitals in Chile with health care providers (HCPs) and PLWH. Data analysis identified five key elements PCC used in HIV clinical practice in Chile. Participants and governance documents highl

Reliance Management in Healthcare for Professional and Elite Athletes: Ethics, Patient-Centered Care, and Governance.

Professional and elite sport represents a high-pressure healthcare environment in which standard principles of patient-centered care, confidentiality, informed consent, shared decision-making, and clinical independence are tested by organizational and performance-related pressures. This narrative review examines reliance management in athlete-physician relationships. Reliance is conceptually distinguished from trust: whereas trust is an interpersonal attitude that can be betrayed, reliance denotes the broader dependence of the athlete-patient on the physician, the care process, and the surroun

TRIAGE-AI: A clinician-facing implementation-readiness framework for evaluating artificial intelligence in cancer pathways.

Artificial intelligence (AI) is increasingly being offered for use in cancer pathways, but clinicians still lack a practical method for judging whether a specific tool is ready for adoption in routine care. Existing standards such as DECIDE-AI, TRIPOD-AI, CONSORT-AI and evidence standards for digital health technologies are essential, but they are primarily directed at developers, trialists and regulators rather than clinical leads making procurement, governance or pathway decisions. This article proposes TRIAGE-AI, a clinician-facing implementation-readiness framework covering six domains: tr

Professional Boundaries and Ethical Obligations in Saudi Arabia An Integrated Sharia, Legal and Clinical Psychology Framework.

BACKGROUND: Professional boundaries in psychotherapy are structural safeguards within a fiduciary, power-imbalanced relationship. In Saudi Arabia, boundary dilemmas arise in a Sharia-informed moral environment and a regulated healthcare system, commonly involving mixed-gender privacy sensitivities, gifts and culturally normative hospitality, dual relationships in close-knit communities, conflicts of interest, and technology-mediated challenges (e.g., messaging platforms and tele-mental health). METHODS: This study uses a secondary-data integrative design combining (i) doctrinal analysis of pu

Conversational AI in Hereditary Cancer Care: Sociotechnical Study of Responsible Design Requirements.

BACKGROUND: Individuals with BRCA1/2 germline variants face complex, preference-sensitive medical decisions under psychological distress. Structural constraints in genetic counseling create support gaps during waiting periods, prompting patients to rely on fragmented or misleading online information. Conversational AI may offer low-threshold, continuous informational support, yet methods for its responsible integration into clinical care remain unclear. OBJECTIVE: This study identifies and systematizes stakeholder-informed sociotechnical design requirements and governance conditions for a cli
