Supporting effective clinical supervision and assessment in the development of higher-level nursing practice.

BACKGROUND: Higher-level practice development within nursing reflects the demands of a changing society. Nurses require not only the underpinning theory, but also an effective and structured clinical community of practice, derived from a collaborative relationship between higher education institutions and clinical partners. AIMS: To reflect on the findings of two qualitative research studies that examined higher-level practice, associated education, practice assessment and required clinical supervision. METHODS: Exploration of the experiences of clinical supervisors supporting students under

Lessons Learned From the Implementation of a Pragmatic Trial Evaluating Non-Pharmacological Approaches to Dementia Care in US Nursing Homes.

RATIONALE: The consistent delivery of effective evidence into nursing home practice has been shown to enhance quality of care and residents' outcomes. Pragmatic trials enable the evaluation of efficacious interventions in a real-world context, including care of residents living with Alzheimer's Disease and Related Dementias (ADRD). Yet, there are limited pragmatic trials conducted in nursing homes and even less evidence on the strategies to successfully conduct such research in this setting. OBJECTIVE: To identify the lessons learned from a pragmatic trial and provide recommendations for futu

Comparison study of home catheter cleaning methods.

Three methods of cleaning urinary catheters for reuse at home by clients on intermittent catheterization programs were studied. Reused catheters were collected from clients, autoclaved, and then incubated in a culture of Escherichia coli broth. Three different isolates of E. coli were used at concentrations ranging from 4.8 x 10(5) to 1.0 x 10(8) colony-forming units per milliliter. The catheters were then rinsed with tap water for 1 minute and soaked in one of three cleaning solutions for 30 minutes. The three cleaning solutions studied were 0.6% hydrogen peroxide, bleach in a 1:4 solution wi

[Nurse involvement in clinical trials with cell therapy].

Research with stem cells has made a qualitative leap since the research group from Minnesota University Stem Cell Institute in the USA published a study in July 2002, which showed that the bone marrow stem cells of adults multiplied indefinitely without losing their ability to differentiate. In this hospital the first implant of stem cells derived from adipose tissue was performed in this hospital in 2003 in an attempt to close a complex perianal fistula in a patient with Crohn's disease. As a result of this first success, a multidisciplinary research team was formed to study the application o

Practice nurse chlamydia testing in Australian general practice: a qualitative study of benefits, barriers and facilitators.

BACKGROUND: Chlamydia infection is a significant public health issue for young people; however, testing rates in Australian general practice are low. Practice nurses (PNs) could have an important role in contributing to increasing chlamydia testing rates. The Australian Chlamydia Control Effectiveness Pilot (ACCEPt), a large cluster randomised control trial of annual testing for 16 to 29 year olds in general practice, is the first to investigate the role of PNs in maximising testing rates. In order to assess the scope for PN involvement, we aimed to explore PN's views in relation to involvemen

[Comparison of two different protocols on change of medication in central venous catheterization in patients with bone marrow transplantation: results of a randomized multicenter study].

Care of central venous catheter (CVC) in patients undergoing bone marrow transplantation (BMT) raises significant problems related to the high risk of local infections, to the immunodeficient status, which in itself is a predisposing factor for systematic blood stream infections. Although frequent changes of CVC dressing might theoretically reduce the incidence of infections, they are also accompanied by significant skin toxicity and patient discomfort. No study has yet addressed these points. The objective of this study was to compare two different time interval protocols for CVC dressing, in

Cystic fibrosis research in allied health and nursing professions.

This report is the result of the "Allied Health and Nursing Professions Working Group" meeting which took place in Verona, Italy, November 2009, which was organised by the European Cystic Fibrosis Society, and involved 32 experts. The meeting was designed to provide a "roadmap" of high priority research questions that can be addressed by Allied Health Professionals (AHP) and nursing. The other goal was to identify research skills that would be beneficial to AHP and nursing researchers and would ultimately improve the research capacity and capability of these professions. The following tasks we

The changing profile of acute childhood illness: a need for the development of family nursing knowledge.

The profile of the acutely ill child and family has undergone dramatic changes in recent years. Advances in medical technology have substantially altered the life-death trajectory for ill children and their families; however, they have also left families to endure increasingly difficult situations with uncertain outcomes for prolonged periods of time. Nurses are interested in understanding how families manage in the face of serious illness, yet a review of the literature has revealed little progress in this area. While the importance of the family has been repeatedly documented, theoretical de

[Increasing the Rate of Completion of the Therapeutic Hypothermia Procedure in the NICU].

BACKGROUND & PROBLEMS: The most effective treatment currently available for perinatal asphyxia-induced hypoxic-ischemic encephalopathy is therapeutic hypothermia, which reduces the mortality rate and neurological disorders in newborns. The earlier this therapy is performed, the better the protective effects on the nerves of the patient. In our neonatal intensive care unit (NICU), we discovered that nurses lack experience caring for patients undergoing hypothermia therapy due to the limited number of cases. In addition, outdated guidelines, the disorganized placement of equipment, and the pauci

Perceptions of the possible impact of dyslexia on nursing and midwifery students and of the coping strategies they develop and/or use to help them cope in clinical practice.

This qualitative grounded theory case study aimed to explore the perceptions of the impact of dyslexia on nursing and midwifery students and of the coping strategies they develop and/or use to help them cope in practice. The questions addressed were: 1. What is the perceived impact of dyslexia on the nursing and midwifery student in clinical practice? 2. How are any difficulties associated with dyslexia managed by the nursing or midwifery student? 3i. What strategies can help and support nursing and midwifery students with dyslexia? 3ii. What are students' and mentors' perceptions of the poste

Nursing treatment of patients with chronic leg ulcers in the community.

A descriptive survey of current reported practice by 146 community nurses for their nursing treatment of leg ulcers was undertaken. Sixty-four per cent of nurses reported they would apply compression bandages to only venous ulcers; in only 23% of cases could the products described achieve an adequate level of compression. A variety of modern wound dressings were used by the nurses; 89% of nurses reported using a combination of different products layered over the ulcer. There is no evidence that this has any beneficial effect and could therefore be a potential waste of money, as well as contrib

[Effects of information on and reflection of women's pain experience during mammography].

At mammography, strong compression of the breasts is required to reduce the radiation dose and to obtain radiographic images of high quality. In recent times the mass media have received a number of letters with complaints about the painfulness of the examination and an unsympathetic reception by the personnel. The aim of this study was to find out what effect information and a special manner of reception ("reflection") had on the experience of pain at mammography. Reflection meant that the X-ray nurse, in an accepting manner, reflected the feelings and facts which the woman expressed. The stu

Simulated IV Pumps in Pathopharmacology: A Safety-Focused Approach for Mastering Medication Safety.

BACKGROUND: Medication safety is a core nursing competency, yet up to half of new graduate nurses are involved in errors. Traditional dosage calculation instruction often lacks clinical context, leaving students unprepared to translate dosage calculation into practice. This project explored the integration of tablet-based simulated IV pumps into a prelicensure nursing pathopharmacology course to strengthen experiential learning and confidence. METHOD: Second-semester accelerated baccalaureate nursing students (N = 88) used simulated IV pump tablets alongside existing dosage calculation curric

Look before you leap: lessons learned when introducing clinical simulation.

Before investing in a human patient simulator, we designed a preliminary study that examined student responses to a laboratory exercise that used lower-fidelity simulation. Our purpose was to compare beginning-level, baccalaureate nursing students' self-reported assessment in the domains of confidence, ability, stress, and critical thinking before and after they participated in the simulation. Results showed statistically significant improvement in all domains for skills in urinary catheterization, intravenous and nasogastric medication administration, and sterile dressing change.
