Application of restricted sequential design in a clinical protocol.

Restricted sequential design is an alternative to fixed-sample analysis as a statistical tool in clinical trials. This paper presents a specific example of the rationale which led to the choice of sequential design in a clinical protocol evaluating granulocyte transfusions in children with leukemia. The main advantage of sequential design is that fewer patients may be necessary to declare statistical significance. Its application is limited to trials where (a) the result of therapy is easily defined, (b) the result of therapy is discernible in a short time interval, and (c) one randomization i

Clinical protocol for occlusal adjustment: Rationale and application.

BACKGROUND: Occlusal adjustment can optimize the result of orthodontics, orthognathic surgery, and comprehensive restoration, and resolve adverse forces to the dentition that affect the entire masticatory system. Mounted diagnostic casts and computerized occlusal analysis offer complementary advantages for evaluating occlusal problems. Predictable occlusal adjustment is facilitated by precise, measured documentation of occlusal force by computerized occlusal analysis. CLINICAL PRESENTATION: A conservative, structural correction of a pronounced, chronic occlusal problem by additive and subtrac

Clinical neurotransplantation protocol for Huntington's and Parkinson's disease.

PURPOSE: The concept of transplantation of neuronal cells to treat Huntington's and Parkinson's diseases is based on the proven principle that dopaminergic and GABA-ergic progenitor neurons (from the human developing ventral mesencephalon and whole ganglionic eminence) can survive, differentiate and functionally integrate into an allogenic host brain. However, several donor and host-specific variables play a major role in the safety and outcome of this procedure. In this paper, we seek to summarize an updated neural transplantation protocol, based on our institutional experience and many years

[Clinical and nursing protocol in the treatment of the decubital ulcers].

The decubital ulcers treatment needs a complex clinical approach. The ulcer healing and the absence of recurrences are not only the result of a correct surgical operation. The purpose of this work is to identify all the clinical and nursing parameters that can provide long lasting healing. We treated 105 decubital ulcers in various body areas (sacralis, ischiatic, trochanteric and calcanear regions) and we defined a clinical protocol that starts from prevention, to the admission, till the convalescence. We obtained satisfactory results with few post-operative complications and recurrences. Our

Protocol for clinical neurophysiologic examination of the pelvic floor.

Clinical neurophysiologic examination of the pelvic floor is performed worldwide, but there is no consensus on the choice of tests, nor on technical details of individual methods. Standardized methods are, however, necessary to obtain their valid application in different laboratories for the purpose of collection of normative data, comparison of patient data and organization of multi-center studies. It is proposed that in patients with suspected "lower motor neuron" type lesions concentric needle electromyography (CNEMG) is the most informative test to detect pelvic floor denervation/reinnerva

Antitumor electrochemotherapy: new advances in the clinical protocol.

BACKGROUND: Electrochemotherapy (ECT) is a new antitumor approach that combines systemic bleomycin (BLM) with electric pulses (EP) delivered locally at the tumor site. These EP permeabilize the cells in the tissue, allow BLM delivery inside the cells, and increase BLM cytotoxicity. As an extension of our initial Phase I trial on patients with head and neck squamous cell carcinoma (HNSCC) permeation nodules, we tested variations of ECT protocol to determine how to improve it. METHODS: Seven patients with multiple and/or large permeation nodules of HNSCC or of salivary or breast adenocarcinoma

Review of the clinical basis and protocol for epidural endoscopy.

Epidural endoscopy is a minimally invasive technology that is now in active clinical trials. This new technique allows the operator to visualize directly the epidural space and contiguous structures, thus allowing for detailed examination leading to a better understanding of the role of epidural adhesion in the development of sciatica.

Identification of key upper-limb muscles during a standardized reach-to-grasp task toward simplified clinical protocols.

INTRODUCTION: Variability in task execution, target properties, and recording procedures has limited the development of standardized upper-limb surface electromyography (sEMG) reference patterns. Establishing reproducible muscle activation profiles is essential for advancing sEMG-based assessment and motor control applications. METHODS: This study investigated the reproducibility of muscle activation during a highly standardized reach-to-grasp task performed under controlled setup conditions in twenty-seven healthy adults. Participants performed repeated executions of the task under controlle

Photon-counting CT in cardiac imaging: multi-institutional guidance on technical principles, clinical evidence, and practical protocols.

Coronary CT angiography (CCTA) is recommended as a first-line test for evaluating suspected coronary artery disease in many patient cohorts. Photon-counting detector CT (PCD-CT) introduces direct energy discrimination, higher geometric dose efficiency, and ultrahigh spatial resolution compared to energy-integrating detector CT, enabling improved coronary lumen visualization and intrinsic spectral imaging. This multi-institutional guidance summarizes technical principles of PCD-CT relevant to cardiac imaging and synthesizes contemporary evidence across key applications: (1) CCTA protocol select

Evaluating progestin-primed and GnRH antagonist ovarian stimulation protocols in PGT-A cycles: implications for clinical practice.

BACKGROUND: Progestin-primed ovarian stimulation (PPOS) has emerged as an alternative to GnRH-antagonist protocols in IVF, yet its impact on embryo chromosomal competence remains controversial. Evidence is particularly limited regarding whether PPOS influences blastocyst euploidy rates and key clinical outcomes in PGT-A cycles, and whether these effects vary according to maternal age and ovarian reserve status. METHODS: This retrospective cohort study included 1,843 PGT-A cycles (666 PPOS, 1,177 GnRH antagonist) performed at a single tertiary IVF center between January 2016 and January 2024.

From Biological Mechanisms to Clinical Outcomes: A Scoping Review Comparing Immediate and Delayed Dental Implant Placement Protocols.

Background/Objectives: Dental implant placement protocols including immediate (IIP) and delayed implant placement (DIP) are likely to affect bone tissue repair and regeneration after the surgery. Despite many benefits of IIP, it has remained unclear whether IIP demonstrates comparable healing processes and outcomes to those observed in DIP. This review aims to summarize and compare biological and clinical outcomes of IIP and DIP, focusing on success and survival rates, periodontal status, esthetics and radiographic outcomes, and biochemical markers. Methods: A literature search of electronic d

Sequential antibody induction for immune tolerance in clinical organ transplantation: a feasibility review of immunosuppressant withdrawal protocols.

Long-term immunosuppressant use after solid organ transplantation causes serious complications, including chronic rejection, infection, malignancy, and metabolic disorders, which has motivated the clinical pursuit of immune tolerance induction. Sequential antibody induction protocols - using peri-transplant antibodies (e.g., alemtuzumab, anti-thymocyte globulin, belatacept, anti-CD40 monoclonal antibodies) combined with phased reduction or withdrawal of maintenance drugs - aim to establish "operational tolerance" or complete tolerance. This review systematically examines the immunological rati
