Coccidioidomycosis is a deep mycotic infection endemic to the southwestern United States. Although it has also been reported to occur in nonendemic areas, it has rarely been reported in Taiwan. We report a case of disseminated coccidioidomycosis which is the first such case reported on this island. The 28-year-old male patient developed a nonproductive cough and fever while travelling in Arizona. Chest roentgenogram revealed infiltration of the right lower lung field and enlargement of the right paratracheal lymph nodes. The titer for the complement fixation test for coccidioidin was 1:32. A 46-year-old soldier of the German airforce stayed in Texas/USA for 6 months. Without clinical complaints the patient revealed nodular lung infiltration and a cavern on a routine-chest-X-ray examination after his return. Serological, cytological, and histological examinations excluded a sarcoidosis, tuberculosis of the lungs and a neoplasm. The finding of precipitating antibodies and a positive skin reaction against coccidioidin led to the diagnosis of a primary pulmonary, spontaneously healed coccidioidomycosis, which did not need any antimycotic therapy.

Results of a study of the prevalence of valley fever among 1128 residents of Tijuana, Baja California are presented. Relapse after apparently successful treatment of coccidioidomycosis has been a problem with both amphotericin B and the azoles. We conducted a retrospective cohort study of 34 patients who required therapy for coccidioidomycosis between 1973 and 1993; 10 relapsed and 25 (one patient received two courses of therapy) did not relapse during follow-up. The interpretation of immunological results in systemic mycoses has been complicated by cross-reactions among specimens from patients with blastomycosis, coccidioidomycosis, and histoplasmosis. Cortisone administered orally, in low dosages for brief periods, promptly suppressed the allergic manifestations accompanying primary pulmonary coccidioidomycosis in 19 cases. A comparative evaluation was performed on the micro- and macrocomplement fixation (CF) tests that are used as standard procedures in the serodiagnosis of blastomycosis, coccidioidomycosis, and histoplasmosis. Fifty-two diabetic patients who underwent pulmonary surgery for coccidioiodmycosis were evaluated by a retrospective study which included classification by stage of disease, status of insulin dependency, and reaction to coccidioidin skin test. We report a case of disseminated coccidioidomycosis in a 39-year-old Japanese male whose illness developed after returning from a trip to an endemic area. He showed positive coccidioidin skin reaction throughout the entire course of his illness. The primary lesion in the lung subsequently spread to the bone. Serologic tests are valuable in the management of patients with suspected or confirmed coccidioidomycosis. Qualitative tests (immunodiffusion, enzyme immunoassay, or latex particle agglutination) permit detection in the serum of the major antibody responses-coccidioidal IgM in early coccidioidomycosis, and complement fixing (CF) IgG, which appears later and is more persistent.
