Management of Peyronie's disease after collagenase (Xiaflex:®).

Although the prevalence of Peyronie's disease (PD) is reported to be 3-9% in men, the true prevalence is likely higher due to under-reporting. Many treatment modalities have been described for PD with varying degrees of success. In this article, we review and summarize the current literature pertaining to all pharmacotherapies (oral, intralesional, iontophoresis, and topical) and minimally invasive treatments available for PD (vacuum, traction device, shock wave therapy, and radiation treatment). Additionally, we discuss emerging therapies for PD that are still in pre-clinical development, inc

Collagenase-3 expression in periapical lesions: an immunohistochemical study.

Collagenase-3 (matrix metalloproteinase-13) is a metalloproteinase (MMP) that is associated with bone lesions and exhibits variable expression patterns in odontogenic cysts; it may play a role in regulating focal proliferation and maturation of jaw cyst epithelium. We studied the localization, staining intensity and distribution of collagenase-3 in 13 periapical granulomas with epithelium, 16 periapical granulomas without epithelium and 10 radicular cysts using archived formalin fixed, paraffin embedded tissues. A monoclonal antibody against human collagenase-3 was used to evaluate its express

Early results of the use of collagenase in the treatment of Dupuytren's contracture.

INTRODUCTION: In recent years, the enzyme collagenase has been developed for the percutaneous treatment of Dupuytren's contracture, allowing management of the condition as an outpatient. We present early results on the treatment of Dupuytren's contracture using injectable collagenase. METHODS: Patients awaiting Fasciectomy for Dupuytren's Contracture were selected for enrolment. Contracture cords were then marked and injected with collagenase in the outpatients department. Twenty-four hours later, patients returned for an extension procedure, performed under regional anaesthesia. Hand therapy

Collagenase clostridium histolyticum injection for the treatment of Dupuytren's contracture.

Collagenase clostridium histolyticum is a novel treatment for Dupuytren's contracture, approved by the U.S. Food and Drug Administration in February 2010. Prior to its availability, surgery was the only treatment for contracture related to this disorder. Dupuytren's disease is a benign, progressive fibroproliferative disorder affecting the palms of the hands. It is characterized by the formation of collagen- rich nodules and cords, which gradually shorten by the action of myofibroblasts, resulting in finger contractures. Intralesional use of clostridial collagenase has been evaluated in a tota

Unidirectional binding of clostridial collagenase to triple helical substrates.

Histotoxic clostridia produce collagenases responsible for extensive tissue destruction in gas gangrene. The C-terminal collagen-binding domain (CBD) of these enzymes is the minimal segment required to bind to collagen fibril. Collagen binding efficiency of CBD is more pronounced in the presence of Ca(2+). We have shown that CBD can be functional to anchor growth factors in local tissue. A (1)H-(15)N HSQC NMR titration study with three different tropocollagen analogues ((POG)(10))(3), ((GPOG)(7)PRG)(3), and (GPRG(POG)(7)C-carbamidomethyl)(3), mapped a saddle-like binding cleft on CBD. Construction and virulence testing of a collagenase mutant of Clostridium perfringens.

Clostridium perfringens produces several extracellular toxins and enzymes, including an extracellular collagenase or kappa toxin that is encoded by the colA gene. To determine if the ability to produce collagenase was a significant virulence factor in cases of gas gangrene or clostridial myonecrosis that are caused by C. perfringens, a chromosomal colA mutant was constructed by homologous recombination and subsequently virulence tested in the mouse myonecrosis model. The results clearly indicate that loss of the ability to produce collagenase does not alter the ability of the mutant to establi

Latent collagenase and gelatinase from human neutrophils and their activation.

Procollagenase M(r) 85,000 (SDS-PAGE) was purified from buffy coat to homogeneity and represents a stable single polypeptide chain forming the entire proenzyme. The procollagenase can be activated by various proteinases, e.g. trypsin, chymotrypsin, cathepsin G, kallikrein and stromelysin and by different mercurial compounds. Proteolytic conversion of the latent enzyme to the active form by chymotrypsin is accompanied by a molecular weight reduction to an apparent M(r) 64,000. This active enzyme lacks the first 79 N-terminal residues. Activation by trypsin leads to a latent intermediate of appa

The involvement of collagenase in the necrosis induced by the bites of some spiders.

1. The midgut extracts of 13 Australian spider species produced cellular disruption in mouse skin in tissue culture conditions. 2. Microbial collagenase and the venoms of some of these species had similar effects. 3. Five venoms also caused severe dermonecrosis in living mice. 4. Pre-mixing the venoms with L-cysteine caused complete in vivo and partial in vitro inhibition of their effects. 5. It was concluded that collagenase is a major factor in the aetiology of necrotic arachnidism.

A perspective on the role of collagenase in recessive dystrophic epidermolysis bullosa.

The pathophysiology of tissue fragility in recessive dystrophic epidermolysis bullosa may be due in part to excessive destruction of interstitial collagens by a structurally altered, but catalytically active, form of human skin collagenase. Therapeutic attempts directed toward reducing the expression of this enzyme have resulted in clinical improvement in some patients with the disease.

Epidermis promotion of collagenase in hypertrophic scar organ culture.

The pathophysiological biology of human hypertrophic scar was examined in a long-term organ culture system. Fresh full-thickness, thin slices of scar were placed in petri dishes. Tissue was successfully maintained for 2 weeks in an environment made up of CMRL-1066 medium, fetal bovine serum, insulin, and hydrocortisone under an environment of 40% O2, 5% CO2, and 55% N2 at 37 degrees C on a rocking platform. Histologically the explants were viable and remained differentiated. The omission of hydrocortisone caused localized destruction of the connective tissue matrix under the epidermal layer. T

Peripheral rheumatoid ulceration and evidence for conjunctival collagenase production.

Two patients with rheumatoid arthritis and peripheral corneal ulcerations were successfully treated by conjunctival resection. The tissues removed were assayed by a variation of the radial diffusion method for tissue collagenases. We used an agarose matrix containing lathyritic rat skin collagen. Wells 3 mm deep were punched in the agarose-collagen and surgical specimens were placed in the wells. Spaces remaining in the wells were filled with balanced salt solution. The assay dishes were incubated for four days at 32 degrees C near 100% humidity. Under these conditions release of collagenase w

Bacterial collagenase harnesses collagen geometry for processive cleavage.

Collagen, the major structural protein in the animal extracellular matrix, forms a triple helix that resists proteolysis and requires specialised enzymes for degradation. Flesh-eating bacteria secrete collagenases that unwind the collagen triple helix and processively trim Gly-X-Y triplet repeats, yet the molecular basis of this process has remained obscure. Here, cryo-electron microscopy reveals how Hathewaya histolytica collagenase ColH engages its substrate and exploits the helix's architecture for catalysis. ColH encircles a single collagen triple helix in a closed-ring conformation and, t

Collagenase-modified polydopamine nanoparticles for safe and effective vitreolysis.

Anomalous posterior vitreous detachment (aPVD) is involved in the pathogenesis of many vitreoretinal disorders. This condition is currently managed by vitrectomy, a routine but invasive surgery which is especially challenging in patients with firm vitreoretinal adhesions. Since 1998, pharmacological vitreolysis emerged as a potential replacement or adjunct therapy to vitrectomy. Over the years, most attention was focused on enzymes, but their use has been limited mainly due to retinal toxicity. To revive the potential of enzymatic vitreolysis, we aimed to immobilize collagenase on the surface

Collagenase Injection versus Limited Fasciectomy for Dupuytren's Contracture.

BACKGROUND: Treatments for Dupuytren's contracture include limited fasciectomy and collagenase injection. Comparisons of the effectiveness of these treatments have been limited. METHODS: We performed an unblinded, multicenter, pragmatic, two-group, randomized, controlled noninferiority trial comparing collagenase injection with limited fasciectomy in persons with moderate Dupuytren's contracture. The primary outcome was the score on the Patient Evaluation Measure-Hand Health Profile (PEM), a questionnaire for assessing hand health as reported by the patient, at 1 year after treatment. Scores

Patient Insight With Collagenase Treatment for Dupuytren: A Prospective Study.

PURPOSE: There appears to be controversy regarding differing patient and physician perceptions of adverse effects (AEs) in the treatment of Dupuytren disease with collagenase clostridium histolyticum (CCH). The aim of this study was to compare the number, type, and severity of AEs perceived and reported by patients and by their physician METHODS: To assess AEs following CCH injection in a standardized way, patients were given a list of predefined complications and asked to rate their severity on a 4-point Likert scale ranging from 1 (serious) to 4 (insignificant). RESULTS: Eighty-five patient

Wide-awake anesthesia in Dupuytren's contracture treated with collagenase.

The injection of collagenase followed by cord manipulation is one of the most popular treatments for Dupuytren's contracture. This is traditionally performed under local anesthesia or regional nerve block potentially with sedation. Neither the treatment with collagenase, nor the wide-awake anesthesia are novel techniques for hand surgeons. Nevertheless, we report the first experience of cord manipulation using the wide-awake approach. In this prospective study, we compared the pain perception of patients who underwent wide-awake anesthesia versus traditional local anesthesia. We recorded the p

Collagenase clostridium histolyticum in Dupuytren's contracture: a systematic review.

INTRODUCTION: In the last few years, the use of collagenase clostridium histolyticum for management of Dupuytren's contracture has increased. The procedure of enzymatic fasciectomy has become popular because it is non-invasive, safe and fast to perform. SOURCES OF DATA: A systematic search was performed on Medline (PubMed), Web of Science and Scopus databases using the combined keywords 'Dupuytren collagenase' and 'Dupuytren clostridium histolyticum'. Forty-three studies were identified. The quality of the studies was assessed using the Coleman Methodological Score. AREAS OF AGREEMENT: The u
