BACKGROUND: Community health workers (CHWs) are an essential cadre in the health systems of many low- and middle-income countries. These workers provide a wide variety of services and are key to ongoing processes of task shifting within human immunodeficiency virus programmes in particular. Ward-based outreach teams (WBOTs) are South Africa's latest iteration of the CHW programme and have been introduced as part of the National Department of Health's Primary Health Care Re-engineering programme.
METHODS: In order to assess the perceived effectiveness of the WBOTs in supporting the ongoing rol

BACKGROUND: The coronavirus disease 2019 (COVID-19) pandemic has significantly impacted family caregivers of older dependents, leading to fatigue, anxiety, depression, and financial burdens, which in turn diminish their quality of life. This study investigated the integration of community participatory systems in older care to address potential COVID-19 recurrence and develop policy proposals for the integrated care of dependent older people.
METHODS: An explanatory sequential design was used, with stratified random sampling across six provinces, resulting in a sample of 440 family caregiver

BACKGROUND AND OBJECTIVES: Research on olfaction and brain neuropathology may help understand brain regions associated with normal olfaction and dementia pathophysiology. To identify early regional brain structures affected in poor olfaction, we examined cross-sectional associations of microstructural integrity of the brain with olfaction in the Atherosclerosis Risk in Communities Neurocognitive Study.
METHODS: Participants were selected from a prospective cohort study of community-dwelling adults; selection criteria included the following: evidence of cognitive impairment, participation in a

In super-aged societies such as Japan, achieving "healthy longevity with well-being" requires not only medical and long-term care services but also a seamless continuum that integrates health promotion, frailty prevention, and community-based support, along with age-friendly physical and social environments that support functional ability, social participation, and independent living. Within this framework, oral frailty (OF)-defined as the accumulation of slight declines in oral function, including tooth loss, chewing and swallowing difficulties, oral dryness, and low articulatory oral motor s

BACKGROUND: Throughout Nigeria malaria is an endemic disease. Efforts to treat malaria can also be combined with other illnesses including pneumonia and diarrhea, which are killing children under five years of age. The use of Rapid Diagnostic Test (RDT) aids early diagnosis of malaria and informs when other illnesses should be considered. Those with positive RDT results should be treated with Artemisinin-based Combination Therapy (ACTs), while those with negative RDTs results are further investigated for pneumonia and diarrhea. Critical health systems challenges such as human resource constrai

This paper investigates the use of unmanned aerial vehicles (UAVs) as flying base stations (BSs) to enhance fifth generation (5G) vehicular communications on highways, where traffic congestion and fluctuating user demand can challenge the capacity of terrestrial infrastructure. While UAV-assisted vehicular networking has attracted significant attention, many existing studies rely on simplified mobility, propagation, or communication models that limit the assessment of practical deployment performance. To address these limitations, we develop a realistic UAV-assisted vehicular networking framew

Community health nurses play a vital role in delivering preventive, promotive, curative, and rehabilitative healthcare services through home visits and outreach programs. The community nursing bag has long served as an essential tool for carrying supplies, equipment, medications, and records required for field-based nursing care. However, conventional community bags face several limitations, including infection-control concerns, excessive weight, poor ergonomic design, environmental impact, and limited integration with digital technologies. These challenges necessitate the modernization of tra

With an increase and intensification of tropical storm events in the future, it becomes paramount for individual communities and homeowners to understand their potential risks of impacts from these events. This study describes an empirical modeling methodology using the City of Norfolk in southeastern Virginia to indicate areas of higher risk based on built infrastructure, property exposure, and temporal records of previous losses. Under this new empirical modeling methodology, a risk score is defined as the properties that may be at higher risk from future flooding events based on elevation d

PURPOSE: Integrated delivery networks (IDNs) enhance care for patients with diabetes by improving communication between specialists who perform eye examinations and primary care providers (PCPs). In this study, we aimed to identify factors associated with receiving eye care outside of an IDN and evaluate how leakage affects coordinated diabetes management.
METHODS: The study included 1,139 patients aged 18-75 with diabetes who received primary care in an IDN and had health insurance coverage provided by an accountable care organization (ACO). Claims data were reviewed to determine the rate at

BACKGROUND: Integrated health services are advocated for primary health care to address non-communicable diseases (NCDs), especially in low- and middle-income countries (LMICs). However, evidence of care delivery models and means of scaling up is limited. This study examines community-integrated NCD service models in China, providing evidence on the steps that China took to reorient health services towards primary health care to address the NCD epidemic.
METHODS: A systematic review identified and included 20 studies (from 3959 records screened from five databases) conducted in various region

Intestinal microbial community plays an important part in maintaining health and skeletal muscle development in livestock. This study is the first of its kind in the world. In order to better understand the relationship between gut microbiota and gene expression in skeletal muscle of rabbits, caecum contents and longissimus dorsi tissues of rabbits at 0 d (S1), 35 d (S2) and 70d (S3) were collected and subjected for 16S rRNA sequencing and transcriptome sequencing. Our results showed that, among three groups of rabbits, Firmicutes and Bacteroidetes were the dominant phyla at the phylum level,

The aim of this article is to present a research-based rationale for NETfacts, a newly developed integrated approach at the individual and the community level in order to mitigate the mental and social sequelae of war and crisis. To this end, we provide a selective review of relevant literature from neuroscience, clinical psychology, and social science. In psychotraumatology, individual avoidance describes the effort to prevent exposure to trauma reminders. Among patients with post-traumatic stress disorder (PTSD), this becomes pathological, exacerbating distress and preventing recovery. This

The purpose of this review was to identify the role and contribution of community-based nurse-led wound care as a service delivery model. Centres increasingly respond proactively to assess and manage wounds at all stages - not only chronic wound care. We conducted an integrative review of literature, searching five databases, 2007-2018. Based on inclusion and exclusion criteria, we systematically approached article selection and all three authors collaborated to chart the study variables, evaluate data, and synthesise results. Eighteen studies were included, representing a range of care models

A series of our studies on the development of the Dementia Assessment Sheet for Community-based Integrated Care System 21-items (DASC-21) were reviewed. Study 1: to examine the distribution of scores and internal reliability of DASC-21, trained nurses and researchers visited the homes of 1341 participants aged 65 years and older and living in the community. The nurses interviewed the participants and, when possible, their family members, to complete the DASC-21. Then, the Mini-Mental State Examination was carried out to select the participants of study 2. Study 2: to examine the concurrent and

BACKGROUND: Despite the fact that HIV-positive women carry an increased risk of developing cervical cancer (CC) in comparison with HIV-negative women, HIV and CC screening programs in many developing countries have remained unintegrated. The objective of this study is to explore perceptions and preferences of community members in Uganda, including women, men, and village health teams, regarding the integration of HIV and CC screening services in a single-visit approach.
METHODS: This qualitative study was conducted in three districts in Uganda. Data were collected through focus group discussi

BACKGROUND: Between 2008 and 2011, Rwanda introduced integrated community case management (iCCM) of childhood illness nationwide. Community health workers in each of Rwanda's nearly 15,000 villages were trained in iCCM and equipped for empirical diagnosis and treatment of pneumonia, diarrhea, and malaria; for malnutrition surveillance; and for comprehensive reporting and referral services.
METHODS: We used data from the Rwanda health management information system (HMIS) to calculate monthly all-cause under-5 mortality rates, health facility use rates, and community-based treatment rates for c

Diabetes is an enormous public health problem with particular concern within Hispanic communities and among individuals with low wealth. However, attempts to expand the public health paradigm to include social determinants of health rarely include analysis of social and contextual factors considered outside the purview of health research. As a result, conceptualization of the dynamics of diabetes health disparities remains shallow. We argue that using a holistic anthropological lens has the potential to offer insights regarding the nature of the interface between broader social determinants, h

BACKGROUND: In Ontario, Black people make up approximately 5% of the population and routinely account for over 20% of new HIV diagnoses. The A/C Study, a community-led cross-sectional study among Black Canadians, was designed to examine the factors that augment HIV risk and vulnerability for Black communities and inform policy and practice in Ontario, Canada, regarding HIV prevention and care for Black people. Knowledge translation (KT) was integrated throughout the research process as a core component of the study.
METHODS: Adhering to the principles of self-determination and autonomy in com
