Compartment Syndrome of the Calf Due to Nicolau Syndrome.

We report a case of Nicolau syndrome in a 15 months old girl following an intramuscular injection of penicillin 6.3.3 in her left buttock. This case is unique because she developed compartment syndrome in her left calf far from her injection site. Her toe's tips gangrened in the course of her ailment. We hypothesized that the compartment syndrome might be produced by a probable intra-arterial injection that had produced embolic obstruction of the small and medium size arteries in her leg or a probable perineural or periarteial injection had produced secondary sympathetic stimulation, extensive

Gluteal compartment and crush syndromes. Report of three cases and review of the literature.

Gluteal compartment syndromes are rare. The pathophysiology and the principles of diagnosis and treatment, however, are the same as those for leg and forearm compartment syndromes. Trauma may not be a salient feature of gluteal compartment syndromes where substance abuse and a prolonged period of unconsciousness, recumbency, or both are more typical. Because of this and the large muscle mass involved, systemic manifestations of a crush syndrome are usually present. Altered mental status and metabolic abnormalities may distract from the primary problem, resulting in delayed diagnosis and treatm

Immediate care of crush injuries and compartment syndromes with the split-thickness skin excision.

Prospective evaluation of the one-stage procedure in 32 patients who suffered from 37 severe friction-avulsion injuries was carried out. Most injuries (81%) were in the lower extremity. Split-thickness skin excision was used for immediate diagnosis of flap vascularity and wound coverage. Split-thickness skin was harvested from the avulsed flap surfaces after they were temporarily sutured back to their original bed. Dermal capillary bleeding served as an indicator for tissue viability, thus delineating a clear line for debridement. All compartments were decompressed, and debridement of all deva

Gluteal compartment syndromes: a report of three cases and management utilizing the Wick catheter.

Contusion or prolonged compression of the buttock produced severe local injury in 3 cases. Rhabdomyolysis with acute renal failure occurred in 2 of these cases, and one of them had an associated sciatic nerve palsy. The affected muscles included the gluteus maximus, gluteus medius, and the tensor fascia latae. Clinically and experimentally, these muscles were shown to be functionally enclosed within separate compartments like the peripheral limb muscles. Prompt decompression in the two cases with pressures exceeding 30 mm Hg resulted in muscle and nerve recovery.

Open 4-Compartment Fasciotomy for Chronic Exertional Compartment Syndrome of the Lower Extremity.

BACKGROUND: Chronic exertional compartment syndrome (CECS) is an underrecognized cause of lower extremity dysfunction in athletes. Open 4-compartment fasciotomy remains the gold standard for surgical management. INDICATIONS: Operative intervention is indicated for patients with clinically and objectively confirmed CECS who fail to respond to conservative management. Contraindications include inadequate nonoperative treatment or unaddressed medical comorbidities. Proper diagnosis requires exclusion of vascular or neurologic causes that can mimic CECS symptoms. TECHNIQUE DESCRIPTION: A 2-incis

Correlation between muscle oxygenation and compartment pressures in acute compartment syndrome of the leg.

BACKGROUND: Near-infrared spectroscopy estimates soft-tissue oxygenation approximately 2 to 3 cm below the skin. The purpose of the present study was to evaluate muscle oxygenation in the setting of an acute compartment syndrome of the leg and to determine if near-infrared spectroscopy is capable of detecting perfusion deficits. METHODS: Fourteen patients with unilateral lower extremity trauma were enrolled after the diagnosis of an acute compartment syndrome was made clinically and confirmed with intracompartmental pressure measurements. Lower extremity muscle compartments were evaluated wit

The thickness of the anterior compartment does not indicate compartment syndrome in acutely traumatised legs?

BACKGROUND: The study was conducted to achieve early detection of increased compartment pressure by comparing the changes between the thickness and pressure within the musculofascial compartments after lower leg injury. METHODS: We included cases of a lower-leg fracture between January and December 2007. Bilateral lower leg compartment pressure and thickness were measured before the surgery and post-anaesthesia, and the surgery was performed within 12 h after the initial trauma. The intracompartment pressure (ICP) was measured with a Wick catheter pressure monitor. The thickness (width) of th

Endoscopic compartment release for chronic exertional compartment syndrome: surgical technique and results.

BACKGROUND: Chronic exertional compartment syndrome is an entity that typically fails nonoperative management and requires operative treatment with fasciotomies for return to activity. Fasciotomies performed through single or multiple incisions may fail to totally release the fascia of the afflicted compartment(s) and may result in injury to neurovascular structures that cannot be visualized. PURPOSE: The authors report results of endoscopic compartment release with the assistance of a balloon dissector in the treatment of chronic exertional compartment syndrome. STUDY DESIGN: Case series; L

Ultrasound in acute compartment syndrome of the extremities.

Acute compartment syndrome (ACS) is a surgical emergency that remains challenging to diagnose. Diagnosis is primarily clinical, with invasive intracompartmental pressure (ICP) monitoring recommended when available. However, clinical findings can be unreliable, and invasive ICP measurement carries procedural risks. Ultrasound has increasingly been investigated as a noninvasive diagnostic method for ACS. This review analyzed all reported uses of ultrasound in the context of ACS. Six primary ultrasound techniques were identified: conventional two-dimensional (2D) ultrasound, Doppler ultrasound, p

Acute atraumatic compartment syndrome of the foot.

Acute atraumatic compartment syndrome (AACS) of the foot is an uncommon, limb-threatening condition that is frequently underrecognized due to its atypical presentation and the absence of high-energy trauma. We report a case of a 32-year-old male presenting with abrupt, severe foot pain and swelling 24 h after playing in an indoor park barefoot, with no history of injury. While initial radiographs, CT, and inflammatory markers were unremarkable, physical examination demonstrated pain disproportionate to findings and increased compartmental tension. Magnetic resonance imaging (MRI) was pivotal i

Acute Limb Ischemia and Compartment Syndrome.

Acute limb ischemia (ALI) and compartment syndrome (CS) are pathologies that can be difficult to identify but have devastating outcomes if not diagnosed in a timely manner. ALI can be missed if the provider fails to perform a pulse examination on every patient and fails to recognize the subtlety of early presentation. CS can be missed if providers are not aware of atypical presentations and fail to consider the diagnosis in certain at-risk patient populations.

A sudden presentation of abdominal compartment syndrome.

Abdominal compartment syndrome (ACS) is defined as sustained intra-abdominal pressure (IAP) exceeding 20 mm Hg, which causes end-organ damage due to impaired tissue perfusion, as with other compartment syndromes [1, 2]. This dysfunction can extend beyond the abdomen to other organs like the heart and lungs. ACS is most commonly caused by trauma or surgery to the abdomen. It is characterised by interstitial oedema, which can be exacerbated by large fluid shifts during massive transfusion of blood products and other fluid resuscitation [3]. Normally, IAP is nearly equal to or slightly above ambi

Renal allograft compartment syndrome: a review.

Renal allograft compartment syndrome (RACS) is the result of extrinsic compression resulting in graft dysfunction and loss due to ischaemia. A literature review was performed by computerized searches from the following data sources Medline, EMBASE, PubMed and Cochrane Library databases. Risk factors include size mismatch between graft and recipient. Intraoperative suspicion should be exercised if there is poor tissue turgor, cyanosis and loss of urine output upon fascial closure. Doppler ultrasound is the modality of choice amongst the literature to aid in diagnosis of RACS. From our study, th

Postoperative lumbar paraspinal compartment syndrome.

Lower lumbar paraspinal muscles constitute a compartment as they are surrounded by distinct fascial and bony boundaries. Lumbar paraspinal compartment syndrome is a rare entity, often caused by intense exercise, but also can be a postoperative complication. We present a 60-year-old man with low back pain, numbness in the left lower back and radicular pain in the left lower extremity, which started after a surgery that involved prolonged positioning on the left side 7 years before, and persisted to the day of evaluation. There was an immediate transient rise in the creatine kinase after surgery

Chronic exertional compartment syndrome of the forearm.

Chronic exertional compartment syndrome (CECS) is an overuse injury characterized by increased intracompartmental pressure during exercise. CECS has been described in the foot, thigh, and trunk, but 95% of cases occur in the lower leg. Interestingly, CECS may also affect the upper extremities and has been best described in the forearms. Unfortunately, due to the rarity of this condition, there is no consensus regarding its diagnosis and treatment. This review seeks to discuss the prevalence, etiology, diagnosis, and treatment of CECS of the forearms, which has been described in the literature.

Recurrent spontaneous compartment syndrome of the thigh.

Acute comparment syndrome (ACS) is a surgical emergency, in which tissue pressure becomes greater than perfusion pressure leading to tissue ischaemia. It is typically a consequence of trauma. We present a case in which a patient suffers blunt trauma to the thigh, but develops ACS 2 years after this injury and consequently endures 10-episodes of ACS (no perciptating event or cause) in the same thigh over 10 years. On the 10th presentation the patient was found to have arteriovenous malformation on MR angiography which were embolised and the fasciotomy wound closed with a split-thickness skin gr

Pelvic compartment syndrome: a systematic review.

Bilateral ureteral obstruction due to traumatic pelvic haematoma and increased pressure in the retroperitoneal space constitute an acute pelvic compartment syndrome. We systematically reviewed the available evidence concerning pelvic compartment syndrome using an online search of the MEDLINE databases OVID and PubMed. There were nine cases of pelvic compartment syndrome. A motor vehicle accident was the most frequent cause of pelvic compartment syndrome. Diagnosis was made using clinical and radiological methods in all cases. Treatment was by surgical decompression in 88% of cases. Observed co
