Increased conjunctival expression of protease activated receptor 2 (PAR-2) in seasonal allergic conjunctivitis: a role for abnormal conjunctival epithelial permeability in disease pathogenesis?

AIMS: Aeroallergen exposure to the conjunctival epithelium in seasonal allergic conjunctivitis (SAC) may induce a cellular stress response that disrupts the barrier properties of the conjunctival epithelium, resulting in allergic disease. Whether such changes occur in SAC is unknown. Epithelial permeability is known to be increased when protease activated receptor 2 (PAR-2) is activated. We evaluated the expression of PAR-2 in patients with SAC-in-season (SACS) and compared it with control non-atopic subjects or those with out-of-season allergic conjunctivitis (OSAC). METHODS: Six SACS, eight

Unveiling geographic conjunctival ulcer: a rare clinical presentation of herpetic eye disease.

A male patient in his 50s presented with a history of sudden onset redness and pain in the right eye for 15 days. He was under prescription of topical antibiotics, steroids and lubricants by the primary treating ophthalmologist for 2 weeks with reported worsening of symptoms. He was noted to have a visual acuity of 20/32 and 20/20 in the right and left eye, respectively. Clinical examination revealed diffuse conjunctival congestion and a focal area of conjunctival blanching which on fluorescein staining showed multiple geographic conjunctival ulcers. Rose Bengal staining revealed a fine cornea

Conjunctival microvascular hemodynamics following vaso-occlusive crisis in sickle cell disease.

Painful vaso-occlusive crisis (VOC) is the clinical hallmark of sickle cell disease (SCD). Microcirculatory hemodynamic changes following painful VOC may be indicative of future development of VOC events in subjects with SCD. The purpose of the present study was to determine alterations in conjunctival microvascular hemodynamics during non-crisis state in SCD subjects with a history of VOC. Conjunctival microcirculation imaging was performed to measure conjunctival diameter (D) and axial blood velocity (V) in 10 control and 30 SCD subjects. SCD subjects were categorized into two groups based o

The eye in bone marrow transplantation. III. Conjunctival graft-vs-host disease.

Of a series of 263 patients undergoing allogeneic bone marrow transplantation (BMT), 24 patients developed conjunctival involvement by graft-vs-host disease (GVHD). In each case, a distinct-appearing conjunctivitis developed that representing GVHD of the conjunctiva. In 19 cases (79%), conjunctival GVHD presented with pseudomembrane formation due to loss of the conjunctival epithelium; in 4 of these cases, the corneal epithelium was lost as well. Nineteen patients developed this feature in association with acute GVHD, and mortality among these patients was 89.5%. Kaplan-Meier analysis of survi

Quantitative analysis of conjunctival vascular alterations: Applications in ocular and systemic disease detection.

The conjunctival blood vessels are the only microcirculatory system on the body surface that can be observed non-invasively. Anatomically interconnected with multiple craniofacial circulatory systems, these vessels can indirectly reflect the blood supply to these areas and the overall state of systemic microcirculation. We synthesize findings from 48 studies spanning 2020-2025. Overall, existing research has found that the conjunctival vascular parameters can change in various diseases such as diabetes, cardiovascular diseases, and autoimmune diseases, and may even precede organic lesions. Objective evaluation of allergic conjunctival disease (with a focus on the application of artificial intelligence technology).

We have summarized the past efforts and results of objective measurement methods for conjunctival hyperemia classification. Severity classification using conjunctival blood vessel occupancy rate, ocular surface temperature analysis, and artificial intelligence have been reported to be clinically useful, as they have been found to correlate with the severity of conjunctival hyperemia by doctors. The AI method using slit lamp microscope images, whose main purpose is to be widely used in daily clinical practice, can be spread all over the world. As a result, it may lay the foundation for clinical

Human conjunctival surface mucins: a quantitative study of normal and diseased (KCS) tissue.

The architecture and distribution of surface mucins were studied qualitatively and quantitatively by transmission electron microscopy in ruthenium red stained biopsies of the human conjunctiva. Six control specimens were compared with three from early keratoconjunctivitis sicca (KCS) and with three specimens from severe KCS cases. The area of the ruthenium red/OsO4/mucin reaction product on the conjunctival epithelial surface was measured by image analysis and the values were expressed in micron2 per micron length of epithelium. The total area of surface mucins was differentiated from the narr
