Episodic memory deficits are almost always the first cognitive impairment in Alzheimer disease (AD). AD is also characterized by a loss of self-awareness. The aim of this article is to give an interpretation of AD patients' episodic impairments through the study of the relationship between memory and the self. Using the Remember/Know paradigm associated with the self-reference effect and emotional valence, we showed that this relationship may be impaired in AD. On the one hand, this could explain AD patients' difficulty accessing autonoetic consciousness, that is to say mentally bring back eve

INTRODUCTION: Accurate assessment of disorders of consciousness (DoC), including minimally conscious state (MCS) and unresponsive wakefulness syndrome (UWS/VS), remains a major challenge in clinical neurorehabilitation. EEG-based biomarkers offer a non-invasive approach for characterizing residual brain activity, yet their discriminative sensitivity may depend on auditory stimulus design.
METHODS: EEG recordings were obtained from 15 male patients with DoC during four auditory stimulation paradigms. Nonlinear, spectral power, and graph-theoretic features were extracted, yielding 136 candidate

BACKGROUND: Disorders of consciousness (DoC) are characterized by a broad decline in background excitatory synaptic activity and varying levels of cerebral network disruption. Repetitive transcranial magnetic stimulation (rTMS), a neuromodulatory technique, is anticipated to assist the recovery of consciousness. Nonetheless, ongoing debates persist regarding its effectiveness, in light of the inconsistent results of recent research.
OBJECTIVE: The purpose of this study is to investigate the efficacy of rTMS in promoting recovery of consciousness in patients with DoC and to probe its impact on

Neuroimaging studies have suggested an important role for the default mode network (DMN) in disorders of consciousness (DoC). However, the extent to which DMN connectivity can discriminate DoC states-unresponsive wakefulness syndrome (UWS) and minimally conscious state (MCS)-is less evident. Particularly, it is unclear whether effective DMN connectivity, as measured indirectly with dynamic causal modelling (DCM) of resting EEG can disentangle UWS from healthy controls and from patients considered conscious (MCS+). Crucially, this extends to UWS patients with potentially "covert" awareness (min

INTRODUCTION: Musical sensory orientation training (MSOT) is an innovative technique to improve the state of consciousness and cognitive function. Compared with traditional arousal therapies, MSOT offers a non-invasive, safe, and easily operable alternative with no side effects. This study aimed to conduct a preliminary investigation into the effect of MSOT in improving consciousness levels in patients with DoC (Disorders of Consciousness), as well as the feasibility of its clinical application, thereby providing reference for future large-sample randomized controlled studies.

PURPOSE OF REVIEW: Survivorship of severe acute brain injuries is increasing worldwide, with many of these injuries resulting in disorders of consciousness (DOC). We aim to review current challenges in achieving goal-concordant care for patients with DOC, while highlighting recent advancements to guide neurologic prognostication.
RECENT FINDINGS: Prognostic uncertainty, cognitive biases among clinicians and surrogate decision-makers (SDM), and a paucity of nuanced neurologic assessments in many settings pose major barriers to accurate prognostication. Short observation times and large variati

Modern medicine has been shaken by the surge of psychedelic science that proposes a new approach to mitigate mental disorders, such as depression and post-traumatic stress disorder. Clinical trials to investigate whether psychedelic substances can treat psychiatric conditions are now underway, yet less discussion gravitates around their use in neurological disorders due to brain injury. One suggested implementation of brain-complexity enhancing psychedelics is to treat people with post-comatose disorders of consciousness (DoC). In this article, we discuss the rationale of this endeavour, exami

Epileptic seizures/post-traumatic epilepsy (ES/PTE) are frequent in persons with brain injuries, particularly for patients with more severe injuries including ones that result in disorders of consciousness (DoC). Surprisingly, there are currently no best practice guidelines for assessment or management of ES in persons with DoC. This study aimed to identify clinician attitudes toward epilepsy prophylaxis, diagnosis and treatment in patients with DoC as well as current practice in regards to the use of amantadine in these individuals. A cross-sectional online survey was sent to members of the I

BACKGROUND: Making an accurate diagnosis in patients with disorders of consciousness remains challenging. 18F-fluorodeoxyglucose (FDG)-PET has been validated as a diagnostic tool in this population, and allows identifying unresponsive patients with a capacity for consciousness. In parallel, the perturbational complexity index (PCI), a new measure based on the analysis of the electroencephalographic response to transcranial magnetic stimulation, has also been suggested as a tool to distinguish between unconscious and conscious states. The aim of the study was to cross-validate FDG-PET and PCI,

PURPOSE OF REVIEW: The burden of proof for establishing diagnosis and prognosis in patients with disorders of consciousness lies with behavioral assessment methods. The current review discusses recent advances in understanding the strengths and weaknesses of this methodology.
RECENT FINDINGS: Behavioral assessment methods remain the 'gold standard' for establishing diagnosis and prognosis in patients with disorders of consciousness, although their psychometric integrity and clinical utility remain largely unproven. While the Glasgow Coma Scale maintains its standing in the trauma setting, the

OBJECTIVE: To describe the characteristics of patients enrolled in disorders of consciousness (DoC) rehabilitation programs and to examine factors associated with improvement beyond measurement error on neurobehavioral function. Outcomes for adults with DoC after severe brain injury are highly variable and difficult to predict. Applying a minimal detectable change (MDC) threshold to change measures can identify help to distinguish true improvement or decline from random fluctuation. We also evaluated whether change in neurobehavioral function during the first 2 weeks of rehabilitation was asso

The human brain controls various cognitive functions via the functional coordination of multiple brain regions in an efficient and robust way. However, the relationship between consciousness state and the control mode of brain networks is poorly explored. Using multi-channel EEG, the present study aimed to characterize the abnormal control architecture of functional brain networks in the patients with disorders of consciousness (DOC). Resting state EEG data were collected from 40 DOC patients with different consciousness levels and 24 healthy subjects. Functional brain networks were constructe

We report 3 cases of spontaneous intracranial hypotension (SIH) associated with consciousness disorder and coma. In patients, SIH was suspected on a computed tomography scan and diagnosed by cerebral magnetic resonance imaging (MRI). Spinal MRI confirmed cerebrospinal fluid leakage. SIH should be seen as an underestimated cause of consciousness disorder and coma, especially in patients with a history of orthostatic headache, spinal injury, or oculomotor signs. Computed tomography scans should be examined for signs of SIH before operating on patients with a spontaneous subdural hematoma. Brain

The technique of transauricular vagus nerve stimulation (taVNS) is methodologically applied by placing electrodes on the auricular region, specifically on the cymba, cavum, or tragus, to stimulate the vagus nerve non-invasively. Additionally, the earlobe, which is not innervated by the vagus nerve, can serve as a placebo site for control stimulation. The stimulation intensity is adjusted between 0.5-3.0 mA, with session durations ranging from 20-60 min, administered either daily or weekly, depending on the study protocol. The total number of sessions varies according to the protocol, extending
