This research introduces an innovative solution to address the challenges of bacterial keratitis and alkali burns. Current treatments for bacterial keratitis and alkali burns rely on the frequent use of antibiotics and anti-inflammatory eye drops. However, these approaches suffer from poor bioavailability and fluctuating concentrations, leading to limited efficacy and potential drug resistance. Our approach presents an adaptive drug-releasing contact lens responsive to reactive oxygen species (ROS) at ocular inflammation sites, synchronously releasing Levofloxacin and Diclofenac. Contact lens-assisted corneal cross-linking (CACXL) was introduced by Jacob et al. in 2012 for treating thin keratoconic corneas using riboflavin soaked soft contact lens to artificially increase the functional corneal thickness. It is advantageous over other thin corneal cross-linking techniques as it works independent of swelling properties of the cornea, is an epi-off technique and does not require additional time, additional expensive equipments or special solutions. The only additional requirement as compared to all other techniques is a UV barrier-free soft contact lens (SoflensTM, B&L)

PURPOSE: To determine the rates of microbial contamination and identify contaminants associated with contact lens (CL) care accessories. To investigate self-reported compliance levels with care of CL accessories and its association with contamination level.
METHODS: A total of 46 CL cases and care solutions from asymptomatic soft contact lens wearers were collected. Samples from the CL cases and care solutions were cultured for microbiological identification and enumeration. A questionnaire regarding compliance to care and hygiene procedures were administered to each subject on their visit.

Thirty-eight patients were examined because of ocular redness, irritation, and corneal changes apparently related to soft contact lens wear. The corneal changes were transient and ranged from faint epithelial opacities to a coarse, punctate epithelial keratopathy. Solutions containing thimerosal had been used by all of the patients for lens care, and 31 responded to an ocular challenge with a thimerosal-preserved lens lubricant. Twenty-seven of these 31 also reacted to thimerosal patch testing. The 31 resumed lens wear with relief of symptoms by using unit-dose, preservative-free saline for le

Cellulose acetate butyrate extended-wear contact lenses were fitted bilaterally on ten New Zealand albino rabbits. We administered flurbiprofen 0.03% solution topically to the experimental eyes and vehicle solution to the contralateral control eyes four times a day in a random, masked fashion beginning 24 hours before the contact lens was fitted. Corneal neovascularization, which developed in all eyes by the 25th day of contact lens wear, was significantly suppressed by flurbiprofen treatment (mean vessel length was 1.5 +/- 0.4 mm for the treated eyes and 3.0 +/- 0.3 mm for the control eyes; P

Acanthamoeba species are an important cause of microbial keratitis that may cause severe ocular inflammation and visual loss. The first cases were recognized in 1973, but the disease remained very rare until the 1980s, when an increase in incidence mainly associated with contact lens wear was reported. There is an increased risk when contact lens rinsing and soaking solutions are prepared with nonsterile water and salt tablets. The clinical picture is often characterized by severe pain with an early superficial keratitis that is often treated as herpes simplex infection. Subsequently a charact

Currently, bacterial conjunctivitis is managed by multiple antibiotic eye-drop solution, which is highly inefficient due to low ocular bioavailability and frequent dosing. Therapeutic soft contact lenses can be used to sustain the release of ocular drugs. However, the conventional soaking method (economic and widely used) showed low drug uptake and high burst release, and the optophysical properties of the contact lens were altered for clinical application. In this paper, novel ofloxacin-loaded niosomes were developed to increase the drug loading capacity of contact lenses while also sustainin

PURPOSE: Twelve volunteers participated in a study designed to measure the overnight corneal edema response with a variety of hydrogel contact lenses. During the study four subjects (5 eyes) experienced a contact lens related acute red eye (CLARE) reaction, which manifested as severe ocular pain, photophobia, corneal infiltration, and conjunctival hyperemia. An additional five subjects (7 eyes) developed corneal infiltrates only. Twelve eyes (of 9 subjects) showed no response.
METHODS AND RESULTS: Upon microbiological examination of the contact lenses and storage solutions, gram-negative bact

A 21-year-old woman developed bilateral keratoconjunctivitis from contact lens wear, which progressed to diffuse corneal scarring and vascularization after the patient refused to discontinue wearing contact lenses. The visual disturbance became so severe that a penetrating keratoplasty had to be performed in one eye for visual rehabilitation. Examination of the penetrating keratoplasty specimen disclosed destruction of Bowman's membrane throughout the superior half of the cornea, which was replaced by a fibrous scar that was only midly chronically inflamed. Deep vascularization occurred within

SIGNIFICANCE: An equivalent 12 months of cleaning did not induce significant changes in the optical aberrations or base curves of scleral lenses.
PURPOSE: This study aimed to test whether an equivalent of 12 months of manual cleaning alters the optical and physical properties of conventional and wavefront-guided scleral lenses.
METHODS: Twelve scleral lenses (four repeats of three designs, termed A, B, and C) were manufactured in Boston XO material: design A, -5.00 D defocus; design B, -5.00 D defocus with -0.153-μm vertical coma; and design C, -5.00 D defocus with a full custom wavefront-gu

Contact lens-associated corneal infiltrative events (CIEs) are presumed sterile events that have complicated contact lens wear for more than 30 years. There is consistent evidence that silicone hydrogel soft contact lenses increase CIE risk by twofold compared with low Dk hydrogel materials. The incidence of CIEs during silicone hydrogel extended wear ranges from 2% to 6% for symptomatic events and from 6% to 25% when asymptomatic events are included. For daily wear, with silicone hydrogels, the incidence of CIEs ranges from 2% to 3% for symptomatic events and from 7% to 20% when asymptomatic

Sight-threatening microbial keratitis associated with contact lens wear remains a serious concern for patients, eye-care practitioners, and the contact lens industry. Several decades of research and some major advances in lens and solution technology have not resulted in a decline in disease incidence. Here, we offer a perspective on the complex pathogenesis of microbial keratitis, the factors that have prevented a better understanding of this disease, and new approaches being used to tackle this important clinical problem.

PURPOSE: To investigate whether contact lenses used after surgery for congenital cataracts act as a depot for dexamethasone, which would allow the prescribed amount of drops to be reduced, and to examine whether the preservative benzalkonium chloride accumulates in the contact lens matrix, which would suggest a need for more frequent replacements.
METHODS: Contact lenses (n = 10) worn by infants treated with dexamethasone eye drops after congenital cataract surgery were analysed with scanning electron microscopy, UV-vis, 1 H-NMR and LDI-MS for chemical deposits and for changes on the contact

The wetting and soaking solutions and contact lens cases of eye clinic patients commonly were contaminated with gram-negative bacteria during their use. Serratia marcescens occurred most frequently in preserved solutions, whereas Pseudomonas aeruginosa was most often recovered from home-prepared saline. The bacteria were recovered at densities of greater than 10(6) cells per ml and typically persisted in the solutions. Eight patients who developed bacterial keratitis during 1986 used solutions contaminated with the etiological agents of the infections. Improper hygienic practices of the patien

Drug-eluting contact lens can substitute the multiple eye drop therapy. However, loading hydrophobic drug like cyclosporine in the contact lens is very challenging, due to low drug uptake (via soaking method); and alteration in the swelling and optical properties which restricts its clinical application. To address the above issues, graphene oxide (GO, large surface area with oxygen containing functional groups) was incorporated in the contact lenses during fabrication. These GO-laden contact lenses (SM-GO-Cys) as well as blank contact lenses (SM-Cys) were soaked in the solution of cyclosporin

Over a 21-month period, we treated corneal ulcers in 11 patients using daily-wear contact lenses and 29 patients using extended-wear contact lenses. Since more patients use daily-wear than extended-wear lenses, this suggests that patients using extended-wear lenses are at greater risk for the development of corneal ulcers. Positive bacterial cultures were obtained from the corneal scrapings of nine of 11 patients using daily-wear lenses and 20 of 29 patients using extended-wear lenses. In the daily-wear group, all 11 patients failed to exercise satisfactory care in using and disinfecting their

Currently, conjunctivitis is treated by frequent high dose administration of sparfloxacin eye drop solution. However, the eye drops are inconvenient because of low bioavailability, short ocular drug residence time, and need of frequent instillation, which lead to patient noncompliance affecting the routine life style of patients. Silicone contact lenses can be used to sustain the release of sparfloxacin. However, the presence of sparfloxacin alters the optical and physical properties of the contact lens. To overcome the issues, a novel polyvinyl pyrrolidone (PVP)-coated sparfloxacin-laden ring
