An emerging new standard of care: initial and continued treatment for patients with signs and symptoms of active periodontal disease.

A growing knowledge of periodontal disease has improved dentists' understanding of the disease's etiology, pathophysiology, and natural history. This increased expertise has allowed treatment strategy and outcomes to improve. General dentists are the first to diagnose and treat the disease and it is important for them to understand both the disease and the new methods of treatment. Locally administered time-released antibiotics target specific microorganisms and may inhibit the action of collagenase, an extremely destructive enzyme that is released into periodontal tissues via the host respons

Continuous sedation in adolescent and young adult cancer patients under home-based end-of-life care.

PURPOSE: This study examined the implementation status of continuous sedation (CS) and its clinical and social background factors in adolescent and young adult (AYA) patients with cancer who received end-of-life care at home. METHODS: We conducted a single-center retrospective observational study with 62 AYA patients with cancer. We compared the CS (S) and non-sedation (NS) groups, analyzing factors contributing to sedation implementation. RESULTS: CS implementation rate was 37.1% (23/62 cases). Participants' median age was 36 (S group) and 33 (NS group) years (p = 0.08). Both groups had sim

Impact of continuity of care under the model of healthcare integration on treatment outcomes and prognostic recovery of orthodontic patients.

Malocclusion is one of the most serious oral problems following dental caries and periodontal disease. Although orthodontic treatment can effectively improve these problems, improper care often leads to poor orthodontic results or complications such as gingivitis. This is because the treatment period is long. Based on this, this study aimed to investigate the effect of continuity of care under the integrated model of health care on the treatment efficacy and prognosis of orthodontic patients. This study used a randomized controlled design. From June 2022 to December 2022, 68 patients who recei

Continuous positive airway pressure in COVID-19-associated respiratory failure: improving patient care with a proforma.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and the disease it causes (COVID-19) has proven to be the greatest challenge to modern global health care, infecting more than 450 million people worldwide and responsible for just over 6 million deaths. Over the past 2 years there have been major advances in the treatment of COVID-19, including a significant reduction in the number of individuals developing severe symptoms, since the introduction of COVID-19 vaccines and the advances in pharmacological treatments. However, for those individuals who become infected with COVID-19 and

Outcomes of COVID-19 patients treated with continuous positive airway pressure outside the intensive care unit.

AIM: We aimed to characterise a large population of coronavirus disease 2019 (COVID-19) patients with moderate-to-severe hypoxaemic acute respiratory failure (ARF) receiving continuous positive airway pressure (CPAP) outside the intensive care unit (ICU), and to ascertain whether the duration of CPAP application increased the risk of mortality for patients requiring intubation. METHODS: In this retrospective, multicentre cohort study, we included adult COVID-19 patients, treated with CPAP outside ICU for hypoxaemic ARF from 1 March to 15 April, 2020. We collected demographic and clinical data

The need for continuous quality assessment for providing optimal comprehensive care for patients with alpha-1 antitrypsin deficiency.

Background: Alpha-1-antitrypsin deficiency (AATD) is an orphan disease that mainly affecting the liver and the lung. This creates difficulties to ensure that comprehensive care is administered to both organ systems. Past assessments of care delivered to patients with AATD demonstrated that improvements are needed. For that reason, we reassessed a population of patients with AATD in a large health care system to see if past findings affected present care. Methods: We performed electronic health record (EHR) reviews on all patients with documented AATD and confirmed the diagnosis by evidence of

Continuous subcutaneous infusion for pain control in dying patients: experiences from a tertiary palliative care center.

BACKGROUND: Continuous subcutaneous infusion (CSCI) via ambulatory infusion pump (AIP) is a valuable method of pain control in palliative care. When using CSCI, low-dose methadone as add-on to other opioids might be an option in complex pain situations. This study aimed to investigate the effects, and adverse effects, of CSCI for pain control in dying patients, with particular interest in methadone use. METHODS: This was an observational cohort study. Imminently dying patients with pain, admitted to specialized palliative inpatient wards and introduced on CSCI, were monitored daily by staff f

Patient expectations of AI-based computerized clinical decision support system for improving continuity of care after hospitalization: a cross-sectional study on rehabilitation after total hip or knee replacement for osteoarthritis, with pilot questionnaire.

Continuity of care after hospital discharge is a common issue for patients who underwent major surgery. Computerized clinical Decision Support Systems (CDSSs) can help physicians stratify patients based on clinical and social characteristics, make predictions about relevant care milestones and outcomes, and make personalized decisions as early as possible, including safe discharge planning, based on timely activation of the relevant services. To enhance the clinical uptake and effectiveness of these technologies, it is crucial to determine whether patients feel that their personal priorities f

Critical care nurses' experiences of managing patients receiving continuous kidney replacement therapy in intensive care units: a qualitative study in Saudi Arabia.

BACKGROUND: Acute kidney injury requiring kidney replacement therapy is a common and life-threatening complication among critically ill patients. Continuous kidney replacement therapy (CKRT) is frequently used in haemodynamically unstable ICU patients and places substantial technical, cognitive, emotional, and relational demands on critical care nurses. Although nurses are central to CKRT delivery, limited qualitative evidence explores their lived experiences of providing technology-embedded kidney support in Middle Eastern ICU contexts. METHODS: A qualitative study using reflexive thematic a

Knowledge, Attitude, and Practice of Clinicians Regarding Continuous Renal Replacement Therapy for Critical Care Patients in Southern Palestine.

Continuous renal replacement therapy (CRRT) is crucial for managing acute kidney injury in critical care settings, particularly when traditional dialysis is unsuitable. In resource-limited regions like Palestine, intensive care unit (ICU) clinicians' knowledge, attitudes, and practices (KAP) significantly impact patient outcomes. This study assessed the KAP of ICU clinicians in southern Palestine to identify gaps and propose improvements. A cross-sectional study in 6 hospitals across Hebron and Bethlehem involved 250 ICU clinicians. Data were collected using a structured questionnaire and anal

Continuous Opioid Infusion Among Patients Who Receive Mechanical Ventilation and Who Transition to Comfort-Focused Care.

FoxO transcription factors (TFs) extend lifespan in invertebrates and may participate in the control of human longevity. The role of FoxO TFs in lifespan regulation has been studied most extensively inC. elegans, where a conserved insulin/insulin-like growth factor signaling (IIS) pathway and the germline both control lifespan by regulating the subcellular localization of the FoxO transcription factor DAF-16. Although the control of FoxO activity through modulation of its subcellular localization is well established, nuclear translocation of FoxO is not sufficient for full FoxO activation, sug
