[Contraindications to lung transplantation: evolving limits?].

In France, the higher frequency of pulmonary sample in organ donors and the enhancement of surgical and perioperative life support techniques, have increased the number procedures and the short term prognosis of lung transplantation (LT). In this setting, the classical contraindications of LT need to be reconsidered. In this article, some of the classical contraindication of LT are confronted to the experience acquired in other solid organ transplantations or from some LT centers. Specific situations such as LT in patients with previous cancer, HIV infection, viral hepatitis, nutritional disor

[Interventional management of secondary retentions after thoracic and abdominal trauma].

BACKGROUND: Percutaneous treatment of secondary thoracic and abdominal retention formations after blunt or penetrating trauma now represents a standard interventional radiological procedure. Various supportive imaging procedures are available, whereby computed tomography is mostly the treatment of choice due to the high diagnostic sensitivity. OBJECTIVE: Based on clinical examples this review article gives an overview of the state of the art minimally invasive interventional treatment of secondary posttraumatic retention formations of the thorax and abdomen. The indications and contraindicati

A Prospective Randomized Study Comparing Mini-surgical Percutaneous Dilatational Tracheostomy With Surgical and Classical Percutaneous Tracheostomy: A New Method Beyond Contraindications.

Although percutaneous dilatational tracheostomy (PDT) is more accessible and less time-demanding compared with surgical tracheostomy (ST), it has its own limitations. We introduced a modified PDT technique and brought some surgical knowledge to the bedside to overcome some standard percutaneous dilatational tracheostomy relative contraindications. PDT uses a blind route of tracheal access that usually requires perioperational imaging guidance to protect accidental injuries. Moreover, there are contraindications in certain cases, limiting widespread PDT application. Different PDT modifications

[Renal biopsy: standard procedure of modern nephrology].

For more than 50 years, renal biopsy has been an important diagnostic procedure in modern nephrology. Increasing perfection of the biopsy technique has made the procedure very safe, with rare complications and few contraindications. Improved histological diagnostics and broader therapeutic possibilities have significantly expanded the indications. The most important indications are acute renal failure, proteinuria > 1 g/d (or 3 g/d) and nephritic urinary sediment. Complications with renal grafts provide additional indications for a renal biopsy. In addition, diabetes and monosymptomatic urinfi

Ross Procedure After Valve-Sparing Root Replacement With Marfan Syndrome.

BACKGROUND: The Ross procedure is one of the optimal surgical options for aortic valve pathologies in relatively young patients. However, its application in patients with Marfan syndrome (MFS) is not only debatable but may even be considered a contraindication. CASE SUMMARY: A 31-year-old woman with MFS had undergone valve-sparing root replacement 2.5 years earlier and now presented for treatment of severe aortic insufficiency (AI). A Ross procedure with stabilization of the aortic root was performed. Two years after the procedure, mild AI was observed; however, the patient was asymptomatic,

The WATCHMAN Device Review: A New Era for Stroke Prophylaxis.

Atrial fibrillation (AF) is a major risk factor for ischemic stroke, accounting for more than 37 million cases worldwide. In AF, the left atrial appendage (LAA) is the most common site of thrombus formation, and its ligation/closure with the WATCHMAN device is a good alternative to long-term oral anticoagulation, especially in patients with contraindications to warfarin. However, the implantation procedure is associated with various risks and complications. A short-term anticoagulant and antithrombotic administration are essential after implantation. However, no consensus has been reached on t

Percutaneous closure of left atrial appendage to prevent embolic events in high-risk patients with chronic atrial fibrillation.

BACKGROUND: Percutaneous closure of the left atrial appendage (LAA) is a novel alternative for the treatment of patients with atrial fibrillation (AF) and with a high risk of stroke who are not eligible for long-term anticoagulation therapy. The aim of this study was to asses the safety, feasibility, and long-term efficacy of this procedure. METHODS: From July 2004 to June 2007, 20 patients (13 male, mean age 69 +/- 8 years) with non-valvular AF (NV-AF) underwent LAA percutaneous closure using the PLAATO system, implanted through a transeptal access. All patients had contraindications to anti

Laser-assisted uvulopalatoplasty: long-term results with a treatment for snoring.

We present a retrospective analysis of the initial results of our institution's evaluation of a new procedure designed to help relieve snoring, as well as having the potential to assist in the treatment of obstructive sleep apnea. The ambulatory, in-office CO2 laser procedure is performed with a local anesthetic. It is a serial procedure that achieves the same end-result as conventional uvulopalatopharyngoplasty, but without many of the associated problems. The indications for the procedure are reviewed, and detailed technical aspects of its performance, including use of new instrumentation, a

Initial experience with a novel percutaneous left atrial appendage exclusion device in patients with atrial fibrillation, increased stroke risk, and contraindications to anticoagulation.

Atrial fibrillation (AF) increases by fivefold a patient's risk for thromboembolic stroke. The main source of emboli in AF is the left atrial appendage (LAA). Therefore, LAA closure could reduce the risk for thromboembolic events in AF. The investigators report the first United States experience with a novel percutaneous LAA closure device, the Lariat snare device, and its outcomes in 21 patients with AF, CHADS2 scores ≥2, and contraindications to anticoagulation. The LAA was closed with a snare containing suture from within the pericardial space. The intraoperative success of the procedure wa

Capsule endoscopy of the esophagus.

Video capsule endoscopy has acquired wide clinical acceptance since its the US Food and Drug Administration approval in 2001. Recently, the technology of video capsule endoscopy has been adapted to other organs in the gastrointestinal tract, including the esophagus and colon. In this review, we discuss esophageal capsule endoscopy (ECE)-the procedure, its indications, contraindications, safety, and future applications. ECE is a minimally invasive procedure that uses special video capsules with ability to acquire images from 2 cameras with high image storing speed of 14 to 18 frames per second.

[Video-assisted minimally invasive parathyroidectomy with median access. Technical changes: case load 1999-2002].

INTRODUCTION: In 1997 a Minimally Invasive Video Assisted Technique for Parathyroidectomy (MIVAP) was developed in the University of Pisa. In this review we examine the last three years of MIVAP (240 cases) in order to identify the advantages and the limits of the procedure after the first-period development of the technique. RESULTS: In our experience, 65% of patients affected by primary hyperparathyroidism (PHPT) turned out to be eligible for MIVAP. During the first years several selection criteria were strictly followed; more recently, some initially absolute contraindications to the opera

[The use of the lingual flap in the primary reconstruction of the hypopharynx].

From September 1991 till January 1994 in ENT Clinic in Cracow 5 patients with far advanced laryngeal cancer involving hypopharynx were treated surgically. Total laryngectomy with partial or total hypopharyngectomy was performed and then the reconstruction of hypopharynx one stage procedure using lingual flap was done. In all patients functional results were acceptable. The main contraindications for using lingual flap is tumor infiltration of the base of tongue.

[Gastroscopy -- a routine procedure in the investigation of diseases of the stomach (author's transl)].

After a short review of the development of endoscopic investigations, the technique of gastroscopy is described and the indications for this procedure are pointed out. This procedure is of equal importance as radio-diagnostic measures in the routine investigation of stomach diseases and is of particular value when used as an emergency procedure and in the follow-up of patients after gastric operations. It also offers great advantages in the diagnosis of stomach tumours since biopsy material can be obtained from the lesion. Stomach haemorrhage due to gastric erosion, oesophagitis and the Mallor

A New Strategy for Transcatheter Left Atrial Appendage Closure With Cerebral Embolic Protection in Patient With Left Auricular Thrombosis and Total Contraindication to Long-Term Anticoagulation.

Atrial fibrillation (AF) is the most common cardiac arrhythmia, occurring in 1%-2% of the general population. An important aspect is the treatment of AF in terms of stroke prevention. In patients with absolute contraindication to long-term anticoagulation due to high bleeding risk, a valuable alternative exists in left atrial appendage (LAA) closure. Unfortunately, thrombus in the LAA is a contraindication to the procedure because of high risk of embolization. We describe a clinical case with permanent AF, absolute contraindication to long-term anticoagulation therapy, and persistent thrombus

Future of left atrial appendage interventions.

Percutaneous left atrial appendage occlusion (LAAO), currently recognized as a procedure with relatively low risk, is increasingly being adopted in clinical practice. However, due to the preventive nature of the procedure and the necessity to compare it with newer and safer oral anticoagulants, further optimization is required to address remaining challenges. These latter include acquiring comprehensive data on safety and efficacy, establishing standardized pre-procedural planning, and simplifying procedural process. Consequently, we have provided an overview that encompasses future opportunit

Ulnar Nerve Anterior Transmuscular Transposition in the Lateral Decubitus Position.

Ulnar nerve anterior transmuscular transposition is a well-accepted surgical technique for the treatment of ulnar nerve entrapment or subluxation at the elbow. The procedure, which addresses both compression and traction forces on the nerve, may be performed with the patient in the lateral decubitus position, allowing direct access to and excellent visualization of the ulnar nerve through an incision on the posteromedial aspect of the elbow. This report reviews the surgical indications and contraindications, pertinent anatomy, patient positioning rationale and method, surgical technique, posto

Vocal Fold Medialization Procedures in Previously Radiated Patients: A Survey of Practice Patterns.

BACKGROUND/OBJECTIVES: Head and neck radiation therapy (HNRT) has traditionally been considered a contraindication to vocal fold medialization procedures. Although safety has been demonstrated, we hypothesize that actual management varies. This study evaluates practice patterns of otolaryngologists regarding vocal fold medialization in patients after HNRT. METHODS: A 25-question survey evaluating respondents' management of patients status post HNRT with vocal fold paresis/paralysis was distributed to 357 otolaryngologists.
