Horizontal fusional responses to stimuli containing artificial scotomas.

Horizontal fusional responses were studied with stimuli containing binocular or monocular, artificial, stabilized, scotomas. Binocular scotomas of 5-deg, 10-deg, and 15-deg diameters were utilized. The fusional responses to scotomatic stimuli were compared with full-field stimulus responses. All responses contained significant motor and nonmotor (sensory) components. Overall motor compensation to stimuli with 10-deg and 15-deg scotomas was reduced, while the overall motor compensation to stimuli with 5-deg scotomas was not. With full-field stimuli and with stimuli containing binocular scotomas

Ophthalmologic features of Parkinson's disease.

Patients with Parkinson's disease (PD) commonly complain of impaired visual function and difficulty reading, despite normal visual acuity. Although previous studies have evaluated contrast sensitivity, color vision, visuospatial processing, visual hallucinations, and ocular movements, none has systematically evaluated the ocular complaints and ocular findings of PD patients. Thirty patients with early untreated PD and 31 control subjects without neurologic or known ocular diseases were ophthalmologically evaluated for the frequency of visual complaints, dry eyes, blepharitis, visual hallucinat

Objective vs subjective measures of fixation disparity for short and long fixation periods.

PURPOSE: Fixation disparity, i.e. the vergence error for stationary fusion stimuli, can be measured objectively with eye trackers and subjectively with nonius lines. Between these two measures, previous studies found differences that tended to be proportional to the amount of forced vergence, i.e. the discrepancy between vergence and accommodative stimulus. We investigate whether objective and subjective fixation disparity might be similar without forced vergence. METHOD: We simultaneously measured fixation disparity with the EyeLink II system and with flashed dichoptic nonius lines in 17 sub

Attenuation of perceived motion smear during vergence and pursuit tracking.

When the eyes move, the images of stationary objects sweep across the retina. Despite this motion of the retinal image and the substantial integration of visual signals across time, physically stationary objects typically do not appear to be smeared during eye movements. Previous studies indicated that the extent of perceived motion smear is smaller when a stationary target is presented during pursuit or saccadic eye movements than when comparable motion of the retinal image occurs during steady fixation. In this study, we compared the extent of perceived motion smear for a stationary target d

The influence of repetitive eye movements on vergence performance.

We measured the peak velocity of convergence eye movement responses in four normal subjects before and after a large number of either repetitive vergence or repetitive saccadic eye movements. A 20% decrease in the mean value of peak velocity was observed in vergence responses after 100 repetitive step vergence eye movements. However, 100 cycles of slow sinusoidal vergence tracking did not induce any notable change in vergence dynamics. Five hundred repetitive saccadic eye movements also caused an approximately 20% decrease in peak velocity. The reduction in peak velocity was related to the num

Relationship between clinical signs and symptoms of convergence insufficiency.

PURPOSE: The percentage of children who are symptomatic has been shown to increase with the number of signs of convergence insufficiency (CI). Our goal was to investigate whether there is a relationship between the severity of the clinical signs of CI and symptom level reported in children with a three-sign symptomatic CI. METHODS: The Convergence Insufficiency Treatment Trial enrolled 221 children with symptomatic CI from ages 9 to 17 years. Inclusion criteria included the following three signs of CI: (1) exophoria at near at least 4Δ greater than at distance, (2) insufficient positive fusio

Emmetropisation in normal and strabismic children and the associated changes of anisometropia.

The purpose of this study was to measure whether emmetropisation failed in children who had strabismus irrespective of their refraction in infancy, and to record simultaneous changes in anisometropia. We also report how often hypermetropia increased before these children presented with esotropia. A total of 2920 infants had a cycloplegic retinoscopy at age 5-7 months and again at 42 months or when defective vision was identified. Changes of refraction in 210 children with strabismus are compared with the remaining 2710 who did not. When the spherical equivalent of the fixing eyes was > +2.75 D

Natural strabismus in monkeys: accommodative errors assessed by photorefraction and their relationship to convergence errors.

PURPOSE: To examine accommodation and accommodative convergence relationships in naturally strabismic monkeys. METHODS: Photorefraction was used to measure accommodative responses of each eye under monocular and binocular viewing conditions. These accommodative results were then compared to assessments of vergence state made under monocular viewing conditions using methods previously described. RESULTS: Accommodation was always accurate under monocular viewing conditions, with the exception of the inability of one myopic eye to focus distant targets. The accommodative response in the two eye

[The accommodation-convergence bar (A=C bar)].

The accommodation-convergence bar was developed as a simple measuring and exercise instrument. For the ophthalmologist, it is a convenient means of measuring accommodation and convergence in the office, while patients can use it for home convergence and fixation exercises which they can monitor themselves. The A = C bar has also been used experimentally to eliminate the functional central scotoma in dyslexic children.

Complex regional pain syndrome type I: associated visual sensorimotor case findings.

OBJECTIVE: The objective was to present new visual sensorimotor findings in a patient with complex regional pain syndrome type I, formerly known as reflex sympathetic dystrophy. DESIGN: Clinical measurements were compared for the following visual sensorimotor tasks before and after 10 minutes of near visual stimulation: accommodation, vergence, and reading eye movements. PATIENT: The patient was a 19-year-old female university student with complex regional pain syndrome type I. RESULTS: All visual sensorimotor findings worsened dramatically after performance of the brief near visual task. I

Vergence adaptation in children and its clinical significance.

BACKGROUND AND PURPOSE: Vergence adaptation has not been well investigated in children even though it may contribute to binocular dysfunction and near work induced asthenopia. METHODS: We compared vergence adaptation in 18 children and 18 young adult subjects by assessing tonic vergence (TV) before and immediately after a period of sustained near fixation, by measuring heterophoria with a synoptophore through 0.5 mm binocular pinholes. Adaptation was induced by a reading task at 15 cm for a continuous 5 minute period. RESULTS: Mean pre-task TV values of 0.70 MA (Meter Angles) and 0.20 MA wer

The effect of perceived distance on accommodation under binocular steady-state conditions.

Spatiotopic cues, such as perceived distance, have little effect on accommodation unless blur has been reduced or eliminated. We investigated the effect of perceived distance on accommodation under binocular steady-state conditions, about which little is known. Blur was reduced but not eliminated by using a stimulus with a moderately low luminance. Accommodation was measured under two conditions: (1) when cues from perceived distance, blur, and convergence were aligned; and (2) when perceived distance was opposed by both blur and convergence. We found a significant difference in accommodation

The effect of monovision lenses on the near-point range of single binocular vision.

Monovision (MV) contact lenses are commonly used to provide optical correction for presbyopia. Based on their review of the literature on fusional mechanisms, Erickson and Schor predicted that monocular blur associated with MV should not be sufficient to disrupt binocular fusion. We found that fusional vergence ranges in presbyopes corrected with MV were not substantially different from those measured under full binocular nearpoint correction. There were small but statistically significant tendencies for the phoria and base-in vergences to be less exo under MV conditions. Comparisons of findin

Spasm of the near reflex.

Four patients with spasm of the near reflex are described. This entity is characterized by transient attacks of convergence, accommodation, and miosis, and is usually seen in young individuals as a functional disturbance. This disease has to be differentiated from other horizontal gaze disturbances, including sixth nerve palsy, divergence paralysis, and insufficiency. Despite extensive investigations, no central nervous system disease was found.

AC/A ratio, age, and refractive error in children.

PURPOSE: To examine how the response AC/A ratio (the amount of accommodative convergence per unit of accommodative response) varies as a function of refractive error and age, to determine whether it is a risk factor for the onset of myopia, and to examine the relation between ocular structural features and the AC/A ratio. METHODS: Accommodation was stimulated by a letter target presented in a Badal system at 0.00, 2.25, and 4.37 D to 828 children aged 6 through 14 years in 1996. Of these, 726 had no myopia in 1996 and were available for examination the following year. Accommodative response a

Characteristics of divergence excess type intermittent exotropia in Asian children.

BACKGROUND: Exotropia is twice as common as esotropia in Asian children, with divergence excess intermittent exotropia contributing more than one half of exotropia cases. In this study, distance-near relationships and ratios of accommodative convergence to accommodation (AC/A) are compared using different measurement methods in Asian children with divergence excess intermittent exotropia. METHODS: Children with intermittent exotropia and a distance deviation exceeding the near by at least 10(Δ) were consecutively recruited. Do bifocals reduce accommodative amplitude in convergence excess esotropia?

BACKGROUND: As therapy with bifocal lenses can generate a condition of acquired hypo-accommodation, we assessed the results of bifocal therapy in children with non-refractive accommodative esotropia in whom near point of accommodation (NPA) was measured before the prescription of the lenses and at the end of the study. METHODS: We examined 28 consecutive patients orthophoric for far, but with esotropia due to excess convergence for near (group 1), and 28 matched controls, orthophoric for far and near (group 2). The patients were prescribed bifocal lenses between the age of 5 and 8. The NPA wa
