Perioperative outcomes after laparoscopic cholecystectomy in elderly patients: a systematic review and meta-analysis.

BACKGROUND: Laparoscopic cholecystectomy is increasingly performed in an ever ageing population; however, the risks are poorly quantified. The study aims to review the current evidence to quantify further the postoperative risk of cholecystectomy in the elderly population compared to younger patients. METHOD: A systematic literature search of PubMed, EMBASE and the Cochrane Library databases were conducted including studies reporting laparoscopic cholecystectomy in the elderly population. A meta-analysis was reported in accordance with the recommendations of the Cochrane Library and PRISMA gu

CONVERSION OF LAPAROSCOPIC CHOLECYSTECTOMY INTO OPEN CHOLECYSTECTOMY: AN EXPERIENCE IN 300 CASES.

BACKGROUND: Laparoscopic cholecystectomy is getting popularity in developing countries especially in Pakistan. Conversion from laparoscopic to open cholecystectomy is also common. This study intends to evaluate the causes of conversion from laparoscopic cholecystectomy to open cholecystectomy and to establish the efficacy and safety of the procedure. METHODS: This descriptive case series was conducted in the department of General Surgery at Social Security Teaching Hospital Islamabad from November 2012 to October 2015. Patients of more than 20 years of age presenting in OPD with symptomatic g

A structured conversion score to guide oncologic colectomy during combined endoscopic-laparoscopic surgery for complex colonic polyps.

BACKGROUND: Management of complex colonic polyps with benign preoperative biopsy findings remains challenging when endoscopic resection is technically difficult or oncologic risk is uncertain. Combined endoscopic-laparoscopic surgery (CELS) offers an organ-preserving alternative to oncologic colectomy (OC), yet clear criteria for intraoperative decision-making are lacking. This study aimed to identify predictors of conversion from CELS to OC and malignant pathology and to develop an intraoperative risk score to guide escalation. METHODS: A single-center retrospective cohort study included con

Open Conversion at Robotic Adrenalectomy: Rates and Predictors.

BACKGROUND: Rates and predictors of open conversion at robotic adrenalectomy (rRA) are unknown. METHODS: We identified patients treated with rRA (National Inpatient Sample 2009-2019). First, temporal trends in open conversion were assessed using Estimated Annual Percentage Changes (EAPC). Second, multivariable logistic regression models were used to identify predictors of open conversion during rRA. RESULTS: Of 2,292 rRA patients, 286 (12.0%) underwent open conversion. The rates of open conversion decreased over time from 39.2% to 10.2% over the study span (EAPC -15.9%; p = 0.0003). Open con

Clinical outcomes of robotic-assisted versus laparoscopic cholecystectomy in nonelective procedures: A systematic review and meta-analysis.

BACKGROUND: Robotic-assisted cholecystectomy (RAC) is increasingly used for patients presenting with acute cholecystitis and other biliary conditions. The safety and efficacy of robotic surgery in elective procedures are well documented; however, evidence supporting its application in urgent and emergent surgery, key components of the acute care setting, remains limited. The aim of this study is to compare the outcomes of nonelective robotic and laparoscopic cholecystectomies. METHODS: A systematic review and meta-analysis were conducted using Embase, Cochrane, and PubMed to identify studies

Outcomes of Laparoscopic Redo Fundoplication in Patients With Failed Antireflux Surgery: A Systematic Review and Meta-analysis.

OBJECTIVE: The aim of this meta-analysis was to summarize the current available evidence regarding the surgical outcomes of laparoscopic redo fundoplication (LRF). SUMMARY OF BACKGROUND DATA: Although antireflux surgery is highly effective, a minority of patients will require a LRF due to recurrent symptoms, mechanical failure, or intolerable side-effects of the primary repair. METHODS: A systematic electronic search on LRF was conducted in the Medline database and Cochrane Central Register of Controlled Trials. Conversion and postoperative morbidity were used as primary endpoints to determi

Routine Bronchoscopy and Fogarty Catheter Occlusion of Tracheoesophageal Fistulas.

INTRODUCTION: Use of a Fogarty catheter for emergent occlusion of tracheoesophageal fistulas (TEFs) has been described for use in unstable neonates. Our purpose was to describe a case series of elective Fogarty catheter occlusion of the TEF. MATERIALS AND METHODS: A formal operative laryngoscopy and rigid bronchoscopy were performed with Fogarty catheter placement into the fistula before surgical intervention. The balloon was inflated and gentle traction provided occlusion of the tract. An endotracheal tube was then placed in a midtracheal location irrespective of fistulous tract location. Th

Laparoscopic appendectomy: Which factors are predictors of conversion? A high-volume prospective cohort study.

UNLABELLED: Appendicitis represents one of the most frequent condition requiring surgery. In Italy almost 0.2% of the population will be affected by acute appendicitis every year. Laparoscopic appendectomy (LA) has gained acceptance over the past years and despite several meta-analyses, randomized studies and retrospective studies have been conducted, the indications and results are still conflicting especially in cases of complicated appendicitis. The aim of our study is to evaluate which factors are related to conversion to open appendectomy (OA) during laparoscopic appendectomy (LA). MATHE

Risk factors for complications after adrenalectomy: results from a comprehensive national database.

PURPOSE: Most knowledge regarding outcome after adrenal surgery stems from retrospective studies reported by highly specialized centres. The aim of this study was to report a national experience of adrenalectomy with particular attention to predictive factors for postoperative complications, conversion from endoscopic to open surgery and length of hospital stay. METHODS: Adrenalectomies reported in the Scandinavian Quality Register for Thyroid, Parathyroid and Adrenal Surgery (SQRTPA) 2009-2014 were included. Risk factors for complications, conversion and hospital stay >3 days were assessed u

Study protocol for a multicenter prospective controlled and randomized trial of transanal total mesorectal excision versus laparoscopic low anterior resection in rectal cancer.

PURPOSE: Compared with the open approach, laparoscopic total mesorectal excision (TME) achieves faster patient recovery, reduces morbidity rates, and shortens hospital stay. However, in laparoscopic low anterior resection (L-LAR), conversion to open surgery is required in almost 20% of cases. Transanal TME (Ta-TME) combined with laparoscopy, also called hybrid natural orifice transluminal endoscopic surgery (NOTES), is a less invasive procedure that can overcome some of the limitations of laparoscopic rectal surgery. In this study, we aim to determine whether Ta-TME has a lower rate of convers

Correlation among ultrasonographic and videoscopic findings of the gallbladder: surgical difficulties and reasons for conversion during laparoscopic surgery.

Laparoscopic cholecystectomy is currently the standard procedure for chronic and acute cholecystitis. The purpose of this prospective study was to evaluate the preoperative ultrasound findings of the gallbladder and correlate those aspects with surgical videoscopic and histopathologic findings and the results concerning intraoperative complications and the conversion index to open surgery. Gallbladder findings were classified into three categories according to the gallbladder wall characteristics and the presence of visible lumen and stones. Simple chronic cholecystitis (type I) and acute chol

Predictors of conversion and outcomes after conversion in laparoscopic redo ileocolic resection for Crohn's disease.

BACKGROUND: Redo ileocolic resection (ICR) for Crohn's disease is technically challenging, and surgeons often debate whether a laparoscopic approach should be attempted given the risk of conversion. We evaluated predictors of conversion and postoperative outcomes following conversion to inform operative decision-making. METHODS: Patients undergoing redo ileocolic resection for Crohn's disease from 2009 to 2021 at a quaternary referral centre were identified from a prospectively maintained database. Cases were categorized as completed laparoscopically, converted or planned open. Predictors of

Predicting conversion of laparoscopic cholecystectomy for acute cholecystitis.

BACKGROUND AND OBJECTIVES: Laparoscopic cholecystectomy can be safely performed in patients with acute cholecystitis. However, the rate of conversion to open cholecystectomy remains higher when compared with patients with chronic cholecystitis. Preoperative clinical or laboratory parameters that could predict the need for conversion may assist the surgeon in preoperative or intraoperative decision making. This could have cost-saving implications. METHODS: A retrospective review of 46 patients undergoing laparoscopic cholecystectomy for acute cholecystitis was performed. Records were assessed

Single-Incision Laparoscopy-First Strategy in Crohn's Disease: Risk Factors for Conversion and Surgical Outcomes.

BACKGROUND: Single-incision laparoscopic surgery (SILS) is an evolving minimally invasive technique for Crohn's disease (CD). However, evidence regarding conversion risk, specifically in SILS, is limited. This study aimed to identify independent predictors of conversion to open surgery. METHODS: We retrospectively analyzed patients with CD who underwent intestinal resection using an SILS-first strategy between April 2018 and October 2025. The patients were classified into conversion and non-conversion groups. Logistic regression analyses were performed to analyze risk factors for conversion t

Factors determining conversion from laparoscopic to open Roux-en-Y gastric bypass.

BACKGROUND: Conversion from laparoscopic to open Roux-en-Y gastric bypass (RYGBP) is expensive and time-consuming. METHODS: Data from our first 1,000 laparoscopic RYGBP was entered into a database (Minnesota Database-Bariatric, Exemplo Medical). All patients met NIH criteria for bariatric surgery. RESULTS: 41 (4.1%) of 1,000 consecutive lapRYGBPs were converted to open. Patients requiring conversion to open surgery, analyzed for predictors, revealed: 1) BMI, waist size, and weight all were significantly greater in patients converted to open bypass; 2) Gender: 9 of 109 males (8.3%) and 32 of

Laparoscopic nephrectomy for urolithiasis: when is better to avoid it.

OBJECTIVE: to investigate the risk factors for conversion to open surgery in laparoscopic nephrectomy (LN) for urolithiasis. METHODS: we reviewed data on all patients over 18 years of age submitted to LN between January 2006 and May 2013 at our institution. We analyzed the Charlson's index, the ASA score, renal function by the equation and stage of MDRD (Modification of Diet in Renal Disease), preoperative computed tomography (CT) findings, complications by the Clavien-Dindo classification and conversion rate. We used logistic regression analysis to determine the risk factors for conversion.

Indications for laparoscopy in the management of intussusception: A multicenter retrospective study conducted by the French Study Group for Pediatric Laparoscopy (GECI).

INTRODUCTION: Surgical management of intussusception is required for cases where enema reduction fails. Some articles report an advantage of the laparoscopic over the open approach, but complications such as serosal tearing and frank perforation have been described. We aim to determine the best indication of laparoscopy in the decisional management tree of intussusception. PATIENT AND METHOD: This is a retrospective chart review of all children with intussusception who failed hydrostatic enema reduction and who underwent immediate laparoscopic management. Cases were compiled from 7 pediatric

Fundus-first dissection: a salvage technique for the difficult laparoscopic cholecystectomy.

BACKGROUND: Fundus-first dissection (FFD) is an established salvage technique for difficult cholecystectomies when the critical view of safety (CVS) is unachievable. However, it is not widely practised due to perceived risks of dissecting inflamed or contracted gallbladders or fibrosed cystic pedicles. This study aimed to evaluate the safety and outcomes of FFD in a large series of difficult laparoscopic cholecystectomies (LC). METHODS: A prospectively maintained database, with retrospective analysis of outcomes of LC performed by a single surgeon between 1992 and 2022. FFD, was employed in N
