Tsunamis hiding in plain sight: spreading depression in clinical neurology.

Spreading depression is a neurophysiological phenomenon that is observed in the central nervous system of many species, from insects to humans. In essence, spreading depression is a slowly propagating wave of mass depolarization (that is, spreading depolarization), aptly termed a 'brain tsunami', which successively engulfs contiguous brain regions, causing transient neuronal hyperexcitability at its leading edge, followed by complete but reversible neuronal silence lasting minutes. This wave cannot be detected in routine scalp EEG recordings, which contributes to its under-recognized status as

Perspectives on spreading depression.

Spreading depression (SD) consists of a transient suppression of all neuronal activity that spreads slowly across regions of gray matter. The paper is divided into three parts. Martins-Ferreira describes 30 years of research on SD in the isolated retina. Much of this work has relied on the prominent intrinsic optical signals that accompany SD in the retina. By inducing SD to propagate in circles with a velocity of 3.7 mm min(-1), it is possible to investigate the finely balanced electrochemical equilibrium that maintains the traveling wave. SD is accompanied by a slow negative extracellular vo

EEG patterns in each phase of hemiplegic migraine: a systematic review and pooled prevalence meta-analysis.

INTRODUCTION: Hemiplegic migraine (HM) is a rare subtype of migraine with aura, and its pathophysiology is similar to that of migraine with aura, primarily involving cortical spreading depression. Electroencephalography (EEG) may serve as a valuable tool for investigating brain function in patients with hemiplegic migraine. Therefore, this study aims to synthesize available evidence through a systematic review and pooled prevalence meta-analysis to assess the types of EEG abnormalities observed in each migraine phase in patients with hemiplegic migraine and to estimate the prevalence of EEG ab

Traveling waves in a model for cortical spreading depolarization with slow-fast dynamics.

Cortical spreading depression and spreading depolarization (CSD) are waves of neuronal depolarization that spread across the cortex, leading to a temporary saturation of brain activity. They are associated with various brain disorders such as migraine and ischemia. We consider a reduced version of a biophysical model of a neuron-astrocyte network for the initiation and propagation of CSD waves [Huguet et al., Biophys. J. 111(2), 452-462, 2016], consisting of reaction-diffusion equations. The reduced model considers only the dynamics of the neuronal and astrocytic membrane potentials and the ex

Spreading depolarization as a therapeutic target in migraine.

Migraine with aura is characterized by recurrent attacks of visual and, occasionally, sensory, language and/or motor disturbances, typically followed by headache. Migraine with aura can be associated with allodynia and vascular and psychiatric comorbidities. The electrophysiological cause of the aura is cortical spreading depolarization, a wave of depolarization that propagates slowly across the cortical surface, producing reversible metabolic and electrochemical perturbations. In this Review, we focus on the relationship of spreading depolarization with migraine aura and migraine headache. Ab

Is canonical SD a clinically relevant model of migraine? Argument pro.

Migraine with aura is a phenotypically heterogeneous disorder characterized by scintillating scotoma, sensory, language, and/or motor disturbance often followed by a severe headache, and cutaneous allodynia. Cortical spreading depression (SD), a neuro-glial slowly propagating depolarizing wave, is the likely electrical event responsible for aura and a headache trigger. While clinical and preclinical observations support these relationships, there remains controversy over the role of SD as a model of migraine. This article as part of a series of debate articles will focus on data supporting SD

History of migraine.

Migraine symptoms were described in ancient Babylonia, and supernatural forces were felt to play a role in etiology and treatment. This changed in the Greco-Roman period, when the (dis)balance of humors was considered in (patho)physiology and treatment based on this. Aretaeus distinguished between cephalalgia, cephalea, and heterocrania. The latter term was changed to hemicrania by Galen. Physicians in the 17th century attributed headache to the meninges, extracranial periost, and cranial blood vessels. As for the pathophysiology, Willis suggested intracranial vasoconstriction with subsequent

Pathophysiology of migraine--from molecular to personalized medicine.

INTRODUCTION: Understanding of migraine pathophysiology has substantially improved over the last two decades. As a result, migraine is now mainly considered to be a disorder of the brain, rather than one of the vasculature or the meninges. PATHOPHYSIOLOGY: Although it remains speculative how exactly they relate to each other, the following three processes are important in migraine: 1. Cortical spreading depression is a wave of intense depolarization, it starts in the occipital lobe, propagates through the brain and is followed by a period of suppressed activity. 2. Activation of the trigemono

Spreading depolarizations pose critical energy challenges in acute brain injury.

Spreading depolarization (SD) is an electrochemical wave of neuronal depolarization mediated by extracellular K+ and glutamate, interacting with voltage-gated and ligand-gated ion channels. SD is increasingly recognized as a major cause of injury progression in stroke and brain trauma, where the mechanisms of SD-induced neuronal injury are intimately linked to energetic status and metabolic impairment. Here, I review the established working model of SD initiation and propagation. Then, I summarize the historical and recent evidence for the metabolic impact of SD, transitioning from a descripti

Spreading depolarization suppression from inter-astrocytic gap junction blockade assessed with multimodal imaging and a novel wavefront detection scheme.

Spreading depolarizations (SDs) are an enigmatic and ubiquitous co-morbidity of neural dysfunction. SDs are propagating waves of local field depolarization and increased extracellular potassium. They increase the metabolic demand on brain tissue, resulting in changes in tissue blood flow, and are associated with adverse neurological consequences including stroke, epilepsy, neurotrauma, and migraine. Their occurrence is associated with poor patient prognosis through mechanisms which are only partially understood. Here we show in vivo that two (structurally dissimilar) drugs, which suppress astr

Migraine and Two-Pore-Domain Potassium Channels.

Migraine is a common, disabling neurological disorder with a genetic, environmental, and hormonal component with an annual prevalence estimated at ~15%. It is characterized by attacks of severe, usually unilateral and throbbing headache, and can be accompanied by nausea, vomiting, and photophobia. Migraine is clinically divided into two main subtypes: migraine with aura, when it is preceded by transient neurological disturbances due to cortical spreading depression (CSD), and migraine without aura. Activation and sensitization of trigeminal sensory neurons, leading to the release of pro-inflam

Interictal hemodynamic abnormality during motor activation in sporadic hemiplegic migraine: An explorative study.

INTRODUCTION: The motor weakness in sporadic hemiplegic migraine (SHM) is a poorly understood aura manifestation. Cortical spreading depression affecting motor excitability and alterations of neurovascular coupling may be integral to the development of migraine aura. METHODS: We studied 10 right-handed SHM patients and 17 healthy controls with functional near-infrared spectroscopy (fNIRS) in the interictal period. Subjects performed a finger opposition task and had real time determination of oxyhemoglobin (OxyHb) and deoxyhemoglobin (deOxyHb) changes. Recordings were completed with 10 left an

Direct electrophysiological evidence that spreading depolarization-induced spreading depression is the pathophysiological correlate of the migraine aura and a review of the spreading depolarization continuum of acute neuronal mass injury.

Spreading depolarization is observed as a large negative shift of the direct current potential, swelling of neuronal somas, and dendritic beading in the brain's gray matter and represents a state of a potentially reversible mass injury. Its hallmark is the abrupt, massive ion translocation between intraneuronal and extracellular compartment that causes water uptake (= cytotoxic edema) and massive glutamate release. Dependent on the tissue's energy status, spreading depolarization can co-occur with different depression or silencing patterns of spontaneous activity. In adequately supplied tissue

Is canonical SD a clinically relevant model of migraine? Argument con.

Cortical spreading depolarization (CSD) is a unique brain response to a neuronal insult, manifested by a massive exchange of intra- and extracellular compartments, loss of ionic gradients, and membrane potential, where neurons are caged in this pathological state for minutes with no activity. This electrochemical tsunami propagates slowly by invading the neighboring intact cortex in the whole hemisphere in rodents, and it is transient in metabolically uncompromised brains. Vascular reactivity of brief hyperemia, prolonged oligemia, increased inhibitory synaptic tonus, intracellular edema, and

Cortical mechanisms in migraine.

Migraine is the second most prevalent disorder in the world; yet, its underlying mechanisms are still poorly understood. Cumulative studies have revealed pivotal roles of cerebral cortex in the initiation, propagation, and termination of migraine attacks as well as the interictal phase. Investigation of basic mechanisms of the cortex in migraine not only brings insight into the underlying pathophysiology but also provides the basis for designing novel treatments. We aim to summarize the current research literatures and give a brief overview of the cortex and its role in migraine, including the

Neocortical in vivo focal and spreading potassium responses and the influence of astrocytic gap junctional coupling.

Raised extracellular potassium ion (K+) concentration is associated with several disorders including migraine, stroke, neurotrauma and epilepsy. K+ spatial buffering is a well-known mechanism for extracellular K+ regulation/distribution. Astrocytic gap junction-mediated buffering is a controversial candidate for K+ spatial buffering. To further investigate the existence of a K+ spatial buffering and to assess the involvement of astrocytic gap junctional coupling in K+ redistribution, we hypothesized that neocortical K+ and concomitant spreading depolarization (SD)-like responses are controlled

Thalamocortical network interruption: A fresh view for migraine symptoms.

Migraine is a multifaceted brain disorder where multisensory disturbances are associated with headache. Yet sensory symptoms are conventionally justified by dysfunctions confined to the cerebral cortex, a perspective through the complex interplay of thalamocortical network would provide the entire picture, more pertinent to the central sensory processing. It is important to consider thalamus as a hub that integrates multiple domains via extensive connections among anatomically and functionally separate cortical areas. Accordingly, cortical spreading depression (CSD), implicated in migraine pat

Spreading ischemia after aneurysmal subarachnoid hemorrhage.

Spreading depolarization (SD) is a wave of mass neuronal and glial depolarization associated with net influx of cations and water. Prolonged SDs facilitate neuronal death. SD induces tone alterations in cerebral resistance arterioles, leading to either transient hyperperfusion (physiological neurovascular coupling) in healthy tissue or hypoperfusion (inverse neurovascular coupling = spreading ischemia) in tissue at risk for progressive damage. Spreading ischemia has been shown experimentally in an animal model replicating the conditions present following aneurysmal subarachnoid hemorrhage (aSA

State-of-the-art preclinical techniques to study the impact of spreading depolarizations in awake rodents.

BACKGROUND: Understanding the mechanisms of pathological brain network activity and the efficacy of therapies requires testing hypothesis in vivo, where brain circuitry remains preserved. Therefore, animal models are a key tool in the study of primary neurological disorders such as migraine, stroke and epilepsy. These models not only have advanced our understanding of the underlying neurobiology of these disorders but have also provided novel pharmacological targets and insights on shared pathophysiological processes such as spreading depolarizations (SD). [Epileptic seizure and migraine attack: A revisit from the "Borderland of Epilepsy" to clinical implementation of infraslow activity/DC shifts in scalp EEG].

Migraine attacks, especially ones with aura, have symptoms similar to epileptic seizures, and the two may sometimes be difficult to differentiate clinically. However, the characteristic minute-by-minute symptom development and progress within 60 ‍min is useful for diagnosis. Although the details of its pathophysiology remain unsolved, cortical spreading depolarization (CSD) is one of the main pathogenetic factors. In epilepsy, clinical data have shown that ictal DC shifts could reflect impaired homeostasis of extracellular potassium by astrocyte dysfunction. Ictal DC shifts were found to be di
